Orifice-view angiography permits us to visualize en face the orifice of the aortic or mitral valve. The radiopaque annulus of bioprosthetic valves assists in permitting the angiographer to position the patient exactly, so that the valve can be seen as if looking directly into the orifice. Orifice-view angiography of porcine bioprosthetic valves has been useful in assessing the size and configuration of the valve orifice. It can reveal a failure of leaflet opening that would indicate degeneration, even when hemodynamic measurements remain equivocal.

รายงานนี้มีวัตถุประสงค์ที่จะประมวลแนวความคิดต่าง ๆ เกี่ยวกับการ ประเมินผลโครงการทันตกรรมชุมชนเพื่อวิเคราะห์ให้เห็นว่า แนวความคิดเหล่านี้ มักจะมุ่งที่จะให้ข้อมูลในทางวิชาการ โดยไม่ได้ให้ความสำคัญเท่าที่ควรแก่การแยกแยะข้อมูลเพื่อประโยชน์แก่การตัดสินใจในงานบริหารระดับต่าง ๆ พร้อมกับเสนอ แนะแนวความคิดในการแยกแยะข้อมูลที่ได้จากการประเมินผลโครงการ เพื่อที่จะ ช่วยให้ข้อมูลนี้สอดคล้องกับความต้องการในทางบริหาร แนวความคิดแก้ไขนี้พอจะสรุปได้ คือ ให้มีการจัดกลุ่มของมาตรการ ประเมินผลออกเป็น 4 กลุ่ม ตามช่วงเวลาของการดำเนินโครงการและความต้องการ ในการใช้ข้อมูลเพื่อการตัดสินใจของผู้บริหาร และให้กำกับชื่อสําหรับเรียกกลุ่ม ข้อมูลเหล่านี้ใหม่เป็น 1. ex-ante evaluation 2. input-output evaluation 3. on-going evaluation 4. ex-post evaluation

ปัจจุบันมีการนำเอา "ไบออปซี่" (biopsy) มาใช้กันอย่างกว้างขวาง ในการวินิจฉัยโรค การวินิจฉัยได้อย่างถูกต้อง นอกจากจะขึ้นอยู่กับความรู้ความ สามารถของแพทย์ผู้ทำไบออปซี่และพยาธิแพทย์แล้ว ยังต้องอาศัยวิธีการจัดการกับ ชิ้นเนื้อตัวอย่างให้เหมาะสมด้วย จากประสบการณ์การทํางานในภาควิชาทันตพยาธิ วิทยา คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย พบว่ามีข้อบกพร่องบาง ประการ เช่น ตัดชิ้นเนื้อตัวอย่างเล็กเกินไป ใส่น้ำยารักษาสภาพไม่ถูกต้อง ให้ ประวัติผู้ป่วยไม่ละเอียดพอ เป็นต้น สิ่งเหล่านี้เป็นอุปสรรคต่อการวินิจฉัยโรคของพยาธิแพทย์ บทความนี้มุ่งหวังที่จะทบทวน อธิบาย วิธีการรวมทั้งเหตุผลและข้อเสนอแนะในการทำไบออปซี่

ความคิดรวบยอดเกี่ยวกับตนเอง ของบุคคลมีมาตั้งแต่เกิดและมีการพัฒนามาเป็นลำดับสิ่งที่มีอิทธิพลต่อการพัฒนาความคิดรวบยอดเกี่ยวกับตนเอง ได้แก่ประสบการณ์ในช่วงแรกของชีวิต และครอบครัว วัฒนธรรม ร่างกาย และสติปัญญา นอกจากนี้ โรงเรียน บทบาทของครู และบรรยากาศในชั้นเรียน จะมีส่วนสำคัญในการพัฒนาความคิดรวบยอดเกี่ยวกับตนเอง การเรียนรู้ความคิดรวบยอด ของบุคคลจะเกิดประโยชน์ในการเข้าใจบุคคลและพฤติกรรมของบุคคลมากขึ้น

