Background and Aim: Nonerosive gastroesophageal reflux disease or nonerosive reflux disease (NERD) is characterized by troublesome reflux-related symptoms in the absence of esophageal erosions/breaks at conventional endoscopy. There are a number of medicines cited in homoeopathic literature which can be used for treatment of symptoms such as heartburn and regurgitation. A pilot study was undertaken to explore usefulness of homoeopathic medicines in treatment of NERD. Methodology: In this study, 78 patients were screened and 34 were enrolled, having symptoms of heartburn and/or regurgitation at least twice a week, having a gastroesophageal reflux disease (GERD) symptom score of more than 4. …

Capecitabine, an oral prodrug of 5-fluorouracil (5-FU), is approved for early-stage and advanced colorectal cancer and metastatic breast cancer. Cardiotoxicity of 5-FU is well described in the literature. However, cardiac adverse effects of capecitabine are poorly described. We report a case of coronary vasospasm induced by capecitabine. A 41-year-old female with metastatic breast cancer presented with chest pain 3 days after starting capecitabine. The chest pain was relieved by rest and exacerbated by exertion. Her physical examination was unremarkable except for a rapid heart rate of 100 bpm. Electrocardiogram test showed no acute ischemic changes. Troponin tests were negative. CT …

We tested procedures for removing adipocere from insect samples to allow identification. An acceptable procedure was determined: (i) Samples were sorted in petri dishes with 75% alcohol to remove any larvae, adult insects, or other soft-bodied material. (ii) Samples of up to 24 puparia were placed in a vial with 15 mL of 95% acetone, capped, and vortexed for a total of 30–90 sec in 10- to 15-sec bursts. วัตถุประสงค์: เพื่อศึกษาผลของการดูแลทางจิตวิญญาณเชิงพุทธต่อความผาสุกทางจิตวิญญาณของผู้ป่วยสูงอายุโรคมะเร็งระยะสุดท้าย\n\nแบบแผนงานวิจัย: การวิจัยแบบกึ่งทดลองชนิดกลุ่มเดียววัดแบบอนุกรมเวลา\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุที่เป็นโรคมะเร็งระยะสุดท้ายทุกชนิด เข้ารับการรักษาเป็นผู้ป่วยในโรงพยาบาลเอกชน ระดับตติยภูมิ ที่มีคุณสมบัติตามเกณฑ์ที่กำหนด จำนวน 30 คน เครื่องมือที่ใช้ในการทดลอง ได้แก่ การดูแลทางจิตวิญญาณเชิงพุทธตามคู่มือในการดำเนินกิจกรรม โดยดำเนินกิจกรรมทุก 2 วัน ครั้งละ 20-30 นาที รวม 7 ครั้ง เป็นเวลาทั้งหมด 2 สัปดาห์ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ แบบประเมินความผาสุกทางจิตวิญญาณ มีค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .87 วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์ความแปรปรวนแบบวัดซ้ำและการเปรียบเทียบรายคู่\n\nผลการวิจัย: ค่าเฉลี่ยคะแนนความผาสุกทางจิตวิญญาณระหว่างดำเนินโปรแกรมการดูแลทางจิตวิญญาณเชิงพุทธและหลังเสร็จสิ้นโปรแกรมฯ สูงกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป: โปรแกรมการดูแลทางจิตวิญญาณเชิงพุทธส่งผลให้ผู้ป่วยสูงอายุโรคมะเร็งระยะสุดท้ายมีความผาสุกทางจิตวิญญาณมากขึ้น โดยมีความผาสุกทางจิตวิญญาณมากขึ้นตั้งแต่อยู่ระหว่างการเข้าร่วมโปรแกรมฯ

