The Federal Aviation Administration emphasized the need to focus on and develop human factors training as early as 1993 in official Human Factors Policy Order 9550.8. The purpose of this study was to conduct a detailed qualitative phenomenological analysis of the risk perceptions and decision-making model of collegiate aviation pilots for unstabilized approaches. The study focused on understanding how collegiate aviation pilots perceive unstable approaches, the risk associated with unstable approaches, and the factors that trigger pilots to execute a go-around. The increasing availability of smartphones allowed people to easily capture and share images on the internet. These images are often associated with metadata, including the image capture time (timestamp) and the location where the image was captured (geolocation). The metadata associated with images provides valuable information to better understand scenes and events presented in these images. The timestamp can be manipulated intentionally to provide false information to convey a twisted version of reality. Eye movement characteristics might provide insights on pilots' mental fatigue during prolonged flight. The visual entropy, eye fixation numbers, and eye fixation durations of ten novice pilots and ten expert pilots were analyzed for a four-hour simulated flight task consisting of four consecutive flight legs. Each flight leg lasted approximately one hour and contained five flight phases: takeoff, climb, cruise, descend, and landing. The pilots maneuvered the simulated B-52 aircraft following instrument flight rules (IFR) in a moderate-fidelity Microsoft Flight Simulator environment. The majority of First Officers and Captains feel that Captains establish the tone of the flight deck microculture. However, the majority of these pilots also report that as First Officers, they have hesitated to share safety-pertinent data and/or experience being silenced after sharing safety data due to a psychologically unsafe flight deck. Utilizing direct feedback from industry pilots, we explore ways that Captains can create a more inclusive microculture that elicits safety voice. The FAA requires airports operating under the Code of Federal Regulations Part 139 to conduct a wildlife hazard assessment (WHA) when some wildlife-strike events have occurred at or near the airport. The WHA should be conducted by a Qualified Airport Wildlife Biologist (QAWB) and must contain several elements, including the identification of the wildlife species observed and their numbers; local movements; daily and seasonal occurrences; and the identification and location of features on and near the airport that could attract wildlife. Past accidents have indicated that first officers (FO) are less likely to identify and correct captain errors than captains are to correct FO errors. Crew resource management (CRM) training was introduced in the late 1970s to improve captain teamwork skills to utilize the FO more effectively and to increase FO willingness to interject to preserve safety. Despite the effectiveness of CRM training programs, there continue to be incidences where subordinate pilots make weak or ineffective attempts to preserve safety.

Fatigue is a pervasive safety hazard in aviation affecting several aspects of a pilot's’ ability to safely perform their jobs. Several factors can contribute to fatigue, including inadequate sleep, stress, long work hours, excessive workload, and inadequate nutritional habits. In addition to flight training, some factors including academic, social, part-time work, and emerging time management skills are unique for Title 14 Code of Federal Regulations (CFR) Part 141 collegiate aviation pilots. Regardless of the type of maintenance performed on aircraft, instructions are to be used to provide the aviation technicians completing the maintenance activities with guidance on, and an outline of, the maintenance items to be performed and completed. However, the use of instructions does not guarantee the correct and proper completion of the maintenance activities as the instructions may be erroneous and/or maintenance personnel can misunderstand, misinterpret, or improperly follow the procedures outlined. The science on mindfulness as it relates specifically to aviation is sparse. It has only been in recent years that mindfulness has been examined as it relates to aviation. That being the case, this paper draws connections from areas that are already well studied with mindfulness; stress, anxiety, attention, and communication. Evidence exists that even short duration mindfulness interventions can have positive effects on mindfulness. The one-day workshop created with this thesis gives pilots a cursory overview of mindfulness as it relates to various areas of commercial aviation. The most frequent obstacle of an aircraft evacuation is the passengers carrying baggage while evacuating. Passengers who insist on taking their carry-on baggage during an emergency evacuation not only slow down the evacuation process but also act as a significant risk to the safety of other passengers. This study investigated the factors that affect passengers’ behavioral intention to evacuate with carry-on baggage and the effects of evacuating with carry-on baggage on the total evacuation time. Abstract Fatigue is a significant contributing factor that reduces human ability and leads to accidents and threatens the safety of aircraft and human lives. Approximately 70% of fatal accidents that occur in commercial aviation operations are due to human factors. Purpose: Aircraft accidents due to wildlife hazards have become a growing safety and economic problem to the Brazilian and international aviation industries. These safety occurrences have resulted in significant direct and indirect economic losses as well injuries and fatalities worldwide. The purpose of this study was to develop empirical information obtained from the analysis of wildlife strike and aircraft operations data in Brazil that could be used for accident prevention efforts.

The presence of human factors in aviation remains a critical area of research given the safety implications of human error. Understanding what specific factors contribute to human error allows managers and operators to take steps to mitigate these hazards.