การศึกษาขนาด รูปร่าง ฟันหน้าของคนไทยกลุ่มหนึ่ง, สุดจิตต์ ทองธรรมชาติ

ในการศึกษาสถิติเกี่ยวกับขนาด และรูปร่างของฟันหน้าบนของชายหญิง อายุตั้งแต่ 20-42 ปี จำนวน 56 คน ทำให้ทราบว่าฟันหน้า central incisor ขนาดใหญ่ที่สุด ความยาวในแนว inciso cervical 12.2 ม.ม. ความกว้างที่สุด ในแนว mesio-distal 9.57 ม.ม. ฟัน lateral incisor ยาวที่สุด 11.4 ม.ม. กว้างที่สุด 6.4 ม.ม. พื้น cuspid ยาวที่สุด 12.3 ม.ม. กว้างที่สุด 8.5 ม.ม. และฟันซีน (cuspid) มีความแตกต่างกันมากในแต่ละคน จากการศึกษาเปรียบเทียบกับผลงานวิจัยของ Arne M. Bjorndal, D.D.S., M.S., William G. Henderson, M.P.H., Ph.D., Arthur Eddy Skidmore, D.D.S., M.S., และ Frank H. Kellner, D.D.S. (BKHS) พบว่าฟันของคนไทยและคน อเมริกันมีความกว้างในแนว mesio-distal ใกล้เคียงกันแต่ความยาวในแนว inciso cervical ในคนไทยสั้นกว่าของคนอเมริกัน จึงทําให้ฟันหน้าของคนไทย โดยเฉลี่ยดูจะป้อมกว่าของชาวอเมริกัน ซึ่งสอดคล้องกับรูปร่างใบหน้าของกะโหลก ในคนไทยส่วน Central incisor มักจะเป็นรูป Tapering

ได้ทำการสัมภาษณ์หัวหน้าครัวเรือนทุกครัวเรือนในหมู่บ้านหินดาษ ตำบลดงน้อย อำเภอราชสาสน์ จังหวัดฉะเชิงเทรา จำนวน 54 คน เป็นชาย 21 คน (คิดเป็นร้อยละ 38.9) และหญิง 33 คน (คิดเป็นร้อยละ 61.1) ในส่วนที่เกี่ยว กับประวัติส่วนตัว การประเมินทันตสุขภาพของตนเอง ปัญหาทันตสุขภาพ การ ใช้บริการทางทันตกรรม บริการทันตกรรมของภาครัฐ และเรื่องเกี่ยวกับฟันปลอม ซึ่งให้ผลของการศึกษาดังนี้ ประชาชนคิดว่าตนเองมีปัญหาโรคฟัน และเชื่อว่า เกิดมาจากพื้นเป็นรู (คิดเป็นร้อยละ 74) สภาพของเหงือก ประชาชนเชื่อว่า อยู่ในสภาพแข็งแรงดี (คิดเป็นร้อยละ 81) ร้อยละ 65 ของผู้ตอบไม่ต้องการการ ใส่ฟันปลอมแทนฟันแท้ที่ถูกถอนไป เมื่อมีปัญหาโรคในช่องปาก เขานิยมไปรับบริ การการถอนฟันจากหน่วยแพทย์เคลื่อนที่ แต่สําหรับพวกที่มีปัญหาแล้วไม่ไปรับบริ การเพราะเชื่อว่าโรคฟันเกิดขึ้นเองและหายเอง หรือเพียงแต่ซื้อยาแก้ปวดมากินก็พอ ประชาชนมีความต้องการ (Demand) คำแนะนำและการบริการทางการป้องกัน โรคเป็นอันดับหนึ่ง การตรวจฟันและการถอนฟันเป็นอันดับรองลงไปตามลำดับ

The porcine bioprosthesis has been the prosthetic valve of choice at Henry Ford Hospital since October 1971. By 1979, 23 cases of degeneration had been seen, and the rate seven years after implantation was 16%. Now, with a ten-year follow-up, there are 41 degenerated valves. After seven years, the percent free of degeneration is 88%, (SE of 2% [standard error]); at eight years, 82% (SE of 2.9%,); at nine years, 80% (SE of 3.4%); and at ten years, the percent free of degeneration is 69% (SE of 6.5%,). The frequency content of heart sounds and murmurs in patients with implanted bioprosthetic valves may reveal evidence of degenerative changes before such changes are clinically apparent. An increased dominant frequency of the heart sound caused by a bioprosthetic valve in either the aortic or mitral positions suggests stiffening of the leaflets. While a musical systolic murmur of a bioprosthetic valve in the mitral position suggests flutter from a torn, insufficient leaflet, limited observations of musical systolic murmurs in the aortic position do not seem to indicate a degenerated valve.

Mechanical forces caused by the dynamics of blood flow in the region of porcine bioprosthetic valves may directly injure the valve as well as formed elements of the blood. Previous in vitro studies in our laboratory evaluated quantitatively the fluid dynamic stresses in the region of such valves. In this paper we discuss our results in relation to fluid dynamic stresses found in patients with normal and diseased natural aortic valves. Immunological Involvement In Porcine Bioprosthetic Valve Degeneration: Preliminary Studies, Rollin H. Heinzerling, Paul D. Stein, Jeanne M. Riddle, Donald J. Magilligan Jr., John J. Jennings

Preliminary animal model studies have indicated that porcine bioprosthetic valves can be recognized by the immune system. This report presents the results of a preliminary investigation on the implanted valves of 27 patients. While none of the 27 demonstrated an immune response to porcine serum proteins, five patients developed antibodies against the implanted valve leaflets. Cells were enzymatically eluted from a degenerated bioprosthetic valve and were classified by means of monoclonal antibodies. Although these cells were not found to be of T-lymphocyte lineage, they did express la determinants and had the morphologic appearance of plasma cells. We therefore conclude that …
