Introduction: Job satisfaction is vital for the optimal functioning of medical practitioners. Herein, we report our experience of restructuring the internship program by identifying the gaps, developing, implementing strategies to overcome gaps and sharing the results of the pre-implementation and post-implementation audit, as an example for establishing a system for improving intern's work-based learning and satisfaction in a university hospital setting.

Aim:

To explore and compare expired CO2 (ECO2) and heart rate (HR), during newborn resuscitation with bag-mask ventilation, as predictors of 24-h outcome.

Methods:

Observational study from March 2013 to June 2017 in a rural Tanzanian hospital. Side-stream measures of ECO2, ventilation parameters, HR, clinical information, and 24-h outcome were recorded in live born bag-mask ventilated newborns with initial HR < 120 bpm. We analysed the data using logistic regression models and compared areas under the receiver operating curves (AUC) for ECO2 and HR within three selected time intervals after onset of ventilation (0-30 s, 30.1-60 s and 60.1-300 s).

Results:

Among 434 included newborns (median birth weight 3100 g), 378 were alive at 24 h, 56 had died. Both ECO2 and HR were independently significant predictors of 24-h outcome, with no differences in AUCs. In the first 60 s …

CONTEXT: Primary care physicians need a strong foundation in diabetes management, as they are the first line of care for patients with this complex disease, which is increasing in frequency in the United States. This foundational training begins in medical school, but its applications become more important during residency.

OBJECTIVES: To quantify osteopathic and allopathic family medicine residents' amount of exposure to diabetes in residency training, investigate referral patterns related to diabetes management, and assess comfort levels with various diabetes treatment modalities.

Multiple gestation has been on the rise because of advancement in assisted reproductive technology. Triplet pregnancy is associated with fetal loss and preterm birth as its major complications. Spontaneous triplet pregnancy is rare. In the case of fetal loss, delayed interval delivery has been used to achieve delivery of the retained fetuses. There is no common approach to delayed interval delivery. Although medical students enter medicine with altruistic motives and seek to serve indigent populations, studies show that medical students’ attitudes towards the undeserved tend to worsen significantly as they go through their medical education. This finding emphasizes the need for medical educators to implement activities such as service-learning that may help mitigate this negative trend. All students at the University of Nevada Las Vegas (UNLV) School of Medicine are required to participate in longitudinal service-learning throughout medical school, and a majority of students interact with the underserved at their service-learning sites. Anecdotal evidence suggested that hopelessness and helplessness (HH) were often reported by undergraduate medical students. It is known that medical students are more susceptible to high levels of stress and depression than other student groups. There is currently concern about suicide rates in students and high drop-out rates in junior doctors. But what can be said of HH within this population? This study was aimed at eliciting medical students’ experiences of HH. An interpretive phenomenological approach was adopted. Participants were recruited from a single medical school. BACKGROUND: Heart failure (HF) risk and the underlying risk factors vary by race. Traditional models for HF risk prediction treat race as a covariate in risk prediction and do not account for significant parameters such as cardiac biomarkers. Machine learning (ML) may offer advantages over traditional modeling techniques to develop race-specific HF risk prediction models and to elucidate important contributors of HF development across races.

METHODS: We performed a retrospective analysis of 4 large, community cohort studies (ARIC [Atherosclerosis Risk in Communities], DHS [Dallas Heart Study], JHS [Jackson Heart Study], and MESA [Multi-Ethnic Study of Atherosclerosis]) with adjudicated HF events. …

Recent data has demonstrated that the pathophysiology of severe COVID-19 infection is associated with a significant pulmonary coagulopathy. Thrombotic complications have been reported in approximately 35-45% of patients with severe COVID-19. Entry of SARS-CoV-2 into the host cells leads to dysregulation in inflammatory signalling pathways, disrupting the normal coagulation mechanism. The hypercoagulability with abnormal clot formation is attributed to the inappropriately elevated immune response, culminating in a ‘cytokine storm’ with high levels of pro-inflammatory cytokines and subsequent thrombosis. The coagulopathy in COVID-19 affects many coagulation parameters such as D-dimer levels, fibrinogen levels, platelet count and prothrombin time. Coagulation parameters must …

The purpose was to consolidate the published scholarly work in the PubMed database on different treatment modalities including vaccine, oxygen supplementation, dexamethasone or steroids, aspirin, heparin, remdesivir, zinc, vitamin c or ascorbic acid, oxygen, anti-malarial like chloroquine and hydroxychloroquine, and azithromycin or macrolides. This study aimed to analyze the effects of physical activities on the motor and cognitive functions of participants with special needs and compare these results with a similar program but organized online because of the health precautions that have been applied due to the COVID-19 pandemic. An intervention consisting of sending an educational brochure aiming to increase high school volunteers' health literacy was then tested and results showed that it improved participants' health outcomes and also could increase the program's effectiveness in the long term.

Though nonbinary and gender nonconforming people are becoming increasingly recognized in North America, specialized healthcare—specifically gender-affirming procedures—are not becoming available to them. Researchers have examined the subjective experiences of these individuals as they have navigated the healthcare system in order to further understand possible limitations for nonbinary and gender nonconforming populations. Methods include assessing the procedures available to participants and their interpersonal experiences with healthcare professionals and family members. Scientific resources on this topic are limited, and the results are overwhelmingly represented by self-report.
