Background: This study was the first phase of a larger study which explored the past, present and future of nursing in early parenting services in Australia.
Aim: The aim of this paper is to describe the history of nursing within an early parenting service in Western Australia (WA).
Methods: Triangulation of multiple data sources was used to summarise the nursing role over 120 years. The history was discovered through a document analysis of archives, including oral histories, organisational documents, focus groups, nurses’ diaries and interviews with nurses.

Background: General practitioners (GPs) and general practice nurses (GPNs) face increasing demands to provide end-of-life care (EoLC) as the population ages. To enhance primary palliative care (PC), the care they provide needs to be understood to inform best practice models of care.
Objective: To provide a comprehensive description of the self-reported role and performance of GPs and GPNs in (1) specific medical/nursing roles, (2) communication, (3) care co-ordination, (4) access and out-of-hours care, and (5) multidisciplinary care.

The utility of the heart rate derived variable TRaining IMPulse (TRIMP) for assessing internal training load in ice hockey players is not clear. Having a reliable measure of internal training load during on-ice training sessions would help coaches program exercise training. This study determined the reliability of TRIMP during on-ice training sessions in ice hockey players. Twelve Division I collegiate male ice hockey players (aged 18–23 years) had their heart rate (HR) data recorded during two on-ice practice sessions separated by two weeks. TRIMP and other descriptive HR variables were compared between sessions. TRIMP demonstrated moderate reliability during on-ice sessions. …

Introduction:In an effort to expand the reach of health-promotion efforts for people with disabilities, a one-on-one health promotion intervention titled Health My Way was piloted. This intervention incorporated health coaching and health-promotion curriculum designed specifically for people with disabilities.
Purpose:The intervention was evaluated for its effects on health behavior change. Additionally, personal sense of meaning and hope were examined as potential mechanisms of influence in the hypothesized behavior change process.

This thesis provides a culturally-comprehensive review of the plants utilized for food in the Bitterroot Salish tribe of northwestern Montana. As part of the larger Confederated Salish and Kootenai Tribes (CS&KT) of the Flathead Indian Reservation, the Bitterroot Salish historically utilized hundreds of plants for food, medicine and hygiene. This thesis aims to highlight food plants and their important cultural components. The information herein is a combination of history, ethnography, linguistics, ethnobotany, and first-hand experience with the current Salish community to provide a holistic framework of understanding traditional food plants today. Objectives: Examining the relationship between workloads (miles ran) of collegiate cross-country (XC) athletes (n=18) and symptoms of injury or illness over the course of 10 weeks. Background: Internet is becoming an increasingly common tool for survey research, particularly among “hidden” or vulnerable populations, such as men who have sex with men (MSM). Web-based research has many advantages for participants and researchers, but fraud can present a significant threat to data integrity.

This study investigated the effect of supramaximal intensities on number of Tabata bouts performed by recreationally trained males. Eleven relatively trained men (mean age = 22.27 ± 1.74 years, height = 1.72 ± 0.07m, mass = 78.74 ± 15.77kg) were assigned the four intensities in a counterbalanced order. Subjects completed a warm up which was then followed by consecutive 20-second bouts of treadmill running with 10 second bouts of rest until fatigue given each intensity (110%,130%,150%,170% of VO2max). Given that ideal number of Tabata bouts is ~8 with an intensity of 170% VO2max, results suggested that …

Purpose: Interleukin-12 (IL12) promotes adaptive type I immunity and has demonstrated antitumor efficacy, but systemic administration leads to severe adverse events (AE), including death. This pilot trial investigated safety, efficacy, and immunologic activity of intratumoral delivery of IL12 plasmid DNA (tavo) via in vivo electroporation (i.t.-tavo-EP) in patients with Merkel cell carcinoma (MCC), an aggressive virus-associated skin cancer.

