A Nonstationary Negative Binomial Time Series With Time-Dependent Covariates: Enterococcus Counts In Boston Harbor, E. Andres Houseman, Brent Coull, James P. Shine

Boston Harbor has had a history of poor water quality, including contamination by enteric pathogens. We conduct a statistical analysis of data collected by the Massachusetts Water Resources Authority (MWRA) between 1996 and 2002 to evaluate the effects of court-mandated improvements in sewage treatment. Motivated by the ineffectiveness of standard Poisson mixture models and their zero-inflated counterparts, we propose a new negative binomial model for time series of Enterococcus counts in Boston Harbor, where nonstationarity and autocorrelation are modeled using a nonparametric smooth function of time in the predictor. On February 23, 2004, a 50-year-old male cleaner died when he fell approximately 35 feet through a temporary roof opening and was impaled on a piece of equipment on the factory floor. He had been cleaning an exhaust fan on a factory roof with a pressure washer. He and two coworkers had removed the fan from its casing creating a temporary roof opening and set the fan beside the roof opening for cleaning. The large subunit of the eukaryotic ribosome possesses a long and protruding stalk formed by the ribosomal P proteins. Four out of five ribosomal P proteins of Trypanosoma cruzi, TcP0, TcP1α, TcP2α, and TcP2β had been previously characterized. Data mining of the T. cruzi genome data base allowed the identification of the fifth member of this protein group, a novel P1 protein, named P1β. To gain insight into the assembly of the stalk, a yeast two-hybrid based protein interaction map was generated. A parasite specific profile of interactions amongst the ribosomal P proteins of T. cruzi was evident. The TcP0 …

Orthopaedic bone cutting processes such as osteotomy and hip arthroplasty involve the cutting of one with the aid of various manual and powered cutting instruments including manual and powered bone saws. The extent of surgically induced bone necrosis at the point of the cutting tool and the bone is mainly due to the frictional heat generated by bone cutting. Bone necrosis has an impact on bone regeneration, and the rate of healing of the bone post operation. The threshold temperature for impaired regeneration of bone is in the range of 44°C to 47°C. Surgeons may use a number of cutting instruments such as osteotomes and chisels to cut bone during operative procedures. The initial loading of cortical bone during the cutting process results in the formation of microcracks in the vicinity of the cutting zone with main crack propagation to failure occurring with continued loading; microcracking acts as a stimulus for main crack formation and has also been shown to occur during the propagation of the main crack. Odorants and pheromones as well as sweet- and bitter-tasting small molecules are perceived through activation of G protein-coupled chemosensory receptors. In contrast, gustatory detection of salty and sour tastes may involve direct gating of sodium channels of the DEG/ENaC family by sodium and hydrogen ions, respectively. We have found that ppk25, a Drosophila melanogaster gene encoding a DEG/ENaC channel subunit, is expressed at highest levels in the male appendages responsible for gustatory and olfactory detection of female pheromones: the legs, wings, and antennae. Background - National Cholesterol Education Program Adult Treatment Panel III guidelines recommended therapeutic lifestyle changes (TLC) and drug therapy to reduce cardiovascular disease (CVD) risk. These guidelines have been revised recently (ATP III-R); however, the risk of CVD mortality within each intervention window and the effects of cardiorespiratory fitness (CRF) and metabolic syndrome on CVD mortality within the framework of the guidelines are unknown.

Methods and Results - Risk factor and CRF data from 19,125 men (aged 20 to 79 years) who attended a preventive medical clinic between 1979 and 1995 were used. Mortality follow-up was completed until December 31, …

Purpose: To compare changes in functional exercise capacity, dyspnea, functional status, and depression in patients after 8 weeks (24 sessions) and 12 weeks (36 sessions) of pulmonary rehabilitation (PR). Methods: A prospective sample of 31 participants in our PR program completed outcome measures prior to and during the eighth and twelfth weeks of the program. The 6-minute walk test (6MWT) and a stairs climbing test (ST) measured functional exercise capacity. Perceived dyspnea (PD) was measured with a 6-20 scale. The Pulmonary Function Status Scale (PFSS) measured functional status, and the Cardiac Depression Scale (CDS) measured depression. Results : Statistically significant …

