The study was to evaluate the effect of pre-operative education for diaphragmatic breathing and coughing techniques in elective open heart surgery patients on the post-operative pulmonary complications, ventilator use duration and post-operative hospital days.The retrospective chart reviewarticle of the 27 elective open heart surgery patients was from March 2008 to April 2009. Background and Purpose: Oxygen uptake efficiency slope (OUES) is an index for evaluating cardiorespiratory fitness without performing maximal exercise and correlates well with the individual's peak oxygen uptake (□O(superscript 2peak)). The purpose of this study was to determine the clinical usefulness of OUES for determining the cardiorespiratory fitness of obese adults without cardiovascular diseases by investigating (1) the correlation between OUES and weight (2) the change in the OUES after a weight-reduction program. Delayed spinal epidural hematoma (SEH) after a spinal compression fracture is a very rare but important cause of symptomatic spinal cord compression. We report a patient with spinal cord compression and cauda equina syndrome due to delayed SEH after a traumatic T12 compression fracture. A 74-year-old male patient had severe back pain without neurologic deficits after a traumatic T12 compression fracture. He developed progressive bilateral lower extremity weakness, sensory loss, and sphincter dysfunction 2.5 months after the injury. Magnetic resonance imaging of the spine revealed a huge spinal epidural hematoma at the level of T12 to L5. He then received …

Objective: To describe a patient with the critical illness polyneuropathy superimposed on diabetic neuropathy and to highlight the importance of early rehabilitation after critical illness. Methods: A 42-year-old man with a previous history of diabetic polyneuropathy developed flaccid tetraplegia with respiratory failure after septic shock due to a liver abscess. This clinical manifestation, combined with the result of an electrodiagnostic study, suggested the acute exacerbation of polyneuropathy. Because a pre-existing polyneuropathy history complicated a differential diagnosis, we performed a comprehensive work-up to rule out other systemic diseases or metabolic disorders that may cause polyneuropathy. This is a casereport about a 53-year-old female patient with a history of chronic back pain, suffered from complete paraplegia shortly after spinal manipulation. CT and MRI images consistently showed ossification of the ligamenta flava in conjunction with bilateral facet joints hypertrophy at the T10-11, T11-12 and L1-2 levels. Spinal cord injury with T12 paraplegia ASIA-A was thus diagnosed. Posterior laminectomy at the T10, T11, L1 and L2 levels and interbody fusion from T10 to L2 spines for fixation were performed. Although not of high incidence, it is an important cause of brain hemorrhage or infarction in pregnant women. According to previous studies, the most common symptom is headache. Numerous serotonergic drugs have been increasingly used in recent years to treat depression, pain, insomnia and obesity. However, concomitant use of these drugs may cause serotonin syndrome. The three main clinical features of serotonin syndrome are neuromuscular abnormalities, autonomic hyperactivity, and mental status changes. A wide variety of the clinical presentations range from: very mild symptoms such as tremor, diarrhea, muscle rigidity, or clonus, and hyperreflexia; to fatal consequences such as disseminated intravascular coagulation or multiple-organ failure. Brain abscess is a neurological emergency that requires immediate high-dose antibiotics or surgical intervention. Brain abscess at the site of a previous hemorrhagic stroke that did not receive surgical intervention is rare. Here, we report the case of a 48-year-old man who developed a brain abscess as infective transformation of a preceding right putaminal hemorrhage, 22 days after the stroke. The patient recovered after surgical treatment and systemic antibiotics therapy. Macrodystrophia lipomatosa is a rare congenital anomaly of localized gigantism, characterized by progressive overgrowth of all mesenchymal components, particularly fibroadipose tissue. The anomaly occurs frequently in the distribution regions of the median nerve in the upper extremity and the plantar nerve in the lower extremity. It is usually noticed at birth with predominately unilateral distal involvement of the digits. The cause of this condition is still unknown. For decades, ultrasonography has shown a high degree of accuracy in the evaluation of soft tissue tumors. In this study, we report the condition of a 80-year-old male patient who had been suffering from left thumb pain for 2 years. The patient had initially been diagnosed with de Quervain's disease, but responded poorly to conservative treatment. An ultrasonographic examination unexpectedly identified a well-margined solid tumor with marked vascularity. Suspicion of a neurogenic tumor led to an excision surgery and finally pathology revealed a Schwannoma. Following excision, the symptoms were entirely ameliorated. Multi-gene interactions likely play an important role in the development of complex phenotypes, and relationships between interacting genes pose a challenging statistical problem in microarray analysis, since the genes involved in these interactions may not exhibit marginal differential expression. As a result, it is necessary to develop tools that can identify sets of interacting genes that discriminate phenotypes without requiring that the classification boundary between phenotypes be convex.

