Admission to an emergency shelter reflects an important outcome in a person's life, and many admissions to an emergency shelter reflects a human and financial cost. This article discusses the evolution of the measurement and understanding of homelessness. It looks at characteristics of people who make use of shelter in a chronic fashion to better understand what might predict risk for future continued use. This includes an analysis of a large dataset of emergency shelter utilization in Detroit Michigan collected between 2008 and 2012. Analysis reveals that fundamental demographic information, individual's interactions with the shelter system (good or bad), and …

African American older adults have historically been identified as vital members of the African American family and community (Hill, 1972; Cox, 2002; Waites, 2008). Unfortunately, research has shown an increasing number of older African Americans are being admitted into nursing facilities (Paul, 2004; Feng et al., 2011). Between the years of 1998-2008, it was projected African Americans living in nursing homes increased by 10.8% (Feng et al., 2011).

Neural Correlates Of Episodic Memory Formation In Children And Adults, Lingfei Tang

The medial temporal lobe (MTL) and prefrontal cortex (PFC) are two key brain regions that support episodic memory formation in both children and adults, but the functional developmental of these regions remains unclear. In this study, we investigated the development of neural correlates of episodic memory formation using functional MRI with a subsequent memory paradigm, administered to a cross-sectional sample of 83 children and adults. We found that MTL subregions showed an age-related increase in activation supporting memory formation of complex scenes. In addition, a functionally defined scene-sensitive region in the posterior MTL also showed similar increase and predicted better …

BACKGROUND: Undernutrition is a major public health problem on the globe particularly in the developing regions. The objective of the current study was to assess the prevalence of undernutrition in different age groups and examine the relationship of the disease to parasitic and socioeconomic factors among communities in Harbu Town, northeastern Ethiopia.
METHODS: Stool samples of the study participants were examined for intestinal helminth infections using the Kato-Katz method. In order to address the problem of an unhealthy workforce, employers often implement disease prevention and health promotion strategies, referred to as worksite wellness programs. The purpose of this study was to examine employees' interests and preferences regarding worksite exercise programs. To collect data, a 12 item survey was developed and administered to employees from a company based in Rochester, Minnesota. A total of 21 participants completed the survey (66.7% male, 28.6% female). The mean age was 51 years old. The findings of the study indicated an interest in worksite exercise programs and that incentives may motivate employees to participate …

Biofeedback intervention can help children achieve correct production of a treatment-resistant error sound, but generalisation is often limited. This case study suggests that generalisation can be enhanced when biofeedback intervention is structured in accordance with a "challenge point" framework for speech-motor learning. The participant was an 11-year-old with residual /r/ misarticulation who had previously attained correct /r/ production through a structured course of ultrasound biofeedback treatment but did not generalise these gains beyond the word level. Post-transcriptional gene regulation plays an important role in controlling gene expression patterns in mammalian cells. CUG-binding and Embryonic Lethal vision-type RNA-binding protein 3 (ETR)-like Factor (CELF1), also known as CUGBP1, is a RNA-binding protein involved in post-transcriptional gene regulation through nuclear (alternative splicing) and cytoplasmic (mRNA turnover and translation) mRNA processing events. Primarily, CELF1 is known for its contribution to the development of myotonic dystrophy (DM1). But, published observations from our laboratory and others determined that CELF1 is overexpressed in head and neck squamous cell carcinoma (HNSCC) as well as multiple other cancers. Cerebral microinfarcts (CMI), microscopic brain lesions caused by blockade of small blood vessels, have recently emerged as a potential determinant of cognitive decline. Though small in size, our recent work demonstrated that a single, strategically placed CMI was sufficient to disrupt sensory input in a behavioral task. However, the means by which such small lesions disrupt brain function remain poorly understood. We imaged vascular function in awake, head-fixed mice using two-photon microscopy to examine the impact of CMI on neurovascular coupling. Objective: Here we present the first video demonstration of reduction corporoplasty in the management of phallic disfigurement in a 17 year old man with a history sickle cell disease and priapism.
Introduction: Surgical management of aneurysmal dilation of the corpora has yet to be defined in the literature. Materials and Methods: We preformed bilateral elliptical incisions over the lateral corpora as management of aneurysmal dilation of the corpora to correct phallic disfigurement.
Results: The patient tolerated the procedure well and has resolution of his corporal disfigurement.

