Retention And Career Success Of Faculty: The Case For Building A Sense Of Belonging To The Academic Medicine Community, Wendy L. Ward
Retention And Career Success Of Faculty: The Case For Building A Sense Of Belonging To The Academic Medicine Community, Wendy L. Ward
Marshall Journal of Medicine
As attrition in the healthcare workforce rises, factors that positively impact retention, engagement, work satisfaction, and vitality for faculty in Colleges of Medicine are of critical importance. One important factor that is less often discussed is the need to develop a sense of belonging and connectedness to the institutional community. Underrepresented minority faculty particularly feel the benefit of feeling like they belong. Recommendations for enhancing faculty connectedness to a community in support of developing a sense of belonging to that community are offered.

Implication Of Melanocortin Receptor Genes In The Familial Comorbidity Of Type 2 Diabetes And Depression., Mutaz Amin, Jurg Ott, Rongling Wu, Teodor T Postolache, Claudia Gragnoli
Division of Endocrinology, Diabetes and Metabolic Diseases Faculty Papers
The melanocortin receptors are G-protein-coupled receptors, which are essential components of the hypothalamic–pituitary–adrenal axis, and they mediate the actions of melanocortins (melanocyte-stimulating hormones: α-MSH, β-MSH, and γ-MSH) as well as the adrenocorticotropin hormone (ACTH) in skin pigmentation, adrenal steroidogenesis, and stress response. Three melanocortin receptor genes (MC1R, MC2R, and MC5R) contribute to the risk of major depressive disorder (MDD), and one melanocortin receptor gene (MC4R) contributes to the risk of type 2 diabetes (T2D). MDD increases T2D risk in drug-naïve patients; thus, MDD and T2D commonly coexist. The five melanocortin receptor genes might confer risk for both disorders. However, they …

Artificial intelligence (AI) has proven time and time again to be a game-changer innovation in every walk of life, including medicine. Introduced by Dr. Gunn in 1976 to accurately diagnose acute abdominal pain and list potential differentials, AI has since come a long way. In particular, AI has been aiding in radiological diagnoses with good sensitivity and specificity by using machine learning algorithms. With the coronavirus disease 2019 pandemic, AI has proven to be more than just a tool to facilitate healthcare workers in decision making and limiting physician-patient contact during the pandemic. It has guided governments and key policymakers …

Knee osteoarthritis (OA) involves peri-articular sarcopenia. The infrapatellar articularis genu (AG) links to the quadriceps femoris (QF) and can be sampled from discarded tissue during arthroplasty. We predict disuse-mediated changes in AG myofiber type ratio and atrophy similar to reports on the QF during OA. OA AGs (n = 40) were preserved and grouped by poor (≤ 85°; n = 11), fair (90°–110°; n = 19), and good (≥ 115°; n = 10) range of motion (ROM). Immunolabeling of slow and fast myosin heavy chains in AG sections allowed comparing distribution and cross-sectional area (CSA) of type-I (T1) and type-II …

Mediation Analysis Of The Effect Of Visuospatial Memory On Motor Skill Learning In Older Adults, Andrew Hooyman, Jennapher Lingo Vangilder, Sydney Y. Schaefer
Physical Therapy Faculty Articles and Research
There is high inter-individual variability in motor skill learning among older adults. Identifying the nature of these individual differences remains challenging due to interactions between participant characteristics (e.g., age, cognition) and task-related factors (e.g., nature of task, level of skill pre-training), making it difficult to determine plausibly causal relationships. This study addresses these competing explanations by using mediation analysis to examine plausible causal inference between visuospatial memory and one-month retention of both gross and fine motor components of a functional upper-extremity task following training. Results suggest that better visuospatial memory results in more retention of fine but not gross motor …

Get Smart: Implementation Of Updated Asthma Guidelines For Pediatric Hospitalists, Alexander Hogan, Kathryn Kyler, Claire Seguin
Presentations
Prescription of Single Maintenance And Reliever Therapy (SMART) for moderate and severe persistent asthma is the most important and actionable improvement in the National Heart Lung Blood Institute (NHLBI) asthma guidelines for pediatric hospitalists. SMART is the use of a single inhaled corticosteroid/long acting beta agonist inhaler both as a daily preventive inhaler and as-needed for asthma symptoms. As hospitalists, we treat patients with poorly controlled asthma who will benefit from SMART asthma action plans. Despite ample evidence supporting use of SMART, implementation of such large changes in practice is challenging. Implementation science and quality improvement principles can be used …

