Background

A hallmark of Parkinson’s disease (PD) is a mismatch between the perceived effort and actual forces exerted during functional activities such as speech. Current evidence supports therapy to help reset this perception of effort, but the neurological underpinnings of such treatments are unclear. This study examined brain activity during tongue movements performed at varying levels of effort to determine (1) which brain areas are involved in each task, and (2) which areas, if any, scale in activation according to effort level. Epigenetic mechanisms, including DNA methylation, mediate the interaction between gene and environment and may play an important role in the obesity epidemic. We assessed the relationship between DNA methylation and obesity in peripheral blood mononuclear cells (PBMCs) at 485,000 CpG sites across the genome in family members (8-90 y of age) using a discovery cohort (192 individuals) and a validation cohort (1,052 individuals) of Northern European ancestry. Background

Health care providers are better able to diagnose depression and initiate treatment when patients disclose symptoms. However, many women are reluctant to disclose depressive symptoms. Little is known about the experience of disclosing depression symptoms in primary care among racially and ethnically diverse women across the life course. We qualitatively explore experiences of disclosure of depressive symptoms to primary care providers among self-identified African American, Hispanic and non-Hispanic White women.

Methods

Twenty-four women with depression were recruited for open-ended interviews. We recorded, transcribed, and coded interviews using inductive content analysis.

This paper addresses the need for better communication skills between physicians and patients. Several communication barriers and communication traps can be overcome with increased education. Trust is critical in a relationship. This paper explores ways to increase trust between physicians and patients by using public relations as a tool to provide awareness of the problem and to build a partnership to improve adherence.

The MR exam revealed multiple intracranialextra-axial masses;isointense on T1W sequence and hypointenseon T1W, T2W, FLAIR, DWI sequence and ADC (b value= 1000 sec/mm2) with avid post contrast enhancement. Diagnosing chronic inflammatory polyradiculopathy in patients with pre-existing diabetic sensorimotor polyneuropathy is a diagnostic challenge. We present a case of a 69 years old who presented with weakness of legs for two months,he was diagnosed as having CIDP on the background of diabetic sensorimotor polyneuropathy, further and extensive workup revealed the final diagnosis of nonsecretory myeloma. Mounting evidence suggests that dietary polyphenol intake can reduce the detrimental effects of various disease processes, and pomegranates have frequently been examined because of their particularly high polyphenol content. This study compared tooth length measurements made on cone beam computed tomography (CBCT) scans and 3-Tesla (3T) magnetic resonance (MR) scans performed with and without an alginate bite registration surrounding the crowns of the teeth. Materials and Methods. One CBCT scan, one MR scan with alginate bite registration, and one MR scan without bite registration were performed on 12 subjects. BACKGROUND: Effective pain management after anterior cruciate ligament (ACL) reconstruction improves patient satisfaction and function.

PURPOSE: To collect and evaluate the available evidence from randomized controlled trials (RCTs) on pain control after ACL reconstruction.

STUDY DESIGN: Systematic review.

METHODS: A systematic literature review was performed using PubMed, Medline, Google Scholar, UpToDate, Cochrane Reviews, CINAHL, and Scopus following PRISMA guidelines (July 2014). Only RCTs comparing a method of postoperative pain control to another method or placebo were included.

OBJECTIVES:

The study aimed to identify the effects of maternal tobacco consumption during pregnancy and other factors on birth outcomes and obstetric complications in Karachi, Pakistan.

DESIGN:

A multicentre hospital-based case-control study.

SETTING:

Four leading maternity hospitals of Karachi.

PARTICIPANTS:

A random sample of 1275 women coming to the gynaecology and obstetric department of selected hospitals for delivery was interviewed within 48 hours of delivery from wards. Cases were women with adverse birth outcomes and obstetric complications, while controls were women who had normal uncomplicated delivery.

