Competency assessment is critical for a myriad of disciplines, including medicine, law, education, and nursing. Many nurse managers and educators are responsible for nursing competency assessment, and assessment results are often used for annual reviews, promotions, and satisfying accrediting agencies' requirements. Credentialing bodies continually seek methods to measure and document the continuing competence of licensees or certificants. Many methods and frameworks for continued competency assessment exist. The portfolio process is one method to validate personal and professional accomplishments in an interactive, multidimensional manner. Based on Working Paper #41: Availability, Characteristics, and Role of Detoxification Services in Rural Areas. Findings: Few rural detox providers exist; 82% of rural residents live in a county without a detox provider. More than half of all rural detox providers serve a 100 mile radius. Travel distances are a barrier to outpatient detox models. Referral options to substance abuse treatment are limited, especially in isolated rural areas.

OBJECTIVE: To establish cephalometric norms of a sample of aesthetically pleasing Pakistani faces and to analyze differences and similarities with accepted standards for Caucasians.

STUDY DESIGN: Analytical cross sectional study.

PLACE AND DURATION OF STUDY: Carried out at Alvi Dental Hospital, Karachi, from August 2007 to February 2008.

METHODOLOGY: Cephalometric tracings were analyzed on a sample of 40 (20 males and 20 females) Pakistani young adults, aged 18-25 years, with full complement of permanent teeth, no previous orthodontic treatment and pleasing profile. Autophagy is an immune effector response against a variety of intracellular pathogens, including Mycobacterium tuberculosis. Mycobacteria survive in macrophages by blocking maturation of the phagosome. Induction of autophagy by the T helper 1 (Th1) cytokine IFN-γ, or amino acid starvation, enables macrophages to overcome the phagosome maturation block and impedes mycobacterial survival. This study investigates the role of T helper 2 (Th2) cytokines Interleukin 4 (IL-4) and Interleukin 13 (IL-13) in autophagy in macrophages and their ability to eliminate intracellular mycobacteria. Background: Group B streptococci (GBS), or Streptococcus agalactiae, are the leading bacterial cause of meningitis and bacterial sepsis in newborns. Here we compared different culture media for GBS detection and we compared the occurrence of different genotypes and serotypes of GBS isolates from the vagina and rectum.

Methods: Streptococcus agalactiae was cultured separately from both rectum and vagina, for a total of 150 pregnant women, i) directly onto Columbia CNA agar, or indirectly onto ii) Granada agar resp. iii) Columbia CNA agar, after overnight incubation in Lim broth.

Background: Gingivitis has been linked to adverse pregnancy outcome (APO). Bacterial vaginosis (BV) has been associated with APO. We assessed if bacterial counts in BV is associated with gingivitis suggesting a systemic infectious susceptibilty.

Methods: Vaginal samples were collected from 180 women (mean age 29.4 years, SD ± 6.8, range: 18 to 46), and at least six months after delivery, and assessed by semi-quantitative DNA-DNA checkerboard hybridization assay (74 bacterial species). BV was defined by Gram stain (Nugent criteria). Gingivitis was defined as bleeding on probing at ≥ 20% of tooth sites.

Total hip and knee replacement surgery are one of the most frequent major surgeries performed in the USA. In 2002, the number of primary hip and knee arthroplasty was 119,000 and 381,000, respectively. By year 2030 the number is projected to increase to 572,000 and 3,481,000 respectively.

Deep wound infection after major orthopedic surgery is one of the most serious complications causing increase in morbidity and mortality and health care costs. Various risk factors for developing infection after hip and knee replacement surgery were described including age, ASA PS status, obesity, patients' comorbidities and surgical technique factors.

This review explores the nature, scope and consequences of loss resulting from mental illness. Losses are described within four key themes: self and identity, work and employment opportunities, relationships, and future-oriented losses. In reflecting upon review findings, several assumptions about loss are illuminated. Findings are situated within the cornerstones of recent mental health reform, specifically a recovery-oriented approach and social inclusion. Particular attention is directed towards notions of risk and responsibility and tensions in realizing the impact of loss within an individualized recovery framework. Implications and recommendations for policy and practice are highlighted.

