The following thesis examines the historical perspective of mental illness, leading to therapeutic interventions for individuals with Borderline Personality Disorder. The need for improved interventions are reflected in treatment failure with both historical and traditional therapies, including repeated hospitalizations, self- injurious behaviors and suicide attempts. This thesis uses the Systems Theory and Ecological Perspective to review and analyze past and present treatments, with special emphasis on the 'Dialectic Behavior Therapy,' (DBT) program initiated in 1993 exclusively for individuals diagnosed with Borderline Personality Disorder.

Experiencing Physical Violence During Pregnancy: Prevalence And Correlates, Vilma E. Cokkinides, Ann L. Coker

Violence during pregnancy directly impacts the mental and physical health of pregnant women. We assessed the prevalence and correlates of physical violence around the time of pregnancy in a representative sample of 6,718 women in South Carolina. Physical violence, defined as "being physically hurt by husband or partner" or "being involved in a physical fight" was reported by 10.9% of recently pregnant women. These were correlates of violence: experiencing increased numbers of stressful life events, being unmarried, having increased parity, being on Medicaid, and having an unwanted pregnancy. Violence against women is a significant public health problem which impacts women, men, and children. Little is known about the frequency or correlates of violence against women in Africa. In this cross-sectional study, we found that 66.7% of 144 women surveyed in a study of AIDS knowledge, attitude, and behaviours, report being beaten by an intimate male partner and 50.7% report having ever been forced to have sexual intercourse; 76.6% of women report either forced sex or intimate partner violence. Circumcised women were most likely to report intimate partner violence and forced sexual intercourse. To improve the health of women …

This study identifies factors associated with client participation in intensive, in-home family preservation services. Our initial focus was on client cooperation, but a review of the literature and preliminary analysis of empirical data convinced us to broaden this focus to consider the diverse ways in which clients participate in treatment.

The clients in our study were involuntary, in that they were referred for family preservation services by a public child welfare agency following substantiated reports of child abuse and neglect. The services were intended as an alternative to removal of a child from the home. The primary caregiver was the …

The purpose of this project was to design and develop a resource manual to help parents work effectively with at-risk, 5th grade students, in the Clover Park School District, Lakewood, Washington. To accomplish this purpose a review of current literature regarding at-risk students and parent involvement was conducted. Additionally, various organizations were solicited for community resources including: United Way, Department of Social & Health Services, Puget Sound Educational Service District, Equal Opportunity Resource Center, and Pierce County Community Action Agency. Finally, all resource materials were compiled, organized, and assembled.

Through the lens of an innovative community-university partnership in the Greater Boston region, the authors analyze how welfare reform organizing overtook a resident-driven empowerment project. Since a major goal of the Chelsea/Dudley Partnership in MA is to support residents in exerting greater power over the practices and policies of community agencies, projects have been initiated in the city of Chelsea and the Dudley neighborhood of Boston to organize, and to strengthen, low income women by training them as welfare advocates. The purpose of this research was to explore the applicability of William E. Cross' Black identity development model and Vivienne Cass' model of lesbian/ gay identity formation to the experience of Black lesbians. Through evaluating their applicability the experience of identity integration was addressed in the experience of Black lesbians whose intersections of identity may conflict within a predominately heterosexual Black community and in the visible lesbian community which is often perceived as predominately White.

