The Patient-Centered Medical Home (PCMH) is an emerging model of health care designed to provide a simpler, more effective health care experience. The model places heavy emphasis on the concept of every patient having a "personal physician" who is the point of access for all health care needs and concerns. The personal physician integrates all relevant health care information to provide the patient with a holistic picture of his health. Two splice sites on the human papillomavirus type 16 (HPV-16) genome are used exclusively by the late capsid protein L1 mRNAs: SD3632 and SA5639. These splice sites are suppressed in mitotic cells. This study showed that serine/arginine-rich protein 30c (SRp30c), also named SFRS9, activated both SD3632 and SA5639 and induced production of L1 mRNA. Activation of HPV-16 L1 mRNA splicing by SRp30c required an intact arginine/serine-repeat (RS) domain of SRp30c. In addition to this effect, SRp30c could enhance L1 mRNA production indirectly by inhibiting the early 3′-splice site SA3358, which competed with the late 3′-splice site SA5639. En bloc radical vulvectomy with bilateral inguinofemoral lymphadenectomy has now been replaced by radical wide excision and selective inguinal lymphadenectomy based on the stage and location of invasive vulvar cancer. Early stage lateral cancers can be effectively treated by radical wide excision and ipsilateral superficial inguinal lymphadenectomy. Lymph node mapping using perilesional injection of radiocolloid and blue dye may identify sentinel lymph nodes which can be removed, thereby avoiding the morbidity of full inguinal lymphadenectomy in selected patients with early stage disease.

Special-population research predicting physical activity (PA) and fitness with minority middle school children from at-risk environments is rare. Hence, the purpose of our investigation was to evaluate the ability of important social cognitive and environment-based measures to predict PA and fitness with children with developmental delay, cognitive, and emotional impairments. Children (N = 89, ages 11-15) completed questionnaires assessing social cognitive and environment-based constructs, self report PA, and completed fitness testing. Correlational results supported some hypotheses. The descriptive and correlational results also indicated commonalities with similar research on non special-population minority middle school children from at-risk environments.

BACKGROUND: In addition to conscientious hygiene and limiting exposure, vaccination is an important public health intervention to limit influenza infection among high risk groups. However, limited research has been conducted among the general population to measure vaccine acceptance against a pandemic strain of influenza, and which specific concerns might hinder acceptance. Because this study was conducted prior to the H1N1 pandemic of 2009, it serves as a baseline measure of the population's perspectives on influenza vaccination. Subsequent studies will shed light on the degree to which a pandemic heightens acceptance.

Many people in the United States have motor impairments due to stroke or other central nervous system (CNS) traumas. Motor impairments also burden the U.S. economy and health care system. Current rehabilitative therapies are moderately effective but can be improved upon. Improvements to rehabilitation protocols are informed by increased understanding of the CNS including understanding interactions between different structures in the CNS and understanding CNS plasticity in response to trauma or skill acquisition.

Universal precautions among healthcare workers (HCWs) and newborn vaccination programs are important public health interventions to reduce the spread of hepatitis and other blood borne pathogens. Objective: The purpose of this study was to examine the recent changes in multiple primary rules for breast and lung cancers and determine their effects on population incidence rates, illustrate specific patterns in multiple primary cancers for breast and lung cancers in New York and compare these patterns to SEER data, and to identify risks for developing multiple primary breast and lung cancers based on data in the New York State Cancer Registry (NYSCR).

This dissertation proposes a conceptual framework that integrates stakeholder and compliance theory and employs a multi-method approach to examine the influence of stakeholders, bureaucratic discretion and external environment on nursing home inspections and enforcement. Negative binomial regression analysis was conducted on a population of 655 nursing homes in New York to identify determinants of variation in compliance and enforcement (as measured by deficiency citations). As hypothesized, organizational characteristics (i.e., nursing home bed capacity, sponsorship, occupancy rates and Medicaid revenue) were significant predictors of regulatory compliance and quality of care. Past animal and human studies have shown that exposure to PCBs can result in a wide range of negative toxic effects. However, only a few studies have investigated the relationship between PCB exposure and attention and impulsivity. The present study examined the relationship between current body burden levels of PCBs and attention and impulsivity in young adults ages 17 to 21 from the Mohawk Nation of Akwesasne. The community is concerned because PCBs from industrial effluent have contaminated the local ecology and entered the Mohawk's food chain. A Breast Cancer Stem Cell Model Created From Mmtv-Pymt Mice Applicable To Human Breast Cancer, Denise Grant Lanza

