The sustainability of lean systems for long term is a major concern across various organizations implementing lean manufacturing methods. This issue can be attributed to inadequate infrastructure, inefficient process management, unsuitable personnel management methods and strategic tools. There is a strong need for addressing the risks for lean system sustainability. The resolution of the risks from a 'soft side' (people) perspective has not been addressed. The primary focus of this study is on the people-related risks.

In spite of the continuing decrease in accident rates in the US Construction industry, accidents are one of the key factors constraining construction industry in terms of cost and time. Fatality rates in the US are more than other developed countries. Research into accident causation included accident occurrence mechanisms, different levels and types of accident causes as and best practices for accident prevention. One of the main concerns for the secure and reliable operation of power systems is the small signal stability problem. In the complex and highly interconnected structure of future power systems, relying solely on operator responses and conventional controls cannot assure reliability. Beginning in the 1970's, power uprates in nuclear power plants began to cause an operational problem called Crud Induced Power Shift (CIPS). Over decades, a method has been developed and refined that has allowed industry to effectively avoid CIPS. However, increasingly challenging economic environments have caused power plants to utilize more aggressive core designs. The problem of CIPS still looms over many reactors as a potential hazard requiring conservative measures. CIPS is due to complex physical and chemical interactions.
