เมตาสแตติคคอริโอคาร์ซิโนมาของช่องปาก : รายงานผู้ป่วย, สุรศักดิ์ ชี้รัตน์

รายงานผู้ป่วยหนึ่งราย เป็นหญิงจีน อายุ 36 ปี มีก้อนเนื้องอกสีแดงจัด ลักษณะอ่อนนุ่ม เลือดออกง่าย งอกขึ้นมาจากแผลถอนฟันของเบ้ากระดูกรองรับฟันที่ไม่หายและเหงือกบริเวณ 22 วินิจฉัยทางคลินิกว่าเป็นมะเร็ง ลักษณะจุลพยาธิวิทยาของเนื้องอกนี้ ประกอบด้วยไซโตทรอโฟบลาสและซินไซติโอทรอโฟบลาสและพบบริเวณที่มีเลือดออกและเนื้อตายจากขาดเลือดอยู่ใต้เยื่อบุผิวช่องปาก ผลการวินิจฉัยทางจุลพยาธิวิทยาเป็น "เมตาสแตติคคอริโอคาร์ซิโนมาของเหงือกและกระดูกแม็กซิลลา บริเวณ 22"

สภาวะโรคฟันผุของเด็กนักเรียนอายุ 12 ปี ในโรงเรียนสังกัดกรุงเทพมหานคร พ.ศ. 2538, จารุวรรณ ตันกุรานันท์

การสํารวจภาคตัดขวางเพื่อศึกษาสภาวะโรคฟันผุของนักเรียนอายุ 12 ปี ในโรงเรียนสังกัดกรุงเทพมหานคร 18 โรง จํานวน 338 คน พบว่า มีอัตราชุกของโรคฟันผุร้อยละ 82.8 ค่าเฉลี่ยฟันผุถอนอุดเป็นซี่ ± ส่วนเบี่ยงเบนมาตรฐานเท่ากับ 2.94±2.20 ต่ำกว่าผลการสํารวจในปี พ.ศ. 2534 โดยกองทันตสาธารณสุข สํานักอนามัยกรุงเทพมหานคร แสดงว่าผลการดําเนินงานทันตสาธารณสุขของกองทันตสาธารณสุข สํานักอนามัย กรุงเทพมหานคร ประสบความสําเร็จในระดับหนึ่ง แต่อัตราฟันผุถอนอุดของฟันกรามแท้ซี่ที่หนึ่งยังคงสูงมาก คือ ปริมาณฟันผุถอนอุดของฟันกรามแท้ซี่ที่หนึ่งบนขวา ฟันกรามแท้ซี่ที่หนึ่งบนซ้าย ฟันกรามแท้ซี่ที่หนึ่งล่างซ้าย และฟันกรามแท้ซี่ที่หนึ่ง ล่างขวา เท่ากับร้อยละ 46.5, 47.9, 63.0 และ 65.4 ตามลําดับ ดังนั้นจึงแนะนําให้ใช้สารเคลือบหลุมร่องฟัน ป้องกันการผุของฟันกรามแท้ซี่ที่หนึ่งเป็นมาตรการหนึ่งของการดําเนินงานทันตสาธารณสุขในกลุ่มเป้าหมายคือเด็กนักเรียนประถมศึกษาของกองทันตสาธารณสุข สํานักอนามัย เพื่อให้ประสบความสําเร็จดียิ่งขึ้น

