The Youth in India: Situation and Needs Study (Youth Study) is the first-ever subnationally representative study conducted to identify key transitions experienced by married and unmarried youth in India. While today’s youth are healthier, more urbanized, and better educated than earlier generations, social and economic vulnerabilities persist. In the course of the transition to adulthood, young people face significant risks related to sexual and reproductive health, and many lack the knowledge and power to make informed sexual and reproductive choices. This working paper reviews the highly diverse regional and country patterns of HIV epidemics and discusses possible causes of the geographic variation in epidemic sizes. The main finding of this analysis is that the HIV epidemic reached a major turning point over the past decade. The peak years of HIV incidence rates are past for all regions, and the peaks of prevalence rates are mostly in the past except in Eastern Europe, where it is expected to peak in 2008. Over the past quarter century fertility has declined rapidly in many developing countries. Projections typically assume that this trend will continue until the replacement level is reached. However, recent evidence suggests that ongoing fertility declines may have slowed or stalled in a number of countries in transition. This Population Council study examines the pace of fertility change in developing countries that have multiple DHS surveys to determine whether ongoing transitions are decelerating or stalling. Internal migration in Southeast Asia raises questions about strains upon traditional systems of support for older adults. While remittances to parents’ households play a role in rural household economies, uncertainty remains regarding whether and under what circumstances children interact with their elderly parents. This paper focuses on the adult children of older persons living in rural Cambodia and Thailand and examines the determinants of personal visits, monetary remittances, and more general forms of household support. This paper explores how poverty affects childbearing patterns in the contemporary developing world. In considering the association between poverty and fertility, we explore one measure of economic status, household asset holdings, in Demographic and Health Surveys conducted in 51 countries since 1990. The results show that the association between poverty and fertility differs according to the measure of fertility considered. We conclude there is much to be gained from distinguishing between the impact of poverty on fertility aspirations and the implementation of those aspirations.

Using data from the 2004 National Survey of Adolescents conducted in Burkina Faso, Ghana, Malawi, and Uganda, this Population Council working paper investigates the timing of two key transitions in adolescence—school exit and premarital sex—among those who remain enrolled in school at the beginning of adolescence (age 12). Girls appear more vulnerable to dropout once they become sexually mature and once they engage in premarital sex. While girls were found to be less likely than boys, at any given age and controlling for other covariates, to have had premarital sex (except in Ghana), school enrollment and the timing of school …

For some time, Chinese government policies have treated rural and urban areas very differently, and a by-product of China’s rapid development seems to be an even greater differentiation between urban and rural social and economic life. This Population Council working paper assesses the effects of primary school characteristics, household characteristics, and recent household economic and demographic shocks on school dropout rates during the first eight grades in rural Punjab and North West Frontier Province, Pakistan. While grade retention has improved over the past six years, dropout rates for girls remain fairly high, particularly at the end of primary school (grade five). The results of this study show clearly the complementary nature of supply and demand factors in determining grade progression in rural Pakistan, particularly for girls. This Population Council working paper assesses the potential roles of late age at marriage and a long period of premarital sexual activity as population risk factors for HIV infection in sub-Saharan Africa. The relationship between marital status and the prevalence and incidence of HIV is examined with ecological data from 33 countries in the region and with individual-level data from nationally representative Demographic and Health Surveys in Kenya and Ghana in 2003. For any given country, the aim of population policy may be narrowly construed as bringing about quantitative changes in the membership of the territorially circumscribed population under the government’s jurisdiction. Marriage And Childbirth As Factors In School Exit: An Analysis Of Dhs Data From Sub-Saharan Africa, Cynthia B. Lloyd, Barbara Mensch

This paper explores the potential importance of marriage and childbirth as determinants of school-leaving in sub-Saharan Africa and identifies some of the common underlying factors that contribute to premature school-leaving and early marriage and childbearing. Results suggests that the reproductive health community should see early marriage as a central area of concern for adolescent reproductive health. Policies that inform parents about the value of starting their children in school on time are likely to have beneficial effects both for grade attainment and for adolescent reproductive health regardless of school quality.

