Rhinocerebral Zygomycosis In Pakistan: Clinical Spectrum, Management, And Outcome, Mubasher Ikram, Moghira Iqbal, Muhammad Aslam Khan, Erum Khan, Mahnaz Shah, Raymond A., Jr Smego

OBJECTIVE: To study the disease spectrum and salient management features of 36 patients with histopathologically-confirmed rhinocerebral zygomycosis seen at our academic center over a 16-year period.

METHODS: Retrospective review of patients admitted to the Aga Khan University Hospital in Karachi, Pakistan from January 1991 to December 2006 with histopathologically-confirmed zygomycosis of the head and neck.

RESULTS: Mean patient age was 40 +/- 5.0 years (range, 34-63 years), and 23 (64%) patients were male. Thirty-two (89%) patients were referred from clinical services other than otolaryngology. Underlying predisposing conditions included diabetes mellitus (21 patients), haematologic diseases (9), and renal failure (6). Twenty …

OBJECTIVES: To assess the performance of 3 risk scores from Japan that were developed to predict, in children with Kawasaki disease, resistance to intravenous immunoglobulin (IVIG) treatment.

STUDY DESIGN: We used data from a randomized trial of pulsed steroids for primary treatment of Kawasaki disease to assess operating characteristics of the 3 risk scores, and we examined whether steroid therapy lowers the risk of coronary artery abnormalities in patients prospectively classified as IVIG resistant.

Background: Previous research has linked alcohol use with an increased number of sexual partners, inconsistent condom use and a raised incidence of sexually transmitted infections (STIs). However, alcohol measures have been poorly standardised, with many ill-suited to eliciting, with adequate precision, the relationship between alcohol use and sexual risk behaviour. This study investigates which alcohol indicator - single-item measures of frequency and patterns of drinking ( > = 6 drinks on 1 occasion), or the Alcohol Use Disorders Identification Test (AUDIT) - can detect associations between alcohol use and unsafe sexual behaviour among male sex workers.

For months in 2003, the world lay under siege by a strain of virus that masqueraded as pneumonia but inflicted a far more lethal effect. By all accounts, the mystery of how the severe respiratory acute syndrome (SARS) virus came to be has remained largely unsolved (Bradsher & Altman 2003). The hypothalamic paraventricular nucleus (PVN) regulates numerous homeostatic systems and functions largely under the influence of forebrain inputs. Glutamate is a major neurotransmitter in forebrain, and glutamate neurosignaling in the PVN is known to mediate many of its functions. Previous work showed that vesicular glutamate transporters (VGluTs; specific markers for glutamatergic neurons) are expressed in forebrain sites that project to the PVN; however, the extent of this presumed glutamatergic innervation to the PVN is not clear. This study combined the use of Loving-Kindness meditation with art therapy for three women suffering from different types of chronic pain. These women ranged in age from 26 to 62 years and were from a variety of backgrounds; two of the women were Caucasian and one woman was originally from Barbados. The participants were recruited from the Department of Physical Medicine and Rehabilitation at Eastern Virginia Medical School. Four sessions were scheduled. Osteomas are benign tumors composed of mature compact or cancellous bone. They represent an uncommon lesion that occurs mainly in craniofacial complex bones. In jaws, they can appear on the bone surface as a polypoid or sessile mass, characterizing a peripheral osteoma (PO), or can be a lesion in the medullar space, then it is called central osteoma. In view of the scarcely reported cases about POs, this article presents a case of PO of the maxillofacial area that was surgically resected using hemicoronal approach.

Depression and other significant mental and behavioral health (MBH) problems have not been well documented in frontier areas of Idaho. It has been suggested that residents of frontier areas access their primary care provider (PCP) for most of their general health and MBH needs. To determine PCPs’ perceptions of MBH issues in frontier areas of Idaho, surveys were sent to all 252 PCPs with a registered practice location in one of Idaho’s 26 frontier counties. Parental satisfaction refers to the extent to which parents are satisfied with their children in relation to parent-child interactions and child behavior. The relationship between parental satisfaction and child behavior problems has been demonstrated extensively in the literature. Children who exhibit increased behavior problems appear to be at increased risk of parental aggression. Maltreating parents evidence greater levels of parental dissatisfaction as compared to caregivers of children who are not maltreated. Thus, low parental satisfaction is a suspected risk-factor for child maltreatment. Little is known about mediators of treatment-seeking in dyspareunia. The general health belief literature as well as some existing qualitative data specific to dyspareunia, however, suggests a number of potentially significant barriers that may delay or prevent women from enlisting the aid of health care professionals. The aim of this study was to investigate influences on dyspareunia treatment-seeking behavior in young women, for whom the consequences of treatment avoidance are hypothesized to be the greatest. Duties of police officers include apprehending suspects, which may involve foot pursuit and firing a weapon at a suspect. Previous studies in biathletes and soldiers suggest that shooting performance is compromised when fatigue is present. Purpose: To investigate the effects of exercise-induced fatigue on shooting performance in police officers. Methods: Nine police officers fired five rapid-fire shots at three targets each with a fifteen second rest period in between each trail. Officers then exercised on a cycle ergometer to 85% maximum heart rate or voluntary fatigue. The pretest shooting was repeated post-exercise. Distances of 4 shots from the center of …

BACKGROUND: In 1976, Hawk and Hazard described the tall cell variant (TCV) of papillary thyroid carcinoma (PTC). While the lesions they described had cytologic features of papillary carcinoma, they showed more aggressive behavior with a greater propensity for extrathyroid extension and lymphovascular invasion than classic PTC. In 1991, Bronner and LiVolsi described a series of patients with TCV that progressed to spindle cell squamous carcinoma (SCSC), a unique form of anaplastic thyroid carcinoma. This study describes the variable clinical and pathologic presentations in 31 patients with anaplastic SCSC arising in association with TCV.

