It is of recent interest in reproductive health research to investigate the validity of a marker event for the onset of menopausal transition and to estimate age at menopause using age at the marker event. We propose a varying coefficient Cox model to investigate the association between age at a marker event, denned as a specific bleeding pattern change, and age at menopause, where both events are subject to censoring and their association varies with age at the marker event. This paper discusses the evidence for pain perception in fish and presents new data on morphine analgesia in fish. Recent anatomical and electrophysiological studies have demonstrated that fish are capable of nociception, the simple detection of a noxious, potentially painful stimulus and the reflex response to this. To prove pain perception, it must be demonstrated that an animal’s behaviour is adversely affected by a potentially painful event and this must not be a reflex response. The Medicare Prescription Drug Proposals And Health Insurance Risk, Dawn Gencarelli

National Health Policy Forum

In order to facilitate a better understanding of the complex issues raised by Senate and House proposals to establish a prescription drug benefit for Medicare beneficiaries, this paper briefly addresses some fundamentals of the health insurance market, defines key risk-sharing mechanisms, including risk corridors and reinsurance, and identifies the relevant risk provisions in the bills. Other issues related to cost management strategies and program design, which may have an impact on cost and adverse selection, are also discussed.

วัตถุประสงค์เพื่อศึกษาความสามารถในการทําซ้ํา (reproducibility) ของการเอียงของศีรษะ (head orientation)ในท่านั่งและยืนโดยใช้และไม่ใช้กระจกเพื่อช่วยกําหนดแนวสายตา วัสดุและวิธีการ อาสาสมัครคือนิสิตทันตแพทย์จํานวน 3 คน เพศชาย 13 คน หญิง 25 คน อายุเฉลี่ยเท่ากับ 2ปี 1 เดือน ทําการถ่ายภาพหน้าด้านข้างด้วยกล้องถ่ายภาพดิจิตอลโดยใช้สภาวะแวดล้อมเดียวกันใน 4 ลักษณะคือ 1) นั่งหลังตรง มองตรง 2) นั่งหลังตรง มองตรงและใช้กระจกกําหนดระดับแนวสายตา 3) ยืนตัวตรง มองตรง 4) ยืน ตัวตรง มองตรงและใช้กระจกกําหนดระดับแนวสายตา ทําการถ่ายภาพทั้ง 4 ลักษณะจํานวน 2 ครั้งในระยะเวลา ที่ห่างกันอย่างน้อย 1 เดือน ภาพดิจิตอลทั้งหมดจะถูกนํามาวัดค่ามุมระหว่าง Glabella-Subnasale Line (GSL) และ True Vertical Line (TVL) ด้วยโปรแกรม Photoshop 7.0 โดยมุมดังกล่าวจะถูกใช้เป็นตัวแทนของการเชียง องศีรษะของกลุ่มอาสาสมัคร ผลการศึกษา พบว่าไม่มีความแตกต่างทางสถิติอย่างมีนัยสําคัญระหว่างมุม GSL-TVL ที่ได้จากการถ่ายภาพ ทั้งสองครั้งในทั้ง 4 ลักษณะ ความแตกต่างของค่าเฉลี่ยของมุม GSL-TVL ระหว่างการถ่ายภาพครั้งที่หนึ่งและสองในท่านั่ง ท่านั่งมองกระจก ท่ายืนและท่ายืนมองกระจกเท่ากับ 1.16 0.50 0.14 และ 0.13 องศาตามลําดับ และมีค่า method error เท่ากับ 2.00 2.05 2.78 และ 3.46 องศาตามลําาดับ สรุป การเอียงของศีรษะในท่ายืนมีแนวโน้มที่จะทําซ้ําได้ง่ายกว่าท่านั่ง ศีรษะมีแนวโน้มที่จะเงยในท่านั่งมากกว่าในท่ายืนและการใช้กระจกกําหนดแนวสายตาอาจมีผลทําให้การทําซ้ําของการเอียงของศีรษะในท่านั่งเป็นไปได้ง่ายขึ้น