After a brief review of the controversy about future trends, this Population Council working paper examines past trends in the components of life expectancy at birth in high-income countries with low levels of mortality. The projection of these components provides the basis for assessing plausible future trends in longevity. Large increases in conventional life expectancy before 1950 are found to be primarily attributable to reductions in juvenile and background mortality. After 1950 the rate of improvement in life expectancy slowed, but senescent mortality fell more rapidly than before, thus becoming the main cause of rising life expectancy at birth. The …

The usual lessons drawn from East Asia’s striking experience of health and fertility transition concern the efficacy of well-designed government programs catering to an existing or ideationally stimulated demand. An alternative interpretation sees the demographic change—and the uptake of services—as a by-product of social and economic development together with, in some cases, strong government pressures. This working paper examines the distribution of household wealth and tests whether associations exist between wealth inequality and health outcomes among older adults living in one of the world’s poorest regions, rural Cambodia. Results confirm difficult economic conditions among most elderly in rural Cambodia. This study suggests there is some validity to generalizing the relationship between wealth inequality and health to extremely poor populations and that a very small difference in wealth makes a relatively large difference in regard to the association with health among those living in impoverished surroundings.

Funding for reproductive health services is stagnant or declining globally, yet population projections, particularly in Africa, indicate that demand for services will increase in the near term. Between 2002 and 2025, for example, the population of women of childbearing age (15–49) is expected to increase by 2 percent annually in sub-Saharan Africa. With this growth will come increased demand for contraception, and other reproductive health services such as antenatal care, safe birthing services, and postpartum care for mothers and children. An examination of fertility trends in countries with multiple DHS surveys found that in the 1990s fertility stalled in mid-transition in seven countries: Bangladesh, Colombia, Dominican Republic, Ghana, Kenya, Peru, and Turkey. An analysis of trends in the determinants of fertility revealed a systematic pattern of leveling off or near leveling in a number of determinants, including contraceptive use, the demand for contraception, and wanted fertility. Findings suggest no major deterioration in contraceptive access during the stall, but levels of unmet need and unwanted fertility are relatively high and improvements in access to family planning methods would therefore be desirable. …

This study’s aims are to: (1) determine the prevalence of functional limitations among older adults in Cambodia using activities of daily living (ADLs); (2) compare limitation prevalence with other countries in the region; (3) estimate active life expectancy; (4) examine standard correlates of functional status and assess whether they are associated with limitation in expected ways. Elderly Cambodians appear more likely to report limitations than their counterparts in neighboring countries. A contribution of the analysis is the examination of a basic measure of health among a population that until recently has been isolated from the rest of the world.

This paper presents findings of a Population Council investigation into the effects of living standards and relative poverty on children’s schooling in urban and rural areas of Senegal. The research shows that in Senegal’s urban areas, living standards exert substantial influence on three measures of schooling: whether a child has ever attended school; whether he or she has completed at least four grades of primary school; and whether he or she is currently enrolled. Sustainability refers to the preservation of human-valued natural capital—the resources that provide environmental services—at a level sufficient to assure the well-being of future generations. This Population Council working paper assess the effects on the total and per capita availability of those services. The degradation of environmental services—exemplified by the overuse of aquifers or (at a global level) of the atmospheric carbon sink—is a significant threat to sustainable development, one that is often exacerbated by population growth. This Population Council working paper examines the causal structure of the relationship between child mortality events and subsequent fertility during a time of rapid decline in fertility in Bangladesh. Results lend support to the hypothesis of an insurance effect, while demonstrating that its demographic significance is likely to be less prominent than that of replacement behavior. Findings indicate that the insurance motive remains intact even after total fertility declined to fewer than three children per woman. This Research Division working paper develops and applies a general framework for the analysis of the period quantum and tempo of life-cycle events, extending methods developed previously by the authors. The existence of tempo distortions is demonstrated in selected period quantum measures such as the total fertility rate and in period tempo measures such as life expectancy. A tempo distortion is defined as an undesirable inflation or deflation of a period quantum or tempo indicator of a life-cycle event, such as birth, marriage, or death, that results from a rise or fall in the mean age at which the event …