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการจัดการกับอาการร่วมกับการใช้สมาธิต่ออาการกำเริบเฉียบพลันของผู้ป่วยโรคปอดอุดกั้นเรื้อรัง\n\nแบบแผนงานวิจัย: การวิจัยกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยโรคปอดอุดกั้นเรื้อรังที่อยู่ในอำเภอพุนพิน และมีคุณสมบัติตามเกณฑ์ที่กำหนด แบ่งเป็นกลุ่มควบคุม 20 ราย และกลุ่มทดลอง 20 ราย โดยจัดให้แต่ละกลุ่มมีความคล้ายคลึงกันด้านเพศ และระดับความรุนแรงของโรค กลุ่มควบคุมได้รับการพยาบาลตามปกติ ในขณะที่กลุ่มทดลองได้รับโปรแกรมการจัดการกับอาการร่วมกับการใช้สมาธิ ซึ่งประกอบด้วย 5 ขั้นตอน คือ 1) การประเมินประสบการณ์เกี่ยวกับอาการกำเริบเฉียบพลัน 2) การให้ความรู้ในการจัดการกับอาการกำเริบเฉียบพลัน 3) การฝึกทักษะในการจัดการกับอาการกำเริบเฉียบพลัน 4) การปฏิบัติในการจัดการกับอาการกำเริบเฉียบพลัน และ 5) การประเมินผลการจัดการกับอาการกำเริบเฉียบพลัน ประเมินอาการกำเริบเฉียบพลันโดยการวัดอัตราการไหลสูงสุดของอากาศขณะหายใจออก (Peak Expiratory Flow Rate: PEFR) โดยใช้ Wright Peak Flow Meter วิเคราะห์ข้อมูลโดยสถิติบรรยาย และสถิติทดสอบที\n\nผลการวิจัย:\n1) ผู้ป่วยโรคปอดอุดกั้นเรื้อรังหลังได้รับโปรแกรมการจัดการกับอาการร่วมกับการใช้สมาธิมีอัตราการไหลสูงสุดของอากาศขณะหายใจออกดีกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\n2.) ผู้ป่วยโรคปอดอุดกั้นเรื้อรังในกลุ่มทดลองมีอัตราการไหลสูงสุดของอากาศขณะหายใจออกเพิ่มขึ้นมากกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n\nสรุป: โปรแกรมการจัดการกับอาการร่วมกับการใช้สมาธิ สามารถลดอาการกำเริบเฉียบพลันในผู้ป่วยโรคปอดอุดกั้นเรื้อรังได้

วัตถุประสงค์: เพื่อบรรยายการรับรู้และการจัดการปัญหาด้านเพศสัมพันธ์ของผู้ป่วยเพศชายโรคไตวายเรื้อรังระยะสุดท้ายที่ฟอกเลือดด้วยเครื่องไตเทียม \n\nแบบแผนงานวิจัย: การวิจัยเชิงคุณภาพโดยใช้ระเบียบวิธีวิจัยแบบปรากฏการณ์วิทยา\n\nวิธีดำเนินการวิจัย: ผู้ให้ข้อมูล คือ ผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่ฟอกเลือดด้วยเครื่องไตเทียมที่มีปัญหาด้านเพศสัมพันธ์ คัดเลือกแบบเจาะจงจากโรงพยาบาลเอกชนแห่งหนึ่ง จำนวน 10 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์เชิงลึก การสังเกต และการบันทึกเทป ข้อมูลที่ได้วิเคราะห์เนื้อหาตามขั้นตอนของ Diekelmann ใส่รหัสข้อมูลโดยดึงเนื้อหาสำคัญ\n\nผลการวิจัย: ประสบการณ์การรับรู้และการจัดการปัญหาด้านเพศสัมพันธ์ของผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่ฟอกเลือดด้วยเครื่องไตเทียม แบ่งออกเป็น 4 ประเด็นหลัก ดังนี้ 1) ชะตาตกเมื่อเจ็บป่วย ซวยที่สุด คือ นกเขาไม่ขัน ซึ่งแบ่งออกได้เป็น 3 ประเด็นย่อย 1.1) ชะตาตกเมื่อเจ็บป่วยด้วยโรคที่รักษาไม่หาย 1.2) เหนื่อยกับการเดินทาง เครียด กังวลเกี่ยวกับการรักษาจนกระทบเรื่องเพศสัมพันธ์ 1.3) ภาวะแทรกซ้อนที่แย่ที่สุด คือ อวัยวะเพศไม่แข็งตัว 2) ปัญหาทางกายที่ว่าแย่หรือจะเท่าปัญหาที่เกิดทางใจ 3) ปัญหาเศรษฐกิจครอบครัวส่งผลต่อเพศสัมพันธ์ และ 4) เพศสัมพันธ์ชีวิตคู่ต้องจัดการและทำความเข้าใจ \n\nสรุป: ผลการวิจัยนี้ ทำให้เกิดความเข้าใจผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่ฟอกเลือดด้วยเครื่องไตเทียมเกี่ยวกับการรับรู้และการจัดการปัญหาด้านเพศสัมพันธ์ของผู้ป่วย สามารถนำผลการวิจัยไปใช้เป็นข้อมูลพื้นฐานในการปฏิบัติการพยาบาล เป็นแนวทางในการวิจัยทางการพยาบาลและพัฒนาองค์ความรู้ด้านปัญหาเพศสัมพันธ์ของผู้ป่วยที่บุคลากรสุขภาพอาจมองข้ามไป