Cancer stem cells are the seeds of tumor growth, but there are limited cell-based methods that exist to study the properties of these cells. To create a model of breast cancer stem cells, we isolated tumors from MMTV-PyMT mice. Two out of the four different cell types isolated survived in culture, CD44+CD24- and CD24+CD49f+CD44low. We found that we could initiate tumors with as few as 10 cells injected subcutaneously in the hind leg or orthotopically in the cleared fat pad with CD24+ cells. However, we could not initiate tumors with injection of CD24- cells. We found a requirement for TICs …

This working paper assesses the extent to which trade barriers, including traditional tariff barriers, non-tariff measures, and red tape, affect wage disparity and employment in Egypt. Using a microeconomic dataset, the author assesses to what extent different trade barriers affected wage disparity and employment in Egypt. This disparity is studied in three dimensions: on gender (males vs. females), qualification (blue vs. white collar workers), and regional (urban vs. rural workers). Functional properties of large conductance Ca2+ activated potassium (BK) channels are determined by complex alternative splicing of the Kcnma1 gene encoding the alpha pore-forming subunit. Inclusion of the STREX exon in a C-terminal splice site is dynamically regulated and confers enhanced Ca2+ sensitivity and channel inhibition via cAMP-dependent phosphorylation. Background: The human papillomaviruses (HPV) are a large family of non-enveloped DNA viruses, mainly associated with cervical cancers. Recent epidemiologic evidence has suggested that HPV may be an independent risk factor for oropharyngeal cancers. Evidence now suggests HPV may modulate the malignancy process in some tobacco- and alcohol-induced oropharynx tumors, but might also be the primary oncogenic factor for inducing carcinogenesis among some non-smokers. More evidence, however, is needed regarding oral HPV prevalence among healthy adults to estimate risk. The goal of this study was to perform an HPV screening of normal healthy adults to assess oral HPV prevalence.

BACKGROUND: Because of the rising expectation of prolonged life in the general population and the recent recognition of undertreated aortic valve disease in the elderly, updating the available results of aortic valve surgery is imperative, especially considering the rapid evolution of the transcatheter valve implantation procedure.

METHODS AND RESULTS: Between 1997 and 2010, 308 patients aged 70 years or older underwent aortic valve replacement (AVR) for aortic stenosis (AS). Short- and long-term results were analyzed and risk factors for long-term mortality were determined. Mean age was 78.5 years and 124 patients were aged 80 or older. This report from the Population Council study “Youth in India: Situation and Needs” presents data from in-depth interviews with mothers and fathers of youth in six states of India regarding communication patterns. The content of parent-child—and parent-daughter—communication is typically intended to ensure that young people conform to prevailing social, gender, and relationship norms. This Population Council working paper investigates the dynamics of the wage gap between men and women in Morocco. Estimates have shown that women earn up to 35 percent less than men in the periods observed. Differences in characteristics between the two sexes, mainly due to the difference in human capital endowments, are explained by only a small proportion of the gap. A negative proportion reported, though, is a kind of breakthrough for women, since it means that in terms of human capital skills, women are better qualified than their male counterparts. This study explores the extremely biased division of labor within Egyptian households. The effects of marriage on women’s market and domestic labor supplies are important aspects of this study conducted by the Population Council for its working paper series on gender and work in the Mideast-North Africa region. New explanations for married women’s low participation rates are proposed. A matching model is estimated to determine how selection into marriage alters the time allocation of women. This working paper reports on a study using Egyptian survey data to determine whether the effect of women's work on their status is mediated by the material transactions that accompany marriage. This perspective posits that marriage payments made to the bride, as well as assets she herself brings to the marriage, enhance her bargaining position. Analysis of the 2006 Egypt Labor Market Panel Survey panel data is used to explore the associations between women’s labor market activity, earnings, absolute and proportional marriage payments, and a measure of women’s status within marriage, namely their decisionmaking power.
