Background Tackling problematic polypharmacy requires tailoring the use of medicines to individual needs and circumstances. This may involve stopping medicines (deprescribing) but patients and clinicians report uncertainty on how best to do this. The TAILOR medication synthesis sought to help understand how best to support deprescribing in older people living with multimorbidity and polypharmacy. Objectives We identified two research questions: (1) what evidence exists to support the safe, effective and acceptable stopping of medication in this patient group, and (2) how, for whom and in what contexts can safe and effective tailoring of clinical decisions related to medication use work …

The SARS-CoV-2 pandemic has had a significant impact worldwide, particularly in middle- and low-income countries. While this impact has been well-recognized in certain age groups, the effects, both direct and indirect, on the neonatal population remain largely unknown. There are placental changes associated, though the contributions to maternal and fetal illness have not been fully determined. The rate of premature delivery has increased and SARS-CoV-2 infection is proportionately higher in premature neonates, which appears to be related to premature delivery for maternal reasons rather than an increase in spontaneous preterm labor. There is much room for expansion, including long-term data …

Impact Of Fast-Track Regulatory Designations On Strategic Commercialization Decisions For Autologous Cell Therapies, Ching Lam, Edward Meinert, Aidong Yang, Zhanfeng Cui

School of Nursing and Midwifery

Background: Regulatory authorities around the world have introduced incentives to improve the speed-to-market of innovative therapies. Aim & methods: To better understand the capacity and portfolio planning decisions of autologous cell therapies and particularly the impact of fast-tracking designations, this paper describes a mixed-integer linear programming approach for the optimization of capacity investment and portfolio selection decisions to maximize the net present value of a candidate portfolio of therapies under different regulatory programs. Results: The illustrative example shows that fast-track designations allow a 25% earlier breakeven, 42–86% higher net present value over a 20-year horizon with earlier upfront capital and …

Abstract STUDY QUESTION What are the intentions of men and women of reproductive age in the UK regarding reproduction and family building? SUMMARY ANSWER We identified six main categories of people; Avoiders, Betweeners, Completers, Desirers, Expectants and Flexers, for whom reproduction education strategies should be tailored differently to suit intentions. WHAT IS KNOWN ALREADY Several studies have highlighted poor fertility awareness across men and women of reproductive age. As the average age of first-time parents continues to rise, there has been a concerted effort from educators, healthcare professionals, charities, reproductive health groups and government policymakers, to improve fertility awareness. 'Mercy, Tenderness, And Compassion': A Study Into The Inclusion Of Acts Of Communal And Familial Support Within Seventeenth-Century Quaker Records Of Sufferings, Toni Wadman Jan 2022

'Mercy, Tenderness, And Compassion': A Study Into The Inclusion Of Acts Of Communal And Familial Support Within Seventeenth-Century Quaker Records Of Sufferings, Toni Wadman

School of Law, Humanities and Social Sciences Theses

This thesis explores the inclusion of encounters of communal and familial compassion and support within seventeenth century Quaker records of sufferings. Whilst previous academic works have used these accounts of communal support to highlight the social integration of Friends within their local communities, little focus has been placed upon the purpose or reasoning behind their presence within Quaker suffering records. This thesis, therefore, examines how Friends perceived and understood these encounters as well as the reasoning and motivation behind their inclusion in their accounts of sufferings. To help answer these questions, this project consults the Original Records of Sufferings, handwritten …

Painting, Playing, Personas: Inserting Change In An Established Painting Practice, Robyn Jean Thomas Jan 2022

Painting, Playing, Personas: Inserting Change In An Established Painting Practice, Robyn Jean Thomas

School of Law, Humanities and Social Sciences Theses

This doctoral research project explores the impact and changes alternative identities —personas— and a play-based method have on my established painting practice while disentangling it from the belief that a single identity authentically makes a painting art. Its methodology provides a path to answering how using personas with 'play' within a painting practice form strategies meeting this project's aims. Three personas were created and applied across the first two stages of the studio inquiry using my painting practice's methods, including reiteration. In the third stage, continuing to paint independent of the personas, their impact and that of the infinite play …

