Background

Little is known about how the severity of injury changes with recurrent events of suspected non-accidental trauma (NAT). Our objective was to determine risk factors for escalating severity of injury in children with multiple events of suspected NAT.

Methods

This retrospective longitudinal cohort study included children from a pediatric Medicaid accountable care organization with ≥ 1 non-birth related episode containing an International Classification of Diseases, Ninth Revision, Clinical Modification or Current Procedural Terminology code for NAT or a skeletal survey between 2007 and 2011. The ability to regenerate complex structures is broadly represented in both plant and animal kingdoms. Although regenerative abilities vary significantly amongst metazoans, cumulative studies have identified cellular events that are broadly observed during regenerative events. For example, structural damage is recognized and wound healing initiated upon injury, which is followed by programmed cell death in the vicinity of damaged tissue and a burst in proliferation of progenitor cells. Sustained proliferation and localization of progenitor cells to site of injury give rise to an assembly of differentiating cells known as the regeneration blastema, which fosters the development of new tissue. Increasing breastfeeding rates and reducing HIV/AIDS are two important global health priorities, and their intersection has led to policies that meet mutual goals of supporting, protecting and promoting breastfeeding and reducing the risk of HIV/AIDS. The World Health Organization has developed and released recommendations, policies, and guidance on infant feeding and the risk of transmission through breastfeeding. This analysis provides a historical, chronological perspective on the global policies for infant feeding in the case of an HIV-positive mother, an analysis of changes, and recommendations for the future. Objective: A secular trend towards increased current and lifetime asthma prevalence has been observed in the US. The current study compared ambient Air Quality Index (AQI) data and asthma prevalence data from all fifty US states.

Methods: Air pollution is measured as AQI by the US Environmental Protection Agency (EPA). County level AQI data are available at the EPA website. Asthma prevalence data for each state are available on the Centers for Disease Control and Prevention (CDC) website. Data from 50 US states (2011-2013) were analyzed for AQI and asthma prevalence. Background: Secondhand smoke (SHS) exposure occurs when people inhale smoke from tobacco products or inhale smoke that has been expelled by other smokers. Exposure to SHS increases the risk of many metabolic and chronic diseases, such as cardiovascular disease.

Purpose: To evaluate the association between SHS and abdominal adiposity in US adults. Methods: Data from 2011-2012 National Health and Nutrition Examination Survey (NHANES) was used to examine the relationship between SHS exposure and abdominal adiposity, Sagittal Abdominal Diameter [SAD]), after adjusting for covariates (age, gender, ethnicity, and income) (n=4,012). Background: United States Air Force (USAF) personnel and retirees participate in aviation both on and off duty. Because aviation mortality is investigated by aircraft ownership, the combined mortality rate for on and off duty flying is unknown, as is aircraft-associated mortality among USAF retirees.

Methods: Mortality data were obtained from the Air Force Mortality Registry (AFMR) and publically available datasets. Air Transport deaths were defined following the Tenth International Classification of Diseases. USAF Active Duty (AD), Guard (ANG), Reserve (ARC) components, aggregate, and retiree rates (1988-2008) were calculated. Sex, age and rank-adjusted AD and retiree rates were compared with US …

This dissertation explores the presence of moral distress and effective interventions to lessen its impact on critical care nurses. Manuscript one was completed prior to entering the doctor of philosophy in nursing program but was instrumental in building the foundation for successive work within this dissertation. An exploratory, descriptive designed study was used to examine moral distress and identify situations in which nurse's experienced high levels of moral distress. Nurses completed a 38-item moral distress scale, a coping questionnaire, and indicated their preferred methods for institutional support in managing distressing situations. Over the past twenty years gynecologic surgery has evolved from primarily abdominal approach to the current focus on minimally invasive surgical techniques. Minimally invasive techniques include vaginal and laparoscopic techniques. Evolution in laparoscopic technology and equipment has exploded to include multiple vessel sealing instruments of varying modalities, instruments with “wristed” capabilities and robotic assisted technology. Single port laparoscopy and natural orifice transluminal endoscopic surgery are other variations of laparoscopic technology.

