Primary care is in a state of crisis due to the lack of clinicians and increasing numbers of insured patients. Encouraging more students to go directly through school for their doctor of nursing practice degree and nurse practitioner (NP) certifications is one proposal to alleviate this crisis. However, this approach would deliver graduates with minimal leadership and clinical experiences directly into practice. One resolution to mitigate this concern would be an NP residency program. Fibromyalgia (FMS) goes undiagnosed in as many as 3 out of 4 people who have the disease. Primary care providers (PCPs) are the first to evaluate patients; therefore, PCPs need to be able to recognize FMS, implement initial treatment, and refer for further consultation. The Fibromyalgia Diagnostic Screening Tool (FDST), a validated instrument to identify FMS, can improve the speed and accuracy of FMS diagnosis. Overweight and obesity in patients with intellectual and developmental disabilities (IDD) are at epidemic levels in the United States. The problem identified for this DNP project is the incidence of overweight and obesity in IDD. The framework used involved the quality model for improvement. For this DNP project a new program called the Daily-Enhanced Physical Activities Program (DEPA) was developed to promote weight loss, reduce sedentary activities, and optimal body mass index (BMI) for IDD patients. Ongoing training for unregulated care providers (UCPs) in Ontario is required to meet the care needs of the aging population. Inconsistencies in standards and options for formal training for UCPs as well as the evolving complex needs of the aging population are factors contributing to needing training programs for UCPs in the workforce. This thesis examined the delivery and transfer of professional learning for UCP training in resident centred care. Breast milk is the ideal source of nutrition for infants for at least the first 6 months of life. Despite women in the United States reaching national objectives for breast-feeding initiation (75%), racial disparities persist. According to the most recent data, 83% of white mothers initiated breast-feeding whereas only 66% African American mothers did so. Breast-feeding initiation may be amenable to perinatal nursing intervention. Breast-feeding may be a practice that helps bridge the racial divide in perinatal outcomes among African American families. This study conducted secondary analysis of data from the National Longitudinal Transition Study-2 (NLTS2) to examine the degree to which student, family, and school constructs predicted self-determination outcomes. Multi-group structural equation modeling was used to examine predictive relationships between 5 student, 4 family, and 7 school constructs developed from NLTS2 data and self-determination outcomes (autonomy, psychological empowerment, and self-realization) across disability groups. The pattern of predictive relationship between the constructs and self-determination outcomes across disability groups was complex. Only one construct—self-concept—showed a positive predictive relationship with all three self-determination constructs across most disability groups. Implications of the complex pattern of …

วัตถุประสงค์: เพื่อศึกษาปัจจัยทำนายความล่าช้าในการมารับบริการที่โรงพยาบาลของผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลัน\n\nรูปแบบการวิจัย: การวิจัยความสัมพันธ์เชิงทำนาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลัน จำนวน 148 คน คัดเลือกกลุ่มตัวอย่างแบบเจาะจง เก็บรวบรวมข้อมูลโดยการสัมภาษณ์ผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลันที่มาตามนัดและนอนโรงพยาบาลด้วยภาวะหัวใจขาดเลือดเฉียบพลันไม่เกิน 2 เดือน โดยใช้แบบสอบถามการตอบสนองต่ออาการต่างๆ มีค่าสัมประสิทธิ์อัลฟ่าของครอนบาคเท่ากับ .67 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสมการถดถอยโลจิสติก\n\nผลการวิจัย: พบว่า ผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลันส่วนใหญ่ (64.2%) เป็นเพศชาย อายุเฉลี่ย 53.59 ปี ประมาณ 2 ใน 3 (64.9%) ของผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลันมีความล่าช้าในการมารับบริการที่โรงพยาบาล โดยรวมตัวแปรที่ศึกษาทั้งหมดร่วมกันทำนายความล่าช้าได้ร้อยละ 52.1 (Pseudo R2 = 0.521, p <0.05) ประกอบด้วย อายุ เพศ การตอบสนองด้านการรับรู้ การตอบสนองทางอารมณ์ การตอบสนองทางพฤติกรรม และการตอบสนองทางอารมณ์หรือพฤติกรรมของบุคคลอื่น โดยมี 5 ตัวแปร ที่สามารถทำนายความล่าช้าในการมารับบริการที่โรงพยาบาล ได้แก่ การตอบสนองทางพฤติกรรมโดยพยายามผ่อนคลาย (OR = 35.566, B = 3.571, p <.05) อยู่กับครอบครัวในขณะเกิดอาการ (OR = 0.041, B = -. 3.191, p <.05) การตอบสนองทางอารมณ์หรือพฤติกรรมของบุคคลอื่นแนะนำให้พักผ่อนและทานยา (OR = 0.056, B = -2.883, p <.01) อยู่ที่บ้านขณะเกิดอาการ (OR = 8.738, B = 2.168, p <0.01) รอให้อาการหายไป (OR = 2.722, B = 1.019, p <0.01)\n\nสรุป: พยาบาลควรส่งเสริมให้ประชาชนมีความรู้ ความเข้าใจเกี่ยวกับอาการแสดงของภาวะหัวใจขาดเลือดเฉียบพลันเพื่อป้องกันความล่าช้าในการมารับบริการที่โรงพยาบาล\n

