วัตถุประสงค์: เพื่อพัฒนารูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 สำหรับกลุ่มเสี่ยงก่อนเบาหวานในชุมชน จังหวัดเชียงราย และเพื่อเปรียบเทียบพฤติกรรมการจัดการตนเอง ดัชนีมวลกาย ระดับน้ำตาลในเลือดหลังอดอาหาร ของกลุ่มเสี่ยงเบาหวาน ระหว่างกลุ่มทดลองและกลุ่มควบคุม\n\nรูปแบบการวิจัย: การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม (participatory action research) และการวิจัยแบบกึ่งทดลองแบบ pretest-posttest control group design\n\nวิธีดำเนินการวิจัย: การศึกษาแบ่งออกเป็น 2 ระยะ ระยะที่ 1 การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมเพื่อวิเคราะห์สถานการณ์การดูแลกลุ่มเสี่ยงเบาหวานในชุมชน ผู้ร่วมวิจัย ประกอบด้วย กลุ่มเสี่ยงเบาหวาน 30 คน กลุ่มบุคลากรทางด้านสุขภาพ 10 คน และกลุ่มตัวแทนจากชุมชน 10 คน เก็บรวบรวมข้อมูลด้วยการสนทนากลุ่ม และการสัมภาษณ์แบบเจาะลึก เพื่อพัฒนาเป็นรูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ระยะที่ 2 เป็นการศึกษาผลของรูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ต่อพฤติกรรมการจัดการตนเองและดัชนีมวลกาย ในกลุ่มเสี่ยงเบาหวาน จำนวน 60 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 30 คน โดยกลุ่มทดลองได้รับการดูแลตามรูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ในกลุ่มเสี่ยงเบาหวาน และกลุ่มควบคุมได้รับการพยาบาลตามมาตรฐาน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติที\n\nผลการวิจัย: รูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ประกอบด้วย 3 กิจกรรมหลัก ได้แก่ 1) กิจกรรมการคัดกรองค้นหากลุ่มเสี่ยงเบาหวานและกิจกรรมให้ความรู้ 2) กิจกรรมการจัดการตนเอง และ 3) กิจกรรมการเยี่ยมบ้าน ซึ่งภายหลังได้รับรูปแบบการป้องกันการเกิดโรคเบาหวาน กลุ่มทดลองมีคะแนนพฤติกรรมการจัดการตนเองดีขึ้นกว่าก่อนการทดลองและดีกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 มีดัชนีมวลกายน้อยกว่าก่อนการทดลองและน้อยกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และมีระดับน้ำตาลในเลือดหลังอดอาหารต่ำกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 แต่ไม่แตกต่างจากควบคุม\n\nสรุป: ผลการวิจัยสามารถนำไปใช้เป็นแนวทางในการจัดบริการเพื่อป้องกันการเกิดโรคเบาหวานชนิดที่ 2 สำหรับกลุ่มเสี่ยงเบาหวานในชุมชน โดยการมีส่วนร่วมของครอบครัว สถานบริการสุขภาพ และชุมชน\n

Single joint kinematic alterations have been identified during gait in those with chronic ankle instability (CAI). The purpose of this study was to compare sagittal plane hip, knee, and ankle kinematics during walking in participants with and without CAI. Twelve individuals with CAI and 12 healthy individuals walked on a treadmill at 1.5 m/s. Three-dimensional kinematics were analyzed using mean ensemble curves and independent t tests. Participants with CAI demonstrated less lower extremity flexion during the absorption phase of stance and the limb placement phase of swing, which may have implications for limb placement at initial contact.

BACKGROUND: We aimed to compare the diagnostic accuracy of the Alere Triage Cardio3 Tropinin I (TnI) assay (Alere, Inc., USA) and the PathFast cTnI-II (Mitsubishi Chemical Medience Corporation, Japan) against the central laboratory assay Singulex Erenna TnI assay (Singulex, USA).