การกลับเป็นซ้ำเป็นปัญหาที่พบได้มากในผู้ป่วยจิตเภทเรื้อรังแม้ว่าผู้ป่วยจิตเภทจะได้รับการรักษาจนอาการทางจิตสงบแต่ก็ยังเกิดการกลับเป็นซ้ำได้ สถานการณ์การกลับเป็นซ้ำของผู้ป่วยจิตเภทเรื้อรังมีแนวโน้มจะเพิ่มขึ้นเรื่อยๆ การเกิดการกลับเป็นซ้ำในผู้ป่วยจิตเภทเป็นปัญหาที่สำคัญทำให้เกิดความผิดปกติของสมองและเกิดอาการทางจิตรุนแรงขึ้นได้ง่าย ส่งผลต่อการตอบสนองต่อการรักษาและการฟื้นหาย ก่อให้เกิดพฤติกรรมรุนแรงและการฆ่าตัวตายในผู้ป่วยได้ นอกจากนี้ยังส่งผลกระทบต่อครอบครัวโดยเฉพาะอย่างยิ่งความรู้สึกเป็นภาระการดูแลของครอบครัวในการดูแลผู้ป่วย รวมทั้งยังทำให้เกิดการสูญเสียค่าใช้จ่ายในการรักษาที่นับเป็นมูลค่ามหาศาลในแต่ละปีของประเทศ ดังนั้นการเรียนรู้สาเหตุและแนวทางการป้องกันการกลับเป็นซ้ำในผู้ป่วยจิตเวชเรื้อรังซึ่งประกอบด้วย การประเมินความรู้และความเข้าใจเกี่ยวกับการกลับเป็นซ้ำ การตรวจสอบอาการเตือนก่อนการกลับเป็นซ้ำ การพัฒนาแผนการปฏิบัติการป้องกันการกลับเป็นซ้ำ การเตรียมการเพื่อปฏิบัติตามแผนการป้องกันการกลับเป็นซ้ำ และการบูรณาการแผนปฏิบัติการป้องกันการกลับเป็นซ้ำสู่ครอบครัวและชุมชน ซึ่งแผนปฏิบัติการในการป้องกันการกลับเป็นซ้ำดังกล่าวจะส่งผลให้บุคลากรทางสุขภาพและผู้ดูแลมีสามารถในการดูแลผู้ป่วยจิตเภทเรื้อรังได้อย่างมีประสิทธิภาพ โดยเฉพาะอย่างยิ่งพยาบาลซึ่งเป็นผู้ที่มีบทบาทสำคัญในการป้องกันและลดการกลับเป็นซ้ำในผู้ป่วยจิตเภท รวมทั้งการดูแลและฟื้นฟูสภาพของผู้ป่วยจึงต้องมีความรู้และเข้าใจในสถานการณ์และแนวทางการป้องกันการกลับเป็นซ้ำของผู้ป่วยจิตเภทเรื้อรังเพื่อให้ผู้ป่วยได้รับการดูแลที่มุ่งเน้นความต้องการของผู้ป่วยส่งผลต่อประสิทธิผลในการป้องกันการกลับเป็นซ้ำนำไปสู่การมีคุณภาพชีวิตที่ดีของผู้ป่วยจิตเวชเรื้อรังต่อไป

วัตถุประสงค์: เพื่อศึกษาประสบการณ์และวิธีการจัดการความมั่นคงทางอาหารของหญิงตั้งครรภ์วัยรุ่นในกรุงเทพมหานคร\n\nรูปแบบการวิจัย: การวิจัยเชิงคุณภาพแบบบรรยาย\n\nวิธีดำเนินการวิจัย: ผู้ให้ข้อมูล คือ หญิงตั้งครรภ์วัยรุ่นที่มารับบริการฝากครรภ์ที่ศูนย์บริการสาธารณสุขแห่งหนึ่ง จำนวน 12 คน อายุ 16-19 ปี อายุครรภ์ 8-20 สัปดาห์ เก็บข้อมูลโดยใช้แบบสอบถามข้อมูลส่วนบุคคล การสัมภาษณ์เชิงลึก การสังเกตและบันทึกภาคสนาม แบบบันทึกอาหารที่รับประทานในระยะเวลา 3 วัน แบบบันทึกภาวะโภชนาการและภาวะสุขภาพของหญิงตั้งครรภ์วัยรุ่น วิเคราะห์ข้อมูลส่วนบุคคลโดยใช้สถิติบรรยาย วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เนื้อหา วิเคราะห์พลังงานและสารอาหารโดยใช้โปรแกรม INMUCAL-Nutrients V.3\n\nผลการวิจัย: พบว่า ประสบการณ์ความมั่นคงทางอาหารของผู้ให้ข้อมูลมี 4 ประเด็นหลัก คือ ด้านคุณภาพและความเชื่อของอาหาร ปริมาณอาหาร ความปลอดภัยของอาหาร ความกังวลเรื่องอาหารและวิธีการจัดการความมั่นคงทางอาหารมี 4 ประเด็นหลัก คือ การจัดการด้านอาหาร ด้านการเงิน ด้านสุขภาพ และการจัดการโดยใช้แหล่งสนับสนุนทางสังคม ส่วนภาวะโภชนาการ จากการบันทึกอาหารที่รับประทานของผู้ให้ข้อมูล 11 คน พบว่าจำนวน 9 คนได้รับพลังงานไม่เพียงพอกับความต้องการของร่างกาย และทุกคนได้รับสารอาหารบางชนิดไม่เพียงพอกับความต้องการของร่างกายคือ โปรตีน แคลเซียม ธาตุเหล็กและ วิตามิน ผู้ให้ข้อมูล 4 คน มีน้ำหนักเพิ่มขึ้นน้อยกว่าเกณฑ์มาตรฐาน ส่วนภาวะสุขภาพพบว่า ผู้ให้ข้อมูล 5 คน มีภาวะโลหิตจาง ทั้งหมดไม่พบภาวะความดันโลหิตสูงขณะตั้งครรภ์\n\nสรุป:ผลการศึกษาครั้งนี้ใช้เป็นแนวทางสำหรับทีมสุขภาพในการส่งเสริมความมั่นคงทางอาหาร รวมทั้งการสร้างเครื่องมือเพื่อคัดกรองความมั่นคงทางอาหารของหญิงตั้งครรภ์วัยรุ่น เพื่อให้สามารถวางแผนและให้การดูแลหญิงตั้งครรภ์วัยรุ่นได้อย่างมีประสิทธิภาพ