Background:
Not much is known about the risk factors of major depressive disorder (MDD) in HIV/AIDS in the African socio-cultural context. Therefore a study was undertaken to examine the prevalence and risk factors of MDD in HIV/AIDS in semi-urban Uganda.
Methods:
A cross-sectional study was undertaken among 618 respondents attending two HIV clinics in Uganda.
Results:
Prevalence of MDD was 8.1%. Factors associated with MDD at univariate analysis only were female gender, family history of mental illness, negative coping style, alcohol dependency disorder, food insecurity and stress; not associated with MDD were social support, neurocognitive impairment, CD4 counts and BMI. …

Purpose: To determine the role of duplex Doppler ultrasonography (DDU) in Patients with acute unilateral renal obstruction. Subjects And Methods:A total of 161 Patients with suspected renal colic due to urolithiasis were evaluated by DDU followed by intravenous urography (IVU). The mean intra-arterial resistive index (RI) and the difference of mean resistive index between both kidneys (delta RI) were determined for each person. An RI value of ≥0.70 and a delta RI value of ≥0.06 were taken as the discriminatory threshold for obstruction. IVU results were considered the " reference standard" against which renal DDU findings were compared.

In investigating the determinants of survival of prostate cancer cohorts, characterization of the cohort in terms of competing underlying causes of death would be appropriate. Atherosclerotic heart disease, bronchitis, acute myocardial infarction, chronic obstructive pulmonary disease, atherosclerotic cardiovascular disease, and stroke were found to be the major causes of death among prostate cancer cohorts aside from prostate cancer. Podocytes are highly differentiated and polarized epithelial cells located on the visceral side of the glomerulus. They form an indispensable component of the glomerular filter, the slit diaphragm, formed by several transmembrane proteins and adaptor molecules. Disruption of the slit diaphragm can lead to massive proteinuria and nephrotic syndrome in mice and humans. CD2AP is an adaptor protein that is important for the maintenance of the slit diaphragm. Together with its paralogue, CIN85, CD2AP belongs to a family of adaptor proteins that are primarily described as being involved in endocytosis and downregulation of receptor tyrosine kinase activity. Institute for Veterans and Military Families
This brief is about the stress that occurs for caregivers of veterans due to secondary trauma. In policy and practice, caregivers can continuously monitor themselves for symptoms of secondary trauma, maintain a balance between professional life and personal life, and promote a supportive culture within the caregiver community; policies should acknowledge that secondary trauma stressors do exist for caregivers and should look for ways to reduce the negative effects of secondary trauma stressors. Suggestions for future research include using econometric models to detect relevant factors for risk of developing secondary trauma stressors.