The Neoliberal Game: An Critical Analysis Of Children's Non Competitive Grassroots Football, Grace Gallacher Jan 2022

The Neoliberal Game: An Critical Analysis Of Children's Non Competitive Grassroots Football, Grace Gallacher

School of Law, Humanities and Social Sciences Theses

This thesis explores the propensity for harms throughout children’s non-competitive football. Adopting ethnographic approaches this project follows a team of under 9’s non-competitive team in a SW FA league, it analyses their private social media page and conducted 18 semi-structured interviews with life-long players, parents, coaches and referees. It advances a critical criminology of sport by presenting an alternative framework to Elias and the Civilising process by using contemporary critical criminology, namely ultra-realism and deviant leisure. By using ultra-realism and deviant leisure this study examines the underlying systemic harm which comes from the neoliberal environment of sport, including children’s non-competitive …

Solo Auto/Biographical Theatre: Towards A Process Of Self-Transformation, Morris Isaac Cohen Jan 2022

Solo Auto/Biographical Theatre: Towards A Process Of Self-Transformation, Morris Isaac Cohen

School of Law, Humanities and Social Sciences Theses

The concept of performance as a transformational experience is not unfamiliar in academic literature; Heddon (2008), Schechner (2013) and Zarrilli (2020) have all written about different aspects of transformations in theatre and performance, including the personal, the social and the political. The specific focus of this Practice as Research PhD is self-transformation and auto/biographical performance. It asks how making, devising and performing a solo auto/biographical piece of work might be a transformational experience for the performer. By transformation, the research refers to an inner shift in perspective on outer circumstances or events, or a change in meaning-making for and by …

Diagrams Of Disturbance: Conceptual Diagrams And The Production Of A Personal Diagram Poetry., Russell Evans Jan 2022

Diagrams Of Disturbance: Conceptual Diagrams And The Production Of A Personal Diagram Poetry., Russell Evans

School of Law, Humanities and Social Sciences Theses

Russell Evans Title: Diagrams of Disturbance: conceptual diagrams and the production of a personal diagram poetry. This thesis is a pursuit of a personal diagram poetry, with a submission of part creative work, part critical exploration of selected diagram poetry over the past 50 years. I examine my own work and that of pioneering diagram poet Jim Rosenberg as variations of the conceptual diagram, and explore wider examples in the work of contemporary visual poets, arriving at a diagrammatics that creates systems of thought from modalities of materiality, spatiality and iconography. I follow Foucault and Deleuze’s pursuit of the diagram …

Rethinking Liveness: An Exploration Of Phenomenological Liveness, Documentation, And Audience, Samantha Sully Jan 2022

Rethinking Liveness: An Exploration Of Phenomenological Liveness, Documentation, And Audience, Samantha Sully

School of Law, Humanities and Social Sciences Theses

This paper explores experiential liveness discourse through the lens of phenomenology, semiotics, and auto-ethnographical research. This thesis examined previous experiential liveness studies and discourses and how these ideologies build a discourse centred around audience/performer relationships, communitas and virtual communities. It explores the interactions between experiential and ontological liveness through documentation, digital archiving and Descartesian dualism. Further, this thesis re-frames core ontological terms within the phenomenological liveness context. It examines event and ritual as experiences and explores my experiences of liveness with reference to previous experiential liveness studies. Finally, this paper examines the importance of experiential liveness as its own discourse …

Development And Validation Of The Short Version Of The Sense Of Humor Scale (Shs-S), Sonja Heintz, Willibald Ruch, Chloe Lau, Donald H. Saklofske, Paul Mcghee