วัตถุประสงค์: เพื่อศึกษาปัจจัยที่มีอิทธิพลต่อพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุไทย ตามกรอบแนวคิด PRECEDE PROCEED Model\n\nรูปแบบการวิจัย: การวิจัยนี้เป็นการวิจัยแบบทำนาย (Predictive research)\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุวัยกลางตำบลบางเพรียง อำเภอบางบ่อ จังหวัดสมุทรปราการ จำนวน 276 คน ได้มาจากการสุ่มอย่างง่าย เก็บรวบรวมข้อมูลโดยแบบวัดความรู้ในการดูแลสุขภาพ แบบสอบถามทัศนคติในการดูแลสุขภาพ การรับรู้คุณค่าในตนเอง การดื่มแอลกอฮอล์ สภาพแวดล้อมที่อยู่อาศัย แรงสนับสนุนทางสังคมจากครอบครัว แรงสนับสนุนทางสังคมจากบุคลากรสาธารณสุข และแบบสอบถามพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุ วิเคราะห์ข้อมูลใช้สถิติเชิงพรรณนาและการวิเคราะห์การถดถอยพหุคูณแบบขั้นตอน\n\nผลการวิจัย: พบว่า มีตัวแปร 6 ตัวแปรสามารถร่วมกันทำนายพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุได้อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 อธิบายความแปรปรวนได้ร้อยละ 27.0 โดยตัวแปรการรับรู้คุณค่าในตนเองเป็นตัวแปรที่มีอิทธิพลสูงสุด (Beta = .266) รองลงมา คือ การดื่มแอลกอฮอล์ (Beta = - .229) ความรู้ในการดูแลสุขภาพ (Beta = .218) สภาพแวดล้อมที่อยู่อาศัย (Beta =.174) ทัศนคติในการดูแลสุขภาพ (Beta =.158) และแรงสนับสนุนทางสังคมจากครอบครัว (Beta = .116) ทั้งนี้ แรงสนับสนุนทางสังคมจากบุคลากรสาธารณสุข พบว่า ไม่สามารถทำนายพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุได้\n\nสรุป: ผู้สูงอายุมีแนวโน้มที่จะกระทำพฤติกรรมส่งเสริมสุขภาพดีขึ้นหากมีความรู้ในการดูแลสุขภาพ มีทัศนคติที่ดีในการดูแลสุขภาพ มีการรับรู้คุณค่าในตนเอง ไม่ดื่มแอลกอฮอล์ มีสภาพแวดล้อมที่อยู่อาศัยที่ดี และได้รับแรงสนับสนุนทางสังคมจากครอบครัว บุคลากรสาธารณสุขสามารถนำทั้ง 6 ปัจจัยมากำหนดเป็นแนวทางในการส่งเสริมพฤติกรรมส่งเสริมสุขภาพให้กับผู้สูงอายุได้\n

