Plasmepsin X (PMX) is an essential aspartyl protease controlling malaria parasite egress and invasion of erythrocytes, development of functional liver merozoites (prophylactic activity), and blocking transmission to mosquitoes, making it a potential multistage drug target. We report the optimization of an aspartyl protease binding scaffold and the discovery of potent, orally active PMX inhibitors with in vivo antimalarial efficacy. Incorporation of safety evaluation early in the characterization of PMX inhibitors precluded compounds with a long human half-life (

Large-scale whole-genome sequencing studies have enabled analysis of noncoding rare-variant (RV) associations with complex human diseases and traits. Variant-set analysis is a powerful approach to study RV association. However, existing methods have limited ability in analyzing the noncoding genome. We propose a computationally efficient and robust noncoding RV association detection framework, STAARpipeline, to automatically annotate a whole-genome sequencing study and perform flexible noncoding RV association analysis, including gene-centric analysis and fixed window-based and dynamic window-based non-gene-centric analysis by incorporating variant functional annotations. In gene-centric analysis, STAARpipeline uses STAAR to group noncoding variants based on functional categories of genes and incorporate …

BACKGROUND: The use of low-value care (LVC) is a persistent problem that calls for knowledge about strategies for de-implementation. However, studies are dispersed across many clinical fields, and there is no overview of strategies that can be used to support the de-implementation of LVC. The extent to which strategies used for implementation are also used in de-implementing LVC is unknown. The aim of this scoping review is to (1) identify strategies for the de-implementation of LVC described in the scientific literature and (2) compare de-implementation strategies to implementation strategies as specified in the Expert Recommendation for Implementing Change (ERIC) and …

BACKGROUND: Precision medicine, optimized interventions, and access to care are catchphrases for the future of behavioral treatments. Progress has been slow due to the dearth of clinical trials that optimize interventions' benefits, individually tailor interventions to meet individual needs and preferences, and lead to rapid implementation after effectiveness is demonstrated. Two innovations have emerged to meet these challenges: fully remote trials and precision clinical trials.
OBJECTIVE: This paper provides a detailed description of Mindful MyWay, a study designed to test online mindfulness training in older adults with depression. Consistent with the concept of fully remote trials using a smartphone app, …

Feasibility Of Cell Phone Surveys In People With Mental Illness Experiencing Homelessness During Covid-19, Tina Thomas, Namrata Walia, Landon Presnall, Jasper Shei, Jane Elizabeth Hamilton
Journal of Digital Psychiatry
People experiencing homelessness (PEH) may be at increased risk for negative physical and mental health outcomes in the context of community spread of COVID-19. Research into the impacts of COVID-19 on this vulnerable population can be affected by inaccessibility and poor engagement resulting in minimal representation in population-based survey data. The increased use of mobile phone technology (mHealth) to provide medical and psychiatric care during the COVID-19 pandemic could provide an effective platform for gathering data from this hard-to-reach group. This paper examines feasibility, via review of the barriers and facilitators, of using a mobile phone intervention to administer a …

Background. The educational experience and success of academic anesthesiologists may be elusive to quantify. We wished to examine the characteristics of a cohort of anesthesiology faculty who were inducted into a medical school master educator academy over the last decade.
Methods. After IRB approval with waiver of consent, all 10 anesthesiology faculty inductees into the academy supplied relevant data for their accomplishments at the time of induction in multiple educational domains, including teaching and assessment, mentoring and advising, evidence of scholarship and production of enduring materials, and educational leadership. These were deidentified and analyzed.
Results. The cohort had evidence of …

Trick to the Treat of Internships and Research
The zoonotic novel coronavirus has posed major challenges to the world’s economy, social development, and public health. Effects in the education and research in the field of healthcare and STEM are accentuated in lower-developed countries or communities where the access to technology and a reliable internet connection are luxuries making it difficult to attain education virtually. The Coronavirus Infectious Disease 2019 (COVID-19) has also disrupted research and training in STEM and healthcare. With significant increases in infection rates there is an urge to redirect projects towards virology research and vaccine development. This comprehensive literature review includes case studies highlighting the …

Trick to the Treat of Internships and Research
The ongoing Coronavirus Infectious Disease 2019 (COVID-19) pandemic has caused a significant disruption in several areas of research within the fields of Anatomy and Physiology. With the whole world coming to a standstill, research of any kind was difficult to pursue, especially in these disciplines requiring kinesthetic/tactile application of concepts or testing hypotheses through hands-on engagement, as the new social guidelines and risks to certain groups of people hindered up-to-date research-data collection. However, with new technology and protocols created or adopted, there has been an effort to better help students and scientists to conduct research remotely; such alternatives cannot effectively …

Given that there still exist cancers that are unresponsive to immunotherapy, high linear energy transfer (LET) radiation offers an alternative solution to bolstering anti-tumor immunity. Linear energy transfer is energy transferred per unit distance. Alpha particles have higher LET than other forms of radiotherapy and can create clustered DNA lesions. A possible component of these lesions is micronuclei, cytoplasmic DNA that can stimulate downstream immunostimulatory mechanisms. Because alpha particles cannot travel far, we utilized diffusing alpha-emitters radiation therapy (DaRT) to diffuse the alpha particles over a larger tumor volume. This occurs through the decay of the radium-224 seed, which forms …

Trick to the Treat of Internships and Research
Autism Spectrum Disorder (ASD) affects a multitude of races and ethnicities as its impact varies from person to person. ASD is typically diagnosed around the age of two to three years old since signs and symptoms of Autism develop then. Increased reporting of Autism in local communities has surged because of better diagnostic tools and criteria as well as public awareness. However, even with these advancements, the African American community suffers a delayed diagnosis for ASD. This issue poses problems for both the parents and children with Autism; opportunities are limited for developmental services by a delayed diagnosis. A literature …

Stroke is the most common cause of epilepsy and ultimately leads to a decrease in the quality of life of those affected. Ischemic and hemorrhagic strokes can both lead to poststroke epilepsy (PSE). Significant risk factors for PSE include age less than 65 years, stroke severity measured by the National Institutes of Health Stroke Scale (NIHSS), cortical involvement, and genetic factors such as TRPM6 polymorphism. The diagnosis of PSE is made by using imaging modalities, blood biomarkers, and prognostic criteria. Electroencephalography (EEG) is currently the gold standard to diagnose PSE, while new combinations of modalities are being tested to increase …
