The purpose of this study was to assess health care professional’s interprofessional collaborative competencies and attitudes towards working in teams and to examine if differences exist between professionals who have completed formal interprofessional education (IPE) and those who have not. This study was guided by the Interprofessional Learning Continuum Model.

Objectives: Food insecurity is an issue in rural communities, particularly among food pantry clients. Food pantries may not provide clients with the opportunity to choose their own foods (client choice), and instead may just give a set amount and type of foods. Lamotrigine is a new generation antiepileptic which blocks sodium channels and can cause significant toxicity in overdose. A case of a three-year-old child who suffered a seizure and required endotracheal intubation after accidental lamotrigine ingestion is presented. The lamotrigine concentration measured 23.2 mcg/mL which is the highest reported after accidental pediatric exposure. A review of the literature regarding pediatric lamotrigine poisoning is also included.

Physically active students are more likely to be fit and learn better in school than sedentary ones. For school-aged children, it is unclear whether physical fitness level is an important determinant for student's activity afterschool. This study examined the association between health-related physical fitness test performance and afterschool physical activity during weekdays. Participants (n = 97, 11-13 years old) completed health-related fitness tests. They wore Actigraph GT3X accelerometers for five consecutive days, recording physical activity participation during after school hours. BACKGROUND: Few researchers have examined the effects of surf programs on children with disabilities. Due to previous research findings, surfing is being used, as the focus of physical activity intervention due to its numerous health and therapeutic benefits.

OBJECTIVE/HYPOTHESIS: The purpose of this study was to explore the effects of an eight-week surfing intervention on various physical fitness measures in 71 children with disabilities such as autism spectrum disorder, down syndrome, global developmental delays, and cerebral palsy. Purpose.

Although walking gait in sighted populations is well researched, few studies have investigated persons with visual impairments (VIs). Given the lack of physical activity in people with VIs, it is possible that reduced efficiency in walking could adversely affect activity. The purposes of this preliminary study were to (1) examine the biomechanics and energetics utilized during independent and guided walking in subjects with VIs, and (2) compare gait biomechanics between people with VIs and sighted controls.

Objective

 To measure the prevalence of burnout among pediatric residents at Karolinska University Hospital (KUH).
 To describe the local drivers of burnout and the protective factors that lead to resilience and workplace engagement.

Conclusions

 Prevalence of burnout among Swedish pediatric residents in this study was 39%, which is lower than reported in the US (50-60%).
 Drivers of burnout were identified across all four domains. They included local features of the learning environment, and many are modifiable.
 Protective factors include perceived control over scheduling, better mental and physical health, lower perceived stress, and being a parent. …

Introduction: Postpartum constipation is a common problem women face after delivery. Vaginal tears, pain medications, hormonal changes, decreased activity, dietary changes, and hemorrhoids can all increase the risk of developing constipation in the postpartum period. Evidence Summary: Constipation can cause pain, discomfort, decreased quality of life, and life-long consequences if left untreated. Nurses have a responsibility to the patient to provide quality care by taking a proactive approach to bowel management. Ovarian cancer remains the leading cause of death among all gynaecological cancers, illustrating the urgent need to understand the molecular mechanisms involved in this disease. Eukaryotic initiation factor 3c (EIF3c) plays an important role in protein translation and cancer cell growth and proliferation, but its role in human ovarian cancer is unclear. Our results showed that EIF3c silencing significantly up-regulated 217 and down-regulated 340 genes. Ingenuity Pathway Analysis (IPA) indicated that the top differentially expressed genes are involved in ‘Classical Pathways’, ‘Diseases and Functions’ and ‘Networks’, especially those involved in signalling and cellular growth and proliferation. Introduction: Nasopharyngeal angiofibroma (NA) characteristically affects adolescent men. Although benign, these tumors can cause local destruction and surgical resection is warranted.

Objective: We present a case of a 62-year-old man with nasal congestion and epistaxis, found to be a right NA on pathology. We also present a review of the available NA staging systems.

Findings: In this case of NA, imaging revealed an aggressive, hypervascular mass in the nasal cavity with involvement of the middle cranial fossa and cavernous sinus. The patient underwent complete endoscopic surgical resection following vascular embolization.