การใช้ชิ้นฟันที่หักติดเข้ากับตัวฟันเดิม นับเป็นทางเลือกหนึ่งในการบูรณะฟันที่แตกหัก ซึ่งเกิดจากอุบัติเหตุ ก่อการบาดเจ็บของฟันหน้าในกรณีที่ผู้ป่วยเก็บชิ้นส่วนฟันที่หักไว้ได้ จัดเป็นการบูรณะฟันเชิงอนุรักษ์ เป็นการ บูรณะฟันที่แตกหักได้อย่างปัจจุบันทันด่วนทั้งในเชิงการบูรณะชั่วคราวหรือถาวร โดยอาศัยระบบบอนด์ดึง (Bonding) และวัสดุเรซินคอมโพสิตช่วยในการยึดติด ใช้เวลาในการบูรณะน้อย, ประหยัดค่าใช้จ่าย, ได้ฟันที่มีสี, รูปร่างและ ทําหน้าที่ได้ดังเดิม ดังเช่นผู้ป่วยทั้ง 2 ราย ซึ่งได้รับการบูรณะฟันด้วยวิธีนี้และประสบผลสําเร็จจากการติดตามผลการรักษาเป็นเวลา 1 ปี

วัตถุประสงค์ ประสิทธิภาพในการบดเคี้ยว (masticatory efficiency) มีความสําคัญอย่างมากต่อสุขภาพร่างกาย ในผู้ป่วยที่มีการสูญเสียฟันธรรมชาติจะมีประสิทธิภาพในการบดเคี้ยวอาหารลดลงซึ่งสามารถแก้ไขได้โดยการใส่ฟันปลอม การประเมินประสิทธิภาพของฟันปลอมให้บรรลุวัตถุประสงค์อย่างแท้จริงจึงมีความสําคัญเป็นอย่างยิ่งโดยทั่วไป ทันตแพทย์มักใช้วิธีการประเมินโดยการสอบถามจากผู้ป่วยซึ่งมิได้ให้ผลอ้างอิงที่ชัดเจน การศึกษานี้จึงมีวัตถุประสงค์เพื่อเปรียบเทียบการประเมินประสิทธิภาพในการบดเคี้ยวในผู้ป่วยสูงอายุโดยการหาความสัมพันธ์ระหว่าง ความสามารถในการบดเคี้ยว (masticatory ability) ซึ่งเป็นการสอบถามจากผู้ป่วย และการแสดงการบดเคี้ยว (mas-ticatory performance) ซึ่งเป็นการทดสอบในห้องปฏิบัติการ ว่าให้ผลไปในทางเดียวกันหรือไม่ วัสดุและวิธีการ ทําการศึกษาในผู้ป่วยสูงอายุ (อายุ 55 ปีขึ้นไป) ที่มารับบริการใส่ฟันปลอมในคลินิกบัณฑิตศึกษา ภาควิชาทันตกรรมประดิษฐ์ คณะทันตแพทย์ศาสตร์ จุฬาฯ จํานวน 62 คน โดยแบ่งเป็น 3 กลุ่ม กลุ่มแรกคือ ผู้ป่วยที่ใส่ฟันปลอมถอดได้ทั้งปากในขากรรไกรบนและล่าง (18 คน) กลุ่มที่สอง คือผู้ป่วยที่ใส่ฟันปลอมบางส่วน ถอดได้สบกับฟันปลอมบางส่วนถอดได้หรือฟันปลอมทั้งปาก (29 คน) และกลุ่มที่สามคือ ผู้ป่วยที่ใส่ฟันปลอมบาง ส่วนถอดได้สบกับฟันธรรมชาติ (15 คน) การทดสอบความสามารถในการบดเคี้ยวทําโดยการประเมินความพึง พอใจในการเคี้ยวอาหารของผู้ป่วยหลังใส่ฟันปลอมจากการตอบคําถาม ส่วนการแสดงการบดเคี้ยวทําโดยการวัด ความละเอียดของอาหารที่เคี้ยวด้วยวิธีร่อนอาหารผ่านตะแกรงโดยให้ผู้ป่วยเคี้ยวแครอทหนึ่งชิ้น (น้ําหนัก 3 กรัม) จํานวน 40 ครั้ง นํามาผ่านตะแกรงชนิดหยาบและละเอียด ตามลําดับ จากนั้นจึงพึ่งให้แห้ง ชั่งน้ําหนัก และ คํานวณการแสดงการบดเคี้ยว นําข้อมูลมาที่ได้มาวิเคราะห์ทางสถิติ จากนั้นหาความสัมพันธ์ระหว่างความสามารถ ในการบดเคี้ยวกับการแสดงการบดเคี้ยวทางสถิติโดยใช้การทดสอบ ไค-สแควร์ ที่ระดับนัยสําคัญ 0.05 โดยใช้เกณฑ์ การแสดงการบดเคี้ยวตั้งแต่ร้อยละ 70 ขึ้นไปถือว่ามีประสิทธิภาพดี ผลการศึกษา การศึกษาพบว่าไม่มีความแตกต่างของการแสดงการบดเคี้ยวในกลุ่มผู้สูงอายุที่ใส่ฟันปลอมประเภท ต่าง ๆ กัน ทั้ง 3 กลุ่ม อย่างมีนัยสําคัญ และความสามารถในการบดเคี้ยวไม่มีความสัมพันธ์กับการแสดงการบดเคี้ยว สรุป ทันตแพทย์ไม่ควรประเมินประสิทธิภาพของฟันปลอมจากการสอบถามผู้ป่วยเพียงอย่างเดียว แต่ควรทําการ ทดสอบเพื่อให้เห็นถึงความสามารถของฟันปลอมในการใช้บดเคี้ยวอาหารอย่างแท้จริง