METHODS: Using the Markers in the Diagnosis of Acute Coronary Syndromes (MIDAS) study population, we evaluated the ability of three different assays to identify patients with acute myocardial infarction (AMI). Fluorescence guided surgery has the potential to positively impact surgical oncology; current operating microscopes and stand-alone imaging systems are too insensitive or too cumbersome to maximally take advantage of new tumor-specific agents developed through the microdose pathway. To this end, a custom-built illumination and imaging module enabling picomolar-sensitive near-infrared fluorescence imaging on a commercial operating microscope is described. The limits of detection and system specifications are characterized, and in vivo efficacy of the system in detecting ABY-029 is evaluated in a rat orthotopic glioma model following microdose injections, showing the suitability of the device for microdose phase 0 clinical trials.

เหตุผลของการทำวิจัย : มีการศึกษามากมายที่พบว่าภาวะแทรกซ้อนทางสูติศาสตร์ของหญิงตั้งครรภ์มีผลกระทบต่อภาวะซึมเศร้าหลังคลอดอย่างไรก็ตามการศึกษาภาวะซึมเศร้าหลังคลอดในมารดาที่มีภาวะแทรกซ้อนทางสูติศาสตร์ในประเทศไทยยังไม่ได้มีการศึกษาวัตถุประสงค์ : เพื่อศึกษาความชุก ตลอดจนปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าหลังคลอดในมารดาที่มีภาวะแทรกซ้อนทางสูติศาสตร์ ที่เข้ารับบริการในแผนกหลังคลอด โรงพยาบาลจุฬาลงกรณ์รูปแบบการวิจัย : การศึกษาวิจัยเชิงพรรณนา ณ เวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : โรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการศึกษา : เก็บรวบรวมข้อมูลจากผู้ป่วยที่มารับบริการที่แผนกหลังคลอด จำนวน116 คน ในช่วง 1 สัปดาห์หลังคลอด และ 4 - 6 สัปดาห์หลังคลอดโดยใช้แบบสอบถามแบบตอบด้วยตนเอง ประกอบด้วย 1) แบบสอบถามข้อมูลส่วนบุคคลทั่วไป 2) ข้อมูลเกี่ยวกับการตั้งครรภ์และการคลอด3) แบบวัดภาวะซึมเศร้า Edinburgh Postnatal Depression Scale(EPDS) 4) แบบสอบถามแรงสนับสนุนทางสังคม Personal ResourceQuestionnaire (PRQ-part II) 5) แบบประเมินสัมพันธภาพระหว่างคู่สมรส Dyadic Adjustment scale (DAS) 6) แบบสอบถามข้อมูลหลังคลอด โดยใช้ Univariate Analysis ได้แก่ t-Test, Chi-squareเพื่อหาปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าหลังคลอดในมารดาที่มีภาวะแทรกซ้อนทางสูติศาสตร์ผลการศึกษา : มารดาหลังคลอดในช่วง 1 สัปดาห์หลังคลอด มีภาวะซึมเศร้าร้อยละ13.7 โดยมีปัจจัยที่สัมพันธ์กับภาวะซึมเศร้าหลังคลอด ได้แก่ เวลาเฉลี่ยที่ทารกเข้านอน, ค่าดัชนีมวลกายของมารดาก่อนตั้งครรภ์,ค่าดัชนีมวลกายของมารดาในปัจจุบัน, สัมพันธภาพระหว่างคู่สมรสที่ไม่ดี และการได้รับการสนับสนุนทางสังคมที่น้อย ส่วนมารดาหลังคลอดในช่วง 4 - 6 สัปดาห์หลังคลอด มีภาวะซึมเศร้าร้อยละ 6.1โดยมีปัจจัยที่สัมพันธ์กับภาวะซึมเศร้าหลังคลอด ได้แก่ ระดับการศึกษา, รายได้, การมีประจำเดือนล่าช้า, การมีระยะเวลาในการคลอดที่ยาวนาน, การตื่นเป็นเวลานานในช่วงกลางคืนของทารก, สัมพันธภาพระหว่างคู่สมรสที่ไม่ดี และการได้รับการสนับสนุนจากสังคมน้อยสรุป : ความชุกของภาวะซึมเศร้าหลังคลอดลดลงตามระยะเวลาหลังคลอดที่1 และ 4 - 6 สัปดาห์ โดยความชุกของภาวะซึมเศร้าในการศึกษานี้มีความใกล้เคียงกับการศึกษาอื่น ๆ และมีปัจจัยที่สัมพันธ์กับภาวะซึมเศร้าหลังคลอดหลายปัจจัย การทราบปัจจัยที่เกี่ยวข้องจะช่วยให้บุคลากรทางการแพทย์สามารถวางแผนการดูแล และให้ความช่วยเหลือได้อย่างเหมาะสมต่อไป.

