วัตถุประสงค์ เพื่อศึกษาว่าที่ระดับค่าความเป็นกรด-ด่างและระยะเวลาเท่าใดของแคลเซียมไฮดรอกไซด์จึงจะ สามารถฆ่าเชื้อ 4 ตัวได้ วัสดุและวิธีการ เตรียมแคลเซียมไฮดรอกไซด์ผสมกับน้ํากลั่นให้ได้ระดับค่าความเป็นกรด-ด่าง 12.5, 12.0, 11.5, 10.5, 9.5, 8.5 และ 7.5 ใส่ลงไปในหลอดที่เตรียมเชื้อทั้ง 4 ตัวคือ เชื้อเอนเทอโรคอคคัส ฟิคัลลิส, เชื้อ แอคติโนมัยชีส วิสโคซุส, เชื้อสแตฟฟีโลคอคคัส ออเรียสและเชื้อสเตรปโตคอคคัส มิวแทนส์ ที่มีความเจือจาง เท่ากับแมคฟาแลนด์นัมเบอร์ 1 ตั้งทิ้งไว้ที่อุณหภูมิห้อง เมื่อครบ 10 นาที, 30 นาที, 1 ช.ม., 6 ซ.ม., 24 ซ.ม., 48 ซ.ม. และ 72 ซ.ม. นําสารในหลอด ใส่ในอาหารเลี้ยงเชื้อ 2 หลอดทุก ๆ ระดับค่าความเป็นกรด-ด่าง โดยมี น้ําเกลือเป็นตัวควบคุม นําอาหารเลี้ยงเชื้อเข้าตู้อบ 37°C เป็นเวลา 48 ช.ม. แล้วอ่านผลโดยดูจากความขุ่น ถ้าขุ่น แสดงว่ามีการเจริญของเชื้อ ทําซ้ํา 3 ครั้ง ตอนที่ 2 เตรียมแคลเซียมไฮดรอกไซด์ให้ได้ค่าความเป็นกรด-ด่าง เหมือนในตอนแรก นําเชื้อที่เจือจางแล้ว ใส่ลงไปในสารที่เตรียมไว้ อ่านค่าความเป็นกรด-ด่างตามเวลาที่เปลี่ยน ไปเหมือนตอนแรก ทําการทดลองซ้ํา 3 ครั้งนํามาหาค่าเฉลี่ย ผลการศึกษา เชื้อทั้ง 4 ตัวสามารถถูกทําลายโดยแคลเซียมไฮดรอกไซด์ที่ระดับค่าความเป็นกรด-ด่าง 12.5 โดย ค่าเฉลี่ยการลดลงของความเป็นกรด-ด่างเริ่มต้นที่ 12.5 มีความแตกต่างจากกลุ่มอื่นอย่างมีนัยสําคัญทางสถิติ (Repeated-Measures ANOVA ที่ p0.00) ส่วนค่าความเป็นกรด-ด่างที่ต่ํากว่า 10 ไม่สามารถทําลายเชื้อตัวใดได้เลย โดยใช้เวลาในการสัมผัสกับแคลเซียมไฮดรอกไซด์แตกต่างกัน เชื้อสเตรปโตคอคคัส มิวแทนส์ ใช้เวลา 30 นาที เชื้อแอคติโนมัยชีส วิสโคซุส …

การศึกษาเปรียบเทียบประสิทธิภาพของสารต่างๆ ที่ใช้ในการดึงเกลือแร่, วิจิตรา วิพิตมากูล, จุลจักร ตั้งยิ่งยง, สุชีรา พงษ์พันธ์