Current therapies for Parkinson's disease (PD), including L-3,4-dihydroxyphenylalanine (L-DOPA), and clinical trials investigating dopaminergic cell transplants, have generated mixed results with the eventual induction of dyskinetic side effects. Although human umbilical cord blood (hUCB) stem/progenitor cells present with no or minimal capacity of differentiation into mature dopaminergic neurons, their transplantation significantly attenuates parkinsonian symptoms likely via bystander effects, specifically stem cell graft-mediated secretion of growth factors, anti-inflammatory cytokines, or synaptic function altogether promoting brain repair. Recognizing this non-cell replacement mechanism, we examined here the effects of intravenously transplanted combination of hUCB-derived plasma into the 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-induced rat model of PD. …

Returning veterans are frequently diagnosed with traumatic brain injury (TBI) and post-traumatic stress disorder (PTSD). Considering a recent case-controlled study of hyperbaric oxygen therapy (HBOT) reporting a reduction in suicidal ideation, we investigated retrospectively three veterans with chronic TBI/PTSD symptoms who were treated with multiple rounds of HBOT with neurophysiological testing performed before and after treatment. Improvements were detected on parameters within neurocognitive domains, including reductions in suicide-related symptoms. These findings independently confirm that HBOT may be effective in treating specific symptoms of TBI/PTSD that are not currently addressed with existing therapeutic approaches.

Targeting neuronal apoptosis after intracerebral hemorrhage (ICH) may be an important therapeutic strategy for ICH patients. Emerging evidence indicates that C1q/TNF-Related Protein 9 (CTRP9), a newly discovered adiponectin receptor agonist, exerts neuroprotection in cerebrovascular disease. The aim of this study was to investigate the anti-apoptotic role of CTRP9 after experimental ICH and to explore the underlying molecular mechanisms. Ganglioside GM1 is a member of the ganglioside family which has been used in many countries and is thought of as a promising alternative treatment for preventing several neurological diseases, including cerebral ischemic injury. The therapeutic effects of GM1 have been proved both in neonates and in adults following ischemic brain damage; however, its clinical efficacy in patients with ischemic stroke is still uncertain. Objective Previous studies have demonstrated that suppression of Nrf2 in Friedreich ataxia tissues contributes to excess oxidative stress, mitochondrial dysfunction, and reduced ATP production. Omaveloxolone, an Nrf2 activator and NF-kB suppressor, targets dysfunctional inflammatory, metabolic, and bioenergetic pathways. The dose-ranging portion of this Phase 2 study assessed the safety, pharmacodynamics, and potential benefit of omaveloxolone in Friedreich ataxia patients (NCT02255435).

Methods Sixty-nine Friedreich ataxia patients were randomized 3:1 to either omaveloxolone or placebo administered once daily for 12 weeks. Patients were randomized in cohorts of eight patients, at dose levels of 2.5–300 mg/day.

We report the case of a seven-year-old boy with an ingested foreign body, which was retained within the appendix for a known duration of ten months, ultimately requiring appendectomy. The ingested foreign body was incidentally discovered by abdominal x-ray at an emergency room visit for constipation. Despite four bowel cleanouts, subsequent x-rays showed persistence of the foreign body in the right lower quadrant. While the patient did not have signs or symptoms of acute appendicitis, laparoscopic appendectomy was performed due to the risk of this foreign body causing appendicitis in the future. The renin–angiotensin system (RAS) is undisputedly well-studied as one of the oldest and most critical regulators for arterial blood pressure, fluid volume, as well as renal function. In recent studies, RAS has also been implicated in the development of obesity, diabetes, hyperlipidemia, and other diseases, and also involved in the regulation of several signaling pathways such as proliferation, apoptosis and autophagy, and insulin resistance. Background: Medication errors account for the most common adverse events and a significant cause of mortality in the USA. The Joint Commission has required medication reconciliation since 2006. We aimed to survey the literature and determine the challenges and effectiveness of medication reconciliation in the trauma patient population.

