มะเร็งเต้านม: ระบาดวิทยา การป้องกันและแนวทางการตรวจคัดกรอง, ภรณี เหล่าอิทธิ, นภา ปริญญานิติกูล

มะเร็งเต้านมเป็นมะเร็งที่เป็นสาเหตุของการเจ็บป่วยที่สำคัญทั้งในประชากรไทยและทั่วโลกโดยเฉพาะประเทศกำลังพัฒนา รวมทั้งประเทศไทยพบอุบัติการณ์การเกิดมะเร็งเต้านมเพิ่มสูงขึ้นเป็นอันดับแรกมากกว่ามะเร็งปากมดลูกมาเกือบสิบปี ในแต่ละปีมีผู้ป่วยมะเร็งเต้านมทั้งรายใหม่และรายเก่า ที่รับการรักษาตามระบบสาธารณสุขเป็นจำนวนมาก ซึ่งจัดเป็นปัญหาที่สำคัญของสาธารณสุขไทยและ ทั่วโลก อย่างไรก็ตามจากการที่มีความก้าวหน้าในเรื่องการตรวจคัดกรองมะเร็งเต้านม ทำให้วินิจฉัยโรคได้ตั้งแต่ระยะแรกและการรักษาทางการแพทย์ ทำให้ผู้ป่วยมะเร็งเต้านมมีอายุที่ยืนยาวมากขึ้นกว่าในอดีต การส่งเสริมสุขภาพและการลดปัจจัยเสี่ยงของการเกิดมะเร็งเต้านมเช่น การออกกำลังกาย งดดื่มแอลกอฮอล์ งดสูบบุหรี่ ในประชากรเพศหญิงก็เป็นแนวทางที่สำคัญในการลดอุบัติการณ์มะเร็งเต้านม และยังอาจช่วยลดโรคระบบอื่น ๆ ร่วมด้วย นอกจากนี้วิธีการส่งเสริมสุขภาพดังกล่าวยังมีส่วนช่วยลดโอกาสที่โรคมะเร็งเต้านมจะกลับเป็นซ้ำอีกด้วย ดังนั้นการส่งเสริมสุขภาพ การรณรงค์ตรวจคัดกรองมะเร็งเต้านม ในประชากรหญิงไทยทั่วไปและกลุ่มที่เสี่ยงจึงเป็นแนวทางที่สำคัญในการป้องกันและรักษาโรคมะเร็งเต้านมที่เป็นปัญหาทางสาธารณสุขอย่างมากในขณะนี้.

Peroxiredoxins (Prxs) are ubiquitous cysteine-based peroxidases that guard cells against oxidative damage, are virulence factors for pathogens, and are involved in eukaryotic redox regulatory pathways. We have analyzed catalytically active crystals to capture atomic resolution snapshots of a PrxQ-subfamily enzyme (from Xanthomonas campestris) proceeding through thiolate, sulfenate, and sulfinate species. These analyses provide structures of unprecedented accuracy for seeding theoretical studies, and show novel conformational intermediates giving insight into the reaction pathway. Based on a highly non-standard geometry seen for the sulfenate intermediate, we infer that the sulfenate formation itself can strongly promote local unfolding of the active site to …

Purpose: To compare community provider reports of adherence to current American Academy of Pediatrics (AAP) autism screening guidelines with their self-reported practices, including implementation of the Modified Checklist for Autism in Toddlers (M-CHAT).

Methods: An online survey was distributed to primary care providers from Kansas, Oklahoma, and North Dakota. The survey collected demographics and self-reported ASD screening practices (including M-CHAT implementation procedures) and knowledge questions.

Results: 133 providers completed the survey (50% pediatricians, 32% int. med/fam. physicians, 18% physician assistants). Of the 96 who reported routinely screening for ASD; 78% reported adhering to AAP guidelines. However, no providers correctly answered …

Body mass index (BMI) can be considered an application of a power law model to express body weight independently of height. Based on the same power law principle, we previously introduced a body shape index (ABSI) to be independent of BMI and height. Here, we develop a new hip index (HI) whose normalized value is independent of height, BMI, and ABSI. Similar to BMI, HI demonstrates a U-shaped relationship to mortality in the Third National Health and Nutrition Examination Survey (NHANES III) population. Abstract

Spontaneous Uterine rupture is associated with massive intra-peritoneal bleed which can be fatal if not recognized. We report a case of 32 year old multigravida at 28 weeks of gestation with history of liver cysts, previous caesarean and uterine curettage, who presented with acute abdominal pain and tenderness; ultrasound revealed placenta percreta. CT abdomen showed haemoperitoneum. The patient underwent emergency caesarean hysterectomy due to uterine rupture at the cornual site.