ปัจจุบันการดูแลรักษาผู้ป่วยเทมโพโรแมนดิบิวลาร์ดิสออเดอร์ (TMD) นิยมใช้วิธีอนุรักษ์และผันกลับได้ (conservative and reversible treatment) การศึกษานี้มีวัตถุประสงค์เพื่อประเมินผลการรักษาผู้ป่วย TMD ที่เคยรับการรักษาด้วยออกคลูซอลสปลินต์ชนิดเรียบจากคลินิกทันตกรรมบดเคี้ยว คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ระหว่างปี พ.ศ. 2531-2537 การสํารวจนี้ใช้แบบสอบถามทางไปรษณีย์ส่งไปให้ผู้ป่วยที่เคยได้รับการรักษานานกว่า 6 เดือน มีผู้ตอบแบบสอบถามทั้งสิ้น 200 ราย (อัตราตอบแบบสอบถามกลับคืน ร้อยละ 50.9) แยกเป็นเพศหญิงร้อยละ 77.0 และเพศชายร้อยละ 23.0 ระยะเวลาเฉลี่ยตั้งแต่เริ่มรักษาจนถึงเมื่อส่งแบบสอบถาม 30.3 ± 13.4 เดือน เมื่อจําแนกผลการรักษาตามอาการสําคัญของผู้ป่วยซึ่งได้แก่ (1) อาการปวด 108 ราย (2) ความผิดปกติในการทําหน้าที่ 56 ราย (3) อาการปวดร่วมกับความผิดปกติในการทําหน้าที่ 36 ราย พบว่าผู้ป่วยมีอาการดีขึ้นเล็กน้อยหรือดีขึ้นจนเป็นที่น่าพอใจหลังจากได้รับการรักษา ร้อยละ 85.2, ร้อยละ 74.1 และร้อยละ 80.6 ตามลําดับ เมื่อวิเคราะห์ผลโดยใช้การทดสอบแบบไคสแควร์ พบว่าอาการสําคัญแต่ละกลุ่มดังกล่าวตอบสนองต่อการรักษาไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p>.05) ผู้ป่วย (ร้อยละ 19.4) ประเมินประโยชน์ของ ออกคลูซอลสปลินต์ในระดับ 8 มากกว่าระดับอื่น (จากสเกล 0 ถึง 10) ในขณะติดตามผลพบว่าอาการที่ยังคงปรากฏในผู้ป่วยมากที่สุด ได้แก่ การมีเสียงที่ข้อต่อขากรรไกร (ร้อยละ 62.5) ผลการสํารวจแสดงว่าทั้งอาการปวดและการทําหน้าที่ผิดปกติของระบบการบดเคี้ยวตอบสนองได้ดีต่อการรักษาด้วยการใช้ออกคลูซอลสปลินต์เป็นหลัก ดังนั้นจึงสนับสนุนการเลือกใช้วิธีอนุรักษ์ในการดูแลรักษาผู้ป่วย TMD ในเบื้องต้น

การศึกษานี้มุ่งที่จะหาความสัมพันธ์ของระยะเวลาที่ทิ้งไว้ตั้งแต่เริ่มผสม กับกําลังตัดขวางที่เปลี่ยนแปลงไปของอะคริลิกเรซิน ชนิดบ่มเองสําหรับใช้ทําถาดพิมพ์ปาก 3 ยี่ห้อ ได้แก่ ฟอร์มาเทรย์ (Formatray), สเปเชียล เทรย์ (Special Tray) และ รูทิเนียม อไครย์ เทรย์ (Ruthinium Acry Tray) โดยสร้างชิ้นตัวอย่างตามมาตรฐาน ไอเอสโอ หมายเลข อาร์ 1567 (ISO R1567) จํานวนยี่ห้อละ 30 ชิ้น โดยแบ่งแต่ละยี่ห้อออกเป็นยี่ห้อละ 3 กลุ่ม ๆ ละ 10 ชิ้น ทิ้งไว้ในอากาศ ที่อุณหภูมิห้องเป็นเวลา 1, 7 และ 14 วันหลังจากเริ่มผสม เมื่อครบกําหนดเวลานําไป ทดสอบด้วยวิธีการตัด 3 จุด (3-Point Bending) ด้วยเครื่องลอยด์ ยูนิเวอร์ซัล เทสติง (Lloyd Universal Testing Machine) แล้วนําค่ากําลังตัดขวางและค่าความสามารถในการถูกดัดงอของวัสดุก่อนหัก นําไปวิเคราะห์ค่าความแปรปรวนแบบทางเดียว และทดสอบด้วย ดันแคน นิวมัลติเพิล เรนจ์ (Duncan's new multiple range test) พบว่าค่ากําลังตัดขวางจะสูงขึ้น เมื่อทิ้งวัสดุไว้นานขึ้น และเมื่อเปรียบเทียบในระยะเวลาเดียวกัน ที่ระดับความเชื่อมั่น 0.05 ค่าเฉลี่ยกําลังตัดขวาง ที่ 1 วัน และ 7 วัน ของ สเปเชียล เทรย์ สูงที่สุด รองลงมาคือ ฟอร์มาเทรย์ และ รูทิเนียม อไครย์ เทรย์ ตามลําดับ ส่วนที่ 14 วัน สเปเชียล เทรย์ สูงที่สุด รองลงมาคือ รูทิเนียม อไครย์ เทรย์ …