วัตถุประสงค์: เพื่อเปรียบเทียบระดับความวิตกกังวลของผู้ดูแลในครอบครัวของผู้ป่วยผ่าตัดเนื้องอกสมองระหว่างกลุ่มเปรียบเทียบและกลุ่มทดลองหลังได้รับโปรแกรมทันทีและหลังได้รับโปรแกรม 1 วัน \n\nแบบแผนงานวิจัย: การวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่างในการวิจัย คือ ผู้ดูแลในครอบครัวที่มารอผู้ป่วยทำผ่าตัดเนื้องอกสมอง หน่วยงานห้องผ่าตัด สถาบันประสาทวิทยา จำนวน 40 คน กลุ่มตัวอย่าง 20 คนแรกจัดเข้าเป็นกลุ่มเปรียบเทียบ 20 คนหลังจัดเป็นกลุ่มทดลอง กลุ่มเปรียบเทียบได้รับโปรแกรมการให้ข้อมูลขณะผ่าตัด ในขณะที่กลุ่มทดลองได้รับโปรแกรมการให้ข้อมูลร่วมกับสุคนธบำบัด เครื่องมือที่ใช้ในการทดลอง คือ โปรแกรมการให้ข้อมูลร่วมกับสุคนธบำบัด เก็บรวบรวมข้อมูลโดยใช้แบบบันทึกข้อมูลส่วนบุคคล และแบบวัดความวิตกกังวล มีค่าสัมประสิทธิ์แอลฟาครอนบาค เท่ากับ .86 วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที \n\nผลการวิจัย: 1) ค่าเฉลี่ยคะแนนความวิตกกังวลภายหลังได้รับโปรแกรมทันทีของกลุ่มทดลองน้อยกว่ากลุ่มเปรียบเทียบ อย่างมีนัยสำคัญทางสถิติ (p< .05) 2) ค่าเฉลี่ยคะแนนความวิตกกังวลภายหลังได้รับโปรแกรม 1 วัน ของกลุ่มทดลองและกลุ่มเปรียบเทียบไม่แตกต่างกัน (p > .05)\n\nสรุป: โปรแกรมการให้ข้อมูลร่วมกับสุคนธบำบัดสามารถลดความวิตกกังวลของผู้ดูแลในครอบครัวขณะรอผู้ป่วยทำผ่าตัดเนื้องอกสมองหลังได้รับโปรแกรมทันที แต่ไม่สามารถลดความวิตกกังวลได้ในระยะหลังผ่าตัด\n\n

วัตถุประสงค์: เพื่อศึกษาสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสังกัดกระทรวงกลาโหม\n \nแบบแผนงานวิจัย: การวิจัยแบบบรรยาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ หัวหน้าหอผู้ป่วยทุกคนที่มีประสบการณ์เป็นหัวหน้าหอผู้ป่วยตั้งแต่ 1 ปีขึ้นไป จำนวน 182 คน ในโรงพยาบาลสังกัดกระทรวงกลาโหม จำนวน 40 แห่ง โดยศึกษาในประชากรทั้งหมด เครื่องมือที่ใช้ในการเก็บข้อมูล คือ แบบสอบถามสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสังกัดกระทรวงกลาโหม เป็นแบบมาตราส่วนประมาณค่า 5 ระดับ ผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 คน หาค่าความเที่ยงด้วยวิธีสัมประสิทธิ์อัลฟาของครอนบาค ได้ค่าเท่ากับ .97 วิเคราะห์ตัวประกอบด้วยวิธีสกัดตัวประกอบหลัก และหมุนแกนตัวประกอบแบบออโธโกนอลด้วยวิธีแวริแมกซ์ \n\nผลการวิจัย: ตัวประกอบสมรรถนะหัวหน้าหอผู้ป่วย โรงพยาบาลสังกัดกระทรวงกลาโหมมีทั้งหมด 10 ด้าน ประกอบด้วย 66 สมรรถนะ ดังนี้ 1) ด้านคุณลักษณะพยาบาลทหาร 2) ด้านการบริหารจัดการ 3) ด้านการปฏิบัติการพยาบาล 4) ด้านการใช้เทคโนโลยีสารสนเทศ 5) ด้านการควบคุมตนเองและเข้าใจผู้อื่น 6) ด้านคุณธรรมและจริยธรรม 7) ด้านการบริหารงานคุณภาพ 8) ด้านการแก้ปัญหา 9) ด้านการวิจัย และ 10) ด้านการประสานงาน โดยมีค่าความแปรปรวนรวมกันร้อยละ 62.04\n\n

