Retinoic acid and thyroid hormone are known to play key roles in the regulation of endochondral ossification. However, the issue of whether these two hormones interact with each other to regulate bone functions remains to be established. We investigated how thyroid hormone and retinoic acid interact to regulate cells involved in endochondral bone formation. We demonstrate that thyroid hormone treatment stimulates differentiation of ATDC5 chondrocytes and promotes formation of mineralized nodules while retinoic acid treatment at high dose inhibits chondrocyte differentiation and formation of mineralized nodules. Furthermore, thyroid hormone induced mineralized nodule formation is inhibited by co-treatment with retinoic acid …

Background: The World Health Organization recommends the promotion of positive cardiovascular behaviors (healthy diet and physical activity), while reducing negative ones (tobacco and alcohol abuse) as critical steps in reducing the burden of cardiovascular diseases. Researchers have highlighted the role of acculturation in predicting such behaviors among immigrants, though similar studies among Asian-American subgroups are lacking. This study examines the putative relationship between acculturation and aforementioned cardiovascular behaviors among six Asian-American subgroups.
Methods: A secondary analysis of the California Health Interview Survey 2007, 2009, and 2011 public access data was conducted. Acculturation was assessed utilizing proxy measures of language spoken …

Background: Sino-implant (II) is a two-rod subcutaneous contraceptive implant used up to 4 years, containing 150 mg of levonorgestrel. We conducted two observational studies of Sino-implant (II) to evaluate its performance in routine service delivery settings.
Methods: We enrolled 1326 women age 18-44 who had Sino-implant (II) inserted at clinics in Pakistan and Kenya. Women were followed-up using either an active or passive follow-up scheme in each study. Study outcomes were: one-year cumulative pregnancy and discontinuation rates; rates of insertion and removal complications; adverse event and side effect rates; reasons for discontinuation; and implant acceptability and satisfaction with clinic services. …

The “Hand of Hope” (Fig. 1) statue was commissioned for display in the street-level lobby of my practice, The Philadelphia Hand Center, by its founders, James M. Hunter, MD, and Lawrence H. Schneider, MD. The work became a hallmark of the practice’s former Walnut Street location as well as a recognizable feature of its neighborhood.
After a sojourn of several years residing next to Thomas Eakins’ portrait of the operative theatre in “The Gross Clinic,” at Thomas Jefferson University, the sculpture has returned to grace our office’s lobby in the historic Benjamin Franklin Hotel.

BACKGROUND: An injury to the ulnar collateral ligament (UCL) of the elbow is potentially career threatening for elite baseball pitchers. Stress ultrasound (US) of the elbow allows for evaluation of both the UCL and the ulnohumeral joint space at rest and with stress.
HYPOTHESIS: Stress US can identify morphological and functional UCL changes and may predict the risk of a UCL injury in elite pitchers.

The default-mode network (DMN) is a major resting-state network. It can be divided in two distinct networks: one is composed of dorsal and anterior regions [referred to as the dorsal DMN (dDMN)], while the other involves the more posterior regions [referred to as the ventral DMN (vDMN)]. To date, no studies have investigated the potentially distinct impact of temporal lobe epilepsy (TLE) on these networks. ภาวะพร่องเอนไซม์กลูโคส 6-ฟอสเฟต ดีไฮโดรจีเนส (G6PD) พบมากในเอเชียตะวันออกเฉียงใต้ ที่มีมาลาเรียชุกชุม ซึ่งเป็นผลมาจากความได้เปรียบในการอยู่รอดต่อการคุกคามของมาลาเรียภาวะพร่องเอนไซม์ G6PD มีสาเหตุมาจากการกลายพันธุ์ของยีน G6PD ส่งผลให้เกิดภาวะเม็ดเลือดแดงแตกเฉียบพลันเมื่อได้รับยาต้านมาลาเรีย ไพรมาควินเป็นยาต้านมาลาเรียที่มีคุณสมบัติในการรักษาและป้องกันการติดเชื้อมาลาเรีย แต่มีผลทำให้ผู้พร่องเอนไซม์ G6PD เกิดภาวะเม็ดเลือดแดงแตกอย่างรุนแรง ด้วยเหตุนี้การตรวจภาวะพร่องเอนไซม์ G6PD และชนิดของการกลายพันธุ์ จึงมีความจำเป็นเพื่อเป็นข้อมูลในการรักษาผู้ป่วยมาลาเรียที่มีภาวะพร่องเอนไซม์ G6PD ได้อย่างมีประสิทธิภาพและเป็นอันตรายต่อผู้ป่วยน้อยที่สุด แต่อย่างไรก็ตามการตรวจคัดกรองภาวะพร่องเอนไซม์ก่อนให้ยาอาจไม่สามารถทำได้ในบางพื้นที่ที่ยังขาดเครื่องมือในการทดสอบ ดังนั้นการสรุปรูปแบบการกระจายตัวของภาวะพร่องเอนไซม์ G6PD รวมทั้งชนิดการกลายพันธุ์ที่พบในพื้นที่ต่าง ๆ ในเอเชียตะวันออกเฉียงใต้จะเป็นประโยชน์ต่อการประเมินความเสี่ยงในการรักษาและควบคุมมาลาเรียด้วยยาไพรมาควินได้.

