Practice Problem: Health care worker (HCW) burnout is a pervasive phenomenon within the healthcare industry, affecting professionals across various disciplines. Emotional exhaustion is a prominent feature, leading to feelings of being emotionally drained, overwhelmed, and unable to cope effectively with the demands of the job. Additionally, burnout often results in a significant lack of motivation among HCWs, who may struggle to find meaning and purpose in their work, exacerbating feelings of frustration and disillusionment. Fear-based disorders, including anxiety and posttraumatic stress disorder, are more prevalent in females than males. Steroid hormones (e.g., estradiol) are involved in fear extinction learning and later recall—a process implicated in the pathophysiology of fear-based disorders. Fear-based disorders commonly emerge during the transition from childhood to adolescence, a period marked by the onset of puberty and a natural increase of steroid hormones. Here, we examined the effects of puberty, sex, and their interaction on extinction recall and underlying neural circuitry in youth. The medial prefrontal cortex (mPFC) is a key region for emotion regulation, and its failure to regulate extinction of aversive memories is associated with Post Traumatic Stress Disorder. Study of how the Infralimbic cortex (IL), a subregion of the mPFC, directly communicates with the Basolateral Amygdala (IL-BLA), has thus far failed to uncover the circuit mechanisms that lead to mPFC-mediated amygdala suppression of firing and diminished fear responses during extinction retrieval. Background

Anhedonia, an impairment in the motivation for or experience of pleasure, is a well-established transdiagnostic harbinger and core symptom of mental illness. Given increasing recognition of early life origins of mental illness, we posit that anhedonia should, and could, be recognized earlier if appropriate tools were available. However, reliable diagnostic instruments prior to childhood do not currently exist.

Methods

We developed an assessment instrument for anhedonia/reward processing in infancy, the Infant Hedonic/Anhedonic Processing Index (HAPI-Infant). Exploratory factor and psychometric analyses were conducted using data from 6- and 12-month-old infants from two cohorts (N = 188, N = 212). …

Introduction: We report on a patient with untreated severe mental illness who presented with a life-threatening emergency: retained products of conception and hemorrhage.

Clinical Findings: A female patient experiencing homelessness developed life-threatening hemorrhage. Her mental illness impaired effective communication and treatment.

Clinical Course: The patient presented with fatigue, vaginal bleeding, and known retained products of conception. Her active mental illness complicated the situation as it limited effective communication and treatment due to delusions. She requested only treatment for an infectious cause of her symptoms. Background: In the United States, 52.8% of Americans aged 18 and older consume alcohol, with 29.5 million suffering from alcohol use disorder (AUD). In Los Angeles, 69.8% of Angelinos aged 12 and older suffer from AUD, and 33% of annual arrests are due to alcohol and other drugs. Alcohol withdrawal syndrome (AWS) develops after abrupt cessation of heavy drinking and is a common occurrence among incarcerated people when newly arrested, resulting in $1.3 billion in healthcare costs. Management of AWS using a fixed-dose therapy of benzodiazepines will improve the utilization of healthcare services in the correctional setting.

Childhood sexual abuse (CSA) is a notable public health concern, affecting a significant proportion of girls in high-income countries. A considerable number of CSA survivors develop Post-Traumatic Stress Disorder (PTSD) by adolescence, which is often considered to be a lifelong disorder with severe emotional and social consequences. Women with CSA-related PTSD display hypervigilance, which is characterized by significantly increased fear network activity and poor top-down control over the amygdala. This meta-analysis examines the potential of intranasal oxytocin (OT) as a treatment for hypervigilance symptoms in women with CSA-related PTSD. Major depressive disorder (MDD) is a mood disorder characterized by persistently depressed mood and/or anhedonia that causes significant functional impairment in daily life. MDD affects nearly 15% of adults in their lifetime, and since 2019, prevalence estimates have increased up to ~30% in young adults (18-25 years old). In addition to its debilitating effects on daily functioning, epidemiological studies link MDD to ~3 times greater risk of cardiovascular disease (CVD), followed by accelerated CVD progression and mortality. Although the mechanisms contributing to MDD-CVD comorbidity are unclear, there is evidence of stress system dysfunction in adults with MDD.

Employing the robust framework of the Social Ecological Model (SEM) and drawing insights from the Social Determinants of Health (SDOH), this research sought to uncover the interplay between SDOH and mental health among adolescents. With a focus on depression, the study unraveled how various social determinants, both individually and collectively, shape the mental well-being of adolescents, leveraging the rich dataset provided by the National Survey of Children’s Health 2018. Alcohol use disorder (AUD) is a prevalent condition and is a leading cause of death for adults aged 20 to 64 (Esser et al., 2022). Patients with AUD who present to acute care hospitals seeking medical treatment are at risk for alcohol withdrawal. Undertreated alcohol withdrawal can progress to delirium tremens, which can result in increased transfers to ICU level of care, increased need for sedative medications, and has been shown to prolong hospital length of stay. More than half of Americans will be diagnosed with a mental illness in their lifetime (CDC, 2021), yet stigma towards mental health affects both patients and providers. Although programs exist to address stigma, improve cultural competency among providers, and educate families on the importance of support, facilities are often limited on programs they provide due to allocation of resources and funds. Without a shift in treatment and programing, stigma will continue to impact patient care and outcome.

Background: Identifying risk factors for suicidal thoughts and behaviors, especially causal ones, will be imperative to improving screening and treatment interventions. To identify replicable brain-based risk factors, there is increasing evidence indicating the need for large sample sizes, in the hundreds to thousands, in order to achieve a sufficient level of statistical power to detect them. This conceptual overview offers a comprehensive overview of systemic pathways that negatively impact the mental health of Black Men throughout their lives. Our argument highlights the importance for counselors and mental health professionals to utilize a thorough social risk assessment that considers these pathways in order to effectively address the mental health needs of Black Men while fostering positive working relationships. This overview strongly advocates for the use of context and structural determinants when evaluating mental health symptoms. Without an appropriate understanding of social risk and determinants, counselors may inadvertently perpetuate disparities by decontextualizing symptomology, and reproducing racist discourse.

Queer women face disproportionately high rates of postpartum depression compared to heterosexual and cisgender individuals. Studies showed that queer women face heightened mental health challenges, including higher rates of depression and anxiety due to factors like discrimination, limited support systems, and negative healthcare experiences. Based on the use of drugs to treat hypertension and tuberculosis for depression, the monoamine hypothesis led to the development of antidepressants in 1952. Serotonin was recognized as a deficiency associated with depression in 1967. As a fine-tuning orchestration of neurotransmitters and neuromodulators, the glutamate system became known using Spravato in 2019 and Auvelity in 2022. In this pilot study, mental health experts rated the importance of 24 science-based recommendations to optimize glutamatergic therapy in depression for a proposed guideline on a Likert scale from zero to four. The life expectancy of individuals with DS has drastically improved over the decades, increasing from 9 years in 1929 to 25 years in 1983 and further reaching 60 years by 2002.