BACKGROUND: An interim analysis of data from the HIV Prevention Trials Network (HPTN) 052 trial showed that antiretroviral therapy (ART) prevented more than 96% of genetically linked infections caused by human immunodeficiency virus type 1 (HIV-1) in serodiscordant couples. ART was then offered to all patients with HIV-1 infection (index participants). The study included more than 5 years of follow-up to assess the durability of such therapy for the prevention of HIV-1 transmission.

เหตุผลของการทำวิจัย : ความเหนื่อยหน่ายในการทำงานจะเกิดขึ้นกับการประกอบอาชีพที่มีความเครียด ส่งผลให้พนักงานหมดกำลัง และขาดแรงจูงใจในการปฏิบัติงานวัตถุประสงค์ : เพื่อศึกษาระดับภาวะเหนื่อยหน่ายในการทำงาน ระดับกลวิธีในการเผชิญปัญหา และปัจจัยที่เกี่ยวข้องกับภาวะเหนื่อยหน่ายในการทำงานของพนักงานองค์การเภสัชกรรมรูปแบบการวิจัย : การศึกษาวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : องค์การเภสัชกรรม กรุงเทพมหานครตัวอย่างและวิธีการศึกษา : เก็บรวบรวมข้อมูลจากพนักงานองค์การเภสัชกรรมตั้งแต่เดือนสิงหาคม ถึง เดือนตุลาคม พ.ศ. 2558 กลุ่มตัวอย่างทั้งหมด 360 คนทำการตอบแบบสอบถาม 4 ส่วน ดังนี้ 1) แบบสอบถามข้อมูลทั่วไป2) แบบสอบถามปัจจัยลักษณะสภาพแวดล้อมในการทำงาน 3) แบบวัดความเหนื่อยหน่ายในการทำงาน 4) แบบวัดการเผชิญปัญหาสถิติที่ใช้ได้แก่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน การทดสอบIndependent t-test One way ANOVA และ Multiple LinearRegressionผลการศึกษา : ผลการศึกษาพบว่ากลุ่มตัวอย่างมีภาวะเหนื่อยหน่ายในการทำงานใน3 มิติ ได้แก่ด้านความอ่อนล้าทางอารมณ์อยู่ในระดับต่ำ (ค่าเฉลี่ย = 2.70ส่วนเบี่ยงเบนมาตรฐาน = 1.43) การลดค่าความเป็นบุคคลในผู้อื่นอยู่ในระดับต่ำ (ค่าเฉลี่ย = 2.48 ส่วนเบี่ยงเบนมาตรฐาน = 1.06) และด้านความรู้สึกไร้ประสิทธิภาพ อยู่ในระดับปานกลาง (ค่าเฉลี่ย = 3.48ส่วนเบี่ยงเบนมาตรฐาน = 0.50) เมื่อวิเคราะห์ถดถอยพหุคูณ พบว่าปัจจัยที่เกี่ยวข้องกับภาวะเหนื่อยหน่ายในการทำงาน ด้านความอ่อนล้าทางอารมณ์อย่างมีนัยสำคัญทางสถิติ ได้แก่ เพศ อายุ ปัจจัยลักษณะสภาพแวดล้อมในการทำงานด้านลักษณะงานที่รับผิดชอบปัจจัยลักษณะสภาพแวดล้อมในการทำงานด้านสัมพันธภาพในหน่วยงาน ตำแหน่งงาน ลักษณะงานที่ปฏิบัติ และกลวิธีในการเผชิญปัญหาแบบหลีกหนี ส่วนปัจจัยลักษณะแวดล้อมในการทำงานด้านสัมพันธภาพในหน่วยงาน, ลักษณะงานที่รับผิดชอบ ตำแหน่งงานและกลวิธีในการเผชิญปัญหาแบบหลีกหนีเกี่ยวข้องกับด้านการลดค่าความเป็นบุคคลในผู้อื่นอย่างมีนัยสำคัญทางสถิติ นอกจากนี้ ตำแหน่งงานมีความสัมพันธ์กับความรู้สึกไร้ประสิทธิภาพอย่างมีนัยสำคัญทางสถิติสรุป : ผลการศึกษานี้เป็นประโยชน์ต่อแผนกทรัพยากรบุคคล ช่วยสร้างมาตรการพัฒนาวิธีการรับมือกับภาวะเหนื่อยหน่ายในการทำงาน และส่งเสริมการใช้กลวิธีการเผชิญปัญหาเชิงบวก เพื่อป้องกันให้พนักงานมีคุณภาพชีวิตที่ดีในการทำงาน.