วัตถุประสงค์ เพื่อเปรียบเทียบประสิทธิภาพของสารที่ใช้ดึงเกลือแร่ 3 ชนิด คือ เอทิลีน ไตเอมีน เตตรา อะซิติก แอซิด (อีดีทีเอ) กรดฟอร์มิก และกรดไนตริกฟอร์มาลีน โดยพิจารณาจากระยะเวลาที่ใช้ ราคา วิธีการเตรียมสาร และการสูญเสียของเนื้อเยื่อ วัสดุและวิธีการ ใช้ฟันกรามใหญ่ที่สาม จํานวน 12 ซี่ แบ่งเป็น 3 กลุ่ม กลุ่มที่ 1 ดึงเกลือแร่โดยใช้อีดีทีเอ กลุ่ม ที่ 2 ใช้กรดฟอร์มิก และกลุ่มที่ 3 ใช้กรดไนตริกฟอร์มาลีน เปรียบเทียบระยะเวลาที่ใช้ดึงเกลือแร่จนสมบูรณ์ซึ่ง ตรวจสอบโดยวิธีการใช้สารเคมี เปรียบเทียบราคาของสารแต่ละชนิดและศึกษาการเปลี่ยนแปลงของเนื้อเยื่อฟัน หลังจากถูกดึงเกลือแร่ ผลการศึกษา กรดไนตริก ฟอร์มาลีนดึงเกลือแร่จากฟันได้เร็วที่สุดคือ 102.0 ชั่วโมง รองลงมาคือ กรดฟอร์มิก 148.5 ชั่วโมง และ อีดีทีเอ 696.8 ชั่วโมง การเปรียบเทียบราคาต่อขบวนการ คือ อีดีทีเอ 395.43 บาท กรดฟอร์มิก 89.10-148.50 บาท และกรดไนตริกฟอร์มาลีน 21.17-30.26 บาท กรดไนตริกฟอร์มาลีนเป็นสารเพียงชนิดเดียว ที่ทําให้เกิดการเปลี่ยนสีและเกิดการสูญเสียเนื้อเยื่อฟัน สรุป กรดไนตริกฟอร์มาลีนใช้เวลาน้อยที่สุดในการดึงเกลือแร่และมีราคาถูกที่สุดแต่มีข้อเสีย คือ อาจทําให้มีการ สูญเสียเนื้อเยื่อฟัน กรดฟอร์มิกซึ่งใช้เวลามากกว่า และมีราคาสูงกว่า เหมาะสมในการใช้งานด้านบริการ เนื่องจาก ไม่ทําให้เกิดการสูญเสียเนื้อเยื่อ แต่อีดีทีเอไม่เหมาะในการใช้งานด้านบริการเพราะใช้เวลานานมากและมีราคาสูง

ผลของแคลเซียมและความเป็นกรดของนมต่อการแตกตัวของฟลูออไรด์, พรศรี ปฏิมานุเกษม, ยุทธนา ปัญญางาม, ชนัญญา เกษประเสริฐ, พธู สุระประเสริฐ

วัตถุประสงค์ เพื่อเปรียบเทียบการแตกตัวของฟลูออไรด์ที่เติมลงในนมชนิดธรรมดา นมชนิดแคลเซียมสูง และนม เปรี้ยวซึ่งมีความเป็นกรดสูง วัสดุและวิธีการ สุ่มผลิตภัณฑ์นมที่มีจําหน่ายในกรุงเทพมหานคร ได้แก่ นมธรรมดา นมแคลเซียมสูง และนมเปรี้ยว จํานวน 10, 5 และ 11 ผลิตภัณฑ์ ตามลําดับ วัดความเป็นกรด-ด่างและปริมาณฟลูออไรด์ตั้งต้นในตัวอย่างนม โดยใช้พีเอชอีเล็คโทรดและฟลูออไรด์อิเล็คโทรด แบ่งนมจํานวน 100 มล เติมฟลูออไรด์ 0.5 มก แล้ววัดปริมาณ ฟลูออไรด์และคํานวณร้อยละของปริมาณฟลูออไรด์ไอออนอิสระที่ปล่อยออกมา เปรียบเทียบค่าเฉลี่ยปริมาณฟลูออไรด์และค่าเฉลี่ยร้อยละของปริมาณฟลูออไรต์ไอออนอิสระที่ปล่อยออกมาระหว่างนมแต่ละชนิดด้วยการ วิเคราะห์ความแปรปรวนที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา นมธรรมดา นมแคลเซียมสูง และนมเปรี้ยว มีค่าความเป็นกรด-ด่าง เท่ากับ 6,470 + 0.086, 6.676 + 0.125 และ 3.869 + 0.121 และมีปริมาณฟลูออไรด์ตั้งต้นเท่ากับ 0.108 ± 0.034, 0.157 ± 0.149 และ 0.314 + 0.140 ส่วนในล้านส่วน ตามลําดับ ภายหลังการเติมฟลูออไรต์ 0.5 มก พบว่านมธรรมดา นมแคลเซียมสูง และ นมเปรี้ยวมีค่าเฉลี่ยร้อยละของปริมาณฟลูออไรด์ไอออนอิสระที่ปล่อยออกมาเท่ากับ 21.36 + 6.67, 13.43 + 10.94 และ 25.70 + 12.78 ตามลําดับ การทดสอบทางสถิติ พบว่า นมแคลเซียมสูงมีร้อยละของปริมาณฟลูออไรด์ไอออน อิสระที่ปล่อยออกมาน้อยกว่านมธรรมดาอย่างมีนัยสําคัญทางสถิติ (p<0.05) แต่นมเปรี้ยวมีร้อยละของปริมาณ ฟลูออไรด์ไอออนอิสระที่ปล่อยออกมาไม่แตกต่างจากนมธรรมดาอย่างมีนัยสําคัญทางสถิติ สรุป ผลิตภัณฑ์นมชนิดแคลเซียมสูงมีผลทําให้ฟลูออไรด์ที่เติมลงไปแตกตัวได้น้อยลง ในขณะที่ความเป็นกรดของนมไม่มีผลต่อการแตกตัวของฟลูออไรด์

