Clinical presentation and complaints of the vulvo-vaginal area are quite prevalent in the pediatric population and the primary care providers the first individuals called upon to evaluate the patients. One of the most difficult tasks for an individual that rarely performs the genitourinary examination is performing one on a pediatric patient. This chapter has been written to aid the primary care provider in successful examination, documentation and diagnosis.

Ovarian tumors are uncommon in the child and adolescent population, but given that cancer is a leading cause of death even in this age group it is important for the general practice physician to be aware of the signs and symptoms. These can include abdominal pain, increased abdominal girth, a palpable mass on pelvic or abdominal exam, nausea and vomiting, decreased appetite, weight loss, constipation, back pain, leg cramps, dyspareunia, urinary symptoms, abnormal uterine bleeding, and ascites, to name a few. Contraception is an important concept for sexually active adolescent females to understand who do not wish to become pregnant. This chapter reviews important, effective, and safe contraceptive methods that can be used by female youth. These methods include abstinence, oral contraception, contraceptive patch. mini-pills, emergency contraceptives, injectable contraception (DMPA), implants (lmplanon), intravaginal ring, vaginal barrier contraceptives, intrauterine devices, and natural family planning. Promotion of sexual responsibility should be the charge of health professionals caring for adolescents and young adults.

Concerns about problems of the breast are often noted in adolescents and their parents. This review discusses issues and disorders of the breast in children and adolescents, starting with basic principles of embryology and adolescent breast development. Concepts that are covered include congenital breast disorders, abnormal timing of breast development, breast asymmetry, underdeveloped breasts, breast atrophy, tuberous breasts, mammary hyperplasia, fibroadenoma, giant fibroadenoma, cystosarcoma phyllodes, intraductal breast papilloma, adenocarcinoma, mastitis, traumatic breast disorders, benign breast disease, fibrocystic change, mastalgia, gynocomastia, and galactorrhea. Onset of menstruation (i.e., menarche) is a determining event of the adolescent female signifying the progress of puberty, which in the majotity of cases is visibly initiated with thelarche (onset of breast development or breast buds). Menarche usually occurs during the earlier time of the second decade of life and ends with menopause, typically in the 5th decade of life. The adolescent can present with a wide variety of menstrual dilemmas and disorders which will be discussed in this chapter.