เหตุผลของการทำวิจัย : มารดาที่บุตรป่วยด้วยโรคปอดเรื้อรัง ที่ต้องใช้ความพยายามและทุ่มเทในการดูแล เนื่องจากผู้ป่วยสามารถมีปัญหาที่เกิดขึ้นได้หลายระบบในร่างกาย ผู้วิจัยเห็นว่าการวิจัยเชิงคุณภาพจะทำให้รู้ถึงอารมณ์ การรับรู้ความรู้สึกนึกคิดของมารดา เพื่อนำผลวิจัยที่ได้มาช่วยรักษา เยียวยาฟื้นฟูจิตใจของมารดาให้คลายความทุกข์วัตถุประสงค์ : เพื่อศึกษาภาวะจิตสังคมในมารดาเด็กโรคปอดเรื้อรังรูปแบบการวิจัย : การศึกษานี้เป็นการวิจัยเชิงคุณภาพสถานที่ทำการศึกษา : หอผู้ป่วยใน แผนกกุมารเวชศาสตร์ โรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการ : ประชากรที่ศึกษา คือ มารดาเด็กโรคปอดเรื้อรัง ที่บุตรรักษาตัวในโรงพยาบาลจุฬาลงกรณ์ เป็นการวิจัยเชิงคุณภาพ (Qualitative research)โดยใช้วิธีการสัมภาษณ์ส่วนบุคคลเชิงลึก (In-depth interview) ผู้ให้ข้อมูลคือ มารดาของเด็กโรคปอดเรื้อรังจำนวน 10 ราย ภาวะจิตสังคมในมารดาเด็กโรคปอดเรื้อรัง ประกอบด้วย 3 ด้าน คือ ผลกระทบที่เกิดขึ้นการมองเห็นจุดแข็ง ข้อดีของตัวเอง และแหล่งสนับสนุนเมื่อบุตรป่วยตั้งแต่รู้ว่าบุตรป่วย จนถึงการรักษา และการดูแลบุตรที่เป็นโรคปอดเรื้อรังผลการศึกษา : จากการศึกษามารดาพบว่าผลกระทบที่เกิดขึ้นกับมารดา ประกอบด้วย1) ด้านร่างกาย 2) ด้านจิตใจ ตั้งแต่รับรู้ว่าบุตรป่วย การตัดสินใจในการให้เจาะคอเพื่อใส่ท่อหลอดคอเทียม, การดูแลบุตร 3) ด้านสังคม4) ด้านเศรษฐกิจการมองข้อดี และจุดแข็งของตัวเองมารดาส่วนใหญ่มารดามองว่าตัวเองมีข้อดี คือ บุตรยังอยู่กับตน ถึงแม้ร่างกายจะไม่แข็งแรงแหล่งสนับสนุนทางสังคมความช่วยเหลือ การสนับสนุนจากครอบครัวและบุคคลใกล้ชิด 1) มีบุคคลที่เป็นที่พึ่งทางใจ 2) มีคนในครอบครัวช่วยดูแลบุตรกำลังใจจากคนรอบข้างตั้งแต่ที่รู้ว่าบุตรป่วยจนถึงกระบวนการรักษา นอกจากนี้มารดายังมีความต้องการด้านต่าง ๆ1) การดูแลบุตร 2) การเข้าใจในความรู้สึก 3) ข้อมูลข่าวสารสรุป : ผลการศึกษา สามารถทำให้เข้าใจปัญหา และผลกระทบที่เกิดขึ้นกับมารดาได้ลึกซึ้งยิ่งขึ้น ภาวะจิตสังคมของมารดาเด็กโรคปอดเรื้อรังด้านร่างกาย จิตใจ และด้านอื่น ๆ สามารถเกิดขึ้นได้ตลอดระยะเวลาตั้งแต่รับรู้ว่าบุตรป่วย มาถึงการรักษา โดยมารดาส่วนใหญ่เรียนรู้การที่จะปรับตัวได้ โดยมองผ่านข้อดีของตนเอง และแหล่งสนับสนุนด้านต่าง ๆที่ตนเองได้รับจากครอบครัว บุคคลใกล้ชิดและทีมสุขภาพ.