อะมิโลเจเนซิส อิมเพอร์เฟกตา เป็นกลุ่มของความผิดปกติทางด้านพันธุกรรมที่มีผลต่อการสร้างเคลือบ ฟันน้ํานมและฟันแท้ ความผิดปกติเกิดเฉพาะที่เนื้อเยื่อชั้นเอ็กโตเดิร์ม รายงานผู้ป่วยนี้อธิบายถึงลักษณะความ ผิดปกติของฟันที่ตรวจพบทางคลินิก ลักษณะภาพรังสี ผลของการวินิจฉัย และการรักษาแบบเฉพาะกาลในฟันแท้ ของเด็กชายไทยอายุ 9 ปี ซึ่งกระทําโดยมีวัตถุประสงค์หลักของการรักษาทางทันตกรรมแก่ผู้ป่วยในรายนี้คือ เพื่อ ป้องกันถึงการสูญเสียของเนื้อเยื่อโครงสร้างฟันจากความผิดปกติดังกล่าว และทําให้การบดเคี้ยวมีประสิทธิภาพดี รวมทั้งทําให้เกิดความสวยงามของใบหน้าและช่องปาก

ความสัมพันธ์ระหว่างการปรากฏของกระดูกทอรัส ที่เพดานปาก และที่ขากรรไกรล่างกับจํานวนฟัน ในคนไทยกลุ่มหนึ่ง, วันดี อภิณหสมิต, ดอลลี่ เมธาธราธิป, สมพร สวัสดิสรรพ์, สุชาติ กาลวสุกุล

วัตถุประสงค์ เพื่อหาความสัมพันธ์ระหว่างการปรากฏของกระดูกทอรัสที่เพดานปาก (TP) และที่ขากรรไกรล่าง (TM) กับจํานวนฟันในคนไทยกลุ่มหนึ่ง วัสดุและวิธีการ ท่าการตรวจช่องปากคนไทย จํานวน 1,350 คน ณ คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย เพื่อสํารวจการปรากฏของ TP และ TM ร่วมกับการบันทึกจํานวนฟัน เพศ และอายุ แบ่งกลุ่มตัวอย่างเป็น 4 กลุ่ม อายุ คือ 13-19, 20-29, 30-39, 40-49, 50-59 และ 60 ปีขึ้นไป ศึกษาความสัมพันธ์ระหว่างการปรากฏของ กระดูกทอรัสทั้งสองกับจํานวนฟันด้วยการเปรียบเทียบความแตกต่างระหว่างจํานวนฟันของคนที่พบและไม่พบ กระดูกทอรัสทั้งสอง วิเคราะห์ความแตกต่างระหว่างกลุ่มด้วยสถิติทดสอบแมนน์ วิทนีย์ยู ที่ระดับนัยสําคัญทางสถิติ 0.05 ผลการศึกษา พบว่าค่าเฉลี่ยของจํานวนฟันบนในคนที่พบและไม่พบ TP ไม่แตกต่างกัน (p = 0.974) ส่วน ค่าเฉลี่ยของจํานวนฟันล่างในคนที่พบ TM มีค่ามากกว่าคนที่ไม่พบ TM (p = 0.001) เมื่อศึกษาในแต่ละเพศ ปรากฏว่าเฉพาะในเพศชายเท่านั้นที่ค่าเฉลี่ยของจํานวนฟันล่างในคนที่พบ TM มีค่ามากกว่าคนที่ไม่พบ TM (p = 0.010) ยิ่งไปกว่านั้นยังพบว่าจํานวนฟันที่สูญเสียไปในเพศชายที่พบ TM น้อยกว่าในเพศชายที่ไม่พบ TM (p = 0.010) เมื่อพิจารณาตามกลุ่มอายุปรากฏว่าค่าเฉลี่ยของจํานวนฟันบนแตกต่างกันในคนที่พบและไม่พบ TP เฉพาะในกลุ่มอายุ 60 ปีขึ้นไป ในขณะที่เมื่ออายุ 30 ปีขึ้นไป ค่าเฉลี่ยของจํานวนฟันล่างในคนที่พบ TM มีค่า มากกว่าคนที่ไม่พบ TM (p < 0.05) ยกเว้นในกลุ่มอายุ 40-49 ปี สรุป พบความสัมพันธ์ระหว่างการปรากฏของกระดูกทอรัสที่ขากรรไกรล่างกับจํานวนฟันล่างในกลุ่มตัวอย่างทั้งหมด และในเพศชาย

