C-Reactive Protein: Potential Significance Of Quantitation In Patients With Chronic Diseases, Hajime Hayashi, Gerald A. Logrippo

Serum C-reactive protein (CRP) levels in 351 patients with certain chronic diseases were assayed by a micro-double immunodiffusion technique developed and used in this hospital. Elevation of CRP values and severity of disease were evident in the following clinical entities: 31 of 54 patients (57%) with liver cirrhosis; 122 of 204 (60%) hepatitis patients; 34 of 52 (65%) patients with gastro-intestinal surgery; 8 of 21 (38%) uremic patients; and all of 20 patients with renal transplantation. There was good correlation between markedly elevated serum CRP values and degree of inflammation and/or physiologic stress during clinical course and recovery.

Genetic Counseling For Hemoglobinopathies: A Model For Large Scale Application, Charles E. Jackson, Esly S. Caldwell, Ellis J. Van Slyck

This report describes a project in a large urban, predominantly black, high school which combined a hemoglobinopathy survey with an educational and genetic counseling emphasis. The faculty and students received information on sickle cell disease explaining its genetic and personal implications. About 29% of the 2,700 students received parental permission and volunteered for blood tests in the initial year. Blood was examined by starch gel electrophoresis during the first year and by cellulose acetate electrophoresis during the second year. Of the 1300 total individuals studied, 7% were found to have sickle cell trait and 2% hemoglobin C trait. The cellulose …

Mostly because of adequate early treatment, large epidermoid carcinomas are not frequently seen today. The case which we documented is unusual because of the huge size of the tumor and the resulting deformity. The photographs illustrate the pre-operative and post-operative appearance. The fact that these tumors can reach enormous size and become malignant bears repeating. Radical excision and skin grafting, we believe, resulted in a cure for this patient. Incidentally, there was a great change in her mental outlook after the removal of this tumor.

The Endoscopic Approach To The Diagnosis Of Upper Gastrointestinal Bleeding, Bernard M. Schuman, Roberto F. Gluckmann, Klaus Anselm, Robert J. Priest

Fifty-nine consecutive patients who were bleeding from the upper gastrointestinal tract were examined endoscopically with fiberoptic instruments. The site and nature of the bleeding were identified in 84% of the patients without significant morbidity or mortality. Endoscopic examination of the esophagus, stomach and duodenum in the bleeding patient should be done within 12 to 24 hours after hospital admission and before barium study of the upper gastrointestinal tract.

Research findings in the last 20 years concerning senile and postmenopausal osteoporosis indicate that the skeletal condition associated with these diseases arises because some systemic factor acts directly upon the bone marrow tissues. There is a secondary tendency to accelerate loss of bone greater than in the normal aging process. The loss occurs only on those surfaces in physical contact with the bone marrow tissue.

The purpose of this study was to evaluate microscopically the effect of topically applied 2% chlorhexidine gluconate on the healing of gingival wounds in dogs.

One week after the teeth were scaled and polished, seven experimental wound sites were prepared in the facial gingiva adjacent to the four premolars on each side of the maxillary arch in six young Labrador dogs. From this point the dogs were fed a soft plaque-inducing diet. The objectives of this study were to develop an animal model for testing oral bone and marrow transplantation that is adaptable to immunological studies; to determine the effect of an isogeneic graft of cancellous bone and marrow on the healing of surgically created defects; and to determine the fate of grafted isogeneic cancellous bone and marrow transplanted into defects. Fourteen CBA/H mice served as experimental animals and bilateral intraoral defects were prepared. The experimental side received isogeneic graft material while the other side served as a control. The purpose of this study was: to develop a model for testing oral bone transplantation; to study the healing of a surgically produced defect with and without the transplantation of strongly allogeneic cancellous bones and marrow; and to determine the fate of the allogenic grafts. Twenty-eight CBA/H mice served as experimental animals. Intra-oral defects were made and in 14 of the animals strongly allogenic cancellous bone and marrow were transplanted. Both dibutyryl cyclic AMP and trypsinized concanavalin A have been shown to inhibit growth in certain cell lines but little testing of these substances has been done using human tumor cell lines. Further, there are at this time no published experimental attempts to correlate the effects on growth with a particular phase of the cell cycle in any cell line.