Depending on the initiating stimulus, cancer cell death can be immunogenic or non-immunogenic. Inducers of immunogenic cell death (ICD) rely on endoplasmic reticulum (ER) stress for the trafficking of danger signals such as calreticulin (CRT) and ATP. We found that nanosecond pulsed electric fields (nsPEF), an emerging new modality for tumor ablation, cause the activation of the ER-resident stress sensor PERK in both CT-26 colon carcinoma and EL-4 lymphoma cells. PERK activation correlates with sustained CRT exposure on the cell plasma membrane and apoptosis induction in both nsPEF-treated cell lines. Our results show that, in CT-26 cells, the activity of …

Costal cartilage is much understudied compared to the load bearing cartilages. Abnormally grown costal cartilages are associated with the inherited chest wall deformities pectus excavatum and pectus carinatum resulting in sunken or pigeon chest respectively. A lack of understanding of the ultrastructural and molecular biology properties of costal cartilage is a major confounder in predicting causes and outcomes of these disorders. Due to the avascular nature of cartilage, chondrocytes metabolize glycolytically, producing an acidic environment. During physical activity hydrogen ions move within cartilage driven by compressive forces, thus at any one time, chondrocytes experience transient changes in pH. A variety …

Chloroviruses exist in aquatic systems around the planet where they infect certain eukaryotic green algae that are mutualistic endosymbionts in a variety of protists and metazoans. Natural chlorovirus populations are seasonally dynamic but the precise temporal changes in these populations and the mechanisms that underlie them have, heretofore, been unclear. We recently reported the novel concept that predator/prey-mediated virus activation regulates chlorovirus population dynamics, and in the current manuscript demonstrate virus packaged chemotactic modulation of prey behavior.

Guns remain a major cause of injury and death among children. We determined pediatric residents’ familiarity with gun safety campaigns and their gun safety counseling practices. We determined pediatric residents’ comfort with the Asking Saves Kids (ASK) campaign, which recommends that parents ask about gun safety and storage where their children play. Methods: Cross-sectional 27-item electronic survey was distributed to three pediatric residency programs in Baltimore, Maryland, USA. Residents were asked to respond to statements using a seven-point Likert scale on familiarity with three gun safety campaigns and their attitudes toward gun safety counseling. Background: Optimal timing to initiate anticoagulation after acute ischemic stroke (AIS) from atrial fibrillation (AF) is currently unknown. Compared to other stroke etiologies, AF typically provokes larger infarct volumes and greater concern of hemorrhagic transformation, so seminal randomized trials waited weeks to months to begin anticoagulation after initial stroke. Subsequent data are limited and non-randomized. Guidelines suggest anticoagulation initiation windows between 3 and 14 days post-stroke, with Class IIa recommendations, and level of evidence B in the USA and C in Europe.

Hexavalent chromium, Cr(VI), is an established human carcinogen that is a worldwide environmental health concern. It is well understood that reactive oxygen species, genomic instability, and DNA damage repair deficiency are important contributors to Cr(VI)-induced carcinogenesis. After decades of research some cancer hallmarks remain understudied for the mechanism of Cr(VI) carcinogenesis. Dysregulated cellular energetics have been established as a hallmark of cancer. Energy pathways that become dysregulated in cancer include mitochondrial respiration, lipogenesis, pentose phosphate pathway, one carbon metabolism, and increased anaerobic glycolysis in the presence of oxygen or ‘Warburg effect’.

BACKGROUND: The United States Centers for Disease Control and Prevention estimates that approximately 60,000 US youth are living with HIV. US youth living with HIV (YLWH) have poorer outcomes compared with adults, including lower rates of diagnosis, engagement, retention, and virologic suppression. With Adolescent Medicine Trials Network for HIV/AIDS Interventions (ATN) support, new trials of youth-centered interventions to improve retention in care and medication adherence among YLWH are underway.