การทดสอบสมการทํานายความกว้างของฟันเขียว และฟันกรามน้อยในคลินิกทันตกรรมจัดฟัน, ปิยารัตน์ อภิวัฒนกุล, วัชระ เพชรคุปต์

วัตถุประสงค์ การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อทดสอบความแม่นยําในการทํานายผลรวมความกว้างของฟันเขี้ยว ฟันกรามน้อยที่หนึ่ง และฟันกรามน้อยซี่ที่สองในคนไทย เมื่อใช้สมการการทํานายที่สร้างขึ้นจากความกว้างของ ฟันกรามแท้ซี่แรกในขากรรไกรเดียวกันของวัชระและปิยารัตน์ เปรียบเทียบกับการทํานายโดยใช้ตาราง Moyers วัน และวิธีการ วัดนาดฟันจากแบบจําลองฟันของผู้ป่วยที่รับการบําบัดรักษาในคลินิกทันตกรรมจัดฟัน ภาควิชา ทันตกรรมจัดฟัน คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย 200 ราย แบ่งเป็นเพศชาย 100 ราย และหญิ 100 ราย หาค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐานของผลรวมความกว้างของฟันเขี้ยวและฟันกรามน้อยทั้งสอง จากค่าจริงที่วัดจากแบบจําลองฟัน นํามาเปรียบเทียบกับค่าที่ได้จากการคํานวณโดยใช้ความกว้างของฟันกรามแท้ ซี่แรกในขากรรไกรเดียวกันตามสมการของวัชระและปิยารัตน์ และจากการใช้ตาราง Meyers (Moyers probability chart) น่าค่าที่วัดได้มาทดสอบความแตกต่าง โดยใช้สถิติ Paired T-test ที่ระดับนัยสําคัญ 0.05 และหาความสัมพันธ์ โดยหาค่าสัมประสิทธิ์สหสัมพันธ์ ผลของการศึกษา พบว่าในขากรรไกรบนของเพศชายและขากรรไกรล่างของเพศหญิงมีความแตกต่างกัน อย่างมีนัยสําคัญทางสถิติระหว่างค่าของขนาดฟันที่ได้จากการใช้ตาราง Moyers กับการใช้สมการวัชระและปิยารัตน์ (P < 0.01) และการใช้ตาราง Moyers กับค่าที่ได้จากฟันจริง (P < 0.01) แต่ไม่พบความแตกต่างกันระหว่าง ขนาดฟันที่ได้รับจากสมการกับค่าจริง ส่วนขนาดฟันในขากรรไกรล่างของเพศชายและขากรรไกรบนของเทศหญิงไม่พบความแตกต่างกันอย่างมีนัยสําคัญทางสถิติระหว่างทุกกลุ่ม แต่ค่าสัมประสิทธ์สหสัมพันธ์แสดงความสัมพันธ์ ระหว่างการใช้สมการวัชระและปิยารัตน์ ใกล้เคียงกับค่าจริงมากกว่าการใช้ตาราง Mayers เมื่อพิจารณาจากช่วง ของความคลาดเคลื่อน (ค่าแตกต่างระหว่างค่าจากการทํานายกับค่าจริง) พบว่าค่าที่ได้จากสมการใกล้เคียงกับ รองขนาดฟันจริงมากกว่าการใช้ตาราง Moyers สรุป จากการศึกษาครั้งนี้พบว่าการใช้สมการวัชระและปิยารัตน์เพื่อทํานายผลรวมความกว้างของฟันเขี้ยวและ ฟันกรามน้อยทั้งสองมีค่าใกล้เคียงกับค่าของฟันจริงมากกว่าค่าใช้จากตาราง Moyers โดยเฉพาะขนาดของฟันในขากรรไกรบนของเพศชาย และขนาดของฟันในขากรรไกรล่างของเพศหญิง

