Background: A diabetes mellitus (DM) patients sometimes exhibit a pseudo-normal diastolic relaxation during echocardiography examination, which may lead to an incorrect diagnosis. Exercise echocardiography help uncover this hidden alteration, but conventional exercise techniques contain movement artifacts. Recently, an isotonic handgrip exercise was introduced and caused hemodynamic and diastolic changes with less movement. Objective: To investigate hemodynamic and left ventricular diastolic that responses to a dynamic handgrip exercise (DHE) in DM patients. Methods: 12 controls and 12 DM patients were recruited and evaluated for hemodynamic and diastolic functions at rest and 3 minutes after performing DHE. Results: DHE increased systolic blood …

Emerging evidence suggests that enlarged perivascular spaces (ePVS) may be a clinically significant neuroimaging marker of global cognitive function related to cerebral small vessel disease (cSVD). We tested this possibility by assessing the relationship between ePVS and both a standardized measure of global cognitive function, the Montreal Cognitive Assessment (MoCA), and an established marker of cSVD, white matter hyperintensity volume (WMH) volume. One hundred and eleven community-dwelling older adults (56–86) underwent neuroimaging and MoCA testing. Quantification of region-specific ePVS burden was performed using a previously validated visual rating method and WMH volumes were computed using the standard ADNI pipeline. Separate …

Perceptions And Attitudes Of Physicians And Midwives Towards Patient Safety At The Maternity Department Of A Tertiary Teaching Hospital In Kenya: A Mixed Method Study, Albert Kiretia Gichuki
Master of Medicine in Obstetrics and Gynecology
Introduction: The maternal mortality ratio (MMR) has declined across the world over the last 17 years. This has been attributed to more women delivering in health facilities with trained birth attendants. Despite this there is still a high maternal mortality rate with over 86% of the mortalities occurring in Sub-Saharan Africa and South East Asia. This has led to increased focus on improving quality of care and patient safety in the health facilities. A positive safety culture, which is the product of individual and group values, attitudes, perceptions and patterns of behaviour that determine the commitment to an organization’s safety …
‘Mapping’ Moral Engagement In The Solution-Focused Approach Through Macintyre’S Model Of Practice, Brian K. Jennings
‘Mapping’ Moral Engagement In The Solution-Focused Approach Through Macintyre’S Model Of Practice, Brian K. Jennings
Journal of Solution Focused Practices
I attempt to answer Trish Walsh’s two questions about the ‘maps’ that might exist for moral engagement in the ‘helping’ professions and how these might relate to the Solution-Focused Approach (Walsh, 2010). I seek to do this by exploring the narrative of the emergence of the Solution Focused Approach from the perspective of Alasdair MacIntyre’s concept of a ‘practice’ (MacIntyre, 1985) with the aim of providing the basis for ‘map’ for moral engagement by Solution-Focused Practitioners. To this end I attempt to interpret the Solution Focused Approach as a MacIntyreian ‘practice’ in which virtues (as ‘human qualities’) emerge out of …

The Undesirable Communication: A Case Of Cerebral Air Emboli In The Setting Of Atrio-Esophageal Fistula Following Atrial Fibrillation Catheter Ablation, Momin Islam, Hussam Al Hennawi, Anwar Khedr, Khaled Harmouch, Mohammad F. Mathbout
Abington Jefferson Health Papers
Atrial-esophageal fistula is an extremely rare condition but is often a deleterious complication following catheter ablation for atrial fibrillation. The associated iatrogenic communication acts as a conduit for air and bacterial translocation, which may lead to cerebral air embolism and polymicrobial sepsis, respectively. Coupled with a history of invasive procedures, the diagnosis is largely based on the accompanying neurological symptoms. In this report, we present the case of a 73-year-old female who presented with neurological deficits attributed to cerebral vascular emboli three weeks after catheter ablation for the treatment of chronic atrial fibrillation.

Using Community-Based Participatory Research To Tailor A Diabetes Education Program To An At-Risk Black Church Congregation In The Mid-Atlantic, Samantha A. Mielke
Doctor of Nursing Practice Projects
Background: Prediabetes and diabetes continue to have a high disease burden in the United States (Lin et al., 2018). Many individuals are unaware of their glucose state or the impact impaired glucose can have on their quality adjusted life years and cost of living. The Centers for Disease Control and Prevention (CDC) has underscored the importance of decreasing the incidence of diabetes and has resources available. Still, many communities, particularly minority populations, can benefit from tailoring programs to their specific populations, where typical approaches may not be as beneficial (Joo & Liu, 2020). Additionally, Black American (BA) congregations often have …

