According to the preliminary investigation the incident is caused due to a pressure applied to the mine from the heavy RAKE (the excavation tool) used by the deminer and the deminer didn’t expect to find a mine in that spot with that depth and may like accident could be avoided by using the metal detector to locate the non visible mine before using the standard RAKE drill.

In 2007 a task force was convened by Cal Poly's Provost to envision The Future of the Library. One of the top recommendations of this report was that the Kennedy Library should foster the concept of “the library as place.” The report stated that “the library needs to be an active space that meets a multitude of academic and social needs,” and recommended that Cal Poly “renovate and expand the library as a multi-use, social and academic center of campus”. Based upon the concepts and procedures of Appreciative Inquiry, Jacqueline Kelm offers Appreciative Living, a qualitative research informed therapeutic program for one. To this end Kelm presents a 28-day plan designed to help individuals achieve greater happiness via a three-step model. She also shares results from a study she conducted with participants who have tried her approach to further demonstrate the usefulness of applying Appreciative Inquiry means to therapeutic ends.