วัตถุประสงค์: เพื่อเปรียบเทียบความพึงพอใจในบริการของผู้ป่วย ระหว่างกลุ่มผู้ป่วยที่จำหน่ายตามปกติ และกลุ่มผู้ป่วยที่ได้รับโปรแกรมการจำหน่ายผู้ป่วยตามแนวคิดแบบลีน ร่วมกับแนวคิดการวางแผนการจำหน่ายผู้ป่วย\n\nแบบแผนงานวิจัย: การวิจัยกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยกลุ่มโรคเรื้อรังทางอายุรกรรมที่ได้รับการจำหน่ายให้กลับบ้าน หอผู้ป่วยพิเศษ โรงพยาบาลเจริญกรุงประชารักษ์ จำนวน 40 คน โดยแบ่งเป็นกลุ่มผู้ป่วยที่ได้รับการจำหน่ายตามปกติ และกลุ่มผู้ป่วยที่ได้รับโปรแกรมการจำหน่ายตามแนวคิดแบบลีนร่วมกับแนวคิดการวางแผนการจำหน่ายกลุ่มละ 20 คน เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมการจำหน่ายผู้ป่วยตามแนวคิดแบบลีนร่วมกับแนวคิดการวางแผนการจำหน่าย เก็บรวบรวมข้อมูลโดย 1) แบบสอบถามความพึงพอใจในบริการของผู้ป่วย 2) แบบบันทึกการปฏิบัติกิจกรรมการจำหน่าย3) แบบบันทึกปริมาณเวลาที่ใช้ในกิจกรรมการจัดการการจำหน่ายผู้ป่วย 4) แบบวัดความรู้เรื่องแนวคิดแบบลีนและ M-E-T-H-O-D เครื่องมือทั้งหมดผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ ได้ค่าความเที่ยงของแบบสอบถามเท่ากับ .95 วิเคราะห์ข้อมูลด้วยสถิติ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที\nผลการวิจัย: ความพึงพอใจในบริการของผู้ป่วยที่ได้รับโปรแกรมการจำหน่ายผู้ป่วยตามแนวคิดแบบลีนดีกว่าความพึงพอใจในบริการของผู้ป่วยที่จำหน่ายตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับที่ .05\n\nสรุป: โปรแกรมการจำหน่ายผู้ป่วยตามแนวคิดแบบลีนส่งผลให้ผู้ป่วยมีความพึงพอใจในบริการมากกว่าการจำหน่ายตามปกติ\n

The population of older adults with chronic disease is increasing, yet little is known about their perception of chronic disease and self-management. To develop successful and sustainable chronic disease self-management interventions in the older adult population, health care providers must first understand older adults’ attitudes toward health status and behavior change. INTRODUCTION: Scholarship is an essential part of academic success. Junior faculty members are often unfamiliar with the grounding literature that defines educational scholarship. In this article, the authors aim to summarize five key papers which outline education scholarship in the setting of academic contributions for emerging clinician educators.
METHODS: The authors conducted a consensus-building process to generate a list of key papers that describe the importance and significance of academic scholarship, informed by social media sources. They then used a three-round voting methodology, akin to a Delphi study, to determine the most useful papers.

INTRODUCTION: Although preventing recurrent violent injury is an important component of a public health approach to interpersonal violence and a common focus of violence intervention programs, the true incidence of recurrent violent injury is unknown. Prior studies have reported recurrence rates from 0.8% to 44%, and risk factors for recurrence are not well established.
METHODS: We used a statewide, all-payer database to perform a retrospective cohort study of emergency department visits for injury due to interpersonal violence in Florida, following up patients injured in 2010 for recurrence through 2012. BACKGROUND:
The use of simulated learning experiences (SLEs) have increased within nursing curricula with positive learning outcomes for nursing students. The purpose of this study is to explore nursing students' perceptions of their clinical decision making (CDM) related to the block sequencing of different patient care experiences, SLEs versus hospital-based learning experiences (HLEs).
METHOD:
A qualitative descriptive design used open-ended survey questions to generate information about the block sequencing of SLEs and its impact on nursing students' perceived CDM.
RESULTS:
Three themes emerged from the data: Preexperience Anxiety, Real-Time Decision Making, and Increased Patient Care Experiences.