ภาวะเหงือกโตที่เกิดจากยาเป็นผลหลังจากที่ได้รับยาบางชนิดเพื่อรักษาโรคทางระบบ ภาวะเหงือกโตเป็นปัญหาของผู้ป่วยในด้านการพูด การเคี้ยวอาหาร การขึ้นของฟันและความสวยงาม มีรายงานเหงือก โตจํานวนมากที่เกิดจากยาในกลุ่มของยาต้านแคลเซียมซึ่งใช้รักษาโรคความดันโลหิตสูงและโรคของหลอดเลือดหัวใจ แอมโลดี้ฟินเป็นยาชนิดหนึ่งที่อยู่ในกลุ่มนี้ เป็นยาค่อนข้างใหม่ที่มีผลข้างเคียงน้อย รายงานฉบับนี้กล่าวถึงผู้ป่วยที่เกิดภาวะเหงือกโตจากยาแอมโลติฟัน การรักษาผู้ป่วยให้หายจากภาวะ เหงือกโตได้ด้วยการขูดหินน้ําลายและการควบคุมการเกิดคราบจุลินทรีย์ โดยไม่ต้องทําศัลยกรรมปริทันต์ร่วมด้วย ในระยะติดตามผลการรักษานาน 1 ปี

Objectives Orth odontic uptake among adolescents appears to increase in Thai society. The purposes of this research were to assess the rate of fixed orthodontic treatment experiences among first-year students at Chulalongkorn University (CU) and to identify its associating factors. Materials and methods A cross-sectional survey was conducted on a stratified-clustered sample of 800 first-year CU students. About 635 completed questionnaires were returned, giving a 79.4% response rate. Results Approximately 32.8 % of subjects experienced fixed orthodontic treatment; divided into 20.6% with orthodontic treatment completed, 8.5% with undergoing orthodontic treatment, and 3.6% with orthodontic treatment discontinued. Objectives To explore the relations of the positions of impacted mandibular third molar and the operation time to the complications after surgical removal of the impaction. Materials and methods Before surgery, all biostatistic data of the patients and radiographic assessment of the impacted tooth were recorded. From 84 healthy patients, 98 impacted teeth were surgically removed by the dental students in the fifth or the sixth year under a careful supervision of one oral and maxillofacial surgeon. Chief Complaints, Dental Health Status And Dental Treatment Needs In Elderly Patients, Aree Jainkittivong, Vilaiwan Aneksuk, Robert P. Langlais

Objective The aims of this study were to investigate the principal oral complaints, dental health status and dental treatment needs in elderly patients. Materials and methods The study sample consisted of 484 dental patients who were 60 years of age and older. Interviews were conducted to identify the oral complaints whereas the dental health status and dental treatment needs were assessed by oral examination. Results There were 17 principal oral complaints identified; and the four leading problems were faulty prostheses (21.1%), pain or/and swelling (16.3%), tooth loss or inefficient chewing (16.1%), and broken teeth and restorations (11.6%). การปลูกถ่ายฟันให้ตนเองกับการรักษาทางทันตกรรมจัดฟัน, ศิริชัย ธรรมชาติอารี, จินตนา ศิริชุมพันธ์

การปลูกถ่ายฟันให้ตนเองเป็นวิธีการหนึ่งที่ใช้ร่วมกับการรักษาทางทันตกรรมจัดฟันในกรณีที่มีการขาดหายไปของฟันแท้ มีฟันฝังหรือฟันคุด การพิจารณาผู้ป่วยที่จะรับการปลูกถ่ายฟันต้องประเมินสภาพร่างกายที่ไม่เป็น ข้อห้ามสําหรับการผ่าตัด หน่อฟันที่จะปลูกถ่ายควรมีการสร้างความยาวรากฟันได้ประมาณสามในสี่ส่วน โดยไม่มี ความผิดปกติของรูปร่างรากฟัน และตําแหน่งที่จะปลูกถ่ายควรมีความหนาของกระดูกเบ้าฟันโดยรอบอย่างน้อย 0.5 มิลลิเมตร การปลูกถ่ายฟันสามารถกระทําได้หลายบริเวณ คือ บริเวณฟันตัด ฟันเขี้ยว ฟันกรามน้อยและฟันกราม ซึ่งพบความสําเร็จถึงร้อยละ 50-97 ภายหลังการปลูกถ่ายฟันจะยึดฟันให้อยู่นิ่งเพื่อให้เนื้อเยื่อรอบฟันเกิดการซ่อมแซม ซึ่งกระทําได้หลายวิธี คือ การมัดด้วยลวด การใช้เครื่องยึดฟันอะคริลิก การยึดโดยใช้กรดกัดเคลือบฟันร่วมกับการ ใช้เรซินคอมโพสิต การใช้เครื่องมือทางทันตกรรมจัดฟันและการใช้วัสดุเย็บแผล ภาวะแทรกซ้อนหลังการผ่าตัดที่ ควรเฝ้าระวัง ได้แก่ การติดเชื้อ การตายของเนื้อเยื่อในโพรงฟัน การละลายของรากฟันทั้งจากภายในและภายนอก สําหรับการหายของเนื้อเยื่อในโพรงฟันให้พิจารณาจากการตีบตันของเนื้อเยื่อในโพรงฟันในระยะเวลา 6 เดือนแรกหลังการปลูกถ่ายฟัน เนื่องจากการทดสอบความมีชีวิตของฟันด้วยกระแสไฟฟ้าในระยะเวลาดังกล่าวยังได้ผลไม่แน่นอนอย่างไรก็ตาม ระยะเวลาที่เหมาะสมในการเคลื่อนฟัน ควรกระทําเมื่อมีการหายของเนื้อเยื่อปริทันต์และก่อนที่ เนื้อเยื่อในโพรงฟันจะเกิดการตีบตันทั้งหมด คือ ประมาณ 3-9 เดือนภายหลังการปลูกถ่ายฟัน และควรระมัดระวัง ในกรณีหมุนฟินที่มีหลายราก เพราะพบอัตราการตายของเนื้อเยื่อในโพรงฟันเพิ่มขึ้น