Purpose: To determine if patients with renal cell carcinoma (RCC) with levels III and IV tumor thrombi are receive any reduction in complication rate utilizing veno-venous bypass (VVB) over cardiopulmonary bypass (CPB) for high level (III/IV) inferior vena cava (IVC) tumor thrombectomy and concomitant radical nephrectomy.
Materials and Methods: From May 1990 to August 2011, we reviewed 21 patients that had been treated for RCC with radical nephrectomy and concomitant IVC thrombectomy employing either CPB (n =16) or VVB (n=5). We retrospectively reviewed our study population for complication rates and perioperative characteristics.

Main findings: A 26-year-old man suffering from partial priapism was successfully treated with a regimen including pentoxifylline, a nonspecific phosphodiesterase inhibitor that is often used to conservatively treat Peyronie's disease.
Case hypothesis: Partial priapism is an extremely rare urological condition that is characterized by thrombosis within the proximal segment of a single corpus cavernosum. There have only been 36 reported cases to date. Although several factors have been associated with this unusual disorder, such as trauma or bicycle riding, the etiology is still not completely understood. Treatment is usually conservative and consists of a non-steroidal anti-inflammatory and anti-thrombotic.

Ureteral-arterial fistula (UAF) is an exceedingly rare but life-threatening condition warranting emergent intervention. Prompt recognition and management of UAF in suspect patients presenting with gross hematuria are required for a successful outcome. We report a rare subset of UAF involving the bilateral common iliac arteries. The patient underwent successful endovascular stent-grafting to correct the arterial defect and delayed open repair of the ureteral strictures. Timely management has benefited from the collaboration of the involved medical teams, which included emergency medicine, urology, and interventional radiology.

Horseshoe kidney (HSK) is the most common renal anomaly. Reports of the incidence of renal cell carcinoma (RCC) in HSK are conflicting. Very few cases of isthmus-located RCC have been reported in the literature. We report a unique case of an isthmus-located RCC. Proper vascular and tumor imaging prior to surgery is key to successful tumor removal.

Objective: The objective of this selective EBM review is to determine whether or not there is efficacy with combination treatment benzoyl peroxide and clindamycin for progressive macular hypomelanosis in adults.
Study Design: A review of three English language studies. One published in 2006 and two in 2011. Includes two RCT’s and one randomized left-right comparison study.
Data Sources: Researched articles via PubMed and Cochrane database. All articles were published in peer-reviewed journals. Two randomized, controlled trials and one randomized left-right comparison study were used.

OBJECTIVE: The objective of this selective EBM review is to determine “Is prophylactic oxybutynin safe and effective in reducing the severity of palmar hyperhidrosis in adults?”
STUDY DESIGN: This review is based on two randomized controlled trials (RCTs) and one non-randomized crossover study published in 2008, 2011, and 2012. The studies compared the safety and efficacy of oxybutynin in reducing the severity of palmar hyperhidrosis.
DATA SOURCES: All articles used were published in English, in peer-reviewed journals, and found using PubMed, Ebscohost, and Cochrane Review databases.

Objectives—To characterize antiretroviral therapy (ART) adherence across different programmes and examine the relationship between individual and programme characteristics and incomplete adherence among ART clients in sub-Saharan Africa.
Design—A cross-sectional study.
Methods—Systematically selected ART clients (≥18 years; on ART ≥6 months) attending 18 facilities in three countries (250 clients/facility) were interviewed. Client self-reports (3-day, 30-day, Case Index ≥48 consecutive hours of missed ART), healthcare provider estimates and the pharmacy medication possession ratio (MPR) were used to estimate ART adherence. Rickettsiae are obligately intracellular bacteria that are transmitted to vertebrates by a variety of arthropod vectors, primarily by fleas and ticks. Once transmitted or experimentally inoculated into susceptible mammals, some rickettsiae may cause febrile illness of different morbidity and mortality, and which can manifest with different types of exhanthems in humans. Veridical stereoscopic depth depends on matching corresponding image points. This requires solving the stereo correspondence problem: how are true matches distinguished from false ones? Conventional algorithms select true matches on the basis of feature detection [what do you mean by ‘feature detection’ here? Is there some more specific term?] and adherence to natural statistics. They reject false matches as noise. We propose here an alternative that uses the signals present in false matches to delineate the true solution. When visualized in a Keplerian array, binocular matches are symmetrically reflected about an axis that is a potential solution. Properties such as …