เหตุผลของการทำวิจัย : การตั้งครรภ์ในวัยรุ่นนับเป็นปัญหาสังคมที่ส่งผลกระทบต่อหลายภาคส่วน โดยเฉพาะผลกระทบทางด้านสุขภาพอนามัยและด้านจิตใจของหญิงวัยรุ่นตั้งครรภ์และบุตรที่จะคลอดออกมา การศึกษาเพื่อให้ได้ข้อมูลปัญหาเชิงจิตสังคมและสิ่งแวดล้อม รวมทั้งความจำเป็นด้านร่างกายและจิตสังคมในหญิงวัยรุ่นตั้งครรภ์ อาจมีส่วนช่วยเป็นแนวทางในการป้องกันและแก้ไขปัญหาได้อย่างตรงต่อความต้องการและมีประสิทธิภาพวัตถุประสงค์ : เพื่อศึกษาปัญหาจิตสังคมและสิ่งแวดล้อมรวมทั้งความช่วยเหลือและการสนับสนุนจากครอบครัวและบุคคลใกล้ชิด และศึกษาความต้องการด้านร่างกาย ความต้องการด้านจิตใจ และความต้องการด้านสังคมของหญิงวัยรุ่นตั้งครรภ์ ที่มาฝากครรภ์ที่คลินิกฝากครรภ์วัยรุ่น แผนกสูติกรรม โรงพยาบาลจุฬาลงกรณ์รูปแบบการวิจัย : การวิจัยเชิงคุณภาพสถานที่ทำการวิจัย : คลินิกฝากครรภ์วัยรุ่น แผนกสูติกรรม โรงพยาบาลจุฬาลงกรณ์กลุ่มตัวอย่างและวิธีการศึกษา : หญิงวัยรุ่นตั้งครรภ์ที่ฝากครรภ์ต่อเนื่องที่คลินิกฝากครรภ์วัยรุ่นแผนกสูติกรรม โรงพยาบาลจุฬาลงกรณ์ โดยการเลือกกลุ่มตัวอย่างเป็นการเลือกเจาะจงจากประชากรดังกล่าว จำนวน 18 ราย ใน3 ไตรมาส ไตรมาสละ 6 เครื่องมือที่ใช้ในการวิจัยประกอบด้วย1) แบบบันทึกข้อมูลส่วนบุคคล 2) แนวคำถามที่ในการสัมภาษณ์ระดับลึกเพื่อใช้เป็นแนวคำถามประกอบการสัมภาษณ์ผลการศึกษา : พบว่าหญิงวัยรุ่นตั้งครรภ์ต้องออกจากเรียนกลางคัน และต้องการกลับไปศึกษาต่อในระดับที่สูงขึ้นไปภายหลังจากที่คลอดบุตร พบปัญหาการตัดสินใจจะพักอาศัยอยู่กับครอบครัวเดิมหรือครอบครัวสามี ปัญหาเศรษฐกิจพบว่าหญิงวัยรุ่นตั้งครรภ์ ที่ไม่ได้ประกอบอาชีพไม่มีรายได้ประจำ หญิงวัยรุ่นตั้งครรภ์มีความต้องการด้านร่างกายคือต้องการคำแนะนำการดูแลตนเอง/ผู้ช่วยเหลือเลี้ยงดูบุตร ด้านอารมณ์/จิตใจ ต้องการความเอาใจใส่จากบุคคลใกล้ชิดและการสนับสนุนให้กำลังใจ และความต้องการทางด้านเศรษฐกิจและสังคม คือต้องการกลับสู่ระบบการศึกษาและต้องการประกอบอาชีพที่มั่นคงสรุป : ปัญหาเชิงจิตสังคมและสิ่งแวดล้อมที่พบในหญิงวัยรุ่นตั้งครรภ์ล้วนส่งผลกระทบต่อสุขภาพจิตของหญิงวัยรุ่นและบุตรในครรภ์การศึกษาถึงรายละเอียดของปัญหา จะสามารถใช้เป็นข้อมูลพื้นฐานในการวางแนวทางในการให้ความช่วยเหลือ ตลอดจนการจัดหาทรัพยากรทางสังคมเพื่อให้ความช่วยเหลือได้ตรงกับความต้องการ และเพื่อวางแนวทางป้องกันปัญหาการตั้งครรภ์ในภาวะที่ไม่พร้อม.

