The human T-cell lymphotropic virus type 1 (HTLV-1) infects and transforms CD4+ lymphocytes and causes adult T-cell leukemia/lymphoma (ATLL), an aggressive lymphoproliferative disease that is often fatal. Here, we demonstrate that the HTLV-1 pX splice-variant p30II markedly enhances the transforming potential of Myc and transcriptionally activates the human cyclin D2 promoter, dependent upon its conserved Myc-responsive E-box enhancer elements, which are associated with increased S-phase entry and multinucleation. Enhancement of c-Myc transforming activity by HTLV-1 p30II is dependent upon the transcriptional coactivators, transforming transcriptional activator protein/p434 and TIP60, and it requires TIP60 histone acetyltransferase (HAT) activity and correlates with the …

Gating of voltage-dependent K(+) channels involves movements of membrane-spanning regions that control the opening of the pore. Much less is known, however, about the contributions of large intracellular channel domains to the conformational changes that underlie gating. Here, we investigated the functional role of intracellular regions in Kv4 channels by probing relevant cysteines with thiol-specific reagents. We find that reagent application to the intracellular side of inside-out patches results in time-dependent irreversible inhibition of Kv4.1 and Kv4.3 currents. In the absence or presence of Kv4-specific auxiliary subunits, mutational and electrophysiological analyses showed that none of the 14 intracellular cysteines is …

The most important stakeholders in patient safety are alert and mobilized frontline health care staff. At Clarian Health Partners, Indianapolis, IN, clinicians in the Safe Passage Program work jointly with unit staff, physicians, and other departments to continuously improve the level of patient, employee, and visitor safety. Medical error reporting at Clarian has tripled. The enthusiasm and passion of Safe Passage clinicians is both inspiring and energizing.

The purpose of this column is to describe the implementation of an international multisite Roy adaptation model-based study of women’s perceptions of and responses to cesarean birth. The need for the study arose from the concern that women’s childbearing needs may not be met to their full satisfaction, especially if the infant is born by cesarean. Serendipity and networking played a part in the selection of four study sites in the United States (Boston, Milwaukee, Norfolk, Oklahoma City) and two in other countries (Finland, Australia). Anatomic aberrations of the femur and tibia secondary to trauma, congenital defects, and prior surgery present challenges for the reconstructive knee surgeon because of an altered mechanical axis and distorted anatomic landmarks. Five patients with arthritis of the knee and extra-articular femoral and/or tibial deformity, retained hardware, or intramedullary (IM) implants underwent total knee arthroplasty using a computer navigation system. The navigation system obviated the need for an IM guide, and the normal mechanical axis of the patients was restored. Extensive dissection for hardware removal or osteotomy was not necessary in these patients. In these 5 cases, a navigation system …

Chronic airway infection is a hallmark of cystic fibrosis (CF) and many CF patients are infected persistently by Staphylococcus aureus. Leiomyosarcoma of the larynx is an extremely rare neoplasm; only about 50 cases have been reported in the English-language literature. We report a new case of laryngeal leiomyosarcoma in a 65-year-old man. The metastatic work-up was negative, and the patient underwent total laryngectomy. He remained disease-free 12 months postoperatively.

The correlation between cholinergic sensitivity and the level of stratification for ganglion cells was examined in the rabbit retina. As examples, we have used ON or OFF alpha ganglion cells and ON/OFF directionally selective (DS) ganglion cells. Nicotine, a cholinergic agonist, depolarized ON/OFF DS ganglion cells and greatly enhanced their firing rates but it had modest excitatory effects on ON or OFF alpha ganglion cells. As previously reported, we conclude that DS ganglion cells are the most sensitive to cholinergic drugs. This study examined the usefulness of Social Cognitive Theory (SCT) to model fast-food intake, fruit/vegetable consumption, and regular physical activity among a group of Chinese-American children in a Southeastern Virginia urban area.

An observational cross-sectional survey research design and snowball sampling were used. The survey employed items from the Health Behavior Questionnaire, the Youth Risk Behavior Survey, and a 24-hour Diet Recall Form. Eighty-four children completed the self-administered survey; 54 of these were enrolled in a Chinese Weekend School. The mean age of the children was 10.5 years (range 6-18) and 43% were girls. Optic neuritis has been described among the toxic effects of Ethambutol. This side effect is dose related. The mean duration of Ethambutol induced optic neuritis (EON) is three months. We report a case of EON after few days of exposure to Ethambutol and the symptoms resolved after discontinuation of Ethambutol. This most likely represents an idiosyncratic reaction which is different as compared to dose related optic neuritis.

In this case report, we would like to present an alternative surgical technique for the treatment of the bilateral prominent mandibular angles by using a custom-made resin splint (template). The prefabricated template provided a precise outline for the osteotomy, which alleviated the uncertainty of the osseous cut. Furthermore, masseter muscle myomectomy was not necessary in this case which decreased morbidity, and procedure and recovery time, while assuring a more symmetric outcome.

The epithelioid cell histiocytoma (ECH) is a polypoidal benign tumor of superficial connective tissue that is often diagnosed as a pyogenic granuloma. ECHs are speculated to originate from dermal dendritic subunits and are composed of 2 primary cell populations, ie, CD34⁺ primitive fibroblastic dendrocytes and factor XIIIa⁺ histiocytes. Although dendritic subunits are distributed throughout most collagenous tissues inclusive of oral mucosa, to date, all reported cases of ECH have been cutaneous lesions. ECHs' putative pathogenesis entails activation of CD34⁺ “sentinel” reserve dendrocytes, followed by an influx of histiocytes and mast cells. Juxtacrine communication increases release of wound …

This study examined what topics (including experiences, feelings, and thoughts) people define as positive self-disclosure. The study also looked at reasons people generate for self-disclosing versus not disclosing something positive. Male and female students spontaneously described a past experience or feeling they perceive to be personal and positive. They then indicated whether or not they disclosed about these experiences or feelings to their father, mother, same-sex friend, and a past or present significant other/spouse. These descriptions were coded into one of eight categories: Religion, Family Development, Friendship, Sex, Romance, Self-Confidence, Achievement, and Helping Behavior, plus a Miscellaneous category. This study used key informant interviews and student survey data in 508 U.S. communities to examine relationships between the prevalence of community and non-classroom-based school substance prevention strategies and teen substance use rates. After controlling for covariates, analyses indicated that: (1) adult-supervised after-school activities were significantly related to lower past 30-day cigarette smoking and both past 30-day alcohol use and binge drinking; (2) unsupervised after-school recreational facilities were significantly associated with both lower past 30-day cigarette smoking and current daily smoking; (3) community activities to reduce substance use were significantly related to lower binge drinking; and (4) student organizations to …