วัตถุประสงค์: เพื่อออกแบบ สร้าง และตรวจสอบคุณภาพเครื่องมือคัดกรองความเสี่ยงต่อการ\nเกิดภาวะสับสนเฉียบพลันในผู้สูงอายุหลังผ่าตัด\n\nแบบแผนงานวิจัย: การวิจัยเชิงบรรยาย\n\nวิธีดำเนินการวิจัย: การพัฒนาเครื่องมือแบ่งเป็น 2 ระยะ คือ ระยะที่ 1 การสร้างแบบประเมินความเสี่ยงต่อภาวะสับสนเฉียบพลันหลังผ่าตัดสำหรับผู้สูงอายุ ประกอบด้วยปัจจัยเสี่ยงต่อการเกิดภาวะสับสนเฉียบพลันที่ได้จากการทบทวนวรรณกรรมและการทบทวนข้อมูลย้อนหลังภาวะสับสนเฉียบพลันหลังผ่าตัดของผู้สูงอายุจากบันทึกทางการพยาบาล และบันทึกอาการ ได้ปัจจัยเสี่ยง 7 ข้อ คือ อายุที่เพิ่มขึ้น ประวัติติดสุรา ภาวะซึมเศร้าก่อนผ่าตัด การได้รับยากลุ่ม Benzodiazepines, Anticholinergics ความผิดปกติของเชาวน์ปัญญา การคิด การรับรู้ ระดับอิเล็กโตรไลท์ในเลือดไม่สมดุล และระดับฮีโมโกลบิน ฮีมาโตรคริตที่ผิดปกติ จากนั้น จึงจัดทำเครื่องมือ ตรวจสอบความตรงจากผู้เชี่ยวชาญ 5 คน ผลค่าความตรงตามเนื้อหาเท่ากับ 1.0 ระยะที่ 2 การตรวจสอบแบบประเมินความเสี่ยงต่อภาวะสับสนเฉียบพลันหลังผ่าตัดสำหรับผู้สูงอายุ โดยนำไปทดลองใช้กับกลุ่มตัวอย่าง คือ ผู้ป่วยสูงอายุที่มีอายุ 60 ปีบริบูรณ์ขึ้นไป ทั้งเพศชายและเพศหญิงที่เข้ารับการผ่าตัดทางศัลยกรรมออร์โธปิดิกส์แผนกผู้ป่วยในโรงพยาบาลระดับตติยภูมิเขตกรุงเทพฯ จำนวน 300 คน\n \nผลการวิจัย: ค่าสัมประสิทธิ์สหสัมพันธ์ระหว่างแบบประเมินภาวะสับสนเฉียบพลันหลังผ่าตัดสำหรับผู้สูงอายุและเครื่องมือมาตรฐานมีความสัมพันธ์กันสูง (r = .84) และมีค่า z = 0.2 และ z1= 0.33 แสดงว่าความสามารถในการทดสอบของเครื่องมือทั้งสองชนิดไม่มีความแตกต่างกัน ได้ค่าความไวร้อยละ 88.46 - 92.0 ค่าความเฉพาะร้อยละ 100 แสดงว่าเครื่องมือที่สร้างขึ้นใหม่นี้สามารถค้นหา และคัดกรองผู้ป่วยที่มีความเสี่ยงต่อภาวะสับสนเฉียบพลันได้\n\nสรุป:แบบประเมินความเสี่ยงต่อภาวะสับสนเฉียบพลันหลังผ่าตัดสำหรับผู้สูงอายุเป็นเครื่องมือที่มีความไวและค่าความเฉพาะที่ดีสามารถคัดกรองภาวะสับสนเฉียบพลันหลังผ่าตัดในผู้สูงอายุได้ \n