The 16 species and subspecies of the Ambystomidae of the United States are placed into six groups according to their throat morphology as follows: The Dicamptodon group, the Rhyacotriton group, the A. Annulatum–A. cingulatum group, the A. gracile-A. jeffersonianum-A. mabeei-A. macrodactylum-A. maculatum group, the A. talpoideum group, the A. opacum-A. texanum group, and the A. rosaceum-A. t. tigrinum-A. t. melanostictum-A. t. nebulosum group. Dicamptodon appears to be the most primitive group and Rhyacotriton the most specialized. Based on the morphology of the throat region it is suggested that A. rosaceum be placed in the tiger salamander group as a subspecies …

The main purpose of this investigation was to test the endodontic sealing abilities of silver cones, N2, warm gutta percha, chemically softened gutta percha, laterally condensed gutta percha and Cavit under carefully controlled conditions. Evaluations were done by radiographic, macroscopic, microscopic, and autoradiographic examinations.

The four points which must be considered when evaluating the efficacy of sealing root canal systems, in vivo, were presented and discussed.

An analysis has been made of the basic ecology of aspen forests of the mountainous areas of Utah and northern Arizona. Forty-nine study areas have been examined in respect to density, height and age of the tree cover, tree reproduction, and composition and biomass production of the understory. A graphic model of interspecific assocations among prevalent understory species has been prepared by a cluster-analysis procedure based on presence and absence of species in 1,225 small quadrats (0.25 m²) uniformly distributed among the 49 stands. The model separates species of relatively stable as opposed to rapidly seral aspen forests. …

Lipid is associated with all of the major serum protein fractions of both the mallard duck and man. however, whereas the ρ lipoproteins predominate in man with little lipo-albumin evident, according to available literature, in ducks the lipo-albumin constitutes more than half of the serum lipoproteins, and a substantial lipo-prealbumin fraction is evident. Most studies of lipo-proteins are carried out on human serum primarily for the purpose of diagnosis of abnormalities. The cytology of rectal imprints of 105 control subjects and 79 patients with various rectosigmoid diseases has been studied. Our modified slide applicator was employed and found to be efficient, safe and easy to use. The cytological method has been more accurate than evaluation of the macroscopic appearance of the rectal mucosa only and therefore more efficient in the early diagnosis and follow-up of inflammatory bowel diseases. If used during routine proctosigmoidoscopy, it can help to determine the true incidence of proctocolitis in the population at large.

Gout and pseudogout, two metabolically different diseases, occur rarely in the same individual. A patient is reported who featured calcium pyrophosphate dihydrate and monosodium urate cryslals simultaneously in the synovial fiuid exudate which was removed during an acute arthritic attack of the left knee. The patient represents the seventh such case reported in the literature. The pathogenetic differences of gout and pseudogout are reviewed as they relate to the role of the crystal.

Choline salicylate is the anhydrous choline salt of salicylic acid. It possesses the therapeutic effects of aspirin. Clinical trials suggest that it is less irritating to the gastric mucosa. A double-blind gastroscopic study of the acute effects on the gastric mucosa of choline salicylate, aspirin and matching placebo was carried out. Using the fiberoptic gastroscope. the stomach was examined and photographed before, and ten minutes after, the patient had swallowed the tablets. Results were evaluated on the basis of combined endoscopic and photographic observations. This study suggests that choline salicylate in its tablet form causes less gastric mucosal injury than …

Cecal diverticula, although rare, may result in acute or chronic diverticulitis. Rarely does acute hemorrhage arise from cecal diverticula. The diagnosis of acute diverticulitis usually is evident at operation and an appropriate resection is usually feasible, although the local status and general condition of the patient modifies treatment in some instances. Chronic cecal diverticulitis is usually difficult to distinguish from carcinoma and a resection is justified.

Four of 13 patients with idiopathic hypoparathyroidism had associated congenital anomalies of branchial origin. Three had characteristic hypernasal speech. One of these patients exhibited a cleft palate and the other two had functional and anatomic anomalies of the velo-pharyngeal musculature which explained the speech disturbance. The fourth patient represents the twenty-second recorded case of the III and IV pharyngeal pouch (DiGeorge's) syndrome, manifested by absent parathyroids and thymus glands associated with unusual facial features and cardiovascular anomalies. Our four patients exhibited a total of 15 congenital anomalies of branchial origin. By including our patients with those reviewed from the literature, …