Helicobacter pylori is a gram-negative bacteria that is responsible for causing chronic gastritis, ulcers in the stomach and intestine, and eventually even gastric lymphoma or cancer. Multiple treatment options have been recommended for treating an H. pylori infection based on kidney function, previous antibiotic exposure, and whether or not the infection is recurrent. In animal models, anxiety-liked behaviors were demonstrated in ovariectomized or stress-exposed subjects. สปอร์เห็ดหลินจือมีสารสำคัญออกฤทธิ์กระตุ้นการทำงานของเซลล์ในระบบภูมิคุ้มกัน เซลล์เหล่านี้ใช้เวลาในการฟื้นตัวมากกว่า 9 เดือนหลังจากผู้ป่วยมะเร็งหยุดยาเคมีบำบัด จึงนำมาสู่การศึกษาครั้งนี้โดยมีวัตถุประสงค์เพื่อศึกษาผลของสปอร์เห็ดหลินจือที่มีต่อระบบภูมิคุ้มกันในผู้ป่วยมะเร็งเต้านม ผู้ป่วยมะเร็งลำไส้ใหญ่หรือทวารหนักหลังครบการรักษาด้วยยาเคมีบำบัดในช่วงเดือนพฤศจิกายน พ.ศ. 2561 ถึงธันวาคม พ.ศ. 2562 การศึกษานี้เป็นการศึกษาทางคลินิกแบบสุ่ม ปกปิด 2 ทาง มีการเปรียบเทียบกับกลุ่มควบคุม โดยกลุ่มตัวอย่างที่เข้าร่วมการศึกษาจนครบการศึกษาทั้งสิ้น 44 ราย แบ่งโดยการสุ่มเป็น 2 กลุ่มเท่า ๆ กัน ได้แก่ กลุ่มที่ได้สปอร์เห็ดหลินจือที่ผ่านการกะเทาะขนาด 250 มิลลิกรัมและกลุ่มที่ได้ยาหลอก รับประทานผลิตภัณฑ์ครั้งละ 1 แคปซูล วันละ 3 ครั้ง ก่อนอาหาร เป็นระยะเวลา 8 สัปดาห์ วัดระดับอิมมูโนโกลบูลิน (IgG, IgM, IgA และ IgE) ความสมบูรณ์เม็ดเลือด ค่าการทำงานของตับและไต อาการไม่พึงประสงค์ ณ สัปดาห์ที่ 0, 4 และ 8 รวมไปถึงประเมินคุณภาพชีวิตด้วยแบบประเมิน FACT-G ก่อนและหลังเข้าร่วมการศึกษา พบว่า สัปดาห์ที่ 8 กลุ่มที่ได้สปอร์เห็ดหลินจือมีการเปลี่ยนแปลงของระดับอิมมูโนโกลบูลิน เม็ดเลือดแดง เม็ดเลือดขาวชนิด neutrophil basophil eosinophil monocyte และเกล็ดเลือดไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก ในขณะที่จำนวนเม็ดเลือดขาวทั้งหมดและ absolute neutrophil count ในกลุ่มที่ได้สปอร์เห็ดหลินจือมีระดับเพิ่มขึ้นอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก (p-value= 0.031 และ 0.021 ตามลำดับ) ผู้เข้าร่วมการศึกษาทั้ง 2 กลุ่มมีการเปลี่ยนแปลงของค่าการทำงานของตับและไตอย่างไม่มีนัยสำคัญตลอดระยะเวลาการศึกษา อาการไม่พึงประสงค์ที่พบ เช่น ปากแห้ง คอแห้ง คลื่นไส้ เป็นต้น มีความรุนแรงระดับน้อย นอกจากนี้กลุ่มที่ได้สปอร์เห็ดหลินจือมีคุณภาพชีวิตไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก กล่าวโดยสรุป การรับประทานสปอร์เห็ดหลินจือในขนาดที่ศึกษามีความปลอดภัยสามารถเพิ่มจำนวนเม็ดเลือดขาวทั้งหมดและ ANC ของผู้ป่วยมะเร็งหลังได้รับยาเคมีบำบัดได้ แต่ไม่สามารถเพิ่มระดับอิมมูโนโกลบูลินได้อย่างมีนัยสำคัญ

การพัฒนาผลิตภัณฑ์โทนเนอร์จากสารสกัดใบฝรั่งเพื่อยับยั้งแบคทีเรียบนผิวหนังของเยาวชนภายหลังการออกกำลังกาย, ทิทยุต วงษาเนาว์