วัตถุประสงค์: เพื่อพัฒนารูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 สำหรับกลุ่มเสี่ยงก่อนเบาหวานในชุมชน จังหวัดเชียงราย และเพื่อเปรียบเทียบพฤติกรรมการจัดการตนเอง ดัชนีมวลกาย ระดับน้ำตาลในเลือดหลังอดอาหาร ของกลุ่มเสี่ยงเบาหวาน ระหว่างกลุ่มทดลองและกลุ่มควบคุม\n\nรูปแบบการวิจัย: การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม (participatory action research) และการวิจัยแบบกึ่งทดลองแบบ pretest-posttest control group design\n\nวิธีดำเนินการวิจัย: การศึกษาแบ่งออกเป็น 2 ระยะ ระยะที่ 1 การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมเพื่อวิเคราะห์สถานการณ์การดูแลกลุ่มเสี่ยงเบาหวานในชุมชน ผู้ร่วมวิจัย ประกอบด้วย กลุ่มเสี่ยงเบาหวาน 30 คน กลุ่มบุคลากรทางด้านสุขภาพ 10 คน และกลุ่มตัวแทนจากชุมชน 10 คน เก็บรวบรวมข้อมูลด้วยการสนทนากลุ่ม และการสัมภาษณ์แบบเจาะลึก เพื่อพัฒนาเป็นรูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ระยะที่ 2 เป็นการศึกษาผลของรูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ต่อพฤติกรรมการจัดการตนเองและดัชนีมวลกาย ในกลุ่มเสี่ยงเบาหวาน จำนวน 60 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 30 คน โดยกลุ่มทดลองได้รับการดูแลตามรูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ในกลุ่มเสี่ยงเบาหวาน และกลุ่มควบคุมได้รับการพยาบาลตามมาตรฐาน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติที\n\nผลการวิจัย: รูปแบบการป้องกันการเกิดโรคเบาหวานชนิดที่ 2 ประกอบด้วย 3 กิจกรรมหลัก ได้แก่ 1) กิจกรรมการคัดกรองค้นหากลุ่มเสี่ยงเบาหวานและกิจกรรมให้ความรู้ 2) กิจกรรมการจัดการตนเอง และ 3) กิจกรรมการเยี่ยมบ้าน ซึ่งภายหลังได้รับรูปแบบการป้องกันการเกิดโรคเบาหวาน กลุ่มทดลองมีคะแนนพฤติกรรมการจัดการตนเองดีขึ้นกว่าก่อนการทดลองและดีกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 มีดัชนีมวลกายน้อยกว่าก่อนการทดลองและน้อยกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และมีระดับน้ำตาลในเลือดหลังอดอาหารต่ำกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 แต่ไม่แตกต่างจากควบคุม\n\nสรุป: ผลการวิจัยสามารถนำไปใช้เป็นแนวทางในการจัดบริการเพื่อป้องกันการเกิดโรคเบาหวานชนิดที่ 2 สำหรับกลุ่มเสี่ยงเบาหวานในชุมชน โดยการมีส่วนร่วมของครอบครัว สถานบริการสุขภาพ และชุมชน\n

Single joint kinematic alterations have been identified during gait in those with chronic ankle instability (CAI). The purpose of this study was to compare sagittal plane hip, knee, and ankle kinematics during walking in participants with and without CAI. Twelve individuals with CAI and 12 healthy individuals walked on a treadmill at 1.5 m/s. Three-dimensional kinematics were analyzed using mean ensemble curves and independent t tests. Participants with CAI demonstrated less lower extremity flexion during the absorption phase of stance and the limb placement phase of swing, which may have implications for limb placement at initial contact.

BACKGROUND: We aimed to compare the diagnostic accuracy of the Alere Triage Cardio3 Tropinin I (TnI) assay (Alere, Inc., USA) and the PathFast cTnI-II (Mitsubishi Chemical Medience Corporation, Japan) against the central laboratory assay Singulex Erenna TnI assay (Singulex, USA).