Transcranial direct current stimulation (tDCS) is the current technique used clinically to attenuate the effects of various neurological related deficits and augment functions such as memory and learning. The cellular and molecular mechanisms behind tDCS remain largely unknown and this study provides some of the first insights into the mechanisms behind tDCS. Direct current stimulation has been used to increase levels of long term potentiation (LTP) ex vivo suggesting that this stimulation has an effect on the LTP mechanisms of action. Previous animal experiments have indicated that administration of fluoxetine and simvastatin at 20-26 hours post-stroke decreases the volume of ischemic infarcts. This experiment expanded on previous experiments by adding ascorbic acid to the post-stroke regimen, initiating simvastatin pre-stroke, and adding a third initiation time frame (48-54 hours). Male retired breeder Sprague-Dawley rats were on simvastatin for 7 days prior to stroke induction. Combined medications of 5 milligrams/kilogram of fluoxetine, 1 milligram/kilogram of simvastatin and 20 milligrams/kilogram of ascorbic acid were orally administered at 6-12 hours, 20-26 hours, or 48-54 hours, respectively, following stroke induction. Haldane's Rule and Darwin's Corollary to Haldane's Rule are the observations that heterogametic F1 hybrids frequently are less fit than their homogametic siblings and that asymmetric results often are obtained from reciprocal hybrid crosses. In Caenorhabditis, Haldane's Rule and Darwin's Corollary have been observed in several hybrid crosses, including crosses of Caenorhabditis briggsae and Caenorhabditis nigoni. Fertile F1 females are obtained from reciprocal crosses. However, F1 males obtained from C. nigoni mothers are sterile and F1 males obtained from C. briggsae die during embryogenesis. We have identified cbr-him-8 as a recessive maternal-effect suppressor of F1 hybrid male-specific lethality …

Background: Although significant epidemiological evidence indicates that cigarette smoke exposure increases the incidence and severity of viral infection, the molecular mechanisms behind the increased susceptibility of the respiratory tract to viral pathogens are unclear. Adenoviruses are non-enveloped DNA viruses and important causative agents of acute respiratory disease. The Coxsackievirus and adenovirus receptor (CAR) is the primary receptor for many adenoviruses. We hypothesized that cigarette smoke exposure increases epithelial susceptibility to adenovirus infection by increasing the abundance of apical CAR. Treatment adherence rates are low among adult mental health patients. The purpose of this project is to utilize current literature regarding treatment adherence in adult mental health patients in order to identify nursing interventions to address treatment adherence and to create an educational presentation for mental health nurses regarding these interventions. The educational presentation will be created and presented to local mental health nurses in the Miami Valley, and a poster will be produced as a reference.

Mathematical models of drug action and disease progression can inform pediatric pharmacotherapy. In this tutorial, we explore the key issues that differentiate pediatric from adult pharmacokinetic (PK) / pharmacodynamic (PD) studies, describe methods to calculate the number of participants to be enrolled and the optimal times at which blood samples should be collected, and therapeutic drug monitoring methods for individualizing pharmacotherapy. The development of pediatric‐specific drug dosing dashboards is also highlighted, with an emphasis on clinical‐relevance and ease of use.

Introduction

The two most significant impediments to renal allograft survival are rejection and the direct nephrotoxicity of the immunosuppressant drugs required to prevent it. Calcineurin inhibitors (CNI), a mainstay of most immunosuppression regimens, are particularly nephrotoxic. Until less toxic antirejection agents become available, the only option is to optimize our use of those at hand.

Increasing concussion awareness and empowerment of these entities is critical to ensure that concussions are properly managed, risk is reduced, and effects are mitigated.

Viral Hemorrhagic Septicemia virus (VHSv) is an RNA rhabdovirus, which causes one of the world's most serious fish diseases, infecting >80 freshwater and marine species across the Northern Hemisphere. A new, novel, and especially virulent substrain—VHSv-IVb—first appeared in the Laurentian Great Lakes about a decade ago, resulting in massive fish kills. It rapidly spread and has genetically diversified. Discerning spatial macroecological patterns in freshwater fishes has broad implications for community assembly, ecosystem dynamics, management, and conservation. This study explores the potential interspecific covariation of geographic range (Rapoport's rule) and body size (Bergmann's rule) with latitude in North American sucker fishes (Cypriniformes: Catostomidae). While numerous tests of Rapoport's and Bergmann's rules are documented in the literature, comparatively few of these studies have specifically tested for these patterns, and none have incorporated information reflecting shared ancestry into analyses of North American freshwater fish through a hierarchical model.
