Background: Coronavirus disease 2019 (COVID-19) presented many new challenges within assisted living communities (ALCs) and exacerbated preexisting challenges. With high quality of care in ALCs as the continuous goal to achieve, it is important to recognize challenges faced in these communities, especially those related to staffing and COVID-19.
Objective: The objective was to investigate issues related to staffing and the effect of COVID-19 in ALCs, including staff absence, leadership, impacts on staff, and impacts on staff retention.

New Horizons was founded in 1998 by Father Don, a Greek Orthodox priest who did street outreach with homeless youth outside a donut shop near Pike Place Market (New Horizons, n.d.). He discovered that youth would rather live on the backstreets of Seattle than seek shelter and help from current service providers. After conversations with the youth and young adults (YYA), Father Don started an institution that specifically targeted homeless youth and the unstable housed population. Objectives: Qualitative studies nested within clinical trials can provide insight into the treatment experience, how this evolves over time and where improved supportive care is required. The purpose of this qualitative study is to describe the lived experiences of men with advanced prostate cancer participating in the TheraP trial; a randomised trial of 177Lu-PSMA-617 compared with cabazitaxel chemotherapy. Methods: Fifteen men with advanced prostate cancer were recruited from the TheraP clinical trial with interviews conducted at three timepoints during the trial. There is currently no effective treatment for dementia, of which Alzheimer's disease (AD) is the most common form. It is, therefore, imperative to focus on evidence-based preventive strategies to combat this extremely debilitating chronic disease. Nitric oxide (NO) is a key signalling molecule in the cardiovascular, cerebrovascular, and central nervous systems. Vegetables rich in nitrate, such as spinach and beetroot, are an important source of NO, with beneficial effects on validated markers of cardiovascular health and an association with a lower risk of cardiovascular disease. Nonoperative management (NOM) is increasingly utilized for rectal cancer patients with a clinical complete response (cCR) following total neoadjuvant therapy (TNT). The objective of this pilot study was to determine whether FDG-PET/MRI alters clinical response assessments among stage I-III rectal cancer patients undergoing TNT followed by NOM, relative to MRI alone. This prospective study included 14 subjects with new rectal cancer diagnoses. Imaging consisted of FDG-PET/MRI for initial staging, post-TNT restaging, and surveillance during NOM. Two independent readers assessed treatment response on MRI followed by FDG-PET/MRI. Inter-reader differences were resolved by consensus review. Network control theory is increasingly used to profile the brain's energy landscape via simulations of neural dynamics. This approach estimates the control energy required to simulate the activation of brain circuits based on structural connectome measured using diffusion magnetic resonance imaging, thereby quantifying those circuits' energetic efficiency. The biological basis of control energy, however, remains unknown, hampering its further application. To fill this gap, investigating temporal lobe epilepsy as a lesion model, we show that patients require higher control energy to activate the limbic network than healthy volunteers, especially ipsilateral to the seizure focus. BACKGROUND: The aim of this study was to develop artificial intelligence (AI) guided framework to recognize tooth numbers in panoramic and intraoral radiographs (periapical and bitewing) without prior domain knowledge and arrange the intraoral radiographs into a full mouth series (FMS) arrangement template. This model can be integrated with different diseases diagnosis models, such as periodontitis or caries, to facilitate clinical examinations and diagnoses.

The purpose of this quantitative study was to investigate the relationship between exposure to pelvic health content in physical therapy school curricula and Doctor of Physical Therapy (DPT) students’ comfort levels discussing sexual health issues with future patients. As holistic, autonomous health care providers, physical therapists must be confident in addressing all aspects of health with patients, including sexual health. Unpredictability is increasingly recognized as a primary dimension of early life adversity affecting lifespan mental health trajectories; screening for these experiences is therefore vital. The Questionnaire of Unpredictability in Childhood (QUIC) is a 38-item tool that measures unpredictability in childhood in social, emotional and physical domains. The available evidence indicates that exposure to unpredictable experiences measured with the QUIC predicts internalizing symptoms including depression and anxiety. Background: Caffeine is available in an ampoule, used via parenteral and enteral routes in preterm neonates to treat apnea of prematurity (AOP) in neonates of gestational age ≥ 35-40 weeks. A longer duration of therapy has a higher risk of medication non-adherence due to higher costs and inappropriate dosage forms. Pharmaceutically compounded oral caffeine (PCC) could be an appropriate alternate dosage form. The researchers aimed to determine the impact of PCC on medication-related factors influencing medication adherence (MA) and the frequency of hospital readmission with apnea (HRA) in preterm neonates.

Endoplasmic reticulum-mitochondria contacts (ERMCs) are restructured in response to changes in cell state. While this restructuring has been implicated as a cause or consequence of pathology in numerous systems, the underlying molecular dynamics are poorly understood. Here, we show means to visualize the capture of motile IP3 receptors (IP3Rs) at ERMCs and document the immediate consequences for calcium signaling and metabolism. IP3Rs are of particular interest because their presence provides a scaffold for ERMCs that mediate local calcium signaling, and their function outside of ERMCs depends on their motility. Purpose/Aim: The aim of this project was to create an all encompassing stroke core measure report in one easy to access location which would decrease time spent completing audits.
Problems: • Feedback regarding audit tool determined it to be cumbersome and tedious • Each audit required nurse leader to enter each patient record to search for the information • Completion of the audit took a considerable amount of time which removed the nurse leader from the bedside and patient care.