Laromustine is a chemotherapeutic sulfonylhydrazine prodrug used in clinical trials against acute myeloid leukemia. Though laromustine showed some success in clinical trials, more experiments are needed to understand the hematological toxicity and the molecular mechanisms of patients’ resistance. This project aims to develop a strategy to identify compounds synergistic with laromustine in cultured leukemia cells from a library of 450 FDA-approved compounds through a forward chemical genetic screen. To optimize the screen, the cell seeding density, doubling time, and dose response curves were determined. Alcohol consumption is one of the most common health risk behaviors which is a leading cause of various health problems and even to death globally. Excess alcohol use results in 2.5 million deaths every year. Around 320,000 young people aged between 15 and 29 years die from alcohol-related causes, resulting in 9% of all deaths in that age group. การวิจัยนี้เป็นการวิจัยเชิงวิเคราะห์แบบเก็บข้อมูลย้อนหลัง เพื่อประเมินการสั่งใช้ยากลุ่มสแตตินในทางปฏิบัติเพื่อป้องกันโรคหัวใจและหลอดเลือด และศึกษาประสิทธิผลการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ได้รับยาต้านไวรัส ณ โรงพยาบาลกระทุ่มแบน มีผู้ป่วยเข้าร่วมการวิจัย 551 ราย โดยผู้ป่วยส่วนใหญ่เป็นเพศชาย ร้อยละ 52.5 เมื่อประเมินการสั่งใช้ยากลุ่มสแตตินตามแนวทางเวชปฏิบัติการใช้ยารักษาภาวะไขมันผิดปกติเพื่อป้องกันโรคหัวใจและหลอดเลือด พ.ศ. 2559 มีผู้ป่วยที่ควรได้รับยากลุ่มสแตตินตามคำแนะนำเพื่อป้องกันโรคหัวใจและหลอดเลือด 198 ราย (ร้อยละ 35.9) แต่ได้รับยากลุ่มสแตตินเพียง 37 ราย (ร้อยละ 6.7) และไม่ได้รับยากลุ่มสแตตินสำหรับป้องกันปฐมภูมิและทุติยภูมิ 155 ราย (ร้อยละ 28.1) และ 6 ราย (ร้อยละ 1.1) ตามลำดับ เมื่อจำแนกตามระดับความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในการป้องกันโรคหัวใจและหลอดเลือดชนิดปฐมภูมิ พบว่า ผู้ป่วยที่ระดับความเสี่ยงปานกลางและสูงถึงสูงมากได้รับการสั่งใช้ยากลุ่มสแตตินไม่ตรงตามคำแนะนำมากกว่าตรงตามคำแนะนำในแนวทางเวชปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติ (P<0.01 และ P<0.01 ตามลำดับ) สำหรับข้อมูลประสิทธิผลของการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ระยะเวลา 3, 6 และ 12 เดือน พบว่า กลุ่มผู้ป่วยที่ไม่ได้รับยากลุ่มสแตตินและกลุ่มผู้ป่วยที่ได้รับยากลุ่มสแตตินมีผลต่างของระดับแอลดีแอลในเลือด ซึ่งมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<0.01) ผลจากการศึกษานี้แสดงให้เห็นว่า อัตราการสั่งใช้ยากลุ่มสแตตินเพื่อป้องกันโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวีที่ไม่เหมาะสมตามเกณฑ์คิดเป็นร้อยละ 37.7 แม้ว่ายากลุ่มสแตตินจะมีประสิทธิผลในการลดระดับไขมันในเลือดและมีความปลอดภัยในการใช้ร่วมกับยาต้านไวรัส ดังนั้นควรมีการพิจารณาเพิ่มการสั่งใช้ยากลุ่มสแตตินอย่างสมเหตุผลตามเกณฑ์ เพื่อป้องกันความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวี

การศึกษาครั้งนี้เป็นการวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาการตีความตามแนวคิดของมาร์ติน ไฮเดกเกอร์ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ ผู้ให้ข้อมูลจำนวน 11 คน โดยคัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึก การบันทึกเทปและนำข้อมูลมาถอดคำต่อคำ การสังเกต และการจดบันทึกในระหว่างการสัมภาษณ์ ร่วมกับวิเคราะห์เนื้อหาตามแบบวิธีการของแวน มาเนน ผลการศึกษาสามารถแบ่งเป็น 2 ส่วน ได้แก่ การให้ความหมายของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ และประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ โดยการให้ความหมายของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ คือ การดูแลเอาใจใส่เพื่อไม่ให้กลับป่วยซ้ำ เกิดจากความรักใคร่ ห่วงใยและผูกพัน ผู้ป่วยคือคนสำคัญ มีคุณค่า และมีความหมายต่อผู้ดูแล พบ 2 ประเด็นย่อย คือ 1. การทำหน้าที่ด้วยความรักความห่วงใย 2. เกิดความผูกพันรักใคร่ในครอบครัว ส่วนประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ พบ 3 ประเด็นหลัก และ 6 ประเด็นย่อย ดังนี้ 1.การรับรู้ว่าผู้ป่วยผิดปกติและแสวงหาการรักษา 1.1) การรับรู้ว่าผู้ป่วยมีอาการผิดปกติ 1.2) แสวงหาการรักษาที่เหมาะสมกับผู้ป่วย 2. การทำใจยอมรับสิ่งที่เกิดขึ้น 2.1) หลากหลายความทุกข์ถาโถมเข้ามาในชีวิต 2.2) การทำใจยอมรับเพื่อตนเองและผู้ป่วย 3. การดูแลฟื้นฟู และป้องกันไม่ให้ผู้ป่วยกลับป่วยซ้ำ 3.1) การทำหน้าที่ตอบสนองทางด้านร่างกาย จิตใจ และสังคมของผู้ป่วย 3.2) ให้ผู้ป่วยได้กลับมาประกอบอาชีพ และมีรายได้เป็นของตนเอง ผลการศึกษาครั้งนี้ทำให้เข้าใจประสบการณ์ของผู้ดูแลหลักในการดูแลผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำ ตั้งแต่ผู้ป่วยจิตเภทเริ่มมีอาการทางจิต การดูแลฟื้นฟูเพื่อป้องกันการกลับป่วยซ้ำ และผลกระทบโดยตรงที่เกิดขึ้นกับผู้ดูแลหลัก เป็นแนวทางสำหรับพยาบาลและสหวิชาชีพ สามารถนำไปพัฒนาระบบการดูแลทั้งผู้ป่วยจิตเภทที่ไม่กลับป่วยซ้ำและผู้ดูแลหลักอย่างมีประสิทธิภาพและเหมาะสมเพื่อป้องกันการกลับป่วยซ้ำ