การคงอยู่ของฟลูออไรด์บนผิวเคลือบฟัน ภายหลังการเคลือบฟลูออไรด์เจล, พิเชียร อังจันทร์เพ็ญ, สุภัทรา อมาตยกุล, ศิริพร โชติไพบูลย์พันธุ์, ปรัชญา แหบคงเหล็ก, วรวัธน์ พุกจรูญ

วัตถุประสงค์ เพื่อศึกษาการคงอยู่ของฟลูออไรด์บนผิวเคลือบฟันภายหลังการเคลือบด้วยแอ ดูเล ฟอสเฟต ฟลูออไรด์เจล วัสดุและวิธีการ อาสาสมัครจํานวน 14 คน อายุระหว่าง 20 ถึง 22 ปี ได้รับการเคลือบฟันกรามน้อยซี่บนด้านแก้ม ด้วยแอซิดูเล ฟอสเฟตฟลูออไรด์เจลความเข้มข้นร้อยละ 1.23 เก็บตัวอย่างผิวเคลือบฟันบริเวณกึ่งกลางของฟัน กรามน้อย บนด้านแก้มโดยวิธีใช้กรดกัด ทั้งก่อนการเคลือบและหลังการเคลือบฟลูออไรด์เจลที่ 30 นาที 2, 4 และ 8 สัปดาห์ วัดความเข้มข้นของฟลูออไรต์และแคลเซียมในตัวอย่างผิวเคลือบฟันโดยใช้ฟลูออไรด์อิเล็กโตรดและเครื่อง อะคอมมิกแอบขอบขันสเปกโตรโฟโตมิเตอร์ วิเคราะห์ข้อมูลโดยใช้สถิติวิลคอกชนไซนต์แรงค์เทสท์ และฟรีดแมนเทสท์ ทีระดับนัยสําคัญ 05 ผลการศึกษา ปริมาณฟลูออไรด์บนผิวเคลือบฟันก่อนการเคลือบมีค่าเท่ากับ 2,1072498 ส่วนในล้านส่วน ภายหลังการเคลือบที่ 30 นาที 2, 4 และ 8 สัปดาห์ ปริมาณฟลูออไรต์บนผิวเคลือบฟันมีค่าเท่ากับ 6,5252699, 4,7512966, 2,906-863 และ 1,792-486 ส่วนในล้านส่วนตามลําดับ เมื่อวิเคราะห์ทางสถิติพบว่าปริมาณฟลูออไรด์ บนผิวเคลือบฟันที่ 30 นาที, 2 และ 4 สัปดาห์ภายหลังการเคลือบฟลูออไรด์เจล แตกต่างกับปริมาณฟลูออไรด์บน ผิวเคลือบฟันก่อนการเคลือบอย่างมีนัยสําคัญทางสถิติ (p < .01) ไม่พบความแตกต่างระหว่างปริมาณฟลูออไรด์บน ผิวเคลือบฟันก่อนการเคลือบและที่ 8 สัปดาห์หลังการเคลือบ สรุป ฟลูออไรด์บนผิวฟันภายหลังการเคลือบฟันด้วยแอซิดูเลตฟอสเฟตฟลูออไรด์เจล สามารถคงอยู่ได้นานกว่า 4 สัปดาห์ แต่ไม่เกิน 4 สัปดาห์