The genetic determinants of fasting glucose (FG) and fasting insulin (FI) have been studied mostly through genome arrays, resulting in over 100 associated variants. We extended this work with high-coverage whole genome sequencing analyses from fifteen cohorts in NHLBI's Trans-Omics for Precision Medicine (TOPMed) program. Over 23,000 non-diabetic individuals from five race-ethnicities/populations (African, Asian, European, Hispanic and Samoan) were included. Eight variants were significantly associated with FG or FI across previously identified regions MTNR1B, G6PC2, GCK, GCKR and FOXA2. We additionally characterize suggestive associations with FG or FI near previously identified SLC30A8, TCF7L2, and ADCY5 regions as well as APOB, …
A Dosimetric Evaluation Of Skin Dose In Post Mastectomy Radiation Therapy: A Comparison Of Static Field Intensity Modulated Radiation Therapy And Volumetric Modulated Arc Therapy, Dusti M. Henion
Masters Projects
Introduction
Post mastectomy radiation therapy can cause significant skin reaction for patients treated with bolus. The elimination of bolus as a standard practice for this treatment has been suggested in recent literature. Additionally, volumetric modulated arc therapy (VMAT) may deliver a comparable plan with the benefit of reducing treatment times for this population compared to static field intensity modulated radiation therapy (IMRT).
Methods
This study is a retrospective dosimetric analysis of ten right-sided postmastectomy radiation therapy patients. The subjects had previously completed a course of radiation therapy consisting of static field IMRT with the use of bolus every other day …
A Quantitative Investigation At Dose To Tissue When Using Physical Constraints Versus Biological Based Constraints In Pancreatic Carcinoma Patients, Kelly R. Alley
Masters Projects
Introduction
The purpose of this study was to compare dose to organs at risk as well as PTV coverage. Physical objectives, biological objectives, and a combination of the two were utilized to compare which technique provided the best coverage of the PTV while also preserving the greatest amount of healthy tissue.
Methods
Each randomly selected patient had three separate plans created for them. The first plan used only physical based constraints in the form of upper objectives. The second plan used only biological based constraints in the form of gEUD objectives. The third plan used a combination of both upper …

Depressed individuals are twice as likely to develop Alzheimer’s disease (AD) as compared to controls. Brain amyloid-β (Aβ) deposition is believed to have a major role in AD pathogenesis but studies also suggest associations of Aβ dynamics and depression. The aim of this study was to test if plasma Aβ levels are longitudinally associated to late-life depression. We measured plasma levels of amyloid-β1-40 (Aβ40) and amyloid-β1-42 (Aβ42) peptides longitudinally for three consecutive years in 48 cognitively intact elderly subjects with late-life major depressive disorder (LLMD) and 45 age-matched cognitively healthy controls. We found that the Aβ42/Aβ40 plasma ratio was significantly …

Partners With Lower Relationship Commitment Are More Likely To Use Technology During Couple Leisure Time, Justin Matthew, Brandon T. Mcdaniel Phd
Health Services and Informatics Research
The constant availability of digital devices can create opportunities for digital connection or distraction in couple relationships. Prior research has investigated the effects of personal technology use during shared couple time, but how the relational process of commitment may be tied to personal technology use during shared couple time is not yet fully understood. In the current study, we examined whether individuals’ level of relationship commitment would be associated with their likelihood of using technology while with their partner during shared leisure time in a sample of 1065 adult participants. Participants (Mean age = 35.73 years) from across the U.S. …

The HLA-DRB1*03:01 allele is a major genetic risk factor in systemic lupus erythematosus (SLE), but the mechanistic basis of the association is unclear. Here we show that in the presence of interferon gamma (IFN-γ), a short DRB1*03:01-encoded allelic epitope activates a characteristic lupus transcriptome in mouse and human macrophages. It also triggers a cascade of SLE-associated cellular aberrations, including endoplasmic reticulum stress, unfolded protein response, mitochondrial dysfunction, necroptotic cell death, and production of pro-inflammatory cytokines. Parenteral administration of IFN-γ to naïve DRB1*03:01 transgenic mice causes increased serum levels of anti-double stranded DNA antibodies, glomerular immune complex deposition and histopathological renal …

Overall Survival With Brentuximab Vedotin In Stage Iii Or Iv Hodgkin's Lymphoma, Stephen M Ansell, Nancy L Bartlett, Et Al.
2020-Current year OA Pubs
BACKGROUND: Five-year follow-up in a trial involving patients with previously untreated stage III or IV classic Hodgkin's lymphoma showed long-term progression-free survival benefits with first-line therapy with brentuximab vedotin, a CD30-directed antibody-drug conjugate, plus doxorubicin, vinblastine, and dacarbazine (A+AVD), as compared with doxorubicin, bleomycin, vinblastine, and dacarbazine (ABVD). A planned interim analysis indicated a potential benefit with regard to overall survival; data from a median of 6 years of follow-up are now available.
METHODS: We randomly assigned patients in a 1:1 ratio to receive up to six cycles of A+AVD or ABVD. The primary end point, modified progression-free survival, has …