เหตุผลของการวิจัย : คุณภาพการดูแลถือว่าเป็นเป้าหมายหลักในการให้บริการสุขภาพโดยให้ความสำคัญต่อมุมมองการรับรู้ และประสบการณ์ของผู้ป่วยต่อคุณภาพการดูแลที่ได้รับ ซึ่งมีหลายปัจจัยที่เกี่ยวข้องได้แก่ความต้องการมีส่วนร่วมในการจัดการอาการ การรับรู้ความรุนแรงของอาการเพศ จำนวนครั้งเข้ารับการรักษาวัตถุประสงค์ : ศึกษาความสัมพันธ์ระหว่างความต้องการมีส่วนร่วมในการจัดการอาการ การรับรู้ความรุนแรงของอาการ เพศ จำนวนครั้งเข้ารับการรักษา และการรับรู้คุณภาพการดูแลของผู้ป่วยมะเร็งระหว่างเข้ารับการรักษาในโรงพยาบาลรูปแบบการวิจัย : การศึกษาความสัมพันธ์เชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : หอผู้ป่วยมะเร็งก้อนทูมวิทยาและมะเร็งโลหิตวิทยาของโรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการศึกษา : ศึกษาในผู้ป่วยมะเร็งจำนวน 97 ราย ใช้วิธีเลือกกลุ่มตัวอย่างแบบเจาะจงรวบรวมข้อมูลโดยใช้ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความต้องการมีส่วนร่วมในการจัดการอาการ แบบประเมินอาการที่พบบ่อยในโรคมะเร็ง 10 อาการและแบบประเมินคุณภาพการดูแลในมุมมองของผู้ป่วยผลการศึกษา : กลุ่มตัวอย่างส่วนใหญ่เป็นเพศชาย ร้อยละ 61.9 ความต้องการมีส่วนร่วมในการจัดการอาการอยู่ในระดับ passive มากที่สุด(ร้อยละ 47.4) การรับรู้คุณภาพการดูแลของกลุ่มตัวอย่างอยู่ในระดับสูงตรงกับความต้องการมากที่สุด (ร้อยละ 43.3) ผลวิเคราะห์ความสัมพันธ์พบว่า การรับรู้ความรุนแรงของอาการและจำนวนครั้งเข้ารับการรักษา มีความสัมพันธ์กับการรับรู้คุณภาพการดูแลอย่างมีนัยสำคัญทางสถิติ (χ2 = 10.50, P =.03 ; χ2 = 6.79, P =.009 ) สำหรับเพศและความต้องการมีส่วนร่วมในการจัดการอาการไม่มีความสัมพันธ์กับการรับรู้คุณภาพการดูแลอย่างมีนัยสำคัญทางสถิติ(rpb= .92 และ χ2 = 8.32, P =.08) ตามลำดับสรุปผลการวิจัย : พยาบาลควรเน้นการดูแลจัดการเพี่อลดความรุนแรงของอาการในผู้ป่วยมะเร็งขณะเข้ารักษาตัวในโรงพยาบาล โดยเฉพาะกลุ่มผู้ป่วยที่เข้ารับการรักษาบ่อยครั้งซึ่งอาจมีการรับรู้คุณภาพการดูแลลดลง.

