Programs Of Assertive Community Treatment (Pact): A Critical Review, Tomi Gomory

Advocates of Programs of Assertive Community Treatment (PACT) make numerous claims for this intensive intervention program, including reduced hospitalization, overall cost, and clinical symptomatology, and increased client satisfaction, and vocational and social functioning. However, a reanalysis of the controlled experimental research finds no empirical support for any of these claims. Instead, there is evidence that the program is both coercive and potentially harmful. The current promotion of PACT appears to be based more on professional enthusiasm for the medical model than upon any benefit to the clients.

The higher prevalence of depression in women is coupled with a higher prevalence of pain complaints. Growing evidence suggests that the comorbidity of these conditions is also proportionately higher in women than men. This paper critically reviews the empirical findings relating to gender differences in comorbid pain and depression as well as findings in support of hypothesized etiologic factors that could explain why women may be more susceptible than men to comorbidity. The empirical evidence for biogenic, psychogenic, and sociogenic explanatory models is presented, and an integration of these models is proposed as a guideline to both research and clinical …

This report, produced jointly by the British Union for the Abolition of Vivisection and Compassion in World Farming, fills a number of significant gaps in the current debate about xenotransplantation.

In this report we also summarise the ethical and welfare issues concerning experiments on animals for xenotransplant research and their possible use as source animals for organs. Both these aspects are responsible for much pain and distress caused to many animals. We prefer the term “source animals” to “donor animals”, because animals do not choose to donate their organs for xenotransplantation.

How spouses support one another may be important in understanding and preventing marital distress, but has received relatively little attention. Instead, the behavioral model of marriage and corresponding treatment protocols have focused on the importance of good conflict management skills in preventing and treating marital distress. This paper outlines recent research indicating that couples social support skills predict marital outcome two years later, above and beyond conflict management skills. These results indicate that successful prevention and treatment programs may need to incorporate support skills training as well as conflict management training. We evaluated the side effects induced by injection of Freund's adjuvant (FA)and alternative adjuvants combined with different antigens. Rabbits and mice were injected subcutaneously, intramuscularly (rabbits) and intraperitoneally (mice)with different adjuvants (FA, Specol, RIBI,TiterMax, Montanide ISA50)in combination with several types of antigens (synthetic peptides, autoantigen, glycolipid, protein, mycoplasma or viruses). The effects of treatment on the animals' well-being were assessed by clinical and behavioural changes (POTand LABORASassays) and gross and histopathological changes. In rabbits, treatment did not appear to induce acute or prolonged pain and distress. Mice showed behavioural changes immediately after (predominantly secondary) immunization. Objective: The role of group therapy in treatment of posttraumatic stress disorder (PTSD) has been traditionally restricted to issues of self-esteem and interpersonal relationships, rather than primary symptoms of the disorder. In this study, the authors examined the effectiveness of a 16-week trauma-focused, cognitive-behavioral group therapy, named Interactive Psychoeducational Group Therapy, in reducing primary symptoms of PTSD in five groups (N=29) of multiply traumatized women diagnosed with chronic PTSD. Method: The authors made assessments at baseline, at 1-month intervals during treatment, at termination, and at 6-month follow-up by using self-report and structured interview measures of PTSD and psychiatric symptoms. Most existing cephalometric analyses are based on norms for the Caucasian population. Therefore, they cannot be properly applied for Mexican or Hispanic patients. Ethnic variations in the normal positions of the maxilla and mandible influence and alter the diagnosis and treatment plan for each of the different type of malocclusions. Previous studies have addressed the difference between the Caucasian norms and the Hispanic norms, this indicates the variety of their skeletal, dental and facial features.

This study investigates gender differences in the barriers and incentives that are most influential in the coronary patient's decision to participate in cardiac rehabilitation (CRPs) and suggests strategies to counter these barriers. Patient surveys were administered to consecutive English-speaking attenders and referred nonattenders to a cardiac rehabilitation center at a university healthcare system in Toronto, Canada. A survey questionnaire, constructed from a literature review and advice from key informants, examined potential factors affecting decisions to engage in CRPs. One129 attenders at a CRP and 61 referred nonattenders completed the questionnaire. Physician recommendation was reported to be the most important factor …

Over the years, numerous proposals have been made in the counselor education (CE) literature about what counseling philosophy can best lead the profession to a distinctive professional identity (Bauman & Waldo, 1998; Fong & Lease, 1994; Guterman, 1994). An issue in this debate is whether psychiatric diagnostic training forms a part of a counseling philosophy (i.e., model-for-helping) and professional identity that is more focused on client psychopathology than on normal developmental issues. This study explored how training and experience in psychiatric diagnostic categories (PDCs) influenced counselors’ development of their counseling philosophy and professional identity.

Mexican Americans have a higher incidence of obesity than non-Hispanic whites, yet this population seems to be less concerned with maintaining an ideal body weight than non-Hispanic whites. This puts the Mexican Americans population at higher risk for illnesses associated with increased body fat and obesity. One important factor found to influence health maintenance behavior is perceived self-efficacy, or the degree to which the individual feels capable of effectively managing his or her behavioral outcome. This study examines the moderating effect of health competency and health value on body weight in individuals undergoing acculturation.

For bottom line oriented payors such as government and business, the focus is on measuring cost. As hospitals account for over one-third of health care expenses, hospital cost per admission is a common measure of performance.

Many environmental and organizational factors come into play in determining hospital cost per admission. Adolescents often make adult decisions regarding their lifestyle and behavior with little support from their family and community. In this study the researcher examined the difference in risk-taking behaviors between genders and the role that families and communities play in reducing risk-taking behaviors. In the study I further analyzed self-reports of numbers of development assets those students reporting participation in high-risk behaviors. Information about how operators use their eyes while interacting with visual displays is often an overlooked aspect of human-computer interaction. Such information is fundamental to assessing the quality of software interfaces and understanding the cognitive processes that underlie operator behavior. Other research evaluating information displays evolved from using reaction time and subjective data as dependent variables to using oculometric measures. In the current research conventional performance measures are coupled with oculometric measures to evaluate the influence display characteristics and cognitive variables have on performance.

The purpose of this study was to determine whether women with breast cancer and the presence of the drug tamoxifen, are more severely depressed than women with breast cancer and the absence of tamoxifen. Average Beck Depression Inventory (BDI) scores did not differ between the two sub-groups; however, two significant group differences were found. First, 10 women in the tamoxifen group had scores of zero (versus 4 in the absence of prescribed tamoxifen group). It is well established that repeated, high doses of 3, 4-methylenedioxymethamphetamine (MDMA) result in the long-term depletion of serotonin levels and destruction of serotonergic terminals in various locations in the brains of a variety of species. Further, it is also well known that concomitant injections of the serotonin reuptake inhibitor, fluoxetine, prevents this deterioration. It has recently been noted that such MDMA neurotoxicity disrupts stimulus control in rats trained to discriminate MDMA from saline in a drug discrimination procedure (Schechter, 1991a).

In the early days of the United States, care of the disabled elderly outside the home meant the public almshouse. By the 1920s, private, nonprofit homes for the aged were prevalent. More recently, private, for-profit facilities have grown to dominate the field.

For-profit ownership has been controversial. Underlying the controversy is the concern that quality might be lowered in order to enhance profit.
