Role Of Intrathecal Rituximab And Trastuzumab In The Management Of Leptomeningeal Carcinomatosis, Anthony J. Perissinotti, David J. Reeves

OBJECTIVE: To review evidence for the use of intrathecal rituximab and trastuzumab in the management of leptomeningeal carcinomatosis.

DATA SOURCES: A search of MEDLINE (1966-July 2010) and International Pharmaceutical Abstracts (1970-July 2010) was performed using search terms intrathecal, trastuzumab, rituximab, and monoclonal antibody. Additionally, American Society of Clinical Oncology, San Antonio Breast Conference, American Association for Cancer Research, and American Society of Hematology meeting abstracts were searched.

STUDY SELECTION AND DATA EXTRACTION: Publications were reviewed for inclusion. Those reporting use of rituximab and trastuzumab intrathecally are reviewed and include 1 Phase 1 trial, 2 small prospective studies, 1 case series, …

The aim of the study was to evaluate Diabetes Obesity - Wellness Opportunities Program (DOWOP) for overweight and obese adolescents aged eight to 14 years.

The study was conducted over 14 weeks at Victor J. Cassano, Sr. Health Center in Dayton, Ohio. Thirteen weekly nutrition, exercise and cognitive presentations were held. Cooking class was held on week thirteen. Participants completed food and activity diaries. The program sessions focused on healthy eating, increasing physical activity, decreasing sedentary behavior and supporting participants maintain healthy lifestyle.

Nurses recorded BMI and waist circumference at baseline, week 7 and week 14. Program administrators were interviewed …

In this project, psychiatric nurses were trained in patient-centered narrative interviewing approaches and placed in a variety of healthcare sites and conditions to function as consultants. Primary care clinicians (PCCs) in those sites selected patients that each clinician felt was frustrating or difficult and arranged for the consultant to interview them. The consultants then presented patient information to the PCC, data typically difficult to obtain using a standard medical interviewing format. The consultant and PCC discussed the new information provided. The PCC than determined if the information provided was valuable, whether it changed their clinical decision-making regarding this patient's diagnosis …

Sputum Neutrophils As A Biomarker In Copd: Findings From The Eclipse Study, Dave Singh, Lisa Edwards, Ruth Tal-Singer, Stephen I. Rennard

INTRODUCTION: The percentage of neutrophils in sputum are increased in COPD patients, and may therefore be a biomarker of airway inflammation. We studied the relationships between sputum neutrophils and FEV1, health status, exacerbation rates, systemic inflammation and emphysema, and long term variability at 1 year.

METHODS: Sputum samples were obtained from 488 COPD patients within the ECLIPSE cohort. 359 samples were obtained at baseline, and 297 after 1 year. 168 subjects provided samples at both visits. Serum interleukin-6 (IL-6), IL-8, surfactant protein D and C-reactive protein levels were measured by immunoassays. Low-dose CT scans evaluated emphysema.

With the push to streamline medical records and the use of electronic medical records system technology on the rise, successful implementation is important. The push is due in part for the need to reduce unnecessary paperwork, increase reimbursement, increase quality of medical records and increase overall quality of care. Factors to consider are the effects of implementation on quality of care, quality of medical records, physician and staff perceptions, working conditions, patient satisfaction, cost and barriers. Implementation may have a positive, negative or neutral effect on these factors making the need to review literature and report outcomes significant.

The following is an excerpt of this article: Neural tube defects (NTD) are the most common types of birth defects. Research shows that folic acid taken periconceptionally greatly reduces the risk of having a NTD affected child. This paper will explain the role that folate plays in the metabolism, specifically in synthesizing methionine. It will bring evidence to show that methionine is crucial for normal neural tube development. In addition, it will explore the genetic factor involved in folate metabolism and possible folate deficiency.

The following is an excerpt from the article: Ischemia is defined as an inadequacy of blood flow to tissue. Ischemia can deprive tissue of oxygen and metabolic substrates and it can also prevent the removal of waste products. If the ischemia is maintained over enough time the tissue will lose its homeostasis and eventually die causing an infarct. Retinal ischemia occurs when the blood supply to the retina does not meet the metabolic needs that are required to sustain the retina. This can lead to retinal damage and severe vision loss. Ischemia is caused by occluded blood vessels.

Peanut allergy is generally detected early in life and is commonly associated with other atopic disorders such as asthma, eczema, and rhinitis. The prevalence and pervasiveness of peanut allergies is increasing worldwide, and most peanut allergic patients have lifelong sensitivities to peanuts (de Leon et al, 2008). Pulmonary artery embolism (PE) is a rare and life-threatening complication in the neonate. We describe two previously stable infants who incurred respiratory failure from PE. The first case is a late preterm infant with gastroschisis with autopsy confirmation of PE presumably as a result of a central venous line (CVL) with thromboembolism. The second case is an extreme low birth weight infant with sudden onset of respiratory failure, abnormal chest x-ray, disseminated intravascular coagulation (DIC), and echocardiographic confirmation of a thrombus at the junction of the superior vena cava and right atrium (SVC/RA) at the site of a previous CVL. …

The global increase in the incidence of obesity has emerged as one of the most serious public health risks in recent years. Despite the enormity of the obesity pandemic, there are currently only two FDA-approved therapies for its treatment and these drugs exhibit modest effi cacy and have limiting side effects. Prieurianin is a plant limonoid product that deters feeding in insect larvae. We investigated in this study the effects of prieurianin on weight loss and adipogenesis. Our results showed that prieurianin causes weight loss by reducing energy intake in obese mice on highcalorie diet. We also found that prieurianin …