เหตุผลของการทำวิจัย : สัตว์บำบัดนับเป็นศาสตร์ใหม่ในวงการแพทย์ทางเลือกของไทยเนื่องจากสัตว์เป็นสิ่งมีชีวิตที่ใกล้ชิดกับมนุษย์และเป็นสิ่งมีชีวิตที่สามารถมอบความรักให้ได้โดยไม่มีเงื่อนไข จึงสอดคล้องกับความต้องการของเด็กกำพร้า จึงอาจจะเป็นอีกทางเลือกหนึ่งที่น่าสนใจที่จะเป็นส่วนช่วยเหลือหรือพัฒนาจิตใจของเด็กกำพร้าให้มีความเข้มแข็ง เห็นคุณค่าในตนเอง ไม่ซึมเศร้า และมีความสุขในการใช้ชีวิตมากขึ้นวัตถุประสงค์ : เพื่อศึกษาผลของการใช้โปรแกรมการบำบัดด้วยกระต่ายที่มีต่อความรู้สึกเห็นคุณค่าในตนเอง, ภาวะซึมเศร้า และความสุขของเด็กกำพร้ารูปแบบการวิจัย : การวิจัยครั้งนี้เป็นการวิจัยแบบสุ่มตัวอย่างเปรียบเทียบ (RandomizedControlled Trial; RCT) ระหว่างประชากร 2 กลุ่ม คือ กลุ่มทดลองและกลุ่มควบคุมในช่วงเวลาก่อนและหลังที่ประชากรจะได้รับการรักษาโดยการใช้กระต่ายช่วยบำบัดสถานที่ทำการศึกษา : สถานสงเคราะห์เด็กหญิงบ้านราชวิถีตัวอย่างและวิธีการศึกษา : แบ่งกลุ่มตัวอย่าง จำนวน 30 คน อายุ 12 - 15 ปี โดยใช้วิธีการสุ่มแบบเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 15 คน โดยกลุ่มทดลองเข้าร่วมกิจกรรมการบำบัดด้วยกระต่าย จำนวน 10 กิจกรรมเป็นเวลา5 วัน ส่วนกลุ่มควบคุมดำเนินชีวิตตามปกติ การศึกษานี้ใช้แบบวัดความรู้สึกเห็นคุณค่าในตนเอง แบบประเมินภาวะซึมเศร้าในเด็ก และแบบวัดความสุข โดยศึกษาระยะก่อนและหลังการทดลอง สถิติที่ใช้วิเคราะห์ข้อมูลใช้โปรแกรมสำเร็จรูป SPSS วิเคราะห์ความแตกต่างของอายุ คะแนนความรู้สึกเห็นคุณค่าในตนเอง คะแนนภาวะซึมเศร้าและคะแนนความสุขของทั้งสองกลุ่มในระยะก่อนการทดลองด้วยสถิติindependent sample t-test และโปรแกรม Stata SE 11.0 วิเคราะห์ผลของกิจกรรมการบำบัดด้วยกระต่ายในระยะก่อนการบำบัด หลังการบำบัด และระยะติดตามผล 1 เดือน ด้วยสถิติ Generalized EstimatingEquations (GEE)ผลการศึกษา : ก่อนเข้าร่วมกิจกรรมบำบัดด้วยกระต่าย กลุ่มทดลองและกลุ่มควบคุมมีคะแนนการเห็นคุณค่าในตนเอง ภาวะซึมเศร้า และความสุขไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ ที่ p - value เท่ากับ 0.494, 0.722และ 0.288 ตามลำดับ แต่ภายหลังการได้รับกิจกรรมการบำบัด พบว่ากลุ่มทดลองมีคะแนนการเห็นคุณค่าในตนเองเพิ่มขึ้น คะแนนภาวะซึมเศร้าลดลง และคะแนนความสุขเพิ่มขึ้น คือค่า p - value เท่ากับ0.514, 0.001 และ 0.003 ตามลำดับ ดังนั้นกลุ่มทดลองหลังจากเข้าร่วมกิจกรรมการบำบัดด้วยกระต่าย มีคะแนนภาวะซึมเศร้าลดลงและคะแนนความสุขเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05ซึ่งขนาดอิทธิพล (effect size) …