การรักษาคลองรากฟันในผู้ป่วยจัดฟัน: รายงานผู้ป่วย, สุมาลี ส่งไพศาล

ปัญหาหนึ่งที่พบมากในผู้ป่วยที่ได้รับการรักษาด้วยเครื่องมือทันตกรรมจัดฟันชนิดติดแน่นคือ ความ ไม่เอาใจใส่ในการดูแลรักษาสุขภาพของช่องปาก ซึ่งนําไปสู่การเกิดโรคฟันผุ และในบางกรณีอาจจําเป็นต้องรักษา คลองรากฟัน ถ้าพื้นที่ใช้เป็นหลักยึดในการเคลื่อนฟันมีรอยโรครอบปลายรากฟันขนาดใหญ่ อาจมีผลต้องเปลี่ยนแผนการรักษาด้านทันตกรรมจัดฟัน นอกจากนี้ การรักษาคลองรากฟันในฟันเหล่านี้มีข้อควรพิจารณาที่ต้องวางแผน การรักษาร่วมกับทันตกรรมจัดฟัน และสําคัญอย่างยิ่งที่ต้องติดตามผลการรักษาจนแน่ใจว่าเกิดการหายอย่างสมบรูณ์ รายงานฉบับนี้กล่าวถึงผู้ป่วยหญิงไทยอายุ 19 ปี ที่ใส่เครื่องมือจัดฟันชนิดติดแน่นและขาดการรักษาตามกําหนด นัดหมายเป็นเวลา 1 ปี ผู้ป่วยได้กลับมาหาทันตแพทย์อีกครั้งด้วยอาการปวดฟันกรามซี่ที่หนึ่งล่างซ้ายซึ่งเป็นฟันที่ ใช้เป็นหลักยึดในการเคลื่อนฟัน ตรวจพบฟันโยกและเหงือกบวม ภาพรังสีแสดงถึงเงาโปร่งรังสีขนาดใหญ่บริเวณ จุดแยกรากและรอบปลายรากฟัน หลังจากรักษาคลองรากฟันซี่นี้ไปแล้วเป็นระยะเวลา 6 เดือน ประเมินจากภาพ รังสีพบว่าเริ่มมีการหายของรอยโรค จึงอุดคลองรากฟันแล้วส่งผู้ป่วยกลับไปรักษาต่อทางด้านทันตกรรมจัดฟัน การ ประเมินผลจากภาพรังสีภายหลังการรักษาเป็นเวลา 1 ปี พบว่ามีการหายของพยาธิสภาพรอบรากฟัน และฟันซี่นี้ สามารถใช้เป็นหลักยึดสําหรับการเคลื่อนฟันทางทันตกรรมจัดฟันได้โดยไม่เกิดผลแทรกซ้อน

อิทธิพลของรสและอุณหภูมิของเครื่องดื่มชาเขียว ต่ออัตราการไหลของน้ําลาย, ระวีวรรณ ปัญญางาม, พรศรี ปฏิมานุเกษม

