The aim of this study was to describe clinical profiles and outcomes of children admitted directly from the Emergency Room (ER) to the Pediatric Intensive Care Unit (PICU) of academic hospital. The medical records of all children (1 month to 16 years) admitted in PICU from ER, from January 2011 to December 2012 were reviewed. Of the 26,774 patients seen in the ER during the study period, 468 (1.7%) were admitted to the PICU which constituted about 41.5% (468/1127) of all the total PICU admissions. Isoniazid (INH) is the mainstay of management against Mycobacterium tuberculosis. INH-induced acute pancreatitis is an uncommon association and with dearth of literature on it. We are reporting a case of an 11 years old girl who developed acute pancreatitis after 2 weeks of antituberculous therapy. An INH free regimen was started. She was discharged and followed without complications.

With global obesity on the rise, the need for research concerning the complexity of risk factors is more important than ever. While the increase of the high-calorie, low-nutrient food supply correlates with rising weights, new studies across disciplines reveal the intricacy of the environmental and circumstantial phenomena that influence obesity rates and behaviors for at-risk individuals. AIMS Public Health is pleased to present this special issue entitled “Obesity and Food Intake in Vulnerable Populations” in support of this research. Accumulating evidence suggests that interleukin-1 (IL-1) signaling plays an essential role in the pathogenesis of heart failure by inducing cardiomyocyte apoptosis, but the mechanisms of this process are poorly defined. We further explored these molecular pathways.
We isolated cardiomyocytes from neonatal mice and then cultured and stimulated them with murine IL-1β in vitro. Cell apoptotic ratios were measured by means of flow cytometry. Expression of effector molecules was analyzed by means of enzyme-linked immunosorbent assay, Western blotting, and real-time quantitative polymerase chain reaction. Pulmonary arteriovenous fistula is a rare disease. To the best of our knowledge, prenatal diagnosis of a fistula between the left pulmonary artery and the left pulmonary vein has not been described in the medical literature. We report a case of the prenatal diagnosis of a left pulmonary artery-to-pulmonary vein fistula, followed by successful neonatal surgical repair.

We describe transcatheter aortic valve implantation in a patient who had severe peripheral artery disease. The patient's vascular condition required additional preliminary peripheral intervention to enable adequate vascular access.
A 78-year-old man with severe aortic stenosis, substantial comorbidities, and severe heart failure symptoms was referred for aortic valve replacement. The patient's 20-mm aortic annulus necessitated the use of a 23-mm Edwards Sapien valve inserted through a 22F sheath, which itself needed a vessel diameter of at least 7 mm for percutaneous delivery. Left ventricular hypertrophy in aortic stenosis is considered a compensatory response for the maintenance of systolic function but a risk factor for cardiac morbidity and death. We investigated the degree of left ventricular mass regression after implantation of the sutureless Medtronic 3f Enable® Aortic Bioprosthesis.
We studied 19 patients who, from May 2010 through July 2011, underwent isolated aortic valve replacement with the 3f Enable bioprosthetic valve, with clinical and echocardiographic follow-up at 6 months. The mean age was 77.1 ± 5.1 years (range, 68–86 yr); 14 patients were women (73.7%); and the mean logistic EuroSCORE was 15.4% ± 11.8%. …

Background
Gluten sensitivity and the incidence of Celiac disease are ever increasing. The aim was to measure the effectiveness of an enzyme based dietary supplement on the reduction of symptoms encountered upon the consumption of gluten.
Results
The data showed a statistically significant improvement when compared to the placebo group in the following categories: pain, bloating, emptying of bowels, hunger pains, rumbling of stomach, lower energy levels, headaches, and food cravings. There was no statistically signficant improvement in the following symptoms: comfort, nausea, and loss of appetite.
Conclusion
Overall, the dietary supplement, Glutalytic, reduced many of the symptoms that are …

Type 2 diabetes (T2D) is a complex metabolic disease that is more prevalent in ethnic groups such as Mexican Americans, and is strongly associated with the risk factors obesity and insulin resistance. The goal of this study was to perform whole genome gene expression profiling in adipose tissue to detect common patterns of gene regulation associated with obesity and insulin resistance. Aim:
To assess the relationship between cognitive status and self-reported symptoms of depression, anxiety and stress of older adults living in an underserved rural South American population.
Methods:
Community-dwelling Atahualpa residents aged ≥60 years were identified during a door-to-door census, and evaluated with the Depression Anxiety Stress Scale-21 (DASS-21) and the Montreal Cognitive Assessment (MoCA). We explored whether positivity in each of the DASS-21 axes was related to total and domain-specific MoCA performance after adjustment for age, sex and education.

Chronic psychological stress impairs antibody synthesis following influenza vaccination. Chronic stress also increases circulating levels of proinflammatory cytokines and glucocorticoids in elders and caregivers, which can impair antibody synthesis. The purpose of this study was to determine whether psychological stress increases ex vivo cytokine production or decreases glucocorticoid sensitivity (GCS) of peripheral blood leukocytes from healthy college students. A convenience sample of Reserve Officer Training Corps (ROTC) students completed the Perceived Stress Scale (PSS). This analysis compares the staffing implications of three measures of nurse staffing requirements: midnight census, turnover adjustment based on length of stay, and volume of admissions, discharges and transfers.
Background
Midnight census is commonly used to determine registered nurse staffing. Unit-level workload increases with patient churn, the movement of patients in and out of the nursing unit. Failure to account for patient churn in staffing allocation impacts nurse workload and may result in adverse patient outcomes.

