Advances in Clinical Medical Research and Healthcare Delivery

Major depressive disorder (MDD) is a prevalent and debilitating mental illness that affects millions globally. Traditionally, the pathophysiology of MDD was thought to be due primarily neurotransmitter imbalances. However, emerging research highlights the role of neuroinflammation in MDD pathogenesis. Elevated levels of pro-inflammatory cytokines, microglial activation, and blood-brain barrier dysfunction have been implicated in the etiology of MDD. Additionally, biomarkers such as C-reactive protein and neuroimaging findings further support the neuroinflammatory hypothesis. This growing body of evidence has led to increased interest in novel treatment approaches, including anti-inflammatory medications and lifestyle interventions, as potential therapeutic strategies. Despite a wide array of therapeutic modalities, many patients with Major Depressive Disorder (MDD) experience no significant improvement in symptoms with traditional treatment. Given the prevalence of Treatment Resistant Depression (TRD), other alternative therapeutic modalities for MDD are being researched. Repetitive Transcranial Magnetic Stimulation (rTMS) is one modality that continues to show successful therapeutic outcomes for those with TRD. The aim of this study is to highlight a case of rTMS being used on a patient with TRD, autism spectrum disorder (ASD), post-traumatic stress disorder (PTSD), and substance use.

Purpose

This quality improvement project aimed to implement Lean strategies to optimize the intake process in a psychiatric emergency department, thereby reducing patient wait times, minimizing left-without-being-seen (LWBS) occurrences, and improving access to timely psychiatric care.

Background

The project was conducted in a hospital-based psychiatric emergency department in Southeast Texas, serving adults and children six years and older.

Lean methodology and PDSA cycles were employed to refine the intake process and eliminate inefficiencies. Background: Obsessive-compulsive disorder (OCD) affects up to 2.5% of the population with significant impacts on everyday functioning and relationships. First-line treatment includes selective serotonin reuptake inhibitors (SSRI) and cognitive behavioral therapy; however, studies have found that up to 40-60% of patients do not respond to SSRIs. ECT is an alternative approach that has demonstrated limited efficacy in OCD.

A Long Non-Coding Erna Forms R-Loops To Shape Emotional Experience-Induced Behavioral Adaptations, Rose Marie Akiki

Emotional experiences often evoke neural plasticity that supports adaptive changes in behavior, including maladaptive plasticity associated with mood and substance use disorders. These adaptations are supported in part by experience-dependent activation of immediate-early response genes, such as Npas4. We discovered that a conserved, long non-coding enhancer RNA (lnc-eRNA) transcribed from an activity-sensitive enhancer produces DNA-RNA hybrid R-loop structures that support 3D chromatin-looping of the enhancer and proximal promoter and stimulus-induced, rapid Npas4 gene induction. We also show that this Npas4 lnceRNA and its R-loops are required for the development of behavioral adaptations produced by chronic psychosocial stress or cocaine exposure, …

Medical imaging devices have enabled doctors to render images of the brain without cutting into the body. These images are colloquially called “brain scans.” Through journalism and mass dissemination online, brain scans have become an example of Michael Calvin McGee’s “ideograph,” a language term that subtly takes on outsized political and symbolic meaning to enforce state power. In conversation with theories of new materialism, I situate the brain scan as an ideograph within Jenny Edbauer’s model of rhetorical ecologies. People with some types of post-stroke aphasia often experience depression as well as loss of speech. Music therapy has unique properties that allow it to be used therapeutically to treat both speech impairment and depression yet it is rarely used to treat both disorders. This literature review seeks to examine the underlying causes for this discrepancy, review methods used by practitioners to treat both disorders and suggest future directions for additional research and implications for the treatment of patients with aphasia who have both language impairment and depression.

Temporary anxiety is a healthy response to stress; however, continuous anxiety can threaten an individual’s day-to-day living by eliciting negative responses. In the study, monozygotic 84 (MZ) and 153 dizygotic (DZ) adolescent twins pairs, ages 15 to 20 years, participated in anxiety-provoking tasks and subjective experiences of anxiety were measured using subjective units of distress (SUDS). The twin participants were recruited through the Mid-Atlantic Twin Registry (MATR) at Virginia Commonwealth University. The American Psychiatric Association and academic psychiatry in the United States have two conflicts of interest that may affect their assessment of psychiatric drugs and their development of diagnostic and clinical care guidelines: payments from pharmaceutical companies and guild interests. Until recently, the proposed solution to industry-academic relationships has been transparency. However, cognitive dissonance research reveals that disclosure is not a solution because cognitive biases are commonplace and difficult to eradicate. Aboriginal Policy Research Consortium International (APRCi)

Objective: To describe and characterise treated psychotic disorders in the Indigenous populations of Cape York and the Torres Strait.

Design: Cross-sectional analysis of patients with a psychotic disorder identified by treating psychiatrists.

Setting and participants: Indigenous patients aged 􏰀 15 years in Cape York and Torres Strait communities receiving treatment for a psychotic disorder over 3 months in 2010.

Main outcome measures: Prevalence of psychosis diagnoses, intellectual disability, and substance use comorbidities.

Results: 171 patients were included. The prevalence rate in this population was 1.68%, higher for males (2.60%) than females (0.89%), and twice as high in the Aboriginal (2.05%) …
