วัตถุประสงค์ เพื่อศึกษาถึงผลของซีลเลอร์อุดคลองรากฟันชนิดที่มียูจีนอลเป็นส่วนผสมต่อค่าความแข็งแรงเฉือน ของเรซินคอมโพสิตกับเนื้อฟันรวมทั้งเปรียบเทียบผลของวิธีการทําความสะอาดเนื้อฟันที่ปนเปื้อนด้วยซีลเลอร์โดยการเปรียบเทียบค่าความแข็งแรงเฉือนหลังจากการทําความสะอาดผิวฟันด้วยวิธีต่าง ๆ กัน 3 วิธี วัสดุและวิธีการ นําฟันกรามแท้ซี่ที่ 3 ของมนุษย์ที่ไม่มีรอยผุ จํานวน 50 ซี่ ที่ถูกถอนออกมาตัดบริเวณด้าน บดเคี้ยวจนลึกถึงชั้นเนื้อฟันให้ได้แนวระนาบ ฝังฟันลงในท่อพลาสติกด้วยอะคริลิกเรซิน โดยให้ผิวฟันด้านที่ถูกตัด อยู่ระดับเดียวกันและขนานกับขอบของท่อพลาสติก แบ่งฟันที่เตรียมเสร็จแล้วจํานวน 5 กลุ่ม ๆ ละ 10 ซี่ ด้วยวิธี สุ่มอย่างง่าย ผิวหน้าฟันของกลุ่มที่ 1 (กลุ่มควบคุมลบ) ไม่ถูกทาด้วยซีลเลอร์อุดคลองรากฟันชนิดที่มียูจินอลเป็น ส่วนผสม ส่วนในกลุ่มที่ 2-5 ที่ผิวหน้าของฟันถูกนํามาทาด้วยซีลเลอร์ และทําความสะอาดผิวฟันด้วยวิธีต่างๆ ดังนี้ กลุ่มที่ 2 เช็ดออกด้วยสําลีแห้ง (กลุ่มควบคุมบวก) กลุ่มที่ 3 เช็ดออกด้วยสําลีชุบแอลกอฮอล์ความเข้มข้น ร้อยละ 70 กลุ่มที่ 4 เช็ดออกด้วยสําลีชุบกรดอะซิติกความเข้มข้นร้อยละ 5 และกลุ่มที่ 5 เช็ดออกด้วยสําลีชุบ แอซีโทน ภายหลังจากที่ทําความสะอาดผิวหน้าฟันแล้วจึงก่อเรซินคอมโพสิตบนผิวฟันให้เป็นทรงกระบอก (เส้นผ่านศูนย์กลาง 5 มิลลิเมตร สูง 2 มิลลิเมตร) โดยการใช้สารยึดติดระบบโททอลเอทช์ ฟันที่บูรณะเสร็จแล้ว นํามาทดสอบด้วยเครื่องทดสอบความแข็งแรงเฉือนที่ความเร็วหัวกด 0.5 มิลลิเมตรต่อนาที ผลการศึกษา ค่าเฉลี่ยความแข็งแรงเฉือนของกลุ่มที่ 4 มีค่าเฉลี่ยมากที่สุดคือ 18.59 เมกะพาสคาล ตามมาด้วย กลุ่มที่ 1 (17.98 เมกะพาสคาล) ในขณะที่กลุ่มที่ 2 ให้ค่าเฉลี่ยน้อยที่สุด (14.42 เมกะพาสคาล) ผลการวิเคราะห์ ทางสถิติโดยการใช้การวิเคราะห์ความแปรปรวนทางเดียว พบว่ากลุ่มที่ 1 และกลุ่มที่ 2 กลุ่มที่ 2 และกลุ่มที่ 4 กลุ่ม ที่ 3 และกลุ่มที่ 4 แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p < 0.05) สรุป การปนเปื้อนด้วยซีลเลอร์ชนิดที่มียูจินอลเป็นส่วนผสม และวิธีการทําความสะอาดเนื้อฟันแบบต่าง ๆ มีผลต่อค่าเฉลี่ยความแข็งแรงเฉือนโดยวิธีการทําความสะอาดเนื้อฟันที่ปนเปื้อนด้วยซีลเลอร์ชนิดที่มียูจินอลเป็น ส่วนผสมด้วยการเช็ดออกด้วยสําลีชุบกรดอะซิติกความเข้มข้นร้อยละ 5 เป็นวิธีที่มีประสิทธิภาพมากที่สุด (ว ทันต จุฬาฯ 2554;34:203-12)