Sexual health is the state of well-being regarding sexuality. Sexual health is highly valued and associated with overall health. Overall health and well-being are more than the absence of disease or dysfunction. Health care systems adopting whole health models of care need to incorporate a holistic assessment of sexual health. This includes assessing patients' sexual orientation and gender identity (SOGI). If health systems, including but not limited to the Veterans Health Administration (VHA), incorporate sexual health into whole health they could enhance preventive care, promote healthy sexual functioning, and optimize overall health and well-being. Assessing sexual health can give providers …

Due to workforce demands, new undergraduate nurses are hired directly into fast-paced units and are expected to manage complex patients with rapidly changing conditions and respond to time-sensitive situations. It is important for nurse educators to prepare undergraduate nurses for transition into clinical practice upon graduation. Simulation has been a valuable tool to provide experiential learning and promote clinical decision-making. The next iteration of improving clinical simulation as an experiential learning modality for nursing students is Immersive Virtual Reality (IVR): a realistic, immersive simulation in a 3-dimensional environment that is experienced by body movements and hand controllers. IVR can incorporate …

Approximately 75% of active adults in the U.S. do not meet the recommended levels of overall physical activity (CDC, 2021a). Given the beneficial impact of physical activity on health, an 8-week long, evidence-based group exercise program—Fit & Strong! (F&S!)—was created to improve the health of older adults (Hughes et al., 2004, 2006, 2010). Despite the clear physical benefits of F&S!, it remains unknown what motivates F&S! participants to initially participate in the program and also throughout the program. Drawing from core notions of socioemotional selectivity theory (SST; Carstensen, 2006), research has found that older adults are highly motivated to exercise …

The atmosphere is composed of several layers, each with its own distinct environment varying in temperature, pressure, and levels of UV radiation. Quantifying these varying parameters proves to be useful in understanding atmospheric composition in greater detail. Variance in the composition of the atmosphere allows for the study of the evolution of physical phenomena at different altitudes. Our group quantified this variance using a high-altitude weather balloon and designed an experimental method to observe the nature of sound propagation through varying altitudes. The goal was to develop an altitude-dependent model of the speed of sound by using an open-air, microcontroller-based …

In Chicago, city officials continue to locate polluting industries in predominately Black and Hispanic/Latinx neighborhoods. Lower income families are disproportionately exposed to greater amounts of toxic chemicals such as lead (Pb). While the City of Chicago focuses its effort in removing industrial sites from the North Side, a predominately white community area, sources of industrial pollution are more likely to be found in communities of color. Due to the prevalent sources of Pb on the Southwest side, residential soil is often highly disturbed and contaminated. Pb is a serious neurotoxin, and ingestion or inhalation of Pb contaminated soil and dust …

Phosphorus is a major element needed for plant growth. Excess phosphorus from agricultural runoff and urban stormwater can enter waterways, resulting in growth of toxic algal blooms that are deleterious to local fauna. One way to remove phosphorus is by diverting agricultural and urban runoff into wetlands. One such wetland is Prairie Wolf Slough (PWS), located in north suburban Chicago, IL. Stormwater enters PWS through an inlet channel and exits through an outlet pipe into the Chicago River. A 16-year study documented that a significantly greater mass of phosphorus leaves PWS than enters through the inlet. This study accounted for …

Synthesis Of Triaminopyrimidine Derivatives For Inhibition Of Inflammatory Caspases, Marianna C. Haddad, Caitlin Karver
DePaul Discoveries
Caspases are cysteine-dependent aspartic proteases whose functions are connected to different mechanisms of cell death and inflammation. As caspase-1 plays a significant role in the immune response, its activity has been of interest as a target for inhibitor development as its inhibition will reduce the levels of pro-inflammatory cytokines in inflammatory diseases, thus minimizing the symptoms that arise and serving as a possible therapeutic approach. Prior work discovered a family of potent inhibitors of caspase-1 with a common triaminopyrimidine scaffold. To further explore structure-activity relationship profiles of potential inhibitors of caspase-1, two different syntheses were attempted to create two new …

Inflammatory caspases are important enzymes in the initial immune response to pathogens. A common method of studying caspase activity is through the use of fluorogenic assays, in which the cleavage of a substrate by an enzyme results in the emission of fluorescence. Fluorogenic assays require the use of fluorophores: molecules that will fluoresce upon the cleavage of a substrate.
