This report summarized recent studies on the basic structures of a myofascial trigger point (MTrP). A MTrP is a hyperirritable spot in a palpable taut band of skeletal muscle fibers. Spot tenderness, recognized pain, and taut band are now demonstrated to be reliable diagnostic criteria of an MTrP. Referred pain (ReP) or local twitch response (LTR) is a confirmatory sign of MTrP. There are multiple loci in an MTrP region. The basic unit of an MTrP is the MTrP locus which contains a sensory component (the sensitive locus) and a motor component (the active locus). Although it has variable reliability, the bedside clinical swallowing evaluation (CSE) of dysphagia has long been accepted as the basic screening tool in the examination of the swallowing function of stroke patients. The purpose of this study was to define the reliability of the CSE by comparing it to the standard videofluoroscopic study of swallowing (VFSS). Thirty six stroke patients within a period of two years were included in this study, and all of them received the CSE and VFSS simultaneously. Three of them had a normal VFSS, whereas 21 (58%) aspirated during the VFSS examination. Ten of the people …

nfants with a birth weight less than 1500gm (very low birth weight, VLBW) are more likely to have disabling cerebral palsy(CP). The aim of this study was to identify prenatal, perinatal, postnatal risk factors for CP in VLBW infants and to determine the usefulness of cranial ultrasonographic (us) findings in predicting CP.The study group comprised 159 VLBW infants born at Chang Gung Memorial Hospital in Kaohsiung in the period from 1989 to 1996. CP was diagnosed via clinical evaluation in twenty-five (15.7%) of these Children. Paraplegia Following Epidural Anesthesia: A Casereport, Yu-Ching Lin, Yen-Ho Wang, Kou-Mou Huang, Tien-Shang Huang

This article reports the case of a 36-year-old woman who experienced a sudden sharp pain in her lower back and became paraplegic after epidural anesthesia for a cesarean section. Three months later, the appearance of an ascending sensory level despite motor recovery raised the prssibilite of another lesion causing the new neurologic symptoms. Magnetic resonance imaging revealed arachnoiditis with subarachnoid cysts that severely compressed the thoracic spinal cord. Surgical decompression relieved the neurologic symptoms at first, but the symptoms soon deteriorated due to re-formation of adhesions and probably intrinsic cord damage. การทดลองนี้ มีวัตถุประสงค์เพื่อศึกษาผลการแช่แข็งตัวอ่อนกระบือปลักไทย ที่เก็บจากแม่กระบือ ปลักพันธุ์ดีที่ถูกคัดออกจากฝูง โดยทําการแช่แข็งตัวอ่อนกระบือปลักจำนวน 29 ตัวอ่อน ด้วยวิธีมาตรฐาน ใช้อัตราการลดอุณหภูมิ 0.5°c ต่อนาที จากอุณหภูมิ -6.5°c ถึง -30 °c ก่อนนำลงแช่ในไนโตรเจนเหลว น้ํายา แช่แข็งตัวอ่อนที่ประกอบด้วย 10% (v/v) กลีเซอรอล + 0.1 M ซูโครส ในสารละลาย Dulbecco's modified phosphate buffered saline (D-PBS) หลังเก็บไว้นานประมาณ 1 ปี ละลายตัวอ่อนและดึงสารป้องกันการเกิด ผลึกน้ำแข็งออกจากตัวอ่อนแบบ 3 ขั้นตอนในน้ำยาสำหรับละลายตัวอ่อน โดยค่อย ๆ ลดความเข้มข้นของ กลีเซอรอลจาก 5% (v/v) ไปยัง 2.5% และ 0% ตามลำดับ อย่างละ 5 นาที พบว่ามีตัวอ่อนกระบือ จำนวน 25 ตัวอ่อน (86.2%, 25/29) ที่มีคุณภาพและลักษณะสามารถนำไปใช้ย้ายฝากได้ แบ่งเป็นระยะมอรูล่า 15 ตัวอ่อน (83.3%, 15/18) มอรูล่าระยะรวมตัวแน่น (compact morula) จำนวน 4 ตัวอ่อน (100%, 4/4) เป็นตัวอ่อน ระยะบลาสโตซีสระยะแรก (early blastocyst) 2 ตัวอ่อน (100%, 2/2) และ ระยะบลาสโตซิส (blastocyst) 4 ตัวอ่อน (100%, 4/4) ย้ายฝากตัวอ่อนจำนวน 21 ตัวอ่อนให้กระบือตัวรับ 10 ตัว ตัวรับตั้งท้อง 1 ตัว (10%, 1/10) จากผลการศึกษานี้ …

