It strives to teach students design skills through providing solutions for individuals, communities, and organizations in the surrounding area while mirroring engineering industry standards. Future Perspectives In Acute Myocarditis Complicated By Cardiogenic Shock, Giovanni A. Tavecchia, Iside Cartella, Enrico Ammirati

Acute myocarditis is an inflammatory disease of the myocardium with a highly variable clinical course. Fulminant myocarditis (FM) represents the most threatening scenario with hemodynamic compromise and cardiogenic shock at presentation. Despite medical advances and the availability of promising mechanical circulatory support (MCS), FM is burdened by a dismal prognosis. Early referral to tertiary hospitals with MCS facilities and prompt diagnosis with endomyocardial biopsy are critical steps toward optimal management. Moreover, beyond supportive care, the prevention of irreversible myocardial damage with immunomodulating therapies must be proven in clinical trials. The Surviving Sepsis Guidelines can serve as a structure to help educate and create a set of recommendations on how to care for patients through this complicated pathway of shock. Designing a cardiogenic shock bundle could reduce the variability of care and possibly improve survival. Also, a more standard protocol would allow a review of the outcomes and a system to change practice nationally when new data or technology becomes available. This could create a continuous quality improvement cycle. The pulmonary artery (PA) catheter can be a useful tool in the management of patients with cardiogenic shock; however, there are challenges with the use of this catheter, and clinicians must balance the risks and benefits. In addition, clinicians must properly interpret data generated from a PA catheter in the context of other data to optimize a patient's hemodynamics.

There has been a swift uptake in the use of teletherapy since the start of the COVID-19 pandemic, which has corresponded with an increase in clinical scholarship focused on conducting systemic therapy in an online format. A majority of this scholarship offers ideas for adapting therapeutic tasks developed around in-person contact for a remote format. The current article moves beyond adapting and offers ideas for remote systemic therapy that are born from our experiences of evolving through teletherapy. The mental health professions require innovation in order to advance theory and practice. This article presents reimagined therapeutic work that joins the metaphysical element of Gene Keys with the largely accepted theoretical approach of Bowen Family Systems Theory. We propose the integration of metaphysics as a way to facilitate higher levels of differentiation – particularly in clients who have an expanded worldview inclusive of metaphysics. The reader is asked to stretch their beliefs and perspective as they read through this paper.

Counseling and Family Therapy Scholarship Review
The purpose of this transcendental phenomenological study was to explore the lived experiences of MFT trainee shame, self-criticism, and self-compassion. Additionally, this study also sought to understand how those experiences may affect a trainee’s clinical work as a first-time practicum student. Interviews were conducted with 15 trainees in a graduate program who were performing therapy at a practicum site. Utilizing Moustakas’ transcendental phenomenology, six essential themes emerged: (1) shame and self-criticism are interrelated and can affect therapeutic presence; (2) self-criticism can have a positive impact on clinical work; (3) trainees use metaphors to describe shame and self-criticism (4) self-compassion …

This article is the fruit of a systematic scoping review, according to PRISMA precepts. Its objective was to explore the use of stories and other symbolic genre on family therapy settings. Thus, it explains how the researchers made the selection of articles in phases or steps, in a total of six, having a final result of 11 articles, in a time-lapse of 28 years. There is no shortage of therapeutic modalities and interventions at the therapist’s disposal. Psychotherapy, because it claims to treat the individual, is necessarily multifaceted and complex. The dimension of language which structures much of our experience should be considered as a starting point for a psychotherapeutic technique. French psychoanalyst Jacques Lacan developed a theory and technique of the talking cure beginning from this assumption: that language structures our experience. In the early 1950s he sought to recover a linguistic Freud: to re-center psychoanalytic psychotherapy on language itself. In this paper, I sketch the theoretical reasons for this turn in Lacan …