Renal Cell Carcinoma Of Pelvic Kidney With Atypical Nodal Metastasis: A Diagnostic Challenge, Roshan-E-Shahid Rana, Muhammad Hammad Ather
Section of Urology
Ectopic pelvic kidney is a known congenital anomaly; however, the presence of renal cell carcinoma (RCC) in an ectopic kidney is rare with the evidence available in the form of a few case reports only. In this case report, we present a case of metastatic RCC in the pelvic kidney which became a diagnostic challenge because of atypical contrast-enhanced computed tomographic (CT) characteristics and unusual pattern of lymph node involvement including cervical lymph node in the absence of visceral metastasis. Because of its unusual location and uncertain vascular anatomy, ectopic kidney poses a surgical challenge. Owing to the rarity of …
The Role Of Bkca Channel Expression And Functional Regulation On The Development Of Pulmonary Hypertension In Type 2 Diabetic Individuals, Tahirah Nimi
PCOM Biomedical Studies Student Scholarship
Human pulmonary artery smooth muscle cells (HPASMC) maintain the vascular tone and exhibit myogenic reactivity in the pulmonary arteries. The BKCa channel, being the predominate K+ channel within the HPASMC, is important in regulating the pulmonary arterial pressure and because the BKCa channel’s activity can be modulated by various molecules, studies have evaluated the role that altered BKCa channel expression and function plays in the dysfunction of the HPASMC during type 2 diabetes (T2D). The results from those studies provided a basis for this project. In this study, cell cultures, western blot experiments, and cAMP assays …

Background: Maternal psychological stress has been linked to preterm birth. However, the dierential contribution of psychological stress versus stress hormones is not clear. Studies focus primarily on perceived stress and cortisol, with few assessing its inter-convertible hormone cortisone. Furthermore, little is known about the potential moderating roles of obstetric risk and fetal sex in the relationship between maternal stress and gestational length. This gap in knowledge is particularly evident for rural women who typically experience chronic multiple stressors during pregnancy. We explored the relationship of hormonal and psychological stress to gestational length and the eects of obstetric risks and …

Introduction: Previous surveys of male sex workers (MSW) in sub-Saharan Africa have not fully documented the HIV and sexually transmitted infections (STIs) rates and vulnerabilities by age category.
Methods: The bio-behavioral survey of MSW in Nairobi, Kenya, utilized respondent-driven sampling to recruit MSW. Structured interviews captured MSW's behavioral aspects, and biological tests for HIV and other STIs.
Results: Analysis of the two age categories, 18–24 years (younger MSW) and 25 years and above (older MSW), shows that of all participants, a significantly higher proportion of younger MSW (59.6% crude, 69.6% RDS-adjusted) were recruited compared to older MSW …
A Patient Risk Minimization Model For Post-Disaster Medical Delivery Using Unmanned Aircraft Systems, Mark J. Simpson
A Patient Risk Minimization Model For Post-Disaster Medical Delivery Using Unmanned Aircraft Systems, Mark J. Simpson
Doctoral Dissertations and Master's Theses
The purpose of this research was to develop a novel routing model for delivery of medical supplies using unmanned aircraft systems, improving existing vehicle routing models by using patient risk as the primary minimization variable.
The vehicle routing problem is a subset of operational research that utilizes mathematical models to identify the most efficient route between sets of points. Routing studies using unmanned aircraft systems frequently minimize time, distance, or cost as the primary objective and are powerful decision-making tools for routine delivery operations. However, the fields of emergency triage and disaster response are focused on identifying patient injury severity …

Preoperative Education For Cardiac Surgical Patients Benchmark Study, Jackson W. Payne
MSN Capstone Projects
Heart surgery is a term that scares a lot of people and rightfully so, as the heart is one of, if not the, most vital organ in the human body. So naturally, one can imagine the amount of fear and anxiety that patients feel when they are told they need heart surgery. This fear and anxiety is not unwarranted, either. Heart surgery, in most cases, is a major and highly complex operation with a lot of potential complications and negatives. Despite this, the majority of the time the benefits do outweigh the risks and patients elect to go forward with …

Approximately 30% of elderly adults are cognitively unimpaired at time of death despite the presence of Alzheimer's disease neuropathology at autopsy. Studying individuals who are resilient to the cognitive consequences of Alzheimer's disease neuropathology may uncover novel therapeutic targets to treat Alzheimer's disease. It is well established that there are sex differences in response to Alzheimer's disease pathology, and growing evidence suggests that genetic factors may contribute to these differences. Taken together, we sought to elucidate sex-specific genetic drivers of resilience.
We extended our recent large scale genomic analysis of resilience in which we harmonized cognitive data across four cohorts …
