The total complex of immune expression is an interplay between nonspecific antimicrobial humoral systems plus specific antibodies and accessory factors. These systems are backstopped by the phagocytic functions of PMNs. If these fail, mononuclear phagocytes respond as a second line of defense to carry out chronic engagements. In addition to a direct activation process by "substrate," macrophages may be activated and mobilized by a lymphocyte-mediated immunologic reaction which probably involves either a "lymphotoxin" and/or a specific antibody cytophilic for macrophages. Immunologically activated lymphocytes appear to be the primary effector cells of anti-tissue (transplantation) cellular immunity, whereas immunologically activated macrophages appear …

However, each of us has his own particular probability of sampling psychiatric emergencies, depending upon who he is and what he does. Physical Illness: The Family And The Physician, Marc H. Hollender

When confronted by the specific stress of a close relative's illness, the family members' reactions may be schematically divided into three stages: 1) disorganization; 2) reintegration; and 3) adjustment. Either disorganization can be intensified or adjustment can be fostered, depending on the physician's understanding of the feelings the family members experience and his willingness and ability to be helpful. The family members' responses, in turn, will influence the patient's reaction to illness or his recovery and rehabilitation. Accordingly, good medical practice requires that the physician include the family in his total treatment plan whenever possible.

Both the anxiety-defense model and the cognitive model provide a scientific basis for practical handling of the surgical patient. Emerging from both models is a humanistic approach to the patient based on understanding and a nonjudgmental attitude as well as on a knowledge of where the patient is psychologically and a willingness to meet him on his ground. The rewards in terms of a reduction of anxiety, depression, and human suffering can be enormous.

In summary, sexual relationships involve two factors: lustful gratification and emotional and physical closeness. Lustful gratification involves sexual identity which is determined by: 1) biological sex; 2) gender identity; 3) child-parent relationships; 4) cultural value systems; and 5) sexological knowledge. Sexual problems are due to ignorance of the sexual facts of life and/or to contamination of the sexual area by other needs, wishes, or fears. A detailed understanding of the presenting complaints and the sexual history of the patient will enable the physician to make a differential diagnosis. Isolated organ perfusion provides us with an excellent tool for investigating the physiology and the function of a variety of individual organs. It enables us to explore these organs, their pharmacologic reactions, their ability to withstand various stresses, their pathology after stress and their suitability for preservation and transplantation. Isolated perfusion has allowed us to better understand hemodynamics, effects of various perfusates, and toxicity of drugs on individual organs. We have recently been enlightened about the vital role of platelets and endothelial integrity. Unilateral Aphakia And Contact Lenses, Herbert Wiesinger

The binocular functions of a group of 60 patients with unilateral aphakia were tested after fitting with a contact lens. Results showed a high degree of extramacular fusion in the group. However, only 50% of these patients obtained satisfactory stereoacuity levels. There was no relation between the time elapsed from cataract extraction to contact lens fitting and the amount of ultimate stereoacuity obtained. Some factors presenting common obstacles to successful fitting of aphakic contact lenses are discussed.

The Role Of Preservation In Transplantation, Folkert O. Belzer

Although it is too early to draw any definite conclusions from these results, as a period of two years is probably necessary to evaluate cadaver kidney transplantation in humans, it appears that our well-matched patients have much fewer problems than our poorly matched patients. It is hoped that by preservation methods such as those discussed, improved methods of tissue typing, and better immunosuppressive therapy, we will be able to raise the survival level of cadaver kidney transplantations to the level between 80 and 85% presently being obtained in transplantations using living, related donors.

Briefly, it will be postulated that a major function of thyroid hormone is for slowly developing adaptations of the circulatory system to environmental changes, and that many thyroid hormone effects are secondary to altered perfusion of tissues. There is no known in vitro effect of thyroid hormone which can reasonably explain any major number of the in vivo effects (Tata, 1964).

In Great Britain, until recently, domestic gas was made from coal and was always referred to as "coal gas." Today we are beginning to use natural gas, and the term coal gas is being replaced by "town gas." Most of the domestic supply, however, is still coal gas, which has a high though variable percentage of carbon monoxide (Mant, 1964). In any event it is always present in a highly lethal concentration. Under most of the existing statutes, therapeutic abortion is only permitted to preserve the life of the mother. Current data reveal that, when matching is very good, there is also very good correlation in living related donor results, particularly in the sibling donor source. When there is slight mismatching, correlation is not as good; however, many patients seem to do well in spite of some mismatch. Correlation in the unrelated cadaver group is not as precise or significant. The current status of tissue typing is still not as precise as it should be, in order for us to rely on it as a selection criterion for renal homotransplantation. Osteopetrosis, or marble bone disease, is a disturbance of skeletal development in which the rate of bone resorption fails to keep pace with the rate of bone formation. Bone matrix accumulates excessively throughout the skeletal system, causing damage to neighboring tissues--particularly the dental, hematopoietic and nervous tissues. The line of investigation on osteopetrosis which has led to the current point of view that the thyroid gland represents the primary side of the disturbance was initiated soon after an experimental animal became available.

Without further detailing the methodology, I would like to emphasize one point, i.e., a psychiatric crisis need no longer be considered the point of departure for a family member, but rather can be regarded as an entrée for helping the entire family to cope with interpersonal conflicts. With the kind of help offered in a mental health center, families as total units can often attain a better adjustment than before, and the illness stemming from interpersonal difficulty will not rest entirely on the scapegoat member. A model of shared power has been suggested as a means of most effectively mobilizing the professional mental health manpower and of stimulating the growth of expertness. It is only one alternative among many. The behavioral scientists constantly are studying the best ways to harness human effort for the social good. The application of their findings to the problem of interdisciplinary collaboration may be a valuable means of improving patient care.