In the fight against HIV/AIDS, the Behavior Change Communication (BCC) model stands as international best practice in preventive education. Ideally, a BCC intervention aims to changes behaviors and attitudes by facilitating group negotiation and introspection, with a resultant improvement in health. However, introducing this best practice model to a group of youth in Nairobi resulted in suboptimal outcomes: they were unaccustomed to expressing themselves in the ways prescribed by the class. The purpose of this study was to examine the relationships between mother-son connectedness, substance use, serious delinquency, violent delinquency, and criminal justice system involvement in young men. This study uses data from The National Longitudinal Study of Adolescent Health (ADD Health), a school-based study of students in grades 7–12 in the United States. Following DNA damage, nuclear p53 induces the expression of PUMA (p53 upregulated modulator of apoptosis), a BH3‑only protein that binds and inhibits the anti‑apoptotic BCL‑2 repertoire, including BCL‑xL. Structural investigations of PUMA and the BCL‑xL×PUMA BH3 domain complex by X‑ray crystallography and nuclear magnetic resonance (NMR) spectroscopy reveal a novel, PUMA‑induced, domain‑swapped dimerization of BCL‑xL that requires a π‑stacking interaction between PUMA W71 and BCL‑xL H113. PUMA is an intrinsically disordered protein, but upon interaction with BCL‑xL, PUMA W71 and the PUMA BH3 domain residues fold into an alpha helix and subtly remodel BCL‑xL to trigger its dimerization. The vast majority of periodontal pathogens are classified as gram negative bacteria. These bacteria have an outermost membrane consisting of lipopolysaccharide (LPS), a molecule that can act as an endotoxin and elicit an immune response. The lipopolysaccharide structure consists of three major regions: oligosaccharide, core, and lipid A. Oligosaccharide is exposed on the cell surface, and lipid A anchors LPS in the outer membrane. It has a wide array of biological effects like cell proliferation, survival, migration, apoptosis invasion, wound healing and angiogenesis. Autotaxin (ATX) was identified as an autocrine tumor cell motility factor from A2058 melanoma conditioned medium. ATX has lysophospholipase D enzyme activity and is responsible for the cleavage of lysophophatidylcholine (LPC) leading to the generation of LPA. The primary objective of this study was to determine the effects of age, gender and vowel height on the temporal and aerodynamic aspects of nasal airflow segments related to velar coarticulation in the normal speech of children and adults. Ex vivo gene transfer has been used to improve the outcome of islet transplantation for treating type I diabetes. RNA interference is an effective approach for reducing gene expressions at the mRNA level. The application of RNA interference to improve the outcome of islet transplantation was reviewed in Chapter 2, where I summarized biological obstacles to islet transplantation, various types of RNAi techniques, combinatorial RNAi in islet transplantation, and different delivery strategies.

Tumor angiogenesis, the creation of new blood vessels, is the characteristic of solid tumors and crucial for their development. Iodinated contrast agents are used to increase the X‑ray detectability of zone of angiogenesis, and thus providing a means for tracking tumor growth.

The overall objective of this project was to evaluate the performance of Kodak CR 7400, a high‑resolution compact computed radiography (CR) system in detection of Omnipaque‑240, an iodinated contrast agent, in a phantom mimicking small animal tumor model.

The facet joints of the lumbar spine are one of the sources of low‑back pain that affects a great portion of the population. Minimally‑invasive (MI) procedures have been becoming more popular in the surgical decompression of the spine because they offer shorter recovery time and involve removal of smaller amounts of important structures. Emergency physicians are key users of health information exchanges (HIE). Understanding their perspectives on the usability of HIE is important if the full potential of the HIE is to be achieved. The literature identified that emergency physician experiences with HIEs are unexplored areas requiring further studies. The purpose of this study using grounded theory methods was to understand the perspectives of emergency physicians concerning the usability of HIEs. Spectinamides are novel amide derivatives of the antibiotic spectinomycin that have emerged as a new class of agents to treat tuberculosis. These agents showed potent in vitro activity against Mycobacterium tuberculosis (MTB) compared to spectinomycin and in a preliminary in vivo study in interferon gamma (IFN‑γ) knockout mice, spectinamide Lee1329 reduced the lung bacillary load of TB comparable to streptomycin. We hypothesized that the application of an iterative pharmacokinetics and pharmacodynamics (PK/PD) guided approach would facilitate the optimization of these lead compounds suitable for further development.