การเปรียบเทียบเสถียรภาพของสีของเฟลสปาติกพอร์ซเลนที่ขัดผิวด้วยวิธีต่าง ๆ, วัลลภัทน์ แสนทวีสุข, อิศราวัลย์ บุญศิริ

วัตถุประสงค์ของการศึกษานี้เพื่อเปรียบเทียบเสถียรภาพของลีเฟลสปาติกพอร์ซเลน วิต้า โอเมก้า 900 ที่ขัดผิวด้วย วิธีต่างๆ 3 วิธี และการเคลือบผิวแบบธรรมชาติ วัสดุและวิธีการ โดยเตรียมชิ้นงานพอร์ซเลน เป็นแผ่นรูปวงกลม จํานวน 80 ชิ้น แบ่งเป็น 4 กลุ่ม ๆ ละ 20 ชิ้นนํา ไปเคลือบผิวแบบธรรมชาติ และขัดผิว 3 วิธี (หัวขัดเซราไหล, ชุดหัวขัดโซฟูและครีมขัดผสมกากเพชร, ชุดหัวขัดใช และหัวขัดไตฟินิช) ประเมินเสถียรภาพของสีตามข้อกําหนดเลขที่ 69 ว่าด้วยเซรามิกทางทันตกรรมของสมาคม ทันตแพทย์แห่งสหรัฐอเมริกา โดยแบ่งชิ้นงานในแต่ละกลุ่ม 10 ชิ้น จุ่มในสารละลายเมทธิลีนบลู อีก 10 ชิ้น จุ่มใน น้ํากลั่นเพื่อเป็นกลุ่มควบคุม วัดค่าสีในระบบสี ไฮยี ก่อนและหลังจุ่มสารละลายด้วยเครื่องสเปกโตรโฟโตมิเตอร์ นํามาคํานวณค่าความแตกต่างของสี ประเมินความเรียบด้วยกล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราด และเปรียบเทียบความแตกต่างของสีของแต่ละกลุ่มด้วยการวิเคราะห์ความแปรปรวน ผลการศึกษา กลุ่มที่เคลือบผิวแบบธรรมชาติให้ผิวเรียบที่สุดและมีเสถียรภาพของสีดีที่สุด การขัดผิวด้วยชุดหัวขัด โซฟและหัวขัดไตฟินิชให้ผิวที่เรียบกว่าวิธีอื่นและมีเสถียรภาพของสีไม่แตกต่างจากการขัดด้วยชุดหัวขัดโชฟุ และครีม ผสมกากเพชร (p > 0.05) สรุป การเปลี่ยนสีของชุดหัวขัดใชฟและหัวขัดไตฟินิชนั้นสามารถยอมรับได้ทางคลินิก แต่กลุ่มที่ขัดผิวด้วยหัวขัด เซราไพลมีการเปลี่ยนสีที่สามารถสังเกตเห็นได้ โดยความแตกต่างของสีมากกว่าทุกกลุ่ม อย่างมีนัยสําคัญทางสถิติ(p < 0.05)

บทความนี้มีวัตถุประสงค์เพื่อรวบรวมเนื้อหาต่าง ๆ ที่เกี่ยวข้องกับการประเมินระยะการเจริญเติบโตของ ร่างกายและใบหน้าในช่วงวัยเริ่มเจริญพันธุ์ จากภาพรังสีมือ-ข้อมือ (hand-wrist radiograph) และจากกระดูก คอในภาพรังสีกะโหลกศีรษะด้านข้าง (lateral cephalometric radiograph) เปรียบเทียบความแตกต่างของ กลุ่มที่มีการเจริญเต็มที่เร็ว (early maturers) กับกลุ่มที่มีการเจริญเต็มที่ช้า (late maturers) รวมถึงความสัมพันธ์ ระหว่างการเจริญเติบโตของร่างกายกับกระดูกขากรรไกร