Steroid hormones, such as oestrogen, mediate their effects via activation of oestrogen regulated genes using nuclear receptors. Selective oestrogen receptor modulators (SERMs), such as Tamoxifen, are used to treat oestrogen responsive breast cancers, hctioning to act as oestrogen antagonists, preventing the oestrogen receptor biding DNA and blocking gene expression. However, Tamoxifen has been identified as an oestrogen agonist in other tissues which can often lead to secondary tumors in the years following the treatment. The CDC estimates that one in 20 patients admitted to the hospital is a carrier of methicillin-resistant Staphylococcus aureus (MRSA). Staphylococci are commonly found on the skin and mucous membranes within the anterior nares, which provides the principle reservoir for this organism. These organisms can go on to cause surgical site infections in hospitalized patients. Mupirocin is an effective topical medication used to eliminate nasal carriage of Staphylococcus aureus (S. aureus). This study examined the rates and risks of premature neonates >2500grams developing intraoperative non-euglycemia events (IONEE). A retrospective chart review of 26 premature neonates >2500 grams who underwent surgical procedures between January 1 and December 31, 2009 was conducted. Statistical analysis was done using Chi square and t-tests. Ten of the 26 subjects (38%) experienced an IONEE. Hyperglycemia was the primary IONEE that was noted in the neonates. (Mean: 143.19; sd: 56.041) Length of surgery was significantly longer in those premature neonates with IONEE than those with euglycemia (71.7 0± 27.03 vs. 45.62 ± 17.98 minutes). Omi/HtrA2 is a mitochondrial serine protease that is known to translocate to the cytoplasm upon induction of apoptosis and to activate caspase-dependent and caspase-independent cell death. The molecular mechanism of Omi/HtrA2's function is not clear but involves degradation of specific substrates. These substrates include cytoplasmic, mitochondrial, as well as nuclear proteins. We have isolated a new Omi/HtrA2 interactor, the THAP5 protein. THAP5 is a fifth member of a large family of transcription factors that are involved in cell proliferation, apoptosis, cell cycle control, chromosome segregation, chromatin modification and transcriptional regulation. Whether insomnia, a known correlate of depression, predicts depression longitudinally warrants elucidation. The authors examined 555 Wisconsin Sleep Cohort Study participants aged 33-71 years without baseline depression or antidepressant use who completed baseline and follow-up overnight polysomnography and had complete questionnaire-based data on insomnia and depression for 1998-2006. Using Poisson regression, they estimated relative risks for depression (Zung scale score >= 50) at 4-year (average) follow-up according to baseline insomnia symptoms and polysomnographic markers. Twenty-six participants (4.7%) developed depression by follow-up. Background: The Rotavirus Efficacy and Safety Trial was a placebo-controlled Phase III study that evaluated the safety and efficacy of a three-dose pentavalent rotavirus vaccine (RV5) including its effect on healthcare utilization for rotavirus gastroenteritis (RVGE). The per-protocol (PP) analyses, which counted events occurring 14 days after dose 3 among infants without protocol violations, have already been published. This paper evaluates the consistency of the healthcare utilization results based on the modified intention to treat (MITT) analyses with the PP analyses. There are continued interests to examine the relationship between maternal employment and childhood obesity. However, little research has been focused on childhood obesity among immigrant families with working mothers. This study used the Panel Survey of Income Dynamics (PSID) and Child Development Supplement (CDS) in 2002 and compared the impact of maternal employment on childhood obesity between immigrant families and US-born families. The positive effect of maternal employment on children’s body weight status was much stronger in immigrant families than in US-born families. The renin-angiotensin system (RAS) plays a pivotal role in the pathogenesis of cardiovascular diseases such as hypertension, myocardial infarction, heart failure, and stroke. The overall purpose of this study was to evaluate the effects of unilateral vestibular disorders and aging on functional performances of activities of daily living and vestibular rehabilitation exercises by examining the correlations among actual and perceived functional measures, the kinematic measurement differences among young healthy adults, older healthy adults, and older adults with unilateral vestibular deficits, and the correlations between kinematic and functional measures. This study was designed to compare the writing motivation of students with specific language impairments with their non-disabled peers. Due to the cognitive and linguistic demands of the writing process, students with language impairments face unique difficulties during the writing process. It was hypothesized that students with specific language impairments will be more likely to report lower levels of perceived writing competence and be less autonomously motivated to write. Data for 685 children ages 6-11 was obtained from the National Health and Nutrition Examination Survey (NHANES). Objective : To survey the clinical memorable experiences, which were suggested to intraoperative malignant hyperthermia (MH) among Thai anesthesiologists. Material and Method : Questionnaires for a national survey of dantrolene requirement were sent by the Royal College of Thai Anesthesiologists (RCAT) to all Thai anesthesiologists (855 members) in 2007. One hundred and two (11.93%) questionnaires were returned. There were 25 anesthesiologists (24.51%), who had experiences of suspected MH events. วัตถุประสงค์ : เพื่อศึกษาประสิทธิผลของโปรแกรมการปรับเปลี่ยนการดำเนินชีวิตที่มีต่อปัจจัยเสี่ยงโรคหลอดเลือดหัวใจตีบ และคะแนนคุณภาพชีวิต และศึกษาผลของซีรั่มต่อเซลล์บุผนังหลอดเลือดแดงของหัวใจ (human coronary artery endothelialcell, HCAEC)วิธีการศึกษา : ผู้ป่วยโรคหลอดเลือดหัวใจตีบ 30 คน อายุเฉลี่ย 63.28 ปี แบ่งเป็นกลุ่มที่ได้รับการรักษาแบบปกติ (กลุ่ม UC) และกลุ่มที่เข้าโปรแกรมฯ (กลุ่ม LM) วิเคราะห์ปัจจัยเสี่ยงของโรคหลอดเลือดหัวใจตีบ เป็นค่าไขมันรวม oxLDL และ proteincarbonyl ในซีรั่ม ทิ่ baseline 6 เดือน และ 12 เดือน และทดสอบผลของซีรั่มดังกล่าวในเซลล์ HCAEC ตรวจวัดเซลล์มีชีวิตด้วยวิธี MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide] assay และวัดระดับของอนุมูลอิสระที่เกิดขึ้นภายในเซลล์โดยวัดปริมาณแสงฟลูออเรสเซนต์ของสาร2,7-dichlorofluorescein (DCF) ทำแบบสอบถามความสามารถในการดูแลตนเองและแบบสอบถามคุณภาพชีวิต เพื่อประเมินประสิทธิภาพของโปรแกรมฯ และสมรรถนะการทำงานรวมทั้งคุณภาพชีวิตของผู้ป่วยทั้งสองกลุ่ม โดยเปรียบเทียบข้อมูลที่ได้ก่อนและหลังการศึกษา เมื่อติดตามครบ 1 ปีผลการศึกษา : จากผลการศึกษาพบว่าผู้ป่วยกลุ่ม LM มีระดับไขมันในเลือด ระดับของ oxLDLและ protein carbonyl ลดลงหลังจากเข้าโปรแกรมฯ 6 เดือน ในขณะที่เพิ่มสูงขึ้นในผู้ป่วยกลุ่ม UC แต่ที่ 12 เดือนไม่พบความแตกต่างทางสถิติเมื่อเทียบกับ baselineทั้งสองกลุ่มยกเว้นระดับ protein carbonyl ในผู้ป่วยกลุ่ม UC ที่เพิ่มสูงขึ้น และระดับ HDL-C ลดลงอย่างมีนัยสำคัญทางสถิติ และจากการทดสอบผลของซีรั่มที่ได้จากผู้ป่วยต่อเซลล์ HCAEC พบว่าร้อยละของเซลล์มีชีวิตเพิ่มขึ้นเมื่อincubated ด้วยซีรั่มจากผู้ป่วยกลุ่ม LM ที่ 6 เดือน แต่ไม่พบความแตกต่างทางสถิติที่ 12 เดือน การวัดระดับของอนุมูลอิสระที่เกิดขึ้นภายในเซลล์ พบว่าเมื่อincubated ซีรั่มที่ 6 เดือนของผู้ป่วยกลุ่ม LM ระดับของอนุมูลอิสระภายในเซลล์ลดลงจาก baseline แต่เพิ่มขึ้นในผู้ป่วยกลุ่ม UC เมื่อ …

The purpose of this study was to examine the impact of involvement in student organizations as it relates to perceptions of campus climate. The researcher conducted a crosssectional, ex-post facto secondary data analysis of a 2005 institutional survey on diversity at Great Falls State University. The researcher utilized a Chi Square Test for Independence, as well as a Forward Thinking Logistic Regression Model to analyze the data. Background: In light of the child overweight and obesity problem in Australia, resistance training programs have been trialled as an innovative way of assisting children increase lean body mass and reduce body fat. The purpose of this study was to investigate the factors influencing overweight children's participation in a resistance training trial program.

Method: Parent-child pairs who participated in the trial program were invited to take part in a follow-up individual interview to discuss their program experiences. In total, 22 semi-structured interviews were conducted with 11 parent-child pairs.