Objective: To determine the signs and symptoms of acute bacterial meningitis (ABM) in different age grops of a paediatric population.
Methods: The retrospective study comprised patients who had been admitted through the Emergency Department of Aga Khan University Hospital, Karachi with the relevant diagnosis from September 2009 to September 2011. Case record forms were used to collect data from patient files. PURPOSE: To evaluate the high-speed anterior segment optical coherence tomography (AS-OCT) grading of the AS inflammation in patients with ocular inflammation.
METHODS: A retrospective consecutive case series study. Patients with clinically visible AS inflammation in at least one eye underwent AS-OCT (Visante; Zeiss Meditec) with three to eight line scans per eye, performed by a trained masked examiner. The images were reviewed for hyperreflective spots, noise, and artifact, and these were correlated to clinical examination.
RESULTS: Seventy-eight eyes of 41 patients were imaged. Forty-seven eyes had anterior chamber cells on clinical examination, and 68 had hyperreflective spots visible on AS-OCT. …

Objective: The study was conducted to elicit the pathogenetic response of Hygrophila spinosa in homoeopathic potencies on healthy human volunteers. Methodology: The drug Hygrophila spinosa was proved by the Central Council for Research in Homoeopathy (CCRH) through randomized, double-blind, placebo-controlled method. The proving was conducted at three centres viz. Central Research Institute (H), [CRI (H)] Noida, Drug Proving Unit [DPU], Bhubaneswar and Regional Research Institute (H) [RRI (H)], Kolkata. Context: In patients with neurogenic lower urinary tract dysfunction due to Spinal Cord Injury (SCI), recurrent Urinary Tract Infections (UTI), is a frequently encountered clinical problem. Often, conventional preventive measures are not successful. Aims: To treat the patients of SCI suffering from recurrent UTI with classical homoeopathy as add-on to standard urologic care. As biotechnology advances rapidly, a tremendous amount of cancer genetic data has become available, providing an unprecedented opportunity for understanding the genetic mechanisms of cancer. To understand the effects of duplications and deletions on cancer progression, two genomes (normal and tumor) were sequenced from each of five stomach cancer patients in different stages (I, II, III and IV). Objective—Global health training opportunities have figured prominently into medical students’ residency program choices across a range of clinical specialties. To date, however, the national scope of global mental health education has not heretofore been systematically assessed. We therefore sought to characterize the distribution of global health training opportunities in U.S. graduate psychiatric education.
Methods—We examined the web pages of all U.S. psychiatry residency training programs, along with search results from a systematic Google query designed to identify global health training opportunities.

Fatigue in lower extremity musculature is associated with decline in postural stability, motor performance and alters normal walking patterns in human subjects. Automated recognition of lower extremity muscle fatigue condition may be advantageous in early detection of fall and injury risks. Supervised machine learning methods such as Support Vector Machines (SVM) have been previously used for classifying healthy and pathological gait patterns and also for separating old and young gait patterns. Psychiatry is increasingly combining new pharmacogenomic findings with therapeutic drug monitoring (TDM) to improve the safety and efficacy of pharmacotherapy. However, a distinction should be made between “nice to know” and “need to know” pharmacogenomic data because many results are statistically significant in meta-analyses but are not clinically relevant due to their low effect sizes. Some examples will illustrate this integration.

Nutrition is related to the mental well-being of clients. However, the nutritional views and practices of social workers in the mental health field have not been thoroughly explored. By using the post-positivist paradigm, 15 mental health social work students were interviewed for their perspectives regarding nutrition and nutrition’s effect on stress. Their main ideas were identified, connected, and organized to develop a theoretical statement. The resulting theory focused on the development of social work students' perspectives in applying nutrition into their mental health practice. BACKGROUND: Patients are increasingly reliant upon the Internet as a primary source of medical information. The educational experience varies by search engine, search term, and changes daily. There are no tools for critical evaluation of spinal surgery websites.
PURPOSE: To highlight the variability between common search engines for the same search terms. To detect bias, by prevalence of specific kinds of websites for certain spinal disorders.
