So much in our society separates the rest of us from the old that a discussion of the differential aspects of practice with the elderly and their families (many of whom are aging) runs the risk of being redundant as well as a concealed endorsement of the professional biases which afflict us all. The truth of the matter is that important differences do exist between social work practice with the old and with younger generations, differentials which emanate from a sound gerontological knowledge base. Home support services for the aged are emerging as a primary concern in the Title XX planning proposals put forth by states and localities. This is not surprising when one considers that Title XX guidelines require the setting of priorities among non-cash benefit programs that help insure the individual's self-sufficiency within his or her normal environment.
All services under Title XX must relate to one or more of five objectives for the individual client: economic independence; self care; prevention or remedy of neglect, abuse or other conditions which lead to dependence; intermediate community based care and, as a last resort …

A neglected topic in discussions of services for the institutionalized elderly seems to be the question of whether the residents of nursing homes are receiving adequate support from volunteers. Undoubtedly, voluntarism is playing an important part in the provision of services and emotional support for the aged population, among them the elderly that are institutionalized. But because of the child-centeredness and youth-orientation of American society, it is possible that work with the elderly may not be attracting volunteers in proportion to the needs of this population. A Comparison Of Kidney Preservation Methods By Oxidative Phosphorylation Studies, Devprakash Samuel, Stanley G. Dienst
Henry Ford Hospital Medical Journal
An oxygen consumption assay using adenosine diphosphonucleotide (ADP) was performed on canine kidneys to measure the efficiency of oxidative phosphorylation. The purpose of the assay was to establish a respiratory control index (RCI) that could be used to evaluate kidneys preserved by different methods. Of the four methods tested, perfusion yielded the best results after 24-hour preservation. Additionally, four pairs of human cadaver kidneys were evaluated with the same assay. Some correlation existed between the RCI values of the nontransplanted kidney in each pair and the clinical viability of its transplanted mate.

ปัจจุบันประชาชนส่วนใหญ่ของประเทศ โดยเฉพาะในต่างจังหวัดต้องทนทุกข์ทรมานจาก โรคในช่องปากในสภาพจํายอม โดยรัฐไม่สามารถให้ความช่วยเหลือในด้านนี้ได้อย่างทั่วถึง เพราะแม้ แต่ในเมืองหลวงเองก็ยังประสบปัญหาทางด้านทันตกรรมอยู่ ทางหนึ่งที่จะช่วยให้ประชาชนชาวไทยมี ทันตสุขภาพที่สมบูรณ์ได้ก็โดยการประชาสัมพันธ์ เผยแพร่ความรู้ความเข้าใจเกี่ยวกับทันตสุขภาพแก่ประชาชน เพื่อให้สามารถป้องกันรักษาอนามัยในช่องปากของตนเองได้ ซึ่งจะเป็นวิธีการที่ดีที่สุด ประหยัดและเป็นไปได้ไม่ยากนัก เอกสารทางวิชาการนี้จึงเสนอความคิดเห็นเกี่ยวกับการประชาสัมพันธ์ เพื่อเผยแพร่ความรู้ความเข้าใจและเสริมสร้างทัศนคติที่ถูกต้องทางด้านทันตสุขภาพแก่ประชาชน ซึ่งเมื่อประชาชนมองเห็นความสําคัญในเรื่องทันตสุขภาพแล้วก็จะเกิดความต้องการเพิ่มขึ้น จนทําให้รัฐต้อง ดําเนินงานทันตสาธารณสุขของชาติให้เจริญก้าวหน้าขึ้นกว่าที่เป็นอยู่ในขณะนี้

Some of the oral Surgeons might face with the problem of fixation of the jaw particularly in cases of the gun shot resulting in the comminuted fractures. In order to treat this type of patient, the splint has to be in place properly for longer period. Some of the instruments such as Rodger Anderson pins have been imported but the disadvantages can still be noticed. มะเร็งกล้ามเนื้อเรียบของลิ้นหรือไลโอไมโอซาร์โคม่า (Leiomyosarcoma) พบไม่บ่อยนัก ทั้งนี้เพราะโดยลักษณะทางโครงสร้างแล้วกล้ามเนื้อลิ้นไม่ได้ประกอบด้วยกล้ามเนื้อเรียบแต่ประกอบขึ้นด้วยกล้ามซึ่งส่วนใหญ่เป็นพวกกล้ามเนื้อลาย กล้ามเนื้อเรียบที่พบบริเวณลิ้น พบได้สองบริเวณคือที่บริเวณเซอร์คัมวอลเลต แพพิลล่าร์ (Circumvallate papilla) และที่บุอยู่ตามผนังหลอดเลือด รายงานผู้ป่วยรายนี้เป็นการรายงานผู้ป่วยซึ่งชิ้นเนื้อใบออปซี่ (Biopsy) ถูกส่งมาตรวจที่แผนกวิชา ทันตพยาธิวิทยา คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ผลการชันสูตรโดยการดูด้วยกล้อง จุลทัศน์ และย้อมพิเศษ (Special stain) ปรากฏแน่ชัดว่าเป็นไลโอไมโลซาร์โคม่า

