This article explains how two AI systems have been incorporated into the everyday operations of two Singapore public healthcare nation-wide screening programs. The first example is embedded within the setting of a national level population health screening program for diabetes related eye diseases, targeting the rapidly increasing number of adults in the country with diabetes. In the second example, the AI assisted screening is done shortly after a person is admitted to one of the public hospitals to identify which inpatients—especially which elderly patients with complex conditions—have a high risk of being readmitted as an inpatient multiple times in the …

Thyroid nodules can be classified as benign, malignant, or indeterminate, the latter of which make up 10–30% of nodules. Radiofrequency ablation (RFA) has become an attractive and promising therapy for the treatment of benign thyroid nodules. However, few studies have investigated the safety and efficacy of RFA for the management of indeterminate thyroid nodules. In this study, 178 patients with thyroid nodules diagnosed as benign (Bethesda II) or indeterminate (Bethesda III/IV) by preoperative cytopathological analysis were included. Patients in the benign and indeterminate cohorts had similar thyroid nodule volume reduction rates at 65.60% and 64.20%, respectively (p = 0.68). The …

Sustaining patient portal use is a major problem for many healthcare organizations and providers. If this problem can be successfully addressed, it could have a positive impact on various stakeholders. Through the lens of cognitive dissonance theory, this study investigates the role of health professional encouragement as well as patients’ security concerns in influencing continuous use intention and deep structure usage among users of a patient portal. The analysis of data collected from 177 patients at a major medical center in the Midwestern region of the United States shows that health professional encouragement helps increase the continuous use intention and …

Correlation Of Gut Alteration With The Progression Of Experimental Autoimmune Encephalomyelitis (Eae) In C57bl/6 Mice, Al Anood Ahmed Al Naqbi
Theses
Background: Experimental autoimmune encephalomyelitis (EAE) is the mouse disease model of multiple sclerosis (MS), a chronic autoimmune disease targeting the central nervous system (CNS) resulting in neuroinflammation, neuroaxonal degeneration, and demyelination. Autoreactive CD4+ T cells are known for their role in the pathogenesis of MS. CD8+ T cells and NK cells were also found to be associated with the disease.
Aim: There is a limited number of studies investigating the link between gut alterations and immune cells in the gut influencing the outcome of the disease since the vast majority of MS patients experience gastrointestinal (GI) problems. Here, …

Investigating The Role Of Natural Killer (Nk) Cells In Ozanimod-Mediated Remission In Experimental Autoimmune Encephalomyelitis (Eae) In C57bl/6 Mice, Doua Youssef Kamyan
Theses
Experimental autoimmune encephalomyelitis (EAE) is an important model of multiple sclerosis (MS) in rodents. The disease is characterized by axonal damages mediated by activated autoreactive T lymphocytes, microglia, and macrophages. Ozanimod (RPC1063) is an immunomodulator that has been recently approved by the FDA for the treatment of relapsing-remitting multiple sclerosis (2020) and ulcerative colitis (2021). It is a selective agonist of the sphingosine1-phophate receptors (1 and 5) involved in lymphocyte trafficking and expressed on naïve and central memory T and B cells as well as NK cells. Ozanimod acts by inducing internalization and degradation of the S1PR which in turn …
Associated Risk Of Blastocystis Infection In Colorectal Cancer, Lena Abdulbaset Labania
Associated Risk Of Blastocystis Infection In Colorectal Cancer, Lena Abdulbaset Labania
Theses
Blastocystis species is an anaerobic intestinal protozoan seen in humans and a wide range of animals. Only nine Blastocystis subtypes are seen in humans. The pathogenicity of Blastocystis spp. has long been controversial. Recently, a subtype-dependent association between Blastocystis spp. and colorectal cancer (CRC) is being debated. Thus, this thesis aims to assess the possible association between Blastocystis spp. infection and CRC condition compared to cancer outside the gastrointestinal tract (COGT) and a cancer-free control (CF) group. This study is the first of its kind in the UAE. Participants were divided into two groups; cancer patients and CF participants. The …

Capacity Building Among Nursing And Midwifery Professional Associations In East Africa, Stephen Ruhmel, Eunice Ndirangu, Joseph Mwizerwa, Ahmed Sarki, Eunice Pallangyo
School of Nursing & Midwifery, East Africa
Background: Despite improvements in educational opportunities, policy changes, and pay raises in the nursing and midwifery professions in East Africa, poor working conditions, few professional development opportunities, and a general lack of respect for these professions predominate. These issues contribute to a low quality of care among a population with a high burden of communicable diseases. Health professional associations may help to address these challenges by providing a voice for nurses and midwives.
Objective: This study evaluated the impact of a 5-year programme focused on strengthening nurses’ and midwives’ professional associations in East Africa. Methods: This study used …
Strategies Ambulatory Practice Leaders Use In Optimizing Electronic Medical Record Systems, Jeffrey L. Brown
Strategies Ambulatory Practice Leaders Use In Optimizing Electronic Medical Record Systems, Jeffrey L. Brown
All Theses And Dissertations
Electronic medical record systems (EMRs) were quickly adopted by ambulatory care practices due to federal government programs encouraging their rapid adoption and implementation. The accelerated implementation and adoption of these digital care systems introduced new forms of administrative burdens that have become negatively associated with the practice of medicine. Research regarding the challenges related to implementing and adopting EMR systems has been abundant. However, there is little research on EMR optimization strategies within ambulatory practice settings. This qualitative phenomenological study explored the lived experiences of ambulatory practice leaders and the strategies they used to optimize EMR systems. The data collected …