วัตถุประสงค์ ศึกษาอิทธิพลของรสและอุณหภูมิของเครื่องดื่มชาเขียวต่ออัตราการไหลของน้ําลาย วัสดุและวิธีการ อาสาสมัครจํานวน 30 คน อายุ 20-22 ปี มีสุขภาพสมบูรณ์และไม่ใช้ยาทุกชนิดอย่างน้อย 3 เดือน ก่อนเข้ารับการทดลอง ในวันแรก ทําการเก็บน้ําลายในระยะควบคุม โดยให้อาสาสมัครนั่งก้มหน้า ปล่อยน้ําลายให้ ไหลลงในภาชนะเป็นเวลา 10 นาที ในวันที่สองถึงเจ็ต ให้อาสาสมัครดื่มน้ําปราศจากแร่ธาตุ หรือเครื่องดื่มชาเขียว ที่มีอุณหภูมิ 5, 30 หรือ 50 องศาเซลเซียส ชนิดละ 300 มิลลิลิตร ตามลําดับ โดยใช้หลอดดูด แล้วเก็บน้ําลายใน ระยะทดลอง ด้วยวิธีการเติมเป็นเวลา 10 นาที คํานวณอัตราการไหลของน้ําลายทุกระยะเป็นมิลลิลิตร/นาที เปรียบเทียบความแตกต่างของค่าเฉลี่ยของอัตราการไหลของน้ําลายในระยะควบคุม หลังดื่มน้ํา กับหลังดื่มชาเขียว ที่อุณหภูมิต่างๆ ด้วยการวิเคราะห์ความแปรปรวนที่ระดับความเชื่อมั่นร้อยละ 95 หากพบมีนัยสําคัญจึงทดสอบหาคู่ ผลการศึกษา ค่าเฉลี่ยอัตราการไหลของน้ําลายระยะควบคุม เท่ากับ 0.47±0.27 มิลลิลิตร/นาที หลังการดื่มน้ําและ ชาเขียวที่มีอุณหภูมิ 5, 30 และ 50 องศาเซลเซียส เท่ากับ 0.67±0.25, 0.48±0.26, 0.51±0.24, 0.75±0.28, 0.56±0.32 และ 0.60±0.30 มิลลิลิตร/นาที ตามลําดับ อัตราการไหลของน้ําลายหลังดื่มน้ําที่มีอุณหภูมิ 5 องศาเซลเซียสสูงกว่า อัตราการไหลของน้ําลายในระยะควบคุมและหลังดื่มน้ําที่มีอุณหภูมิ 30 องศาเซลเซียสอย่างมีนัยสําคัญทางสถิติ (P < .05) แต่ไม่แตกต่างจากภายหลังดื่มน้ําที่มีอุณหภูมิ 50 องศาเซลเซียสอย่างมีนัยสําคัญทางสถิติ นอกจากนี้ อัตราการไหลของน้ําสายหลังดื่มน้ําที่มีอุณหภูมิ 30 และ 50 องศาเซลเซียส ไม่แตกต่างอย่างมีนัยสําคัญทางสถิติด้วย สําหรับอัตราการไหลของน้ําลายหลังดื่มชาเขียวที่มีอุณหภูมิ 5 องศาเซลเซียส พบว่า สูงกว่าอัตราการไหลของ น้ําลายในระยะควบคุม และหลังดื่มชาเขียวที่มีอุณหภูมิ 30 องศาเซลเซียสอย่างมีนัยสําคัญทางสถิติ (p < .05) และอัตราการไหลของน้ําลายหลังดื่มชาเขียวที่มีอุณหภูมิ 30 และ 50 องศาเซลเซียสแตกต่างกันน้อยมากจนไม่ ปรากฏนัยสําคัญทางสถิติ เช่นเดียวกับผลการทดลองที่ได้จากการดื่มน้ํา อย่างไรก็ตาม เมื่อเปรียบเทียบอัตราการ ไหลของน้ําลายหลังดื่มชาเขียวและหลังดื่มน้ําที่มีอุณหภูมิเท่ากัน พบว่า อัตราการไหลของน้ําลายหลังดื่มชาเขียว สูงกว่าหลังดื่มน้ําอย่างมีนัยสําคัญทางสถิติทุกอุณหภูมิ (p < .05) ที่มีความแตกต่างกันทีละคู่ด้วยวิธีผลต่างอย่างมีนัยสําคัญน้อยที่สุดสรุป รสของเครื่องดื่มชาเขียวสามารถกระตุ้นให้น้ําลายมีอัตราไหลเพิ่มขึ้น และขาเขียวชนิดเป็นกระตุ้นให้น้ําลายมีอัตราไหลเพิ่มขึ้นได้มากกว่าชาเขียวชนิดร้อน

Before implantation into the uterine wall, the mammalian embryo undergoes a period of cell division, cell shape change, and cell differentiation leading to the formation of an outer epithelium, the trophectoderm. The trophectoderm is the part of the embryo that initiates uterine contact and, after transformation to become the trophoblast, uterine invasion. Similar to the kidney nephron, the trophectoderm is a transporting epithelium with distinct apical and basolateral membrane domains; its function is to facilitate transepithelial Na+ and fluid transport for blastocoel formation. The purpose of the study on which this article is based was to determine the current extent of linkages among Canadian community health nursing researchers and decision makers and to identify perceptions around the structure and function of potential networks. A qualitative research design was utilised to develop common themes across focus groups, a workshop and key informant interviews. BACKGROUND: Histories of kidney transplantation rarely mention a series reported by Gordon Murray of Toronto and published by the American Journal of Surgery 50 years ago.

METHODS: The papers and biographies of Gordon Murray were reviewed in the context of knowledge at that time about renal failure management to determine their contribution to transplantation research and to current practice.

The hypothesis that inhibitory control – an aspect of executive functioning – is related to children’s suggestibility was tested. Five- to 7-year-olds (N = 125) participated in a staged event, were suggestively interviewed, and were later given a recognition test. Conflict and interference measures of inhibitory control were taken and compared to children’s ability to identify details from the target event and reject details from non-target sources (i.e., false suggestions, details from prior events). Children with higher than average verbal retroactive inhibition skills were more resistant to suggestions than children with poor inhibitory control. Collectively, age and retroactive inhibition …
