To provide a model for quality care and outcomes management, the Nutrition Care Process (NCP) and standardized language were introduced in 2003 by the American Dietetic Association (ADA). The standardized language, published in the International Dietetics & Nutrition Terminology (IDNT) Reference Manual: Standardized Language for the Nutrition Care Process, is designed to facilitate communication, improve care, and close the gap in health care quality. When registered dietitians (RDs) are oriented to a nutrition care process, improvement in their documentation of nutrition care occurs. PURPOSE: There has been reluctance to use intraoperative transesophageal echocardiography (TEE) in small infants. We assessed the utility and safety of a new miniaturized multiplane micro-TEE probe in small infants undergoing cardiac operations.

DESCRIPTION: Hemodynamic and ventilation variables were prospectively recorded before and after micro-TEE insertion and removal in infants weighing 5 kg or less undergoing cardiac operations.

EVALUATION: The study included 42 patients with a mean weight of 3.6 +/- 0.9 kg (range, 1.7 to 5 kg). All probe insertions were successful. There were no complications or clinically significant changes in hemodynamic or ventilation variables. Information provided by TEE …

Podocytes play a key role in maintaining and modulating the filtration barrier of the glomerulus. Because of their location, podocytes are exposed to mechanical strain in the form of fluid flow shear stress (FFSS). Several human diseases are characterized by glomerular hyperfiltration, such as diabetes mellitus and hypertension. The response of podocytes to FFSS at physiological or pathological levels is not known. We exposed cultured podocytes to FFSS, and studied changes in actin cytoskeleton, prostaglandin E(2) (PGE(2)) production and expression of cyclooxygenase-1 and-2 (COX-1, COX-2). Disease detection and management might benefit from external imaging of disease gene mRNAs. Previously we designed molecular imaging nanoparticles (MINs) based on peptide nucleic acids complementary to cancer gene mRNAs. The MINs included contrast agents and analogs of insulin-like growth factor 1 (IGF-1). Analysis of MIN tumor uptake data showed stronger binding in tumors than in surrounding tissues. We hypothesized that MINs with an IGF-1 analog stay in circulation by binding to IGF-binding proteins. To test that hypothesis, we fit the tissue distribution results of several MINs in xenograft-bearing mice to a physiological pharmacokinetics model. Fitting experimental tissue distribution data …

To promote student-centered learning, a course council was established in a beginning undergraduate nursing course. A student representative was selected by peers to attend a monthly course council meeting with faculty. Representatives were asked to query classmates in their section of eight students regarding opinions, questions, and concerns about the course and then bring those views to the council. In this monthly, small group, open dialogue setting, students spoke freely about experiences in the classroom, laboratory, and clinical settings. The stage was set for reflection, problem solving, and decision making involving students and faculty. Student input led to meaningful course …

The purpose of this study was to compare the time to failure and muscle activation of young and old adults for a sustained isometric submaximal contraction with the dorsiflexor muscles when the foot was restrained to a force transducer (force-control task) compared with supporting an equivalent inertial load unrestrained in the sagittal plane (position-control task). Seventeen young (23.6 ± 6.5 yr) and 12 old (70.0 ± 5.0 yr) adults performed the force-control and position-control tasks at 30% maximal voluntary contraction (MVC) until task failure on separate days. Disparities in healthcare stem from historical, social, institutional, and interpersonal factors--all of which can manifest at the level of the clinical encounter. A growing body of research has addressed implicit bias and, more specifically, the implicit bias involved in aversive racism as a mediator of disparate care. While recent studies have suggested links between disparate treatment and implicit bias, little direct evidence exists for how implicit bias may effect disparate care. Qualitative research on physician understanding of processes by which implicit bias translates into disparate care can help fill this gap and identify areas for further research. Comparison of analgesic efficacy of 3 non-steroidal analgesic drugs (carprofen, vedaprofen and tepoxalin) were performed in 40 dogs after ovariohysterectomy. Oral administration of placebo (sugar pill), carprofen (4.4 mg/kg/day), vedaprofen (0.5 mg/kg/day), or tepoxalin (20 mg/kg/day) for 3 days were performed in dogs recovering from ovariohysterectomy. Numerical pain scores (descriptive and composite pain scores) were measured in blinded fashion by three investigators at 0, 12, 24, 36, 48 and 60 hrs following the recovery. The average descriptive and composite pain scores of dogs treated with carprofen or tepoxalin were significantly lower than that of the control group (p<0.05), however, no significance difference was found between the vedaprofen-treated and control groups (p>0.05). Water temperature was set up at 2 levels; high (Ht, 29.5±0.5oC) and low temperature (Lt, 19.5±0.5oC). Experimental catfish body weight and total length were 12.07±1.41 g and 11.57±0.62 cm respectively. Catfish were divided into 4 groups: high temperature with 0.1% sodium chloride (NaCl) (HtWs), high temperature without 0.1% NaCl (HtW/s), low temperature with 0.1% NaCl (LtWs), and low temperature without 0.1% NaCl (LtW/s). The objectives of the present study were to evaluate the damage of DNA of the frozen-thawed (FT) boar spermatozoa and to investigate the effect of various concentrations of L-cysteine supplementation on the sperm DNA damage. A total of 104 cryopreserved semen samples from twenty-six ejaculates of 16 proven boars were analyzed. Of these samples, each semen sample contained a different concentration of L-cysteine i.e., 0 (n=41), 5 (n=41), 10 (n=11) and 15 (n=11) mM. The aim of this report is to describe the clinical, radiographic and pathological appearances of a persistent gastroesophageal intussusception in associated with idiopathic egaesophagus in an adult dog. A 6-years old, male Golden Retriever was referred for evaluation of chronic regurgitation and vomiting. Radiographic diagnosis of persistent gastroesophageal intussusception (GEI) with megaesophagus was made. BACKGROUND: Osler taught that splenic infarction presents with left upper abdominal quadrant pain, tenderness and swelling accompanied by a peritoneal friction rub. Splenic infarction is classically associated with bacterial endocarditis and sickle cell disease.