We evaluated four dengue diagnostic devices from Alere, including the SD Bioline Dengue Duo (nonstructural [NS] 1 Ag and IgG/IgM), the Panbio Dengue Duo Cassette (IgM/IgG) rapid diagnostic tests (RDTs), and the Panbio dengue IgM and IgG capture enzyme-linked immunosorbent assays (ELISAs) in a prospective, controlled, multicenter study in Peru, Venezuela, Cambodia, and the United States, using samples from 1,021 febrile individuals. Archived, well-characterized samples from an additional 135 febrile individuals from Thailand were also used. Computer-assisted learning, also known as e-learning, has been successfully implemented to educate students in anatomical knowledge as well as transferable skills, such as critical analysis, teamwork, leadership and communication. E-learning allows students to self-teach material at their own paces and provides a platform for team-based laboratory approaches.
Several institutions have already integrated histology and physiology in team based laboratory approaches, but integration of histology and pathology instruction has been done to a lesser extent. Our aim was to develop an e-learning atlas that integrates microanatomy and pathology laboratory for an interdisciplinary pre-clinical medical curriculum.

Pendahuluan: Perubahan status nutrisi dikaitkan dengan tingginya mortalitas pada kelompok pasien penyakit ginjal tahap akhir (PGTA). Berbagai penelitian menunjukkan tingginya prevalensi malnutrisi pada pasien dialisis kronik, namun hingga saat ini, penelitian lebih banyak dilakukan pada pasien yang menjalani hemodialisis. Dengan latar belakang berkembangnya CAPD di Indonesia, perlu diteliti faktor-faktor yang berkaitan dengan kesintasan pasien CAPD untuk menekan tingginya morbiditas dan mortalitas pada kelompok pasien tersebut, salah satunya adalah faktor nutrisi.

Pendahuluan. Prevalensi diabetes global diprediksi akan meningkat sebesar dua kali lipat pada tahun 2030 dari 2,8% menjadi 4,4% (berkisar 366 juta) dengan angka komplikasi kaki diabetes berkisar 10%. Ulkus diabetes dan amputasi ekstremitas bawah merupakan komplikasi diabetes yang akan meningkatkan mortalitas dan morbiditas Setelah lima tahun amputasi pertama, 28-51% pasien akan menjalani amputasi kedua. Penelitian ini dilakukan untuk mengetahui proporsi dan profil pasien yang mengalami reamputasi di Rumah Sakit Cipto Mangunkusumo tahun 2008-2012.

Pendahuluan. Pasien hemodialisis (HD) tergolong kelompok risiko tinggi terinfeksi virus hepatitis C. Penelitian mengenai serokonversi hepatitis C di RS Cipto Mangunkusumo belum pernah dilakukan. Agar transmisi hepatitis C dapat diturunkan, faktor risiko serokonversi hepatitis C penting diketahui. Penelitian ini bertujuan mengetahui proporsi dan faktor risiko serokonversi hepatitis C pada pasien yang menjalani HD di RS Cipto Mangunkusumo.
Metode. Penelitian potong lintang terhadap pasien yang menjalani HD di RS Cipto Mangunkusumo pada bulan Juni-Juli 2011. Pemeriksaan anti-HCV menggunakan Roche Elecsys ECLIA, Analisis multivariat menggunakan regresi logistik.

Pendahuluan: Prevalensi sirosis cukup tinggi di Indonesia yang mayoritas populasinya adalah muslim. Pada saat menjalani puasa Ramadhan yang merupakan kewajiban umat muslim terjadi berbagai proses metabolik seperti penurunan glikokogenesis, peningkatan glukoneogenesis dan ketogenesis, dapat mempengaruhi keadaan klinis, nutrisi dan bokimiawi pasien sirosis hati yang juga mengalami proses hiperkatabolik. Penelitian tentang pengaruh puasa Ramadhan pada pasien sirosis hati di Indonesia belum pernah dilakukan. Latar Belakang: Ulkus kaki diabetik terinfeksi merupakan kasus DM yang paling banyak dirawat di RS, berhubungan dengan morbiditas, mortalitas, biaya yang tinggi dan bersifat multifaktorial. Salah satu faktor yang berpengaruh adalah albumin. Belum ada penelitian yang secara langsung menghubungkan konsentrasi albumin serum awal perawatan dengan perbaikan klinis infeksi ulkus kaki diabetik. Karsinoma hepatoselular (KHS) merupakan tumor ganas hati primer yang menempati urutan ketiga penyebab mortalitas akibat keganasan di seluruh dunia, karena acapkali terdiagnosis pada stadium lanjut. Tidak jarang manifestasi ekstrahepatik mucul pada KHS di antaranya hipoglikemia yang dapat muncul di akhir perjalanan penyakit ketika terjadi kegagalan fungsi hati; namun juga dapat muncul sebagai gejala awal sebelum ditemukan nodul pada hepar. Hipoglikemia dapat meningkatkan morbiditas, menurunkan kualitas hidup, dan menurunkan kesintasan. Pemahaman mengenai patogenesis, kewaspadaan serta diagnosis dini, dan penanganan yang tepat merupakan kunci tatalaksana utama.

Pendahuluan: Prevalensi deifisiensi vitamin D pada wanita 50 tahun ke atas di Indonesia cukup tinggi namun pemeriksaan kadar vitamin D serum sangat mahal. Oleh karena itu, diperlukan alat penyaring defisiensi vitamin D yang cukup ekonomis dan sederhana untuk dikerjakan di layanan kesehatan primer. Metode: Penelitian ini merupakan penelitian diagnostik, menggunakan desain potong lintang (cross-sectional) sehubungan dengan pengambilan data hanya dilakukan sewaktu saja. Penelitian ini dilakukan di Kotamadia Bandung selama Agustus–Oktober 2012.