Leiomyosarcoma are malignant tumors that are arised from smooth muscle cells. Almost all of leiomyosarcoma are arised from smooth muscle that lined the wall of uterus and gastrointestinal tract. Leiomyosarcoma of the oral cavity extreamly rare because there is limited smooth muscle in this area. The origin of leiomyosarcoma of oral cavity is limited to smooth muscle of the blood vessel walls and the circumvallate pappillae of the tongue. The purpose of this report is to document and aditional case of leiomyosarcoma of the tongue in a Thai 42-year-old man. Dental Plaque Grows By In Situ Cell Division, Porjai Ruangsri, Dag Orstavik

In vivo plaque formation was significantly reduced when tooth surfaces were subjected to topical iodine (0.2% I1⁄2 in 2.4% KI) twice daily for three days. Similarly, in vivo plaque formation was significantly reduced on enamel surfaces that had been subjected to ultraviolet irradiation. Control experiments indicated that the plaque inhibitory action of irradiation resided with its interference with bacterial cell division. The results are interpreted to suggest that plaque grows in mass primarily by the division and multiplication in situ by plaque bacteria, not by a continued apposition of salivary microbes.

ศัลยแพทย์ทางช่องปาก มักประสบปัญหาและความยุ่งยากเกี่ยวกับการที่จะหาวิธีตรึงชิ้นของกระดูกขากรรไกรล่างที่หัก เนื่องจากถูกยิงด้วยกระสุนปืน และกระดูกแตกเป็นชิ้นเล็กชิ้นน้อย การตรึงชิ้นของกระดูกก็จําเป็นจะต้องตรึงไว้นานโดยที่จะต้องไม่ทําให้เผือกนั้นหลุด เครื่องมือจากต่างประเทศ เช่น Rodger Anderson pins ก็มีราคาสูงลิ่ว และก็ยังไม่สามารถจะตรึงชิ้นกระดูกไว้ได้นานเกิน 3 เดือน เพราะ pins จะหลวมและหลุด จึงได้คิดประดิษฐ์เฝือกขึ้นมาใช้แทนในต้นทุนที่ราคาถูก อย่างเทียบกันไม่ได้ และผลที่ได้รับกลับทําให้สามารถตรึงชิ้นของกระดูกไว้ได้นานเกินคาด คือ เป็นแรมปี สามารถประดิษฐ์ขึ้นได้ด้วยวิธีง่าย และใช้เวลาไม่นาน

The effect of donor pretreatment on perfused cadaver kidney allografts was evaluated in 40 recipients at Henry Ford Hospital over a two-year period. Ofthe 40,23 received kidneys from donors pretreated with 40 mg/K each of cyclophosphamide and methylprednisolone during the first year of the study and up to 70 mg/K during the second year. Our results indicated that donor pretreatment for five to eight hours did not consistently improve survival rates in pretreated perfused cadaver kidneys following transplantation. The use of cyclophosphamide for donor pretreatment does not prevent the use of continuous perfusion to preserve human kidneys. Dosages up to …

Radionuclide examinations provide considerable information in evaluating patients who have received renal transplants. In the uncomplicated case, baseline data should be obtained so that subsequent changes in renal function can be better documented. Serial examinations with several radiopharmaceuticals represent the best nuclear approach to the transplant patient. This paper describes the application and performance of these tests as currently performed at Henry Ford Hospital.

Eighty-one patients with chronic renal failure associated with or secondary to diabetic nephropathy were treated with dialysis and/or transplant. Twenty-five had juvenile diabetes and 56 had adult onset diabetes. Juvenile diabetics did poorly on hemodialysis with 13 patients having a 19% four-year survival rate, whereas those who had cadaveric transplantation did well with a four-year patient and graft survival rate of 56% in nine patients. The three juvenile diabetics who received related kidney grafts are presently alive with good function. Between 7973 and 7977 we encountered 22 cases of acute renal failure after transplantation in 70 patients who received perfused cadaver kidneys. Nearly two-thirds of 76 nonfunctioning grafts were lost due to subsequent superimposed rejection, often undetected and, hence, untreated. Thirty-one percent of the 76 recovered function. The recovery rate, we believe, can be improved by earlier diagnosis and treatment of rejection and by avoiding invasive diagnostic procedures in the early postoperative period. The effect of HLA matching and other aspects of histocompatibility on renal graft survival were analyzed in 144 patients who received renal transplants at Henry Ford Hospital during the past ten years. Living related transplants with one haplotype match or better demonstrated a higher graft survival rate (at least 88%) at one year than the better matched cadaveric transplant. In cadaveric transplants, the group with fewer than two mismatched antigens appeared to have a higher graft survival rate than those with more than two. There were 116 renal transplants performed on 108 patients over a five-year period at Henry Ford Hospital with three major urological complications. The rate of 2.6% compares favorably with that reported in other series. Careful preoperative urological evaluation together with technically precise ureteroneocystostomy are factors that minimize the incidence of urological complications.
