This issue brief looks at the incidence of error in the health care system, opportunities for a systems-based approach to error reduction, and changes needed in health system culture and training. The lessons of human factors research are considered, with examples of their application in the aviation industry. The paper reviews some error-reduction and patient-safety initiatives undertaken by private-sector organizations and by the Veterans Health Administration.

Advanced practice nurses are essential primary health care providers actively engaged in health promotion, illness prevention, and the management of acute and chronic diseases. The purpose of this study was to measure the quality of life and locus of control among three different groups of migraineurs. A retrospective quantitative study was conducted using Ferrans and Powers' (1985) Quality of Life Index and Martin, Holroyd, and Penzien's (1990) Headache Specific Locus of Control questionnaire. The analysis of 79 adult migraineurs indicated that those who received headache education had a higher quality of life and higher external locus of control scores than …

Data-Based Clinical Decision Making In The Treatment Of An Adolescent With Severe Conduct Problems, Douglass W. Nangle, Rebecca E. Carr, David J. Hansen

This case illustrates the contributions of continuous data monitoring to clinical decision making in the treatment of an adolescent with severe conduct problems. Treatment began with a comprehensive point system that required the continual monitoring of a range of target behaviors. Beginning each session with a review of graphs of the monitored behaviors helped keep the sessions focused and rewarded the client and his parents with visual feedback of their progress. A subjective rating system was also implemented in which the client’s parents quantified their evaluations of his overall behavior on a daily basis. This system enhanced the client’s parents’ …

This is the second installment of the special issue on prosocial and moral development in early adolescence. This issue focuses on social and contextual processes in young adolescents’ prosocial and moral behaviors. In this introductory article, a brief review of the research on parents or family, peers, school environment, culture, and nationality as correlates of prosocial and moral development was presented. The research indicates that the social context plays an important role in prosocial and moral development. Relatively little is known about the incidence and prevalence of injuries and other secondary conditions experienced by adults with developmental disabilities. Understanding the risk factors for secondary conditions and strategies for their prevention is important to people with disabilities, rehabilitation providers, and public health agencies. Many secondary condition prevention strategies require direct involvement of the individual with a disability. However, the nature of some disabling conditions requires help from an intermediary facilitator, frequently known as a “Personal Care Attendant” (PCA) or a “Personal Assistant” (PA).

Study Of The Baig Gene In Bile Acid 7Α-Dehydroxylating Bacteria, Michael David

Recently, intestinal bile acid 7α-dehydroxylating bacteria have been implicated in cholesterol gallstone disease. Eubacterium sp. strain V.P.I. 12708, a bile acid 7α-dehydroxylating bacteria, contains multiple bile acid inducible (bai) genes which are located on a large bai operon. Genes of this operon encode the enzymes responsible for bile acid 7α-dehydroxylation. The baiG gene encodes a bile acid transporter in Eubacterium sp. strain V.P.I. 12708. Utilizing the polymerase chain reaction I determined the presence or absence of baiG-like genes in six bile acid 7α-dehydroxylating strains: Clostridium sp. Strain TN-271, Eubacterium sp. strain 1-10, Clostridium bifermentans strain 1-55, Clostridium sordellii strain Y67, …

Objective: Many hospitals are investing in computer-based provider order-entry (POE) systems, and providers’ evaluations have proved important for the success of the systems. The authors assessed how physicians and nurses viewed the effects of one modified commercial POE system on time spent patients, resource utilization, errors with orders, and overall quality of care.

Design: Survey.

Measurements: Opinions of 271 POE users on medicine wards of an urban teaching hospital: 96 medical house officers, 49 attending physicians, 19 clinical fellows with heavy inpatient loads, and 107 nurses.

Although horses have laterally-placed eyes, there is substantial binocular overlap, allowing for the possibility that these animals have stereopsis. In the first experiment of the present study we measured local stereopsis by obtaining monocular and binocular depth thresholds for renal depth stimuli. On all measures, the horses' binocular performance was superior to their monocular. When depth thresholds were obtained, binocular thresholds were several times superior to those obtained monocularly, suggesting that the animals could use stereoscopic information when it was available. Objective: To assess the association between physical violence during the 12 months before delivery and maternal complications and birth outcomes.

Methods: We used population-based data from 6143 women who delivered live-born infants between 1993 and 1995 in South Carolina. Data on women's physical violence during pregnancy were based on self-reports of partner-inflicted physical hurt and being involved in a physical fight. Outcome data included maternal antenatal hospitalizations, labor and delivery complications, low birth weights, and preterm births. Odds ratios and 95% confidence intervals were calculated to measure the associations between physical violence, maternal morbidity, and birth outcomes.

Nitrous oxide is a gas often used in dental, surgical and veterinary operations as an anesthetic agent. Employees stationed in proximity of fugitive gas emissions may potentially suffer adverse health effects from chronic exposure to this agent. Passive dosimeters can be worn in the breathing zone of potentially affected personnel during exposure periods, then later analyzed to quantify nitrous oxide exposure. This study sought to establish the accuracy of several commercially available passive monitoring devices for nitrous oxide by comparison against an infra-red spectrophotometric reference method.

Previous studies have demonstrated that surface antigen proteins, in particular SAG-1, of Toxoplasma gondii are important to this parasite as attachment ligands for the host cell. An in vitro assay was developed to test whether these ligands and other secretory proteins are involved in the immune response of human cells to toxoplasma. Human monocytes were infected with tachyzoites in the presence of antiparasite antibodies, and their effect on mitogen-induced lymphoproliferation was examined. The presence of antibody to either parasite-excreted proteins (MIC-1 and MIC-2) or surface proteins (SAG-1 and SAG-2) during infection neutralized the marked decrease seen in mitogen-induced lymphoproliferation in …

The purpose of this quantitative descriptive study was to examine the acceptance of an Appalachian community's acceptance of a family nurse practitioner in the emergency department.A conceptual framework of leek Ajzen(1988) was used to predict the acceptance of a family nurse practitioner in the emergency department from a sample of participants in a community.Subjects (n=109) from a sample in Maryville, Tennessee who were shopping at Wal-Mart® were used in this study.A tool in which acceptance was acknowledged was used, and yes and no answers were seen as acceptance or nonacceptance.Descriptive statistics suggest that the sample overall was accepting of the …

Lead poisoning is one of the most common pediatric health problems in the United States. A direct result of industrialization, the toxic substance lead has contaminated our environment. Parents of young children should be aware of the hazards of lead, the environmental sources, the need for annual screening. the implications of screening results. and steps to reduce the chances of exposure. Questionnaires were distributed at a pediatrician's office. Parents with children under six years of age were asked to complete the survey. Sixty-two questionnaires were completed and returned. This grounded theory study explored the experiences of formerly homeless individuals and families who have moved from homelessness into stable housing. This was an ethnographic study and involved the researcher staying in an east coast shelter where some of the formerly homeless participants had become staff members and reside. Data analysis was informed by dimensional analysis. Moving out of homelessness was the studies perspective.