เหตุผลของการทำวิจัย : ประชากรกลุ่มมิลเลนเนียล คือกลุ่มประชากรที่เกิดช่วง พ.ศ. 2523ถึง พ.ศ. 2543 ปัจจุบันทั้งในประเทศไทยและต่างประเทศพบสัดส่วนของประชากรกลุ่มดังกล่าว ในที่ทำงานต่าง ๆ เพิ่มมากขึ้นทุกปีการศึกษาในปัจจุบันพบว่ากลุ่มมิลเลนเนียล มีความสนใจและให้ความสำคัญกับเรื่อง สมดุลงานและชีวิตซึ่งเป็นประเด็นที่มีผลกระทบโดยตรงต่อสุขภาพวัตถุประสงค์ : เพื่อประเมินระดับระดับสมดุลงานและชีวิต และศึกษาความสัมพันธ์ระหว่างระดับสมดุลงานและชีวิตและปัจจัยที่เกี่ยวข้อง ในพนักงานวัยทำงานกลุ่มมิลเลนเนียลรูปแบบการวิจัย : การวิจัยแบบภาคตัดขวางสถานที่ทำการศึกษา : บริษัทธุรกิจพลังงานแห่งหนึ่ง มีที่ทำการ 21 จังหวัดในประเทศไทยตัวอย่างและวิธีการศึกษา : ทำการเก็บข้อมูลพนักงานกลุ่มมิลเลนเนียลทั้งหมด 1,951 คนโดยไม่มีการสุ่มตัวอย่าง ณ บริษัทแห่งหนึ่ง โดยใช้แบบสอบถามประกอบด้วย 2 ส่วน คือ ส่วนข้อมูลปัจจัยส่วนบุคคล และส่วนแบบประเมินสมดุลงานและชีวิต สถิติที่ใช้วิเคราะห์ ประกอบด้วยสถิติเชิงพรรณนา Independent t-test, One-way ANOVA, Pearson’scorrelation และ Multiple linear regressionผลการศึกษา : พนักงานวัยทำงานกลุ่มมิลเลนเนียล ในกลุ่มที่ทำการศึกษา ร้อยละสมดุลงานและชีวิตเฉลี่ย 60.98 คะแนน (SD = 16.0) ปัจจัยที่มีความสัมพันธ์ที่ดีต่อสมดุลงานและชีวิตประกอบด้วยการออกกำลังกาย การนอนหลับ และ ระดับความพึงพอใจต่องานในประเด็นเรื่อง ระยะเวลาการพักผ่อนและทีมงานขณะที่การสูบบุหรี่ ชั่วโมงการทำงานต่อสัปดาห์ และอายุงานในบริษัท เป็นปัจจัยที่มีความสัมพันธ์ที่ไม่ดีต่อสมดุลงานและชีวิตสรุป : ผลการศึกษาแสดงให้เห็นถึงความสัมพันธ์ระหว่างสมดุลงานและชีวิต กับปัจจัยต่าง ๆ ที่เกี่ยวข้อง ซึ่งจะเป็นประโยชน์ในการศึกษาและวางแผนนโยบายที่ส่งเสริมการมีสมดุลงานและชีวิตที่ดีของพนักงานวัยทำงานกลุ่มมิลเลนเนียลต่อไป.