วัตถุประสงค์: เพื่อเปรียบเทียบสมรรถภาพปอดของผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังก่อนและหลังของการเข้าร่วมโปรแกรมการจัดการตนเอง และเปรียบเทียบระหว่างกลุ่มที่เข้าร่วมและไม่ได้เข้าร่วมโปรแกรมการจัดการตนเอง\n \nแบบแผนงานวิจัย: กาวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังที่มีระดับความรุนแรงของโรคอยู่ในระดับ 2-3 จำนวน 40 คน ที่มารับการรักษาแผนกตรวจผู้ป่วยนอก หน่วยโรคปอด โรงพยาบาลจุฬาลงกรณ์ แบ่งเป็นกลุ่มทดลอง 20 คน และกลุ่มควบคุม 20 คน จับคู่ในด้าน เพศ อายุ ระดับความรุนแรงของโรค และชนิดของยารักษาโรคปอดอุดกั้นเรื้อรัง กลุ่มทดลองได้รับโปรแกรมการจัดการตนเอง เป็นระยะเวลา 8 สัปดาห์ และกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการทดลอง ได้แก่ โปรแกรมการจัดการตนเองที่สร้างขึ้นโดยใช้แนวคิดการจัดการตนเองของ Bourbeau (2008) ประกอบด้วย แผนการสอน คู่มือการจัดการตนเอง และวีดิทัศน์การบริหารการหายใจและการออกกำลังกาย ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 คน เก็บรวบรวมข้อมูลโดยใช้เครื่องตรวจสมรรถภาพปอด วัดความเร็วสูงสุดของลมหายใจออก (Peak expiratory flow rate: PEFR) วิเคราะห์ข้อมูลโดยหาค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที\n \nผลการวิจัย: 1) สมรรถภาพปอดของผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรัง ภายหลังได้รับโปรแกรมการจัดการตนเองสูงกว่าก่อนได้รับโปรแกรมจัดการตนเอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n2) สมรรถภาพปอดของผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังภายหลังการทดลอง กลุ่มที่ได้รับ\nโปรแกรมการจัดการตนเองสูงกว่ากลุ่มที่ได้รับการพยาบาลปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n \nสรุป: โปรแกรมจัดการตนเองส่งผลให้ผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังมีสมรรถภาพปอดดีขึ้นและดีกว่าการพยาบาลตามปกติ\n

วัตถุประสงค์: เพื่อศึกษาประสบการณ์การเป็นพยาบาลผู้ป่วยเด็กวิกฤต\n \nรูปแบบงานวิจัย: การวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาเชิงตีความ\n \nวิธีดำเนินการวิจัย: ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานในหอผู้ป่วยวิกฤตกุมารศัลยศาสตร์ โรงพยาบาลตติยภูมิแห่งหนึ่ง ในเขตกรุงเทพมหานคร ติดต่อกันเป็นระยะเวลานาน 5 ปีขึ้นไป จำนวน 14 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึก และการบันทึกเสียงสนทนา ข้อมูลที่ได้นำมา วิเคราะห์ข้อมูลเชิงเนื้อหาตามวิธีการของ Colaizzi\n\nผลการวิจัย: พยาบาลผู้ป่วยเด็กวิกฤตให้ความหมายการเป็นพยาบาลผู้ป่วยเด็กวิกฤตไว้ว่า จะต้องเก่ง มีความรู้ ความเชี่ยวชาญ เป็นคนที่มีความละเอียด รอบคอบ ช่างสังเกต และจะต้องมีใจรักและใส่ใจในการดูแลผู้ป่วยและครอบครัว สำหรับประสบการณ์การเป็นพยาบาลผู้ป่วยเด็กวิกฤต พบว่า ประกอบด้วย 4 ประเด็นหลัก ประเด็นแรก คือ ความสามารถเฉพาะทาง ซึ่งเป็นสมรรถนะในบทบาทหน้าที่เชิงวิชาชีพของพยาบาลผู้ป่วยเด็กวิกฤต ประเด็นที่ 2 ความเครียด ซึ่งเกิดจากการขาดความรู้ และทักษะในการปฏิบัติงาน เครียดจากผู้ร่วมงาน และเครียดจากการติดต่อประสานงานกับแพทย์ ประเด็นที่ 3 การปรับตัว พบว่า พยาบาลผู้ป่วยเด็กวิกฤตจะมีการปรับตัวเข้ากับลักษณะงานที่ทำ ผู้ร่วมงาน และตารางการทำงาน เมื่อปรับตัวได้ ทำให้ความเครียดลดลงและมีความมั่นใจในการทำงานมากขึ้น ประเด็นสุดท้าย พบว่า สิ่งที่ทำให้พยาบาลผู้ป่วยเด็กวิกฤตคงอยู่ในงานได้ เพราะบรรยากาศในการทำงานที่อบอุ่นเป็นกันเอง มีความเอื้อเฟื้อต่อกัน รับรู้ถึงคุณค่าของการเป็นพยาบาล มีความภาคภูมิใจในองค์กรและมีความสุขในการทำงาน และการทำงานเป็นความมั่นคงของครอบครัว\n\nสรุป: ข้อมูลที่ได้จากการศึกษาในครั้งนี้ทำให้เข้าใจถึงประสบการณ์การเป็นพยาบาลผู้ป่วยเด็กวิกฤตได้ชัดเจนมากขึ้น ความรู้ที่ได้รับสามารถนำไปเป็นพื้นฐานให้ผู้บริหารทางการพยาบาลนำไปพัฒนาเป็นกลยุทธ์สร้างความพึงพอใจและเพิ่มอัตราการคงอยู่ในงานของพยาบาลผู้ป่วยเด็กวิกฤต\n

Miscommunication or omission of critical patient information contributes to preventable

medical errors that result in 98,000 patient deaths each year. The hand-off

communication process creates a critical time for the patient as necessary information for
