The purpose of this paper is to present a review of the literature related to the proposition that oppositional defiant disorder and conduct disorder are, in essence, the same disorder, with varying degrees of severity. The author of this paper believes that it would make more sense diagnostically to combine these two diagnoses into one category.

Mutant strains of obese rats are currently being studied to reveal the mechanisms of increased lipid synthesis and decreased lipid oxidation in these rats. It was originally thought that much of the fat production that leads to obesity is the result of metabolic processes in the liver and adipose tissue. Studies by Kahle et al.³ showed that the liver and adipose tissue account for only 22% and 7% respectively, of total fatty acid synthesis. It was also shown that 21 % of total de novo fatty acid was localized in the postural skeletal muscle.

The prolonged presence of human activity in a watershed can disturb the physical and chemical cycles of the lake and its surrounding ecosystems (Henderson-Sellers and Markland 1987). ABSTRACT

Background Obese women with the polycystic ovary syndrome are relatively unresponsive to the induction of ovulation by clomiphene. We hypothesized that reducing insulin secretion by administering metformin would increase the ovulatory response to clomiphene.

Methods We performed oral glucose-tolerance tests before and after the administration of 500 mg of metformin or placebo three times daily for 35 days in 61 obese women with the polycystic ovary syndrome. Women who did not ovulate spontaneously were then given 50 mg of clomiphene daily for five days while continuing to take metformin or placebo. Serum progesterone was measured on days 14, 28, …

Purpose: Identify perceptions of professional autonomy of Board Certified Physical Therapy Specialists (BCPTS) who practiced in managed care environments.

Guiding questions: Do BCPTS perceive an increase in professional autonomy as a specialist than as a generalist? How do the perceptions of professional autonomy vary among each specialization? What variables affect perceptions of professional autonomy among BCPTS?

Method: Two-hundred BCPTS throughout the country were randomly chosen as participants. One-hundred-eight respondents were used as subjects.

Data analysis: Frequency distributions and Pearson's correlation coefficient statistic were utilized.

Results: Majority of respondents reported no increase in professional autonomy as a specialist. Greater than three-quarters …

BACKGROUND

Although trimethoprim–sulfamethoxazole is the drug of choice for the prevention of Pneumocystis carinii pneumonia, many patients cannot tolerate it and must switch to an alternative agent.

METHODS

We conducted a multicenter, open-label, randomized trial comparing daily atovaquone (1500-mg suspension) with daily dapsone (100 mg) for the prevention of P. carinii pneumonia among patients infected with the human immunodeficiency virus who could not tolerate trimethoprim–sulfamethoxazole. The median follow-up period was 27 months.

BACKGROUND

Only 15 to 20 percent of patients with chronic hepatitis C have a sustained virologic response to interferon therapy. We compared the efficacy and safety of recombinant interferon alfa-2b alone with those of a combination of interferon alfa-2b and ribavirin for the initial treatment of patients with chronic hepatitis C.

METHODS

ABSTRACT

Background Standard clinical practice permits the use of either single-chamber ventricular pacemakers or dual-chamber pacemakers for most patients who require cardiac pacing. Ventricular pacemakers are less expensive, but dual-chamber pacemakers are believed to be more physiologic. However, it is not known whether either type of pacemaker results in superior clinical outcomes.

Methods The Pacemaker Selection in the Elderly study was a 30-month, single-blind, randomized, controlled comparison of ventricular pacing and dualchamber pacing in 407 patients 65 years of age or older in 29 centers. BACKGROUND

Interferon alfa is the only effective treatment for patients with chronic hepatitis C. Forty percent of patients have an initial response to this therapy, but most subsequently relapse. We compared the effect of interferon alone with that of interferon plus oral ribavirin for relapses of chronic hepatitis C.