Recent research has shown the possibility of pain perception in fish; therefore, the use of analgesia or “painkillers” should be considered for invasive procedures. However, there is relatively little information on the effectiveness of analgesic drugs nor on the appropriate dose for fish. This study assessed the efficacy of three types of drug: an opioid, buprenorphine, a non-steroidal anti-inflammatory drug (NSAID), carprofen and a local anaesthetic, lidocaine. Each drug was tested at three doses on rainbow trout that were noxiously stimulated and the most effective dose was also given to fish experiencing no pain to investigate side-effects. The purpose of this study was to examine the effects of (a) skill test type, (b) choices, and (c) gender on the situational motivation profiles of adolescents during skill testing in physical education. Participants were 507 students (53% male) aged 12–16 years (M = 13.87; SD = 0.94) attending a suburban junior high school in a western state in the U.S. All participants experienced either a norm-referenced, summative or a criterion-referenced, formative skill test with or without choices. The acoustic startle response is a protective response, elicited by a sudden and intense acoustic stimulus. Facial and skeletal muscles are activated within a few milliseconds, leading to a whole body flinch in rodents. Although startle responses are reflexive responses that can be reliably elicited, they are not stereotypic. They can be modulated by emotions such as fear (fear potentiated startle) and joy (joy attenuated startle), by non-associative learning processes such as habituation and sensitization, and by other sensory stimuli through sensory gating processes (prepulse inhibition), turning startle responses into an excellent tool for assessing emotions, learning, and sensory gating. …

PURPOSE: To describe African American women's experience of being at high risk for breast cancer.

DESIGN: A hermeneutic phenomenological approach was used to guide in-depth interviews and analysis. Methods to ensure trustworthiness and rigor were included.

METHODS: Open interviews were conducted with 20 African American women who were at high risk for breast cancer (family history, personal history, genetic mutation). They were recruited from a cancer risk clinic and community-based settings. Data were transcribed verbatim, and themes were labeled among and between all interviews.

Blast attacks are the commonest form of terrorist attack on civilians. Blast often causes injury from industrial accidents. Blast injury is the most common cause of injury and death in modern combat. A recent review of Canadian fatal casualties in Afghanistan found 81% to have been caused by blast, almost four times the number killed by gun shot wounds. Improvised explosive devices (IED) caused 78% of the blasts with another 20% being due to rocket propelled grenades (RPG).1 The purpose of this review is to outline the mechanisms of injury, to define the surgical response and to understand the implications …

B-Cell Lymphoma Of The Brainstem With Central Neurogenic Hyperventilation, S. Ather Enam, Rushna Ali

Non-Hodgkin's lymphoma of the brainstem is a rare entity. Central Neurogenic Hyperventilation (CNH), an associated manifestation of this disease, is an even rarer event. We report a case of an immunocompetent individual who presented to us with tachypnea and facial nerve palsy. Neuroimaging showed a Cerebellopontine angle tumour which on histopathology showed feature consistent with a Non-Hodgkin's B-Cell Lymphoma. The patient went on to develop severe respiratory alkalosis with findings consistent with CNH. Chemotherapy with Methotrexate was started and high dose Dexamethasone was added to the regimen a month later. Radiologically, the tumour size decreased by 50% but the patient's …

Objectives: To determine the frequency of White Coat Hypertension in patients undergoing ambulatory blood pressure monitoring at a tertiary care center and to compare ambulatory blood pressure profiles of normotensives, white coat hypertensives and hypertensives.
Methods: A descriptive cross-sectional study was conducted which included all adult patients undergoing ambulatory blood pressure monitoring over a 3-year period. Those patients with incomplete data, less than 85% successful BP readings and inadequate number of daytime and nighttime readings were excluded from the study. The recent RAF inhibitor trial with PLX4032/RG7204 in late-stage mutant B-RAF melanoma patients has been lauded as a success story for personalized cancer therapy since short-term clinical responses were observed in the majority of patients. However, initial responses were followed by subsequent tumor re-growth, and a subset of patients showed intrinsic resistance. Bi-directional translational efforts are now essential to determine the mechanisms underlying acquired/secondary and intrinsic resistance to RAF inhibitors.

Scleromyxedema is a systemic disease characterized by lichenoid papules, nodules, and plaques on the skin and often diffuse skin induration resembling the cutaneous involvement of systemic sclerosis. The systemic involvement affects the musculoskeletal, pulmonary, cardiovascular, gastrointestinal, and central nervous systems, and the disorder is commonly associated with a paraproteinemia. Involvement of the kidney is rare and not considered a feature of the disease. Here, we describe an unusual case of scleromyxedema complicated by the development of scleroderma renal crisis-like acute renal failure with a marked intimal deposition of mucin, mucopolysaccharides, and hyaluronic acid in the intrarenal vessels.

