Lower Double-Wall Puncture Rate During Ultrasound-Guided Internal Jugular Vein Cannulation Using Sharper, Narrower-Gauge, And/Or Length-Optimized Needles: A 6-Year Quality Improvement Clinical Series In Adult Patients, James M. School of Medicine Faculty Publications
Background; During internal jugular vein (IJV) cannulation, needle tip injury to vulnerable subjacent cervical anatomic structures can be prevented if the cannulating needle tip is not permitted, even momentarily, to penetrate the deep portion of the IJV wall, an event known as double-wall puncture (DWP), also called posterior wall puncture. Methods; We conducted a 6-year ultrasound-guided IJV cannulation quality improvement project, seeking to minimize the occur-rence of DWP in 228 adult patients using needles of different gauge and tip sharpness. Most needles were length-optimized to the distance between the skin puncture site and the IJV mid-lumen for a selected angle …

Mental Health And Experiences Of Pregnancy Among Black Women And Birthing People With Type 1 Diabetes (T1d), Madeleine E. Marcus
Doctoral Dissertations
This dissertation explored the potential effects of intersections of oppression (i.e., anti-Black racism, diabetes discrimination, and sexism) on mental health and pregnancy experiences among Black women and birthing people with Type 1 diabetes (T1D). Previous studies about pregnancy and T1D have included mainly white, cisgender women. Three participants qualified for inclusion in the present study and participated in semi-structured interviews, which the researcher analyzed using an interpretative phenomenological analysis (IPA) framework. Participants described feeling uncertain about whether aspects of their pregnancy experiences were related to obstetric and/or medical racism. Similarly, participants sometimes seemed uncertain about how racism may affect their …

Impact Of Interprofessional Student Teams At A Remote Area Medical Event In Rural Appalachia, Emily K. Flores, Karilynn Dowling, Caroline Abercrombie Md, Rick L. Wallace
Journal of Appalachian Health
Introduction: Education in interprofessional collaboration is vital to expand healthcare access, especially in areas of higher disparity. To address this need, interprofessional faculty collaborators incorporated undergraduate and graduate health profession students into teams at an annual Remote Area Medical event in rural Appalachia between 2017 and 2020.
Purpose: This article evaluates the impact of an interprofessional student teams model on both patient care experience and students’ interprofessional collaboration attitudes and behaviors.
Methods: Student volunteers completed pre- and post-event surveys containing questions about demographics, open-ended questions, and questions from two instruments: the Student Perceptions of Interprofessional Clinical Education-Revised Instrument, Version 2 …

Healthcare Providers’ Perceptions Of Deaf-Hearing Interpreter Teams: Impact Of Interpreting Approaches, Julayne Feilbach
Journal of Interpretation
This study explores the perceptions and preferences of healthcare providers who work with Deaf Interpreter-Hearing Interpreter (DI-HI) teams. Healthcare providers depend on interpreters' ability to bridge the communication and cultural gap to assess and treat patients accurately. Although there have been studies on healthcare providers’ perceptions of interpreters to date, none of the research explores the impact of healthcare providers’ perceptions on their experiences with DI-HI teams. To address this, interviews with nine healthcare practitioners were conducted. As part of the interview, participants were shown a video of two interpreting samples to illustrate different approaches to interpreting. Data were analyzed …

Using Ai For Diagnosis And Treatment Plan, Andrea Guthridge
AI Assignment Library
This assignment is to be used for Undergraduate Medical Education Training to consider the role of AI by patients and aspiring physicians. Students will comparing and contrast three self-diagnostic tools WebMD, ChatGPT, and Bard with a case study to determine a diagnosis and treatment plan. Additionally, students will consider concerns, benefits, privacy and accuracy, and how this will impact their practice as physicians.
Burnout As Chronic Disease,

Burnout As Chronic Disease, Benjamin Doolittle
Journal of Wellness
Burnout is endemic. The prevalence of burnout (~50%) among healthcare providers has been stable over the past 20 years. Acknowledging variability from year to year, burnout is pervasive, with no signs of abating. Despite increased awareness of mental health issues, the rise of Chief Wellness Officers, and the expansion of Employee Assistance Programs, there has not been much improvement. We are even burned out from burnout. Survey responses are low and often do not result in action. Longitudinal intervention trials are often small and of short duration. We need a new paradigm to manage physician burnout. A chronic care management …

Effects Of Transition From Closed-Book To Open-Book Assessment On Students’ Scores In A Pharmacokinetics Course, Reza Mehvar, Richard Beuttler
Pharmacy Faculty Articles and Research
Closed-book summative assessment of student learning, common in pharmacy education, is challenging to administer in a remote setting due to the need for costly and intrusive monitoring technology. Therefore, open-book assessments without monitoring have been considered an alternative in remote settings. The present study investigated the effects of the transition from in-person closed-book to remote open-book format on the students’ scores in different assessment categories in a Pharmacokinetics course. The students’ performances in the transition cohort (Transition, n = 96) during the in-person and remote periods were compared with those of an in-person cohort (Control, n = 85) during the …

How Often Do Medical Students Change Career Preferences Over The Course Of Medical School?, Jean-Sebastien Rachoin, M. Olguta Vilceanu, Natali Franzblau, Sabrina Gordon, Elizabeth Cerceo
College of Communication & Creative Arts Departmental Research
Introduction: During the preclinical years, students typically do not have extensive exposure to clinical medicine. When they begin their clinical rotations, usually in the third year, the majority of the time is spent on core rotations with limited experience in other fields of medicine. Students then must decide on their careers early in their fourth year. We aimed to analyze how often medical students change their career preferences between the end of their second and their fourth year. Methods: We conducted a retrospective, cohort study using the American Association of Medical Colleges Year 2 Questionnaire (Y2Q) and Graduating Questionnaire (GQ) …

The Effect Of An Intersectionality Elective On The Perceived Self-Efficacy Of Medical Students In Addressing Health Inequity, Patrick Rollo, Noel Higgason, Kaitlyn Stark, Amina Aslam, Mason Motakef, Olivia Kerr, Asia Mccleary-Gaddy
Teaching in Clinics
Background: Intersectionality is a theoretical framework that contextualizes an individual’s experience as more than the sum of their cultural identifiers.
Aims: The current study investigates the effect of a 10-week intersectionality curriculum supplemented with student-led discussions on medical students’ self-efficacy in addressing bias and health inequity.
Methods: First- and second-year medical students volunteered to participate in a ten-week pilot elective entitled “Intersectionality in Medicine.” Participants enrolled in the course were required to complete a ten-minute online pre-elective and post-elective survey.
Results: Results showed that after completing the elective, medical students were significantly more confident in identifying their personal biases, assessing …