The Australian General Practice Training National Registrar Survey (AGPT NRS) is an annual, national survey of GP registrars currently training in the AGPT program. It collects information via an online questionnaire about registrar satisfaction, experience and future career plans. It also collects information about registrars’ demographics and training contexts and other aspects of their training experience. This survey is part of the Department of Health and Aged Care’s (the Department) monitoring and quality improvement activities. Carum carvi is a well-known herb traditionally used as a spice in Asian countries. Acetaminophen is a known marketed drug mainly used as an analgesic. It has been scientifically proven that consumption of acetaminophen (paracetamol) is associated with liver toxicity if taken in high doses without medical supervision. The present study evaluated the in vivo antioxidant and hepatoprotective efficacy of Carum carvi against acetaminophen-induced hepatotoxicity in Wistar rats. Our results demonstrate that Carum carvi, at doses (mg/kg) of 100 (D1) and 200 (D2), showed inhibitory properties for DNA-sugar damage, lipid peroxidation, DPPH scavenging, and increased reducing potential in a concentration-dependent …

Sample preparation remains both a challenging and time-consuming process in the field of bioanalytical chemistry. Many traditional techniques often require multi-step processes, which can introduce additional errors to the analytical method. Given the complexity of many biological matrices, thorough analyte extraction presents a major challenge to researchers. In the present study, a headspace solid-phase microextraction (HS-SPME) coupled with a GC/Q-ToF-MS method, was developed to quantify in vitro metabolism of β-caryophyllene by both human liver microsome (HLM) and S9 liver fractions. The viridans group streptococci (VGS) are a large consortium of commensal streptococci that colonize the human body. Many species within this group are opportunistic pathogens causing bacteremia and infective endocarditis (IE), yet little is known about why some strains cause invasive disease. Identification of virulence determinants is complicated by the difficulty of distinguishing between the closely related species of this group. Here, we analyzed genomic data from VGS that were isolated from blood cultures in patients with invasive infections and oral swabs of healthy volunteers and then determined the best-performing methods for species identification. Drug-induced liver and kidney damage is an emergent clinical issue that should be addressed. Rosmarinic acid (RA) has obvious anti-inflammatory and antioxidant effects, so we evaluated the anti-inflammatory and antioxidant effects of RA pretreatment on serum and liver and kidney tissues of cisplatin (CP)-treated mice and explored the possible mechanisms. The results showed that RA pretreatment effectively downregulated the serum, liver, and kidney levels of ALT, AST, BUN, and CRE and the inflammatory factors IL-1β, IL-6, and TNF-α, and simultaneously enhanced the total antioxidant capacity of the liver and kidney. Operational and ethical challenges for nursing homes across the United States during the COVID-19 pandemic were daunting, that experience perhaps only a forecast of future epidemics that nursing home administrators and operators may face. This article describes administrator-identified challenges and focuses on how nursing homes might learn from their experiences by increasing flexibility to meet evolving needs, improving quality assurance and disaster planning, using ethics policies and ethical decision-making processes to work through difficult decisions, and leading the way in creating new policies that will make nursing home care safer and more appropriate for patients with ever changing needs.

Grounded in a presupposition that a single explanatory framework cannot fully account for the expansive learning processes that occur during medical residency, the article examines developing physicians’ reflective writing from three disciplinary lenses. The goal is to understand how the multi-dimensional nature of medical residency translates into assembling educational experiences and constructing meaning that cannot be fully explained through a single discipline. An interdisciplinary research team across medical education, communication, and ethics qualitatively analyzed reflective entries (N=756) completed by family medicine residents (N=33) across an academic year. Results provide evidence for moving toward an integrated thematic explanation across disciplines. The …

Extracellular vesicles, including exosomes and microvesicles, offer great opportunities for understanding biological mechanisms and diagnostic prospects for various disorders. Our laboratory explores the potential of extracellular vesicles from multiple sources for mechanistic studies and for therapeutic applications to various neurological disorders. The laboratory has a well-established facility for generating, isolating and characterizing extracellular vesicles and aims to bring these EVs to clinical applications.
The pathological alterations that manifest during early embryonic development due to inherited and acquired factors trigger various neurodevelopmental disorders, including epilepsy, neuronal migration disorders and neurocognitive dysfunctions. Our work also focuses on the potential use of stem …