การศึกษานี้มีวัตถุประสงค์ 1) เพื่อพัฒนาผลิตภัณฑ์โทนเนอร์จากสารสกัดใบฝรั่งที่มีประสิทธิศักย์ยับยั้งเชื้อแบคทีเรีย Staphylococcus aureus ได้เทียบเท่ากับสารอ้างอิงและยับยั้งเชื้อแบคทีเรียได้ตามวิธีมาตรฐานของ BS EN 1276 : 2009 2) ศึกษาความปลอดภัย และ 3) เพื่อศึกษาประสิทธิศักย์ของผลิตภัณฑ์ในการยับยั้งเชื้อแบคทีเรียบนผิวหนังและการลดการหลั่งเหงื่อของเยาวชนภายหลังการออกกำลังกายโดยไม่มีผลกระทบต่อการระบายความร้อน และมีการควบคุมคุณภาพของสารสำคัญทางธรรมชาติในสารสกัดใบฝรั่งรวมทั้งการทดสอบฤทธิ์ในการยับยั้งและฆ่าเชื้อแบคทีเรีย กลุ่มตัวอย่างในการทดสอบการแพ้โทนเนอร์จากสารสกัดใบฝรั่งด้วยวิธีแผ่นแปะเป็นนิสิตชายและหญิง คณะวิทยาศาสตร์การกีฬา จุฬาลงกรณ์มหาวิทยาลัย อายุ 18 – 24 ปี จำนวน 40 คน ได้รับการแปะแผ่นทดสอบบนผิวหนังทิ้งไว้ 24 ชั่วโมง จากนั้นถ่ายภาพบริเวณที่ทดสอบและนำไปใช้ประเมินปฏิกิริยาการแพ้ที่ผิวหนัง กลุ่มตัวอย่างในการศึกษาประสิทธิศักย์ของโทนเนอร์จากสารสกัดใบฝรั่งต่อเชื้อแบคทีเรียบนผิวหนังและการระงับการหลั่งเหงื่อภายหลังการออกกำลังกายเป็นนิสิตคณะวิทยาศาสตร์การกีฬา จุฬาลงกรณ์มหาวิทยาลัย อายุ 18 – 24 ปี จำนวน 64 คน ทุกคนได้เข้ารับการทดสอบพื้นฐาน จากนั้นทำการสุ่มกลุ่มตัวอย่างเพื่อเข้ารับการทดสอบแบบไขว้ปกปิดด้านเดียว (The single - blind, randomized, placebo – controlled crossover trial) เปรียบเทียบระหว่างโทนเนอร์จากสารสกัดใบฝรั่งและสารลวงโดยมีระยะห่างแต่ละการทดสอบ 7 วัน กลุ่มตัวอย่างวิ่งบนลู่กลความหนัก 75 % ของอัตราการเต้นหัวใจสำรอง เป็นเวลา 5 นาที มีการเก็บข้อมูลก่อนและหลังทดลอง คือ น้ำหนักเหงื่อ อัตราการเต้นหัวใจ อุณหภูมิผิวกาย และน้ำมันผิวหนัง วิเคราะห์ข้อมูลด้วยสถิติการวิเคราะห์ความแปรปรวนแบบทางเดียว การวิเคราะห์ความแปรปรวนทางเดียวแบบวัดซ้ำ การวิเคราะห์ความแปรปรวนสองทางแบบวัดซ้ำ การวิเคราะห์ความแปรปรวนร่วมแบบทางเดียว และเปรียบเทียบความแตกต่างแบบรายคู่โดยใช้วิธีของบอนเฟอร์โรนี (Bonferroni) ที่ระดับความมีนัยสำคัญทางสถิติ 0.05 ผลการศึกษา พบว่า โทนเนอร์จากสารสกัดใบฝรั่งมีประสิทธิศักย์ในการยับยั้งเชื้อ Staphylococcus aureus ได้เทียบเท่ากับยา 0.5 % คลอร์เฮกซิดีน มีประสิทธิศักย์ในการยับยั้งเชื้อแบคทีเรียผ่านตามวิธีมาตรฐานตลอดจนแสดงผลการแพ้ที่ผิวหนังอย่างไม่มีนัยสำคัญ มีประสิทธิศักย์ยับยั้งเชื้อแบคทีเรียบนผิวหนังของเยาวชนภายหลังการออกกำลังกายได้ดีที่สุดเมื่อเปรียบเทียบกับสารลวงและการทดสอบพื้นฐาน และสามารถลดการหลั่งเหงื่อได้ดีกว่าสารลวงและการทดสอบพื้นฐานอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 โดยไม่มีผลต่อการระบายความร้อนของร่างกาย สรุป ผลิตภัณฑ์โทนเนอร์จากสารสกัดใบฝรั่งเป็นผลิตภัณฑ์สำหรับใช้ภายหลังการออกกำลังกายที่ลดการหลั่งเหงื่อ ลดกลิ่นเหงื่อและกลิ่นตัว โดยไม่มีผลต่อการระบายความร้อนของร่างกาย และไม่ทำให้เกิดการแพ้ต่อผิวหนัง

Issue addressed: Young parents (<25 years) have lower engagement with health and community services and are more likely to experience negative outcomes in the perinatal period compared to older parents. The aim of this study was to evaluate the short to medium‐term outcomes of the Young Parents Program (YPP), specifically designed to engage and support young parents, using responsive and codesign strategies in a community setting.

Methods: A qualitative case study used data from interviews with participating parents (n = 20) and a focus group with YPP facilitators (n = 5).

Results: The findings report on the following short to medium‐term YPP outcomes for parents and children. Young parents: are engaged in early parenting services that are welcoming, non-judgemental and holistic; build parenting skills, knowledge, confidence and are tuned into their infants’ needs; are empowered to codesign program activities to meet their parenting and non-parenting needs; have developed friendships and a social support network in their local community; and, are linked …

The aim of the systematic review was to determine the effectiveness of organizational-level person-centered care for people living with dementia in relation to their quality of life, mood, neuropsychiatric symptoms and function. ALOIS, the Cochrane Dementia and Cognitive Improvement Group Specialised Register databases, were searched up to June 2018 using the terms dementia OR cognitive impairment OR Alzheimer AND non-pharmacological AND personhood OR person-centered care.