การศึกษาเปรียบเทียบผลทางคลินิกเกี่ยวกับอาการบวมของใบหน้า การอ้าปาก และการอักเสบจากการถอนฟันกรามล่างคุดซี่ที่สามชนิดตั้งตรงขณะที่มีการอักเสบ โดยศึกษาในผู้ป่วย 60 ราย แบ่งเป็น 2 กลุ่ม กลุ่มที่หนึ่งได้รับยาแก้ปวดและยาแก้อักเสบโดยยังไม่ถอนฟัน กลุ่มที่สองได้รับการถอนฟันในทันทีพร้อมกับได้รับยาแก้ปวดและยาแก้อักเสบชนิดเดียวกัน หลังถอนฟันพบว่า การบวมในระหว่างสองกลุ่มมีความแตกต่างอย่างมีนัย สําคัญทางสถิติ (P<0.01) แต่การอ้าปากในระหว่างสองกลุ่มไม่มีความแตกต่างอย่างมีนัยสําคัญทางสถิติ (P>0.01)

The Thai Journal of Veterinary Medicine

A PRRS virus specific antibody among swine breeding farms in Thailand was studied. Serum samples were collected from 15 breeder farms in central and north-eastern part of Thailand during January to March 1995. Of total 449 samples, 286 (64%) were revealed PRRS-antibody indicated the history of PRRS virus exposure. The exposure rate was varied from 20-90% from farm to farm which appeared among replacement gilts, primiparous sows, multiparous sows and boars. The highest prevalence (75%) was in replacement gilts. The Thai Journal of Veterinary Medicine

Thailand has one of the richest freshwater small fauna in the world; over 170 species are currently recognized. However, species that transmit animal diseases, or are suspected to, are relatively few. At present, only 23 freshwater snail species are implicated or suspected in animal disease transmission. These species are restricted to two orders and seven families. The conventional colorimetric method of cholinesterase activity determination in blood and tissues of mammals was applied to investigate the enzyme activity in muscle and nervous cell of aquatic animals which are economically important, such as, tiger shrimp (Penaeus monodon fabricus) and giant perch (Lates calcarifer). Decreased enzyme activity in aquatic animals may indicate the contamination of insecticide in water resources. This study demonstrated that the method was suitable for detecting toxicity of organophosphate insecticide due to its convenience, simplicity and no sophisticated or expensive instruments needed. Canine Pregnancy Examination By Ultrasonography(การตรวจการตั้งท้องสุนัขด้วยอัลตร้าซาวด์), Sudson Sirivaidyapong

B-mode, Real-Time Ultrasound with 5 MHz Linear Array Transducer were used for pregnancy examination. Twenty five female dogs with different gestation period were examined. During early gestation period (19-29 days), round or oval embryonic vesicles with embryo in embryonic fluid were seen. Embryonic heart activity was detected at about 21-22 days of gestation and was seen easily in the middle and late gestation. At 30-35 days of gestation, the embryo developed hypoechoic head and body with embryonic movement. After 36 days of gestation the hyperechoic skeleton, hypoechoic organs and embryonic activities can be clearly identified. A continuing incidence of abortion record was investigated in a 1700 sow-breeding farm during 1992 through the first three quarters of 1995. The abortion rate per pregnant sows per year were 2.5, 3.7, 3.9 and 3.2% respectively compared to 0.3% recorded in 1991. Six months before, the farm has introduced the first batch of replacement gilts from a domestic breeding supplier into the farm. No obvious clinical signs were observed in sows. Laboratory evidences did not show any significant association with any conventional infection of the agents related the abortion.