Polypharmacy is a highly prevalent problem in older persons, and is challenging to assess and improve due to variations in definitions of the problem and the heterogeneous methods of medication review and reduction. The purpose of this review is to summarize evidence regarding the prevalence and impact of polypharmacy in geriatric oncology patients and to provide recommendations for assessment and management. Polypharmacy has somewhat variably been incorporated into geriatric assessment studies in geriatric oncology, and polypharmacy has not been consistently evaluated as a predictor of negative outcomes in patients with cancer. The burden of HIV/AIDS and other transmissible diseases is higher in prison and jail settings than in the non-incarcerated communities that surround them. In this comprehensive review, we discuss available literature on the topic of clinical management of people infected with HIV, hepatitis B and C viruses, and tuberculosis in incarcerated settings in addition to co-occurrence of one or more of these infections. Methods such as screening practices and provision of treatment during detainment periods are reviewed to identify the effect of community-based treatment when returning inmates into the general population. To date, the only published reports of bone mineral density (BMD) in MPS IV involve patients with MPS IVA; no reports exist describing BMD for MPS IVB. In this prospective study of BMD in three patients with MPS IVB, BMD was acquired by dual-energy X-ray absorptiometry (DXA) at whole body (WB), lumbar spine (LS), and lateral distal femur (LDF). Functional abilities, ambulatory status, medical history, and height z-score were evaluated. Three patients with MPS IVB (two females), aged 17.7, 31.4 and 31.7 years, were evaluated. The purpose of this study was to investigate the joint- and load-dependent changes in the mechanical demands of the lower extremity joints during the hang power clean (HPC) and the jump shrug (JS). Fifteen male lacrosse players were recruited from an NCAA DI team, and completed three sets of the HPC and JS at 30%, 50%, and 70% of their HPC 1-Repetition Maximum (1-RM HPC) in a counterbalanced and randomized order. Writers generally benefit from word processing technology, and the use of other forms of formal writing such as typewriters is archaic. The first stand-alone spell checker programs originated in the early 1980s, and by 1995 they were embedded within word processing programs such as Word 95 (1). With the ubiquity of such software, spelling errors in the medical literature should be extinct. Background: Postpartum women need effective contraception. Concerns have been raised that use of progestogen-only contraceptives (POCs) may affect breastfeeding performance and infant health outcomes.
Objectives: We investigated the clinical outcomes of breastfeeding duration, initiation of supplemental feeding and weaning, as well as infant outcomes including infant growth, health and development among breastfeeding women using POCs compared with breastfeeding women not using POCs.
Search strategy: We searched the PubMed database for all articles published from database inception through December 2014.

Estimates of biological age based on DNA methylation patterns, often referred to as "epigenetic age", "DNAm age", have been shown to be robust biomarkers of age in humans. We previously demonstrated that independent of chronological age, epigenetic age assessed in blood predicted all-cause mortality in four human cohorts. Here, we expanded our original observation to 13 different cohorts for a total sample size of 13,089 individuals, including three racial/ethnic groups. In addition, we examined whether incorporating information on blood cell composition into the epigenetic age metrics improves their predictive power for mortality. Lifting greater loads during weightlifting exercises may theoretically be achieved through increasing the magnitudes of net joint impulses or manipulating the joints’ effective mechanical advantage (EMA). The purpose of this study was to investigate muscle-specific EMA and joint impulse as well as impulse-momentum characteristics of the lifter-barbell system across a range of external loads during the execution of the clean. Headache is a common symptom of post-concussion syndrome which may persist in a substantial portion of patients contributing to a poor quality of life and disability. Background
Countdown to 2015 (Countdown) supported countries to produce case studies that examine how and why progress was made toward the Millennium Development Goals (MDGs) 4 and 5. Analysing how health-financing data explains improvements in RMNCH outcomes was one of the components to the case studies.
Methods
This paper presents a descriptive analysis on health financing from six Countdown case studies (Afghanistan, Ethiopia, Malawi, Pakistan, Peru, and Tanzania), supplemented by additional data from global databases and country reports on macroeconomic, health financing, demographic, and RMNCH outcome data as needed.