TGF-β-induced endothelial-to-mesenchymal transition (EndoMT) is a newly recognized source of profibrotic activated myofibroblasts and has been suggested to play a role in the pathogenesis of various fibrotic processes. Endothelin-1 (ET-1) has been implicated in the development of tissue fibrosis but its participation in TGF-β-induced EndoMT has not been studied. Here we evaluated the role of ET-1 on TGF-β1-induced EndoMT in immunopurified CD31+/CD102+ murine lung microvascular endothelial cells. Porcine deltacoronavirus (PDCoV) is a newly identified virus that has been detected in swine herds of North America associated with enteric disease. The aim of this study was to demonstrate the pathogenicity, course of infection, virus kinetics, and aerosol transmission of PDCoV using 87 conventional piglets and their 9 dams, including aerosol and contact controls to emulate field conditions. STUDY DESIGN: Systematic review.

OBJECTIVE: To determine the incidence, pathogenesis, and clinical outcomes related to neurogenic fevers following traumatic spinal cord injury (SCI).

METHODS: A systematic review of the literature was performed on thermodysregulation secondary to acute traumatic SCI in adult patients. A literature search was performed using PubMed (MEDLINE), Cochrane Central Register of Controlled Trials, and Scopus. Using strict inclusion and exclusion criteria, seven relevant articles were obtained.

RESULTS: The incidence of fever of all origins (both known and unknown) after SCI ranged from 22.5 to 71.7% with a mean incidence of 50.6% and a median incidence of 50.0%. …

"The residency accreditation process can be a stress-inducing experience to many program directors. In 2014, the postgraduate year 1 (PGY1) pharmacy residency accreditation standards were updated from the previous 2005 version. The new standards were formulated to streamline program requirements with the intention of creating a more transparent accreditation process.1 The American Society of Health- System Pharmacists (ASHP) is the accrediting body for PGY1 residencies, PGY1 community pharmacy residencies, PGY1 residencies in managed care pharmacy, and PGY2 pharmacy residency programs in advanced practice areas. Background: Accumulating evidence supports a positive correlation between the presence of antiphosphatidylethanolamine (aPE) autoantibodies and clinical symptoms of antiphospholipid syndrome (APS). However, there is a lack of standardized ELISA-based method for detecting aPE. The current study was conducted to investigate the dependence of aPE ELISA on lipid concentration and composition of PE antigens. Methods: A range of ELISA conditions were examined by varying the concentrations of egg PE and by substituting egg PE with combinations of synthetic DOPE and DSPE. Selecting medical hardware can be a difficult and permanent decision for patients that they are often unprepared for. The authors explore the use of pairwise comparison techniques to better inform medical decision making in an application of choosing between three major cochlear implant manufacturers. This paper appears to be the first study to apply a pairwise comparison decision making approach for cochlear implant device selection. Also, unlike many medical decision making studies that are developed by the healthcare professional, this model and analysis was conducted entirely by the patient for the purpose of making the real-world decision of a device. …

We assessed the appropriate geographic scale to apply an area deprivation index (ADI), which reflects a geographic area’s level of socioeconomic deprivation and is associated with health outcomes, to identify and screen patients for social determinants of health. We estimated the relative strength of the association between the ADI at various geographic levels and a range of hospitalization rates by using age-adjusted odds ratios in an 8-county region of New York State. The 10-km local ADI estimates had the strongest associations with all hospitalization rates (higher odds ratios) followed by estimates at 20 km, 30 km, and the regional scale. …

Optically derived tissue properties across a range of breast densities and the effects of breast compression on estimates of hemoglobin, oxygen metabolism, and water and lipid concentrations were obtained from a coregistered imaging system that integrates near-infrared spectral tomography (NIRST) with digital breast tomosynthesis (DBT). Image data were analyzed from 27 women who underwent four IRB approved NIRST/DBT exams that included fully and mildly compressed breast acquisitions in two projections—craniocaudal (CC) and mediolateral-oblique (MLO)—and generated four data sets per patient (full and moderate compression in CC and MLO views). This descriptive qualitative study explored data from debriefs of all newly hired nurses at 3, 6, and 12 months posthire during a newly designed transition-to-practice program at a pediatric hospital. Four major themes emerged: preceptors, education process, adaptation to the organization, and role transition. Supportive factors included staged orientation, limited preceptors, mentors, regular communication with leaders, and a culture of teamwork. Stressors included too many preceptors, mentorship needs, floating, communication challenges, and organizational changes.

Morphine prescribing is limited by its high addiction tendency and other serious effects. Recent animal’s biological studies on Mu 6TM splice variants, which mainly include G1, G2 and Mu3, supported a high safety and potency profiles of IBNtxA as potential alternative of Morphine. Nevertheless, there is no high-resolution structures of these 6TM variants, and the detailed structural features and dynamic characteristics of these splice variants remain elusive.