OBJECTIVES: To describe the contemporary experience of splenic infarction.

METHODS: We conducted a chart review of inpatients diagnosed with splenic infarction in a Jerusalem hospital between 1990 and 2003.

RESULTS: We identified 26 cases with a mean age of 52 years. Common causes were hematologic malignancy (six cases) and intracardiac thrombus (five cases). Only three cases were associated with bacterial endocarditis. In 21 cases the splenic …

Masking Level Difference (MLD) tests are an established component of auditory processing test batteries; however, normative data for these tests vary according to procedure. In this study, forty normal, native-English speaking adults between the ages of 18 and 26 were tested for MLD via a newly developed computer software program using both an adaptive procedure (MLDA) and a Bekesy procedure (MLDB). The results from the two procedures were analyzed for sex differences and compared with each other. POLN is a nuclear A-family DNA polymerase encoded in vertebrate genomes. POLN has unusual fidelity and DNA lesion bypass properties, including strong strand displacement activity, low fidelity favoring incorporation of T for template G and accurate translesion synthesis past a 5S-thymine glycol (5S-Tg). We searched for conserved features of the polymerase domain that distinguish it from prokaryotic pol I-type DNA polymerases. A Lys residue (679 in human POLN) of particular interest was identified in the conserved 'O-helix' of motif 4 in the fingers sub-domain. A defining characteristic of solid tumors is the capacity to divide and spread under conditions of nutrient deprivation and limited oxygen availability. These microenvironmental stresses arise from structural abnormalities in tumor vessels that lead to aberrant microcirculation. Hypoxia acts as a physiological “selection pressure” in the progression of cancer by activating pathways and enhancing the expression of specific genes in tumor cells which eventually diminish their apoptotic potential. In order to isolate local strain of spore-forming bacteria having probiotic properties, 164 isolates of the sporeforming bacteria were isolated from 152 samples including duodenum, jejunum, ileum and cecum collected from 38 local backyard chickens raised in the northern part of Thailand. These isolates were preliminarily screened for antibiotic susceptibility and antimicrobial acitivity. Only antibiotic-susceptible isolates with antimicrobial activity were further characterized for the ability to resist certain harsh environments. Identification of the potential isolates was carried out by morphological and biochemical studies, followed by analysis of 16S rDNA sequence. Endotoxemia from sepsis can injure the gastrointestinal tract through mechanisms that have not been fully elucidated. We have shown that LPS induces an increase in gastric permeability in parallel with the luminal appearance of secretory phospholipase A2 (sPLA2) and its product, lysophosphatidylcholine (lyso-PC). We proposed that sPLA2 acted on the gastric hydrophobic barrier, composed primarily of phosphatidylcholine (PC), to degrade it and produce lyso-PC, an agent that is damaging to the mucosa. BACKGROUND: Obesity is a systemic disorder associated with an increase in left ventricular mass and premature death and disability from cardiovascular disease. Although bariatric surgery reverses many of the hormonal and hemodynamic derangements, the long-term collective effects on body composition and left ventricular mass have not been considered before. We hypothesized that the decrease in fat mass and lean mass after weight loss surgery is associated with a decrease in left ventricular mass.

In this paper, we present the Cellular Dynamic Simulator (CDS) for simulating diffusion and chemical reactions within crowded molecular environments. CDS is based on a novel event driven algorithm specifically designed for precise calculation of the timing of collisions, reactions and other events for each individual molecule in the environment. Generic mesh based compartments allow the creation / importation of very simple or detailed cellular structures that exist in a 3D environment. Multiple levels of compartments and static obstacles can be used to create a dense environment to mimic cellular boundaries and the intracellular space. The CDS algorithm takes into …