Persons who are deaf face a number of challenges with regard to vulnerability for substance use disorders. Moreover, accessible treatment for this condition can be difficult to establish and maintain. The Deaf community may be one of the most disenfranchised groups in America in regard to appropriate access to substance use disorder (SUD) prevention and treatment services. This article reviews findings related to substance use disorder and treatment for this condition among persons who are deaf. A secondary data analysis of the 2001 National Survey of Veterans (NSV) for 2075 Gulf War-era veterans was conducted to investigate whether the GI Bill (the Servicemen's Readjustment Act of 1944, most recent provisions of which have been entitled the Montgomery GI Bill and the Post 9/11 GI Bill), considered as a social welfare policy, demonstrated protective effects for veterans with disabilities in terms of successful re-entry and sustained enrollment in higher education. Probiotic supplementation is now being focused as an alternative method to control fish diseases worldwide. This study investigates the in vitro efficacy of a human-derived probiotic, Lactobacillus rhamnosus. The first results of the screening for antimicrobial activity using agar spot test and disc diffusion showed that L. rhamnosus has a broad range against twelve isolates (n=12) of Gram-positive and Gram-negative pathogenic bacteria in fish and frogs : Streptococcus iniae (n=4), Streptococcus agalactiae (n=3), Aeromonas hydrophila (n=3), Chryseobacterium indologenes (n=1) and Edwardsiella tarda (n=1). To study the acute and subacute pulmonary effects of diesel exhaust particles (DEPs), mice were intratracheally instilled with 25, 50 or 100 μg of DEPs for dose response experiments. Histological alterations were determined at 3 days post-exposure. 50 or 100 μg of DEPs produced mild to moderate pulmonary inflammation and tissue injury characterized by infiltration of neutrophils and active alveolar macrophages (AMs), focal alveolitis and particle-laden AMs accumulation. Ultrastructural studies of treated animals showed the dissociation of basement membranes and erosion of type I alveolar epithelial cells. Effects of permethrin at a prophylactic dose for ectoparasite infection on cholinesterase (ChE) activity was evaluated. Eight healthy dogs were exposed to a spot-on formulation of permethrin on day 0. The results of serum ChE activity in all of the dogs which was measured at 24 h after exposure to permethrin significantly decreased when compared to those before exposure. However, the serum ChE activity in all the dogs which was measured at 36 and 96 h after exposure to permethrin showed no significant difference as compared to those before exposure. Matrix extracellular phosphoglycoprotein/osteoblast factor 45 (MEPE/OF45) was cloned in 2000 with functions related to bone metabolism. We identified MEPE/OF45 for the first time as a new co-factor of CHK1 in mammalian cells to protect cells from DNA damage induced killing. We demonstrate here that MEPE/OF45 directly interacts with CHK1. Knocking down MEPE/OF45 decreases CHK1 levels and sensitizes the cells to DNA damage inducers such as ionizing radiation (IR) or camptothicin (CPT)-induced killing. Over-expressing wild-type MEPE/OF45, but not the mutant MEPE/OF45 (depleted the key domain to interact with CHK1) increases CHK1 levels in the cells and increases the resistance of the …

Background: Group A beta hemolytic streptococcus (GABHS) pharyngitis is a common childhood illness. Penicillin remains the gold standard therapy, but macrolides are indicated for the penicillin allergic patient, and are often used for convenience.

Methods: We conducted a surveillance study of children with pharyngitis and positive streptococcal rapid antigen testing from 10/05 to 10/06 at 2 sites (A & B). Demographics, treatment, and resistance data was collected and compared to previous data from 2002. Erythromycin (EM) resistance was determined by disk diffusion and E-test on 500 isolates. The amyloid-beta 42 (Abeta42) is thought to play a central role in the pathogenesis of Alzheimer's disease (AD). However, the molecular mechanisms by which Abeta42 induces neuronal dysfunction and degeneration remain elusive. Mitochondrial dysfunctions are implicated in AD brains. Whether mitochondrial dysfunctions are merely a consequence of AD pathology, or are early seminal events in AD pathogenesis remains to be determined. Here, we show that Abeta42 induces mitochondrial mislocalization, which contributes to Abeta42-induced neuronal dysfunction in a transgenic Drosophila model. The cAMP-responsive transcription factor CREB functions in adipose tissue and liver to regulate glycogen and lipid metabolism in mammals. While Drosophila has a homolog of mammalian CREB, dCREB2, its role in energy metabolism is not fully understood. Using tissue-specific expression of a dominant-negative form of CREB (DN-CREB), we have examined the effect of blocking CREB activity in neurons and in the fat body, the primary energy storage depot with functions of adipose tissue and the liver in flies, on energy balance, stress resistance and feeding behavior. Natal teeth are an infrequent occurrence at birth. Often these teeth are extracted because they are very mobile and pose a risk of aspiration. This is a rare case in which a natal tooth was extracted by the pediatrician with his fingers. A root fragment remained and out of this developed a large pulp polyp. This relationship has not been previously reported.

Background: In sub-Saharan Africa, few services specifically address the needs of women in the first year after childbirth. By assessing the health status of women in this period, key interventions to improve maternal health could be identified. There is an underutilised opportunity to include these interventions within the package of services provided for woman-child pairs attending child-health clinics.

Methods: This needs assessment entailed a cross-sectional survey with 500 women attending a child-health clinic at the provincial hospital in Mombasa, Kenya.