Data on road traffic accident (RTA) injuries and their outcome are scarce in Pakistan. This study assessed patterns of RTA injuries reported in Rawalpindi city using standard surveillance methods. All RTA injury patients presenting to emergency departments of 3 tertiary care facilities from July 2007 to June 2008 were included. RTA injuries (n = 19 828) accounted for 31.7% of all injuries. Among children aged 0-14 years females suffered twice as many RTA injuries as males (21.3% versus 11.4%), whereas this trend reversed for the age group 15-24 years (41.9% versus 21.7%). One-fifth of injuries were either fractures or concussion. …

Primary adenocarcinoma of appendix is an uncommon gastrointestinal malignancy. Similarly, Psoas abscess is an entity, which sometimes requires extensive work-up to reach a diagnosis. Combined presentation of these two rare conditions is not only exceptional, but also diagnostically challenging. We present a case of a lady who presented with recurrent UTI secondary to right sided ureteric obstruction, referred right hip joint pain resulting from a Psoas abscess, which eventually turned out to be a consequence of metastatic adenocarcinoma of appendix, causing pseudomyxoma extraperitonei, with simultaneous intraperitoneal deposits.

Despite impressive numbers of hockey participants, there is little research examining elite female ice hockey players. Therefore, the purpose of this study was to describe the physical characteristics of elite female ice hockey players who were trying out for the 2010 U.S. Women's Ice Hockey team. Twenty-three women participated in the study and were evaluated on: body mass (kg), height (cm), age (y) vertical jump (cm), standing long jump (cm), 1 RM front squat (kg), front squat/body mass (%), 1 RM bench press (kg), bench press/body mass (%), pull ups, and body composition (% body fat). The thermodynamic properties of a model infrared heat reflective helmet were evaluated using an advanced thermal manikin technology. The aluminized model helmet was tested for infrared (IR) radiation attenuation properties. Total manikin heat gain and changes in surface temperature were documented for controlled IR radiation exposure levels. The results illustrate the benefits offered by an aluminized reflective surface in attenuating IR radiation and the advantages of using a spacer harness system to minimize radiant heat transfer from the helmet to the head.

Respiratory dyskinesia is an under-recognized side effect of neuroleptic administration. There are only few studies that have addressed the prevalence of respiratory dyskinesia in patients with tardive dyskinesia. Our case report highlights the need to regularly examine patients on antipsychotics for any evidence of dyskinetic movements including respiratory musculature. Since RD is underrecognized and misdiagnosed, early detection can improve long term prognosis as treatment options are few and usually of only limited effect. Recent research has demonstrated body mass (M) bias in military physical fi tness tests favoring lighter, not just leaner, service members. Mathematical modeling predicts that a distance run carrying a backpack of 30 lbs would eliminate M-bias. The purpose of this study was to empirically test this prediction for the U.S. Army push-ups and 2-mile run tests. Two tests were performed for both events for each of 56 university Reserve Offi cer Training Corps male cadets: with (loaded) and without backpack (unloaded). Results indicated signifi cant M-bias in the unloaded and no M-bias in the loaded condition for both events. …

Family members are important to the well-being of their relatives with substance use disorders or cooccurring substance use and mental disorders. Many caregivers experience high levels of burden, negatively impacting their capacity to provide support to their ill family member. The Andersen health care utilization model (Andersen & Newman, 1973, 2005) was used to identify the impact of predisposing, enabling, and need factors hypothesized to predict caregivers’ likelihood of asking for help and support with their caregiving role. The sample include 82 women recruited from outpatient or inpatient substance abuse treatment centers and 82 family caregivers nominated by these women. …

The proliferation signal inhibitors (PSIs)-sirolimus, everolimus, and temsirolimus-have been associated with a noninfectious pneumonitis characterized by lymphocytic alveolitis and bronciolitis obliterans with organizing pneumonia (BOOP). This condition usually occurs within the first year. Herein we presented a case of a deceased donor renal transplant with interstitial pneumonitis developing 6 years after a switch from tacrolimus to sirolimus due to chronic graft dysfunction. After the addition of intravenous pentamidine due to the suspicion of Pneumocystis pneumonia, there was marked clinical deterioration requiring intubation. Open lung biopsy revealed sirolimus-induced pulmonary toxicity (BOOP) with the additional finding of a drug-induced phospholipidosis (DIPL) that …