Objective: To observe interdepartmental variation in the availability of resources and academic activities within the various neurosurgery programs of Pakistan.
Methods: This was a proforma-based survey of neurosurgery trainees and young neurosurgeons of Pakistan, looking at the academic infrastructure and output of their programs. The proforma was filled by 36 respondents from 11 neurosurgery centers of the country. All these centers were accredited for neurosurgery training in Pakistan.
Results: Out of the 36 respondents, 30 were completing a Fellowship training (FCPS) and six were enrolled for a Master in Surgery (MS) program. About 80% of the participants used the Youman's …

Kesehatan maternal yang tergolong tinggi di Indonesia merupakan indikator keberhasilan pembangunan pada Millenium Development Goals yang terus diupayakan untuk diperbaiki. Penelitian ini bertujuan mengetahui hubungan antara karakteristik sosial, demografi, dan ekonomi dengan tingkat kesehatan ibu. Penelitian yang menggunakan sumber data sekunder Survei Demografi Kesehatan di Jawa Barat yang mencakup 86 blok sensus yang meliputi 2.150 rumah tangga meliputi 1.100 rumah tangga di perkotaan dan 1.050 rumah tangga di pedesaan. Demensia Alzheimer menempati urutan kesembilan penyebab kematian di Amerika Serikat. Demensia adalah kondisi yang sering dialami yang berhubungan dengan berbagai faktor dan gaya hidup terutama diet. Penelitian ini bertujuan untuk mengetahui hubungan defisiensi asupan riboflavin (vitamin B12) dengan demensia pada usia lanjut (usila). Penelitian ini menggunakan desain studi cross sectional dan dilaksanakan pada bulan September 2007 sampai dengan Januari 2008. Sampel penelitian adalah 141 lansia berumur lebih dari sama dengan 60 tahun yang diambil secara purposive sampling. Beberapa tahun terakhir angka obesitas di Indonesia cenderung meningkat. Obesitas terjadi akibat ketidakseimbangan antara asupan dan output energi dalam jangka waktu cukup lama. Tujuan penelitian ini adalah untuk mengetahui faktor risiko kejadian obesitas pada pekerja on shore di perusahaan minyak dan gas. Desain penelitian ini adalah studi cross sectional. Sampel yang diteliti adalah seluruh pekerja on shore (laki-laki maupun perempuan) berusia 35 – 55 tahun yang sesuai dan memenuhi kriteria inklusi dan ekslusi yang telah ditetapkan (n = 378). Provinsi Nusa Tenggara Timur (NTT) memiliki Total Fertility Rate (TFR) tertinggi (4,2 anak) dan yang terendah adalah Provinsi Yogyakarta (1,8 anak). Tujuan penelitian ini adalah mengidentifikasi faktor-faktor yang berhubungan dengan angka fertilitas total di kedua provinsi tersebut. Anak penderita Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) di Indonesia memperlihatkan tren yang semakin meningkat karena meningkatnya proporsi perempuan terinfeksi HIV/AIDS. Pertimbangan pada dampak besar yang dihadapi anak penderita HIV mendorong kebutuhan pengembangan instrumen khusus untuk mengukur kualitas hidup mereka. Penelitian ini bertujuan mengembangkan instrumen kualitas hidup anak penderita AIDS dengan memodifikasi instrumen yang ada sesuai dengan konteks Indonesia. Keberhasilan program Keluarga Berencana Nasional tidak hanya diukur dari peningkatan peserta program, tetapi juga efektivitas dan durasi pemakaian kontrasepsi. Penelitian ini bertujuan menilai pengaruh kualitas pelayanan KB terhadap perilaku penggantian alat kontrasepsi di Indonesia. Penelitian sumber data sekunder histori pemakaian metode kontrasepsi dalam kalender data SDKI tahun 2007 dengan metode analisis regresi logistik dengan efek random. High dietary fat intake is a major risk factor for development of cardiovascular and metabolic dysfunction including obesity, cardiomyopathy and hypertension.
Methods
The present study was designed to examine effect of high fat (HF) diet on cardio-vascular structure and function in spontaneously hypertensive rats (SHR), fed HF diet for 15 weeks, a phenotype designed to mimic metabolic syndrome.
Results
Development of metabolic syndrome like phenotype was confirmed using parameters, including body weight, total cholesterol and blood pressure levels.