Background : Bipolar disorder is a severe, chronic relapsing disorder with high morbidity. A valid screening tool of this disorder is useful for early detection and treatment to reduce the morbidity of the patients. Unfortunately, there is no screening questionnaire for bipolar disorder in Thai language. Objective : To identify imaging features of CT scan for type 1 and type 2 papillary renal cell carcinomas (pRCCs); and, to define the findings that can be used to differentiate between these two subtypes.Materials and Methods : Nineteen pRCC tumors recruited into this study were classified as pathology type 1 or type 2. Staged Operation For Large Malignant Proliferating Trichilemmal Tumor Of The Scalp: A Case Report, Kraisri Chantra, Apichai Angspatt, Supparerk Prichayudh

The authors report a case of large malignant proliferating trichilemmal tumor of thescalp with calvarial destruction undergoing a successful staged operation to completelyexcise the tumor. The reconstruction of the scalp was delayed due to considerable amountof intraoperative blood loss and the negative pressure wound therapy was applied to thewound. The reconstruction with a free an anticoagulant thigh flap was subsequently performed5 days later. Five weeks postoperatively, the patient had transient visual impairment fromdural venous sinus thrombosis that was successfully treated with anticoagulation. On 1 yearfollow up, there was no evidence of tumor recurrence.

Abstract

OBJECTIVE:

To determine morbidity after colostomy reversal at a tertiary care hospital.

METHODS:

The retrospective case series was conducted at the Aga Khan University Hospital, Karachi, and comprised records of patients aged 15 years or more who underwent colostomy reversal from January 2003 to December 2011. Data was collected regarding demographics, procedure dates, indication, as well as type and location of colostomy. Details of colostomy reversal, including pre-operative, intra-operative and post-operative variables were recorded.

Objectives: This study was undertaken to compare the effects of IH with standard allopathic (SA) treatment. Methods: A randomized controlled, open-label, comparative trial, was conducted, in which alcohol dependents were screened verbally using the CAGE scale. The participants 80 patients fulfilling the inclusion criteria were randomized either IH (n=40) or SA (n=40) and treated cum followed up for 12 months. Objectives: To investigate the effect of individualized homoeopathic medicines in improving ADL by reducing pain, stiffness and limiting the disease progress. Materials and Methods: 131 consecutive patients with OA of knee were recruited and followed up for minimum period of twelve months. Two orthopaedic surgeons diagnosed the disease based on clinical examination of the patients. Three trained homoeopathic physicians prescribed individualized homoeopathic simillimum and the patients were evaluated for pain on WOMAC Osteoarthritis Index LK3.1 (IK) survey form measuring pain, stiffness and ADL. This paper describes principles of inclusivity across 11 primarily undergraduate institutions designated as Capstone Awardees in Howard Hughes Medical Institute’s (HHMI) 2012 competition. The Capstones represent a range of institutional missions, student profiles, and geographical locations. Despite the importance of early detection to signal lifesaving treatment initiation for HIV+ infants, early infant diagnosis (EID) services have received considerably less attention than other aspects of prevention of mother to child transmission care. This study draws on baseline data from an on-going cluster randomized study of an intervention to improve EID services at six government hospitals across Kenya. Two logistic regressions examined potential predictors of "on time" (infant ≤6 weeks of age) vs. "late" (≥7 weeks) and "on time" versus "very late" (≥12 weeks) EID engagement among 756 mother-infant pairs. HIV and poverty are inextricably intertwined in sub-Saharan Africa. Economic and livelihood intervention strategies have been suggested to help mitigate the adverse economic effects of HIV, but few intervention studies have focused specifically on HIV positive persons. We conducted three pilot studies to assess a livelihood intervention consisting of an initial orientation and loan package of chickens and associated implements to create poultry microenterprises. We enrolled 15 HIV-positive and 22 HIV-negative participants and followed them for up to 18 months. Over the course of follow-up, participants achieved high chicken survival and loan repayment rates.