Objective The aim of this study is to investigate the pulpal blood flow in streptozotocin-induced diabetic rats by using laser Doppler flowmeter Materials and methods The animal model of streptozotocin (STZ)-induced diabetic rats (i.v. injection of STZ 55 mg/kg BW) was used. This study examined the relationship between hope and health status over a two-year period in individuals who were HIV -infected. One hundred and three individuals completed the surveys at entry into the study, at 12 months, and at 24 months. The Health Status Questionnaire (Wu et.al., 1991) measured Cognitive Functioning, Emotional Functioning, and Physical Functioning. Additionally, the Hope Scale (Snyder, 1991) and the Client Information Form were used to collect data. Background: Childhood obesity has been recognized as the new epidemic in developed countries. Caloric restriction with physical activity is the main therapeutic treatment available for these children. We compared two different dietary protocols to assess treatment efficacy.

Methods: Obese children from the Pediatric Endocrinology clinic were prospectively recruited for the study. Children and their parents were allowed to choose one of two dietary protocols: (1) carbohydrate restricted diet (<30 g/day), with unlimited calories, protein, and fat (High protein, Low CHO Diet), and (2) calorie restricted diet (Low Cal Diet). Anthropometric data were measured at baseline and at the 2 month follow up appointment.

Results: Thirty-seven children completed the study of whom 27 chose High Protein, Low CHO Diet and 10 chose Low Cal diet. No differences in gender ratio, age, or …

Autosomal dominant polycystic kidney disease (ADPKD) is the result of mutations in one allele of the PKD1 or PKD2 genes, followed by "second hit" somatic mutations of the other allele in renal tubule cells. Continued proliferation of clonal cells originating from different nephron segments leads to cyst formation. In vitro studies of the mechanisms of cyst formation have been hampered by the scarcity of nephrectomy specimens and the limited life span of cyst-derived cells in primary culture. Previous studies have investigated the relationship between impaired physical fitness and the decline of living function in the elderly population. This study is to investigate the fitness profile in the middle and older persons, and to observe the effect of age on fitness.This study recruited 108 subjects, with independent ambulation skills in a Taipei community. There were 59 males and 49 females. The average age was 70.0±6.2 year-old in male and 65.9±8.9 year-old in female, respectively. A patient with traumatic rotator cuff tear of right shoulder developed severe myofascial pain syndrome with reflex sympathetic dystrophy (RSD) involving the right upper extremity. She was unable to take any type of manual therapy or needle treatment due to severe allodynia in the whole right upper limb. She visited our pain clinic about 2 months after the onset of trauma. She received myofascial trigger point (MTrP) injection beginning with injection into the MTrP of the right first dorsal interosseous muscle, followed by injecting MTrPs of the right wrist-finger extensors and right anterior deltoid muscles. The development of pelvic lymphocele is usually a complication following renal transplantation as well as pelvic lymphadenectomy for gynecologic or urologic cancers. Pelvic lymphocele after blunt trauma is rare described before. Asymptomatic patients are managed with observation and follow-up imaging studies. Symptomatic lymphoceles need to be treated by means of percutaneous needle aspiration or drainage. We reported a case of low back pain after a fall with delayed onset of ispilateral thigh swelling. Computed tomography revealed a fluid-filled mass in the pelvis. After treatment by CT-guided percutaneous aspiration, all symptoms rapidly resolved without recurrent symptoms in 6 months follow-up.

The effectiveness and safety of hormone replacement therapy (HRT) have been studied since the 1980s. However, much of the available evidence was contradictory, and practitioners were uncertain how to counsel peri- and postmenopausal patients. In this article, recent evidence, both for and against use of HRT in peri- and postmenopausal patients, is compared to past studies with recommendations for practice.

Currently 16% of Americans breastfeed their children for at least 12 months as recommended by the AAP, which is well below the HP 2010 goal of 25%. Breastfed infants receive benefits that can improve their health throughout their lives. The benefits of breastfeeding for children include increased resistance to infectious diseases, such as gastroenteritis, respiratory tract infections, and ear infections. Breastfed children also display lower rates of chronic diseases including diabetes, obesity, asthma, and leukemia. The choice to breastfeed results in economic benefits from lower health care costs and from reduced spending on infant formula. The Healthy People 2010 targets …
