Children With Special Health Care Needs: Minding The Gaps, Christie Provost Peters

National Health Policy Forum

This background paper examines the issue of children with special health care needs and their interaction with the health care system. Results from recent national and state surveys and studies were used to review the medical expenditures, utilization, and insurance coverage of these children. The paper also discusses weaknesses within the private and public delivery and financing systems that may hinder the access of certain families with children with special health care needs to important services.

Nurses have used the intervention of caring for many years, but little attention has been given to describing the phenomenon of nursing presence in the perioperative setting. The purpose of this research was to learn more about the experience of the connection of the family to the nurse who kept them informed during the perioperative period. A hermeneutic phenomenological approach was utilized to identify patterns of caring, connecting, and transpersonal nursing presence described by the family of surgical patients. Five women were interviewed for this study. Transcribed interviews became the phenomenological texts for my hermeneutic analysis. Essential themes were uncovered …

The basis for coexistence of similar species is fundamental in community ecology. One mechanism for coexistence is differentiation of diets. Lytechinus variegatus and Arbacia punctulata coexist in different microhabitats along the Florida gulf coast. Their great difference in morphology might affect their choice of microhabitats and diet. We analyzed diets of both species at 1 offshore and 1 nearshore site where both occurred in relatively equal numbers, an offshore site dominated by A. punctulata and an offshore site dominated by L. variegatus. The isolation and characterization of cDNAs encoding poly(ADP-ribose) glycohydrolase (PARG) enzymes and the amino acid sequences of PARGs from several species are described. PARG is involved in the cellular response to DNA damage and its proper function is associated with the body's response to neoplastic disorder inducing agents and oxidative stress. Expression vectors containing the cDNAs and cells transformed with the vectors are described. Probes and primers that hybridize with the cDNAs are described. Expression of the cDNA in E. coli results in an enzymatically active protein of about 111 kDa and an active fragment of about 59 kDa. Methods …

The 2005 Project Child Health Impact Assessment (CHIA) at the Boston Medical Center (BMC) for the UMass Emerging Leaders Program (ELP) is closely related to one that members of the 2004 ELP cohort assisted with. The 2004 project team provided BMC with preliminary research regarding similar concepts in Europe, Canada and the United States. With this research in hand, the Project CHIA team at BMC decided to research, develop, write and publish pilot analyses that illustrated the impact of public policy on children's health. The purpose of this study was to compare event-related potential (ERP) effects of speech processing and effects in sentence reading elicited by sentences containing gapping structures, or a "missing" verb. N400 and P600 waveforms were collected in 20 adults between 18 and 30 years of age. Two experiments were conducted with each participant. In the two experiments ERP recordings were collected as sentences, some containing gapping structures, were presented to the subjects. In one experiment sentences were presented through headphones in sentences spoken at normal rate and with normal intonation. In the second experiment sentences with the same gapping structures …

Urotensin II (UII) is a cyclic neuropeptide with strong vasoconstrictive activity in the peripheral vasculature. UII receptor mRNA is also expressed in the CNS, in particular in cholinergic neurons located in the mesopontine tegmental area, including the pedunculopontine tegmental (PPT) and lateral dorsal tegmental nuclei. This distribution suggests that the UII system is involved in functions regulated by acetylcholine, such as the sleep-wake cycle. Here, we tested the hypothesis that UII influences cholinergic PPT neuron activity and alters rapid eye movement (REM) sleep patterns in rats. BACKGROUND: HIV Associated Dementia (HAD) is a common complication of human immunodeficiency virus (HIV) infection that erodes the quality of life for patients and burdens health care providers. Intravenous drug use is a major route of HIV transmission, and drug use is associated with increased HAD. Specific proteins released as a consequence of HIV infection (e.g., gp120, the HIV envelope protein and Tat, the nuclear transactivating protein) have been implicated in the pathogenesis of HAD. This paper describes the application of a culture brokering model developed by Mary Ann Jezewski to a cultural conflict within a pediatric clinic. The purpose of the case study was to explore the usefulness of the culture brokering model as a tool in resolving conflict as well as a process to expand cross-cultural understanding and promote connectedness of the patient in the health care system. A practice model for the advanced practice nurse is discussed which combines the cultural brokering model and places it within the framework of Leininger's Theory of Culture Care Diversity and Universality. The woman had driven a gas-powered golf cart from her house on the farm to the horse barn to muck stalls and put horses in the barn. As rising health care expenditures focus government attention on slowing the growth, the pharmaceutical industry comes under increasing pressure to curb prices of ethical drugs. Pharmaceutical price regulations have been implemented in many countries to control pharmaceutical expenditures. Yet, creating innovative drugs requires enormous R&D costs, which in turn require adequate expected economic returns. Since price controls reduce profits and expected returns, as countries invoke stricter price regulations, firms will either move their R&D process into less regulated markets or move out of innovative R&D. As a result of their role in the athletes’ recovery, athletic trainers should have an understanding of the many aspects involved in the psychological effect of an injury, and how to assist an athlete in their psychological recovery. This includes the components of injury, factors affecting recovery, psychosocial intervention strategies, counseling, and referral. Kalirin is a multidomain guanine nucleotide exchange factor (GEF) that activates Rho proteins, inducing cytoskeletal rearrangement in neurons. Although much is known about the effects of Kalirin on Rho GTPases and neuronal morphology, little is known about the association of Kalirin with the receptor/signaling systems that affect neuronal morphology. Our experiments demonstrate that Kalirin binds to and colocalizes with the TrkA neurotrophin receptor in neurons. In PC12 cells, inhibition of Kalirin expression using antisense RNA decreased nerve growth factor (NGF)-induced TrkA autophosphorylation and process extension. Kalirin overexpression potentiated neurotrophin-stimulated TrkA autophosphorylation and neurite outgrowth in PC12 cells at a low …