METHODS

We studied 345 patients with chronic hepatitis C who relapsed after interferon treatment. Background and Methods Although generalized convulsive status epilepticus is a life-threatening emergency, the best initial drug treatment is uncertain. We conducted a five-year randomized, doubleblind, multicenter trial of four intravenous regimens: diazepam (0.15 mg per kilogram of body weight) followed by phenytoin (18 mg per kilogram), lorazepam (0.1 mg per kilogram), phenobarbital (15 mg per kilogram), and phenytoin (18 mg per kilogram). We examine the effect of unions on the earnings of health care workers, with emphasis on the measurement and sources of union wage premiums. Using data constructed from the 1973 though 1994 Current Population Surveys, standard union premium estimates are found to be substantially lower among workers in health care than in other sectors of the economy, and to be smaller among higher skill than among lower skill occupational groups. Longitudinal analysis of workers switching union status, which controls for worker-specific skills, indicates a small impact of unions on earnings within both high and low skilled health care occupations. As clinicians, it is important to investigate various factors which could potentially reduce the occurrence of ankle injuries. Moore and co-workers have previously demonstrated that progestins stimulate the growth of T47D human breast cancer cells. We now investigate the possibility that progestins might transactivate the protooncogcne c-myc as part of the mechanism for growth stimulation. Treatment of T47D cells with the synthetic progestin R5020 results in a rapid, dose-dependent increase in c-myc mRNA. This stimulation is evident as early as 5 minutes, increases up to 4-fold at 1 hour, and then returns toward basal levels. The optimal concentration of R5020 for induction of c-myc gene expression is 10 nM, which is within the physiologically relevant range of hormone …

Sporulation is a developmental process in which temporally distinct sets of genes regulate the processes of meiosis and ascospore formation. Sporulation-specific genes are classified into early, middle, and late genes based on their peak time of expression during sporulation. Regulation of two late genes, SPR1 and SPR2, was studied. The function of SPR2 is unknown. Its regulatory region contains two Mid Sporulation Elements (MSE) repeated in tandem that are sufficient to confer sporulation-specific regulation to a CYCl-lacZ reporter. These sequences have UAS (upstream activation sequence) properties, as each 14 base pair element is sufficient to confer sporulation-specific regulation …

This study explored gender differences in reports of depressive symptoms among adolescents. Self-esteem was examined for its relationship to reports of depressive symptoms and self-consciousness was examined to determine its influence on self-esteem and depression. One hundred four 10th grade students were involved in the study. The Beck Depression Inventory-II, the Imaginary Audience Scale, and the Rosenberg Self-Esteem Inventory were used in the analysis. A hierarchical multiple regression analysis was used to analyze the data. Alpha was set at .05. A positive relationship was found between gender and the variables of depression, abiding self-consciousness, transient self-consciousness, and self-esteem. Females scored …

The link between caring and nursing has been evident since the days of Florence Nightingale. She referred to nursing as an act of charity or "God's work." Nursing within the Judea-Christian tradition was viewed as a mission, ministry or call from God to serve others. Those entering nursing fulfilled a commitment to God through service and care for others. Over time the focus in nursing shifted from the Christian perspective of being "called to be a nurse" to technical and physio­logical reasons for helping people. Physiologists say that ions and neutral solutes can cross biological membranes via “transporters” and “channels.” We tend to think about the difference between transporters and channels in terms of gating mechanisms. Ion channels exhibit a wide range of selectivity properties and permeation rates, but their gating at the most basic level can be thought of in terms of a single barrier or gate acting as a switch. When the gate is closed, ions can’t permeate; when the gate is opened, a permeation pathway for ions allows flux, often at very high rates (up to 10⁸/sec). Transporters and ion …

The rapid re-uptake of extracellular glutamate mediated by a family of high-affinity glutamate transporter proteins is essential to continued glutamatergic signaling and neuronal viability, but the contributions of individual transporter subtypes toward cellular physiology are poorly understood. Because the physiology of glutamate transport in the salamander retina has been well described, we have examined the expression and function of glutamate transporter subtypes in this preparation. cDNAs encoding five distinct salamander excitatory amino acid transporter (sEAAT) subtypes were isolated, and their molecular properties and distributions of expression were compared.
