BACKGROUND: Complications of renal failure may prevent timely evacuation of injured soldiers. Conventional renal replacement therapy is not available in forward surgical units. METHODS: Records of in-theater improvised peritoneal dialysis (IPD) in level III hospitals or forward surgical units in Iraq or Afghanistan were reviewed to determine the following: cause of renal failure and associated injuries; type of dialysate, peritoneal access, and exchange technique; and patient outcome. These data were used to propose method for IPD using commonly available materials. RESULTS: IPD is described in four patients. Cysts that are clearly associated with threat of progressive neurological loss or intractable, unremitting symptoms should be released by a decompression procedure.

In our case series, percutaneous aspiration has been successfully used for 3 of 4 PTs with marked decrease in size of cyst on imaging and clinical improvement.

Obstructing pathology and ultimately the location of the cyst can be major factors in determining the success of the procedure.

Our study demonstrated that a minimally invasive aspiration of a ZP SC can often achieve clinical improvement.

This may save the PT from undergoing an invasive surgical decompression.

Overactivity of the renin-angiotensin system (RAS) is involved in the pathogenesis of hypertension, and an overactive brain RAS has been highlighted in several genetic and experimental models. Until now, angiotensin II (Ang II) was thought to be the main effector of this system, and the angiotensin-converting enzyme (ACE)-Ang II-Ang II type 1 receptor axis was the main target for antihypertensive therapies. A new member of the RAS, ACE2 (angiotensin-converting enzyme type 2), has been identified in organs and tissues related to cardiovascular function (e.g. heart, kidney and blood vessels) and appears to be part of a counter-regulatory pathway to buffer …

The Ithaka Report 2006 investigated trends in the valuation of library functions by faculty. Faculty placed high values on library functions as purchaser and archive even as the valuation of information gateway declined. This poster demonstrates an effort toward expanding the archive function of a hospital library. We will describe our pilot program to digitize historical documents and photographs.

The TFR is a sign of upper motor neuron impairment. It does not typically appear for several days after an injury but in patients with pre-existing myelopathy, an early onset exaggerated Babinski response exhibited as a TFR may occur. It is important not to misinterpret such responses as volitional movements, particularly in patients with cognitive dysfunction where the history and physical examination may be limited. Failure to immobilize the spine and to administer adjunct steroid therapy may be detrimental to the patient. Education of initial response physicians may prevent overlooking an acute SCI.

Background
People with mental illness often experience unique barriers to healthy eating and physical activity. For these clients, interventions should focus on changes in the immediate environment to change behaviors. The purpose of this project was to implement and evaluate policy changes that would limit calorie intake and increase calorie expenditure of clients receiving mental health services.

Context

MicroRNAs (miRNAs) guide Argonaute (AGO)-containing microribonucleoprotein (miRNP) complexes to target mRNAs.It has been assumed that miRNAs behave similarly to each other with regard to mRNA target recognition. The usual assumptions, which are based on prior studies, are that miRNAs target preferentially sequences in the 3'UTR of mRNAs,guided by the 5' "seed" portion of the miRNAs. Here we isolated AGO- and miRNA-containing miRNPs from human H4 tumor cells by co-immunoprecipitation (co-IP) with anti-AGO antibody. Cells were transfected with miR-107, miR-124,miR-128, miR-320, or a negative control miRNA. Channelopathies are defined as inherited diseases, mostly located in an autosomal dominant gene. This review describes the significant number of new gene associated with genetic epilepsy syndromes that have emerged during the last decade, together with additional mutations relating to previously known gene associations. One of the most consistent findings in schizophrenia is the dysfunction of specific subsets of GABAergic interneurons in distributed brain regions including the prefrontal cortex and cerebellum. Analyses of post-mortem tissue from cerebellar hemispheres from 13 schizophrenic patients and 13 matched controls by quantitative real-time PCR (qRT-PCR) revealed that the mRNA levels of both the 67 kDa and 65 kDa isoforms of glutamic acid decarboxylase (GAD67 and GAD65) are decreased. Additionally, the presynaptic GABA transporter GAT-1 and the Golgi cell specific neuromodulator metabotropic glutamate receptor 2 (mGluR2) were decreased in the schizophrenic group. CONTEXT: Living wills have a poor record of directing care at the end of life, as a copious literature attests. Some speculation centers on the questionable correspondence between the scenario described in living wills versus the real-life circumstances that typically arise at the end of life.

OBJECTIVE: To assess the strength of association between responses to a standard living will question and preferences for treatments in six end-of-life scenarios.

BACKGROUND: Mindfulness-based stress reduction (MBSR) programs have consistently been shown to enhance the psychosocial well-being of participants. Given the well-established association between psychosocial factors and immunologic functioning, it has been hypothesized that enhanced psychosocial well-being among MBSR participants would be associated with corresponding changes in markers of immune activity.

OBJECTIVES: The objectives of this study were to examine changes in psychosocial and immunologic measures in a heterogeneous patient sample following participation in a MBSR program.

Staphylococcus aureus is a colonizer of the human skin and mucosa that can cause minor to severe disease. Originally afflicting patients at the extremes of age or those with immunocompromising co-morbidities, S. aureus strains have emerged that cause invasive disease in otherwise healthy populations with no health-care associated risk factors. In addition, these community-acquired strains predominately contain antibiotic resistance genes, making them difficult to treat with conventional interventions. Following a presynaptic action potential, there is a rapid rise of [Ca2+]i in the immediate vicinity of Ca2+ channels that triggers membrane fusion and release of transmitter from vesicles within this microdomain. This presynaptic Ca2+ signal ([Ca2+]pre) then disperses to produce a residual Ca2+ ([Ca2+]res) that decays over the course of tens to hundreds of milliseconds. The [Ca2+]res has important implications in synaptic plasticity and is the basis for working memory storage. Ultimately [Ca2+]res is removed from the cytoplasm either into intracellular organelles or across the plasma membrane into the extracellular environment. Ski and SnoN are two highly related transcription factors that are transcribed from two separate genes named SKI and SKIL, respectively. They are both co-repressors of the Smad-mediated transforming growth factor beta (TGFβ) signaling pathway. Originally they were classified as oncogenes as they have the ability to transform quail fibroblasts. However, they have also been found to have anti-proliferative properties so they are also considered tumor suppressors. Objective: This study investigated a multicomponent cognitive behavioral treatment (CBT) for hoarding based on a model proposed by Frost and colleagues and manualized in Steketee and Frost (2007).

Method: Participants with clinically significant hoarding were recruited from the community and a university-based anxiety clinic. Of the 46 patients randomly assigned to CBT or waitlist (WL), 40 completed the 12-week assessment and 36 completed 26 sessions. Treatment included education and case formulation, motivational interviewing, skills training for organizing and problem solving, direct exposure to nonacquiring and discarding, and cognitive therapy. Measurement of perfusion in longitudinal studies allows for the assessment of tissue integrity and the detection of subtle pathologies. In this work, the feasibility of measuring brain perfusion in rats with high spatial resolution using arterial spin labeling is reported. A flow-sensitive alternating recovery sequence, coupled with a balanced gradient fast imaging with steady-state precession readout section was used to minimize ghosting and geometric distortions, while achieving high signal-to-noise ratio. The quantitative imaging of perfusion using a single subtraction method was implemented to address the effects of variable transit delays between the labeling of spins and their arrival at the …