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาองค์ประกอบและตัวแปรที่อธิบายการดูแลแบบประคับประคองสำหรับผู้ป่วยโรคหลอดเลือดสมอง กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในการดูแลผู้ป่วยโรคหลอดเลือดสมองอย่างน้อย 2 ปีขึ้นไป จำนวน 378 คน ได้จากการสุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในการเก็บข้อมูลคือแบบสอบถามองค์ประกอบการแบบประคับประคองสำหรับผู้ป่วยโรคหลอดเลือดสมอง ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิและหาความเที่ยงของเครื่องมือ โดยใช้สูตรสัมประสิทธิ์อัลฟาครอนบาค ได้ค่าความเที่ยงเท่ากับ 0.95 วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์ตัวประกอบ สกัดตัวประกอบด้วยวิธีตัวประกอบหลัก (Principle Components Analysis) หมุนแกนแบบออโธโกนอล (Orthogonal Rotation) ด้วยวิธีแวริแมกซ์ (Varimax Method) ผลการวิจัยพบว่า องค์ประกอบการดูแลแบบประคับประคองสำหรับผู้ป่วยโรคหลอดเลือดสมองประกอบด้วย 11 องค์ประกอบ บรรยายด้วย 81 ตัวแปร มีค่าความแปรปรวนรวมคิดเป็นร้อยละ 64.28 ได้แก่ 1) องค์ประกอบด้านการดูแลระยะสุดท้าย ประกอบด้วย 10 ตัวแปร อธิบายความแปรปรวนได้มากที่สุด คิดเป็นร้อยละ 9.58 2) องค์ประกอบด้านการมีส่วนร่วมและกำหนดเป้าหมายของการดูแล ประกอบด้วย 12 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 8.88 3) องค์ประกอบด้านการจัดการกับความปวดและอาการข้อติดแข็ง ประกอบด้วย 11 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 8.55 4) องค์ประกอบด้านการส่งต่อและระบบการดูแลต่อเนื่อง ประกอบด้วย 11 ตัวแปร อธิบายความแปรปรวนได้ ร้อยละ 7.69 5) องค์ประกอบด้านการดูแลจิตใจและอารมณ์ ประกอบด้วย 9 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 7.39 6) องค์ประกอบด้านการดูแลจิตวิญญาณ ประกอบด้วย 7 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 5.74 7) องค์ประกอบด้านการดูแลความผิดปกติด้านร่างกายหลังเกิดโรคหลอดเลือดสมอง ประกอบด้วย 6 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 5.26 8) องค์ประกอบด้านการให้ข้อมูลที่มีประสิทธิผล ประกอบด้วย 5 ตัวแปรอธิบายความแปรปรวนได้ร้อยละ 3.96 9) องค์ประกอบด้านการเอื้ออำนวยความสะดวกในระบบบริการประกอบด้วย 4 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 3.68 10) องค์ประกอบด้านการป้องกันการเกิดแผลกดทับ ประกอบด้วย …