We studied dominant hand development in 158 patients with congenital muscular torticollis who had received physical therapy in Chang Gung Memorial Hospital, Kaohsiung, from 1989 to 1995. There were 97 boys and 61 girls in the study group. The congenital muscular torticollis was right side in 84 patients and left side in 74 patients. We also enrolled 512 healthy children, aged from 3 to 5 years old, as the control group. Ninety-one percent of the control subjects were right handed, and nine percent of them were left handed. The study group comprised eighty-nine cerebral palsy patients, 46 males and 43 females. The average age was 6.0±3.4 years old. Pelvic frontal view radiographs were made in a total of 178 hips. The migration percentage and acetabular index were measured by pelvic X-ray in all patients. Either hip subluxation or dislocation was diagnosed according to the criteria of Reimers, who considered a migration percentage greater than 33% to denote subluxation and a percentage greater than 100% to be denoted as dislocation. Parkinson's disease (PD) is a degenerative disorder of the nigrostriatal system, and frequently complicated with swallowing difficulties. This paper reviewed the current studies on the scope of the incidence, the clinical symptoms, the use of videofluoroscopic swallowing study (VFSS), and the management of dysphagia in PD. The awareness of dysphagia in PD patients in previous studies was around 31 to 72 %, however, this figure does not reflect the actual severity of swallowing disorders in this group patients. During 1997-1998, a serological survey of equine infectious diseases in Thailand was undertaken. Blood, sera and blood smears were taken from 400 horses, donkeys and mules out of an equine population of 8,600. Tests were carried out at the National Institute of Animal Health, Bangkok, Thailand, and at the USDA laboratories, USA, for 10 diseases. There was no evidence of the presence of African Horse Sickness, Glanders, Dourine, Surra and Equine Viral Arteritis. There was one reactor to Babesia caballi and 62 to Babesia equi. Cytokines are protein mediators produced by the immune and nonimmune cells that are involved in the regulation of inflammation and immune responses. The availability of several recombinant bovine cytokines have led to the investigation of the potential of cytokines as prophylactic or therapeutic strategies in the control of bovine mastitis, the most expensive disease affecting the dairy industry. The main goals of cytokine therapy are to enhance the immune response in the mammary gland against pathogenic bacteria, to increase the efficacy of antibiotic treatment, and to enhance the specific immunity of mastitis vaccines. ศึกษาความสัมพันธ์ของฮอร์โมนโปรเจสเตอโรน (P4) เอสตราไดออล (E2) และคอร์ติซอล (C) ที่มี ต่อความสมบูรณ์พันธุ์ในโคนมพันธุ์ผสม 10 ตัว ด้วย Analysis of Variance และ Duncan's New Multiple Range Test จากผลการตรวจการตั้งท้องโดยการคลําผ่านทวารหนัก แบ่งโคออกได้เป็น 2 กลุ่ม ๆ ละ 5 ตัวคือ กลุ่ม ผสมติดและกลุ่มผสมไม่ติด วิเคราะห์ระดับโปรเจสเตอโรน เอลตราไดออล และคอร์ติซอลในซีรั่มของโคทั้ง สองกลุ่มในวันที่ 0 (วันที่ผสมเทียม), 4, 8, 12, 16, 20, 25 และ 41 หลังการผสมโดยวิธีเรดิโออิมมิวโนเอสเสย์ พบว่าโคกลุ่มที่ผสมติดมีระดับของโปรเจสเตอโรน และคอร์ติซอลสูงกว่าโคกลุ่มที่ผสมไม่ติด (P<0.05) โคทั้ง สองกลุ่มมีระดับโปรเจสเตอโรน ตำสุดในวันที่ 0 (P<0.05) ไม่พบว่ามีความแตกต่างของระดับเอสตราไดออล ระหว่างกลุ่มและในวันต่าง ๆ ที่ทําการศึกษา

การหาค่า Bioavaialability ของสารอาหาร, ทิพย์วรรณ ปริพัฒนานนท์

ค่า bioavailability ของโภชนะ คือ ปริมาณเปอร์เซ็นต์ของโภชนะที่สัตว์ดูดซึมได้จริง ๆ จากอาหารที่กิน และเป็นส่วนที่นำไปใช้ในขบวนการเมตาบอลิซึม ค่านี้แสดงให้เห็นถึงคุณค่าของโภชนะโดยเปรียบเทียบ ระหว่างสารอาหาร 2 ชนิด การหาค่า bioavailability มีหลายวิธี คือ slope-ratio assay, parallel lines assay, three-point assay และ standard curve assay แต่ละวิธีก็จะมีข้อกำหนด และความเหมาะสมของการใช้ งานแตกต่างกันไป วิธีที่ดีสุดนั้นย่อมเป็นวิธีที่เหมาะสมกับข้อกำหนดมากที่สุด ในการทดลองหาค่า bioavailability ของสารอาหารนั้น slope-ratio assay เป็นวิธีที่นิยมกันที่สุด มีข้อกำหนดอยู่ 2 ข้อ คือ การตอบสนองของ สัตว์จะมีค่าเท่ากันเมื่อไม่ได้เติมโภชนะที่ต้องการเปรียบเทียบทั้งสองชนิดในอาหาร (intersection) และความ สัมพันธ์ระหว่างการตอบสนองและปริมาณโภชนะที่ทดลองต้องเป็นเส้นตรง (linearity)

การศึกษาในครั้งนี้ทำการวิเคราะห์ด้วยวิธีทาง immunohistochemistry เพื่อศึกษาชนิดของneuro- transmitters ที่อาจเกี่ยวข้องกับการนำกระแสประสาทรับความรู้สึกเจ็บปวด และอุณหภูมิ (nociceptive และ thermoreceptive information) luusta trigeminal ganglion a sensory trigeminal nucleus vos medulla oblongata ของงูเขียวหางไหม้ (Trimeresurus albolaris) นำสมองงูเขียวหางไหม้จำนวน 12 ตัว มาแยกเอา brainstem และ maxillomandibular ganglion มาตัด section แล้วทำ immunofluorescent labelling โดยใช้ antibodies ต่อ neurotransmitters 6 ชนิด คือ substance P, calcitonin gene-related peptide (CGRP), galanin, neuropeptide Y, nitric oxide synthase (NOS) และ Somatostatin จากผลการทดลองพบว่า ใน ganglion มีเซลล์ขนาดเล็กถึงขนาดกลางจำนวนหนึ่ง ที่แสดง immunoreactivity ต่อ substance P และ CGRP และแทบจะไม่พบเซลล์ที่ให้ผลบวกต่อ galanin ในส่วนของ medulla oblongata WU substance P immunoreactivity lu fibres oglu descending trigeminal tract uazlu neuropil รอบ ๆ lateral descending nucleus ของ trigeminal nerve และยังพบได้ใน fibres ที่วิ่งพาดข้าม trigeminal subnucleus caudalis …