Prone positioning improves oxygenation in patients experiencing respiratory distress. It is used to decrease mortality in mechanically ventilated patients diagnosed with acute respiratory distress syndrome. However, very little evidence has addressed use of prone positioning in nonintubated patients. The purpose of this study was to assess the impact of early self-proning on oxygenation in patients who had or were suspected of having coronavirus disease (COVID-19).

The Qualitative Report
Understanding participants’ perspectives in qualitative research is contingent on unravelling the essential meaning of their speech. When data are collected in native language and translated into English language, the underlying sociocultural meaning of participants’ speech can be missed. This paper discusses a new contextual coding approach and illustrates its application in research. The technique was used in a phenomenological study in Pakistan and a mixed methods study in Europe. Contextual coding entails a preliminary coding stage involving data reading in native language, choosing socially and culturally relevant words and phrases, and developing preliminary codes. Living in a sonic world, a person with hearing loss experiences difficulty in communicating with others. One of the reasons why some people with hearing loss do not wear hearing aids (HAs) is limited perceived benefit, which may result from HAs that are not fit using “best practices” or because the best practices fail to address individual difficulties experienced by the listener. More than 50% of stroke survivors over 65 years of age experience long-term mobilityloss [50]. Hemiparesis is a form of disability that arises from this, causing paralysis on one side of the body. The long-term effects of hemiparesis include loss of locomotion, muscle spasticity, and excessive use of muscles on the non-paretic side. This leads to a gait pattern that is kinematically and kinetically asymmetric.

Background: The incubation period of an infectious disease is defined as the elapsed time between the exposure to the pathogen and the onset of symptoms. Although both the mRNA-based and the adenoviral vector-based vaccines have shown to be effective, there have been raising concerns regarding possible decreases in vaccine effectiveness for new variants and variations in the incubation period.

The Green Bush Viper, Atheris squamigera, is native to West and Central Africa and has few well reported envenomations. Bite victims experience dizziness, nausea, headache, regional lymphadenopathy, and localized edema. Most reports also detail severe effects including thrombocytopenia, coagulopathy, hemolysis, hemorrhage, or renal failure. Fatalities are reported, but poorly described. There is no specific antivenom for A. squamigera, but non-species specific antivenom has been reported helpful in several cases. We report the case of a 36-year-old woman who was bitten by a green bush viper and was treated with several non-species specific antivenoms. There were no complications to antivenom administration …

Manually delineating upper abdominal organs at risk (OARs) is a time-consuming task. To develop a deep-learning-based tool for accurate and robust auto-segmentation of these OARs, forty pancreatic cancer patients with contrast-enhanced breath-hold computed tomographic (CT) images were selected. We trained a three-dimensional (3D) U-Net ensemble that automatically segments all organ contours concurrently with the self-configuring nnU-Net framework. Our tool's performance was assessed on a held-out test set of 30 patients quantitatively. Objective: The coronavirus disease 2019 (COVID-19) pandemic disrupted healthcare delivery, including prenatal care. The study objective was to assess if timing of routine prenatal testing changed during the COVID-19 pandemic.
Methods: Retrospective observational cohort study using claims data from a regional insurer (Highmark) and electronic health record data from two academic health systems (Penn Medicine and Yale New Haven) to compare prenatal testing timing in the pre-pandemic (03/10/2018-12/31/2018 and 03/10/2019-12/31/2019) and early COVID-19 pandemic (03/10/2020-12/31/2020) periods. Primary outcomes were second trimester fetal anatomy ultrasounds and gestational diabetes (GDM) testing. A secondary analysis examined first trimester ultrasounds.

This study aims to elucidate the scaling law to provide the critical condition on appearance of the downwash pattern of the hot smoke ejected from a chimney in a turbulent cross flow. A specially designed wind tunnel with an active turbulence generator developed by Makita was adopted to offer a quasi-isotropic turbulence field in a lab-scale test facility. Background: Despite improvement in the surgical procedure and strictly following the guidelines for mitral valve replacement (MVR), left ventricular dysfunction still occurs. Novel echocardiographic indices can predict development of LV (left ventricle) dysfunction post MVR. LV-GLS (global longitudinal strain) derived from speckle tracking echocardiography, has been proposed as a novel measure to better depict latent LV dysfunction.

This article describes a collaboration between a branch of the Queens Public Library in NY and an academic medical center that delivered a women’s health educational program with an interprofessional team of faculty, trainees, and students. The team delivered interactive 1-hour long monthly sessions in a Question and Answer format guided by a Powerpoint presentation. The overall goal was to deliver health education workshops on common medical conditions (i.e. diabetes, heart disease) and concerns of interest to women across the lifespan (i.e. infertility, dementia) to improve the health literacy of local community members.