Employed parents of children with serious emotional disorders must manage the competing demands of home and job. Although every situation is unique, all parents cope daily with the interface between employment and home. As the number of single employed mothers and dual-worker households has increased, so has the research on work and family. The fundamental structure of the public child welfare system is that of a coercive apparatus wrapped in a helping orientation. Agencies ostensibly having the mission to help are mandated to ask whether parents can be blamed for their child welfare problems, and these agencies have the power to remove children from their homes. Thus, the public child welfare agency has a dual-role structure: On one hand, the agency attempts to engage in prevention and support, and to promote family preservation; on the other hand, it also has the task of investigating complaints against parents and removing children from them. This thesis is an exploration of the models of relationship that are evident in the Region of Waterloo, between church-affiliated social service agencies and their supporting church bodies. Church-affiliated social service agencies were defined as those that either use Christian terminology or denominational names in their title, that require church representation at a board or corporate membership level, or that receive regular, committed financial or volunteer support from a church body. This is a study of Chinese congregations of Ontario in the context of the history of pastoral care. According to a historical review, there are three major practice paradigms of pastoral care in the Western church. The classical pastoral paradigm emphasizes a Christian message. The clinical paradigm emphasizes personality and relationships. The communal- contextual paradigm tries to cover a wholistic goal namely, message, person, and context. In the nineteenth century, the Chinese church integrated the classical paradigm into contemporary culture. The role of identity for the pastoral counsellor is explored in its function in the development of professional identity. This thesis utilizes Object Relations Theory and Process Theology as conceptual foundations in articulating the unique process of the emerging professional identity of the pastoral counsellor. It is argued that, when grounded within selfobjects that embrace our theological training as pastors, we can as clinicians engage ourselves in the process of change. This study investigated the relationship between favorable employability ratings of hypothetical job applicants with a severe disability and two aspects of employers' perceived organizational context: organizational climate and negotiation latitude, using a cross-sectional, correlational design. A survey including a hypothetical job applicant vignette in one of three conditions: non-disabled, severe physical disability (acquired brain injury), severe psychiatric disability (schizophrenia) was mailed out to a random sample of 1,000 employers selected from a national human resource membership list. Research has indicated that social workers are more politically active than the general public (Wolk, 1981; Parker and Sherraden, 1991), but their effectiveness has been questioned (Mathews, 1982). There are differences among social workers, but explanations of differences between "very active" and "inactive" have relied primarily on practice setting or method. However, research in political science has shown that income, education, involvement in associations, and perceived political efficacy, are significant predictors of who does not participate (Verba, Schlozman, and Brady, 1995).

This study examines the success of Children's Service Society of Wisconsin's (CSSW's) in-home parenting skills training program for parents with developmental disabilities or considered low-functioning the referring county social service agency. The purpose of this program is to improve parenting effectiveness in an effort to maintain a safe and nurturing home environment for children in their parental home. Data were collected by administering a parenting effectiveness questionnaire to Children's Service Society of Wisconsin parenting skills trainers on a pre-test-post-test basis. Linking data from the Panel Study of Income Dynamics with the 1970 and 1980 censuses, the author finds that, in accordance with the social isolation theory, neighborhood conditions greatly affect the likelihood of exiting the Aid to Families with Dependent Children (AFDC) program. Neighborhood conditions are especially strong predictors of leaving the program for African Americans, high school dropouts, and for those who marry. Members of each of these groups stay on welfare significantly longer when they live in the most economically depressed neighborhoods of neighborhoods with a relatively high proportion of manufacturing relative to professional and executive workers. Poor farms, which spread to the Midwestern United States in the nineteenth century, were intended to provide work for their residents. Existing literature indicates that the need for work and the ability of residents to work was limited on Midwestern poor farms and that it decreased with time. In the historical case study of a rural Minnesota poor farm presented here, data support contentions of the literature. Between 1889 and 1935, the Yellow Medicine County Poor Farm expanded and modernized the house, while allowing an originally modern farming operation to stagnate. Twenty female administrators in human service agencies serving diverse ethnic communities were interviewed to provide an understanding of their struggles and leadership styles. Applying both Africentric and feminist theoretical frameworks to inform the political frame advanced in theories of transformational leadership, connecting with community and community building were strategies for providing leadership.

Conservatives generally believe that government largess has created a morally defective welfare subculture. Some argue that excessive welfare payments contribute to high homicide rates by undermining individual responsibility and attachment to traditional social institutions. Liberals, on the other hand, suggest that higher welfare benefits may reduce homicide rates by alleviating the strains of poverty and promoting educational achievement. Drawing on a review of literature and aggregate welfare data, this study examined the relationship between local AFDC benefit levels and homicide rates.