School of Psychology

Abstract. Humor training has become increasingly popular to enhance the “sense of humor” and well-being and to decrease depressive symptoms. Despite the wide applications of these training programs, the assessment of training efficacy has attracted less attention. The Sense of Humor Scale (SHS; McGhee, 1996 , 1999 ) recently was expanded to a long version (SHS-L) to enhance its internal consistency ( Ruch & Heintz, 2018 ). At the same time, there is also the need for a brief version of this scale. The purpose of the present study is to develop a short version (SHS-S) in both German- and …

Abstract: A critical challenge during volcanic emergencies is responding to rapid changes in eruptive behaviour. Actionable advice, essential in times of rising uncertainty, demands the rapid synthesis and communication of multiple datasets with prognoses. The 2020–2021 eruption of La Soufrière volcano exemplifies these challenges: a series of explosions from 9–22 April 2021 was preceded by three months of effusive activity, which commenced with a remarkably low level of detected unrest. Here we show how the development of an evolving conceptual model, and the expression of uncertainties via both elicitation and scenarios associated with this model, were key to anticipating this …

Covid-19 Pandemic Impact On Psychotropic Prescribing For Adults With Intellectual Disability: An Observational Study In English Specialist Community Services, D Naqvi, B Perera, S Mitchell, R Sheehan, R Shankar

Peninsula Medical School

Background Coronavirus disease 2019 (COVID-19) has had a disproportionate impact on people with intellectual disability (PwID). PwID are at higher risk of mental illness and receive psychotropic prescribing ‘off licence' also, to manage distress behaviour. The lockdown and reduction of multidisciplinary face-to-face appointments had an impact on care delivery, the recourse possibly being psychotropic prescribing. It is imperative to comprehend the influence the pandemic had on psychotropic prescribing patterns to enable future planning. Aims The aim was to understand the impact of the pandemic by comparing psychotropic prescribing patterns during the England lockdown with the prescribing patterns before lockdown in …

A Multi-Trophic Marker Approach Reveals High Feeding Plasticity In Barents Sea Under-Ice Fauna, Doreen Kohlbach, Lukas Smik, Simon T. Belt, Haakon Hop, Anette Wold, Martin Graeve, Philipp Assmy

School of Geography, Earth and Environmental Sciences

Microalgae growing within and attached to the bottom of Arctic sea ice (sympagic algae) can serve as a nutritious food resource for animals inhabiting the sea-ice water interface (under-ice fauna), particularly during the bottom ice-algal bloom in spring. As a consequence, under-ice fauna is likely impacted by sea-ice decline and changes in ice-algal primary production. To investigate this, samples of pelagic (=PPOM) and ice-associated particulate organic matter (=IPOM) and the iceassociated amphipods Apherusa glacialis and Eusirus holmii, and polar cod (Boreogadus saida), collected below ridged sea ice at two locations with pronounced differences in productivity in the northern Barents Sea …

Data were transcribed verbatim and analysed using inductive framework analysis. Results: GPs’ attitudes (importance, feasibility, appropriateness, helpfulness, and effectiveness), capacities (comfort, confidence, and knowledge), perceived subjective norms (role expectations), willingness, and …

Experimental Evaluation Of Plastic Wake On Growing Fatigue Cracks From The Analysis Of Residual Displacement Fields, Jm Vasco‐Olmo, Francisco A. Díaz, A Camacho‐Reyes, Neil James, Fernando V. Antunes Jan 2022

Experimental Evaluation Of Plastic Wake On Growing Fatigue Cracks From The Analysis Of Residual Displacement Fields, Jm Vasco‐Olmo, Francisco A. Díaz, A Camacho‐Reyes, Neil James, Fernando V. Antunes

School of Engineering, Computing and Mathematics

A growing fatigue crack gives rise to a plastically deformed wake of material that envelops the crack. In this work, the plastic wake extent during fatigue crack growth is experimentally quantified by analyzing the crack tip displacement fields measured with digital image correlation. A novel technique based on use of a yield criterion is proposed that uses the undamaged state of the specimen as the reference state in the image processing. The plastic wake was identified by differentiation of the residual displacement fields obtained with a near-zero load level to avoid any rigid body motion. The plastic wake extent was …