BACKGROUND: Considering relatives' health history in logistic regression for case-control genome-wide association studies (CC-GWAS) may provide new information that increases accuracy and power to detect disease associated genetic variants. We conducted simulations and analyzed type 2 diabetes (T2D) data from the Framingham Heart Study (FHS) to compare two methods, liability threshold model conditional on both case-control status and family history (LT-FH) and Fam-meta, which incorporate family history into CC-GWAS.
RESULTS: In our simulation scenario of trait with modest T2D heritability (h
CONCLUSIONS: Overall, LT-FH and Fam-meta had higher power than CC-GWAS in simulations, especially using phenotypes that were more prevalent …

No medications are approved for cannabis use disorder (CUD), though a small clinical trial demonstrated that the voltage-dependent calcium channel (VDCC) ligand gabapentin reduced cannabis use in treatment seekers. VDCCs are modulated by cannabinoid (CB) ligands, and there are shared effects between CB agonists and VDCC ligands. This overlapping neuropharmacology and the initial clinical results supported the evaluation of pregabalin, a "next-generation" VDCC ligand, as a CUD medication. Two separate placebo-controlled, double-blind, counterbalanced, within-subjects human laboratory studies tested placebo and 300 (N = 2 females, 11 males; Experiment [EXP] 1) or 450 (N = 3 females, 11 males; …

Common single-nucleotide polymorphisms (SNPs) are predicted to collectively explain 40–50% of phenotypic variation in human height, but identifying the specific variants and associated regions requires huge sample sizes1. Here, using data from a genome-wide association study of 5.4 million individuals of diverse ancestries, we show that 12,111 independent SNPs that are significantly associated with height account for nearly all of the common SNP-based heritability. These SNPs are clustered within 7,209 non-overlapping genomic segments with a mean size of around 90 kb, covering about 21% of the genome. The density of independent associations varies across the genome and the …

Type I conventional dendritic cells (cDC1s) are an essential Ag-presenting population required for generating adaptive immunity against intracellular pathogens and tumors. While the transcriptional control of cDC1 development is well understood, the mechanisms by which extracellular stimuli regulate cDC1 function remain unclear. We previously demonstrated that the cytokine-responsive transcriptional regulator STAT3 inhibits polyinosinic:polycytidylic acid [poly(I:C)]-induced cDC1 maturation and cDC1-mediated antitumor immunity in murine breast cancer, indicating an intrinsic, suppressive role for STAT3 in cDC1s. To probe transcriptional mechanisms regulating cDC1 function, we generated novel RNA sequencing datasets representing poly(I:C)-, IL-10-, and STAT3-mediated gene expression responses in murine cDC1s. Bioinformatics analyses …

Introduction Malignant gliomas are the most common primary malignant brain tumors and are typically treated with maximal safe surgical resection followed by chemoradiation. One of the unintended effects of radiation is depletion of circulating lymphocyte pool, which has been correlated with inferior overall survival outcomes.
Methods A comprehensive and systematic searches of the PubMed, Cochrane Central, and Embase databases were done to assess the studies that have reported radiation-related lymphopenia in high-grade gliomas. Hazard ratios (HRs), odds ratios (OR), and mean differences were represented with Forest plots comparing patients with severe lymphopenia and no severe lymphopenia. Review Manager Version 5.3 …

Background: Ductal carcinoma in situ (DCIS) is treated to prevent subsequent ipsilateral invasive breast cancer (iIBC). However, many DCIS lesions will never become invasive. To prevent overtreatment, we need to distinguish harmless from potentially hazardous DCIS. We investigated whether the immune microenvironment (IME) in DCIS correlates with transition to iIBC.
Methods: Patients were derived from a Dutch population-based cohort of 10,090 women with pure DCIS with a median follow-up time of 12 years. Density, composition and proximity to the closest DCIS cell of CD20+ B-cells, CD3+CD8+ T-cells, CD3+CD8- T-cells, CD3+FOXP3+ regulatory T-cells, CD68+ cells, and CD8+Ki67+ T-cells was assessed with …