หลายครั้งทันตแพทย์มีความจําเป็นที่จะต้องทําครอบฟันหรือทําครอบฟันใหม่ให้แก่ฟันหลักของชิ้นงาน ฟันปลอมบางส่วนถอดได้ชนิดโครงโลหะชิ้นเดิม ปัญหาที่พบมักได้แก่การที่ผู้ป่วยปฏิเสธที่จะทิ้งชิ้นงานฟันปลอม เดิมให้แก่ทันตแพทย์ระหว่างการสร้างครอบฟันและความยากของขบวนการที่จะทําครอบฟันเพื่อให้มีรายละเอียดเข้ากันได้ดีกับชิ้นงานฟันปลอมเดิม บทปริทัศน์วรรณกรรมได้นําเสนอเทคนิคและวิธีการต่าง ๆ ในการแก้ปัญหา ดังกล่าว บางเทคนิคทําขั้นตอนบางส่วนนอกช่องปากและใช้เวลาปรับแต่งในคลินิกเพิ่มเติม บางเทคนิคทําการแต่ง โดยตรงในช่องปาก และบางเทคนิคอาจอาศัยทั้งสองวิธีการ รายละเอียด ข้อดี ข้อด้อยของแต่ละวิธีการได้อธิบายไว้เพื่อเป็นข้อมูลให้แก่ทันตแพทย์ในการตัดสินใจเลือกวิธีการรักษาที่เหมาะสมและมีประสิทธิภาพมากที่สุดให้กับผู้ป่วย

Expression and activity of the Na-K-ATPase within the basolateral membrane domains of the trophectoderm epithelium provide the driving force for accumulation of Na(+) and Cl(-) across the nascent epithelium, mediating fluid movement into the forming blastocoel. Within the trophectoderm of the bovine blastocyst, multiple isozymes of the Na-K-ATPase are expressed. Immunolocalization has demonstrated that the alpha1-isozyme localizes within the basolateral membrane, whereas the alpha 3-isozyme localizes to the apical cell margins. Gene-specific RT-PCR and wholemount indirect immunofluorescence confocal laser scanning microscopy were used to examine expression of the Na-K-ATPase gamma-subunit (a regulatory subunit of the Na-K-ATPase) throughout development of bovine …

OBJECTIVE: There is a dearth of empirical research on physician empathy despite its mediating role in patient-physician relationships and clinical outcomes. This study was designed to investigate the components of physician empathy, its measurement properties, and group differences in empathy scores.

METHOD: A revised version of the Jefferson Scale of Physician Empathy (with 20 Likert-type items) was mailed to 1,007 physicians affiliated with the Jefferson Health System in the greater Philadelphia region; 704 (70%) responded. Construct validity, reliability of the empathy scale, and the differences on mean empathy scores by physicians’ gender and specialty were examined.

Diffuse idiopathic skeletal hyperostosis (DISH) is a common disorder in middle-aged and elderly patients but often ignored. It is characterized by calcification and ossification of the spinal ligaments, as well as entheseal ossification within extraspinal sites. The etiology of DISH is still unknown. It can result in chronic pain and stiffness and occasionally severe disability. It often mimiks with spondylosis and ankylosing spondylitis. We report a 71-year-old male patient who suffered from chronic back and shoulders pain for many years. DISH was diagnosed after a series of examinations. Primary antiphospholipid syndrome is a rare disease of vasculopathy, which is usually found in the stroke partient with onset during childhood and young adulthood. We report a female patient who had a history of multiple spontaneous abortions in the past. However, the diagnosis was not made until a cerebrovascular accident happened to her. After then, she started to receive anticoagulant therapy and rehabilitation program. No further cerebrovascular accident occurred in the following 22 months. By this report, we remind that a female patient with history of frequent abortion needs to be investigated carefully to search the possibility of antiphospholipid syndrome.

Species identification of isolates of the Mycobacterium avium complex (MAC) remains a difficult task. Although M. avium and Mycobacterium intracellulare can be identified with expensive, commercially available probes, many MAC isolates remain unresolved, including those representing Mycobacterium lentiflavum as well as other potentially undefined species. PCR restriction analysis (PRA) of the hsp65 gene has been proposed as a rapid and inexpensive approach.
